Obese patients receive worse care solely based on their weight

”You must lose weight, a doctor told Sarah Bramblette, advising a 1,200-calorie-a-day diet. But Ms. Bramblette had a basic question: How much do I weigh?

The doctor’s scale went up to 350 pounds, and she was heavier than that. If she did not know the number, how would she know if the diet was working?

The doctor had no answer. So Ms. Bramblette, 39, who lived in Ohio at the time, resorted to a solution that made her burn with shame. She drove to a nearby junkyard that had a scale that could weigh her. She was 502 pounds.

One in three Americans is obese, a rate that has been steadily growing for more than two decades, but the health care system — in its attitudes, equipment and common practices — is ill prepared, and its practitioners are often unwilling, to treat the rising population of fat patients.

The difficulties range from scales and scanners, like M.R.I. machines that are not built big enough for very heavy people, to surgeons who categorically refuse to give knee or hip replacements to the obese, to drug doses that have not been calibrated for obese patients. The situation is particularly thorny for the more than 15 million Americans who have extreme obesity — a body mass index of 40 or higher — and face a wide range of health concerns.

Part of the problem, both patients and doctors say, is a reluctance to look beyond a fat person’s weight. Patty Nece, 58, of Alexandria, Va., went to an orthopedist because her hip was aching. She had lost nearly 70 pounds and, although she still had a way to go, was feeling good about herself. Until she saw the doctor.

“He came to the door of the exam room, and I started to tell him my symptoms,” Ms. Nece said. “He said: ‘Let me cut to the chase. You need to lose weight.’”

The doctor, she said, never examined her. But he made a diagnosis, “obesity pain,” and relayed it to her internist. In fact, she later learned, she had progressive scoliosis, a condition not caused by obesity.

Dr. Louis J. Aronne, an obesity specialist at Weill Cornell Medicine, helped found the American Board of Obesity Medicine to address this sort of issue. The goal is to help doctors learn how to treat obesity and serve as a resource for patients seeking doctors who can look past their weight when they have a medical problem.

Dr. Aronne says patients recount stories like Ms. Nece’s to him all the time.

“Our patients say: ‘Nobody has ever treated me like I have a serious problem. They blow it off and tell me to go to Weight Watchers,’” Dr. Aronne said.

“Physicians need better education, and they need a different attitude toward people who have obesity,” he said. “They need to recognize that this is a disease like diabetes or any other disease they are treating people for.”

The issues facing obese people follow them through the medical system, starting with the physical exam.

Research has shown that doctors may spend less time with obese patients and fail to refer them for diagnostic tests. One study asked 122 primary care doctors affiliated with one of three hospitals within the Texas Medical Center in Houston about their attitudes toward obese patients. The doctors “reported that seeing patients was a greater waste of their time the heavier that they were, that physicians would like their jobs less as their patients increased in size, that heavier patients were viewed to be more annoying, and that physicians felt less patience the heavier the patient was,” the researchers wrote.

Other times, doctors may be unwittingly influenced by unfounded assumptions, attributing symptoms like shortness of breath to the person’s weight without investigating other likely causes.

That happened to a patient who eventually went to see Dr. Scott Kahan, an obesity specialist at Georgetown University. The patient, a 46-year-old woman, suddenly found it almost impossible to walk from her bedroom to her kitchen. Those few steps left her gasping for breath. Frightened, she went to a local urgent care center, where the doctor said she had a lot of weight pressing on her lungs. The only thing wrong with her, the doctor said, was that she was fat.

“I started to cry,” said the woman, who asked not to be named to protect her privacy. “I said: ‘I don’t have a sudden weight pressing on my lungs. I’m really scared. I’m not able to breathe.’”

“That’s the problem with obesity,” she said the doctor told her. “Have you ever considered going on a diet?”

It turned out that the woman had several small blood clots in her lungs, a life-threatening condition, Dr. Kahan said.

For many, the next step in a diagnosis involves a scan, like a CT or M.R.I. But many extremely heavy people cannot fit in the scanners, which, depending on the model, typically have weight limits of 350 to 450 pounds.

Scanners that can handle very heavy people are manufactured, but one national survey found that at least 90 percent of emergency rooms did not have them. Even four in five community hospitals that were deemed bariatric surgery centers of excellence lacked scanners that could handle very heavy people. Yet CT or M.R.I. imaging is needed to evaluate patients with a variety of ailments, including trauma, acute abdominal pain, lung blood clots and strokes.

When an obese patient cannot fit in a scanner, doctors may just give up. Some use X-rays to scan, hoping for the best. Others resort to more extreme measures. Dr. Kahan said another doctor had sent one of his patients to a zoo for a scan. She was so humiliated that she declined requests for an interview.

Problems do not end with a diagnosis. With treatments, uncertainties continue to abound.

In cancer, for example, obese patients tend to have worse outcomes and a higher risk of death — a difference that holds for every type of cancer.

The disease of obesity might exacerbate cancer, said Dr. Clifford Hudis, the chief executive officer of the American Society of Clinical Oncology.

But, he added, another reason for poor outcomes in obese cancer patients is almost certainly that medical care is compromised. Drug doses are usually based on standard body sizes or surface areas. The definition of a standard size, Dr. Hudis said, is often based on data involving people from decades ago, when the average person was thinner.

For fat people, that might lead to underdosing for some drugs, but it is hard to know without studying specific drug effects in heavier people, and such studies are generally not done. Without that data, if someone does not respond to a cancer drug, it is impossible to know whether the dose was wrong or the patient’s tumor was just resisting the drug.

One of the most frequent medical problems in obese patients is arthritis of the hip or knee. It is so common, in fact, that most patients arriving at orthopedists’ offices in agonizing pain from hip or knee arthritis are obese. But many orthopedists will not offer surgery unless the patients first lose weight, said Dr. Adolph J. Yates Jr., an orthopedics professor at the University of Pittsburgh School of Medicine.

“There are offices that will screen by phone,” Dr. Yates said. “They will ask for weight and height and tell patients before they see them that they can’t help them.”

But how well grounded are those weight limits?

“There is a perception among some surgeons that it is more difficult, and certainly some felt it was an added risk,” to operate on very obese people, Dr. Yates said. He was a member of a committee that reviewed the risks and benefits of joint replacement in obese patients for the American Association of Hip and Knee Surgeons. The group concluded that heavy patients should first be counseled to lose weight because a lower weight reduces stress on the joints and can alleviate pain without surgery.

But there should not be blanket refusals to operate on fat people, the committee wrote. Those with a body mass index over 40 — like a 5-foot-5-inch woman weighing 250 pounds or a 6-foot man weighing 300 — and who cannot lose weight should be informed that their risks are greater, but they should not be categorically dismissed, the group concluded.

Dr. Yates said he had successfully operated on people with body mass indexes as high as 45. What is behind the refusals to operate, he said, is that doctors and hospitals have become risk-averse because they fear their ratings will fall if too many patients have complications.

A lower score can mean reductions in reimbursements by Medicare. Poor results can also lead to penalties for hospitals and, eventually, doctors.

A recent survey of more than 700 hip and knee surgeons confirmed Dr. Yates’s impressions. Sixty-two percent said they used body mass index scores as cutoffs for requiring weight loss before offering surgery. But there was no consistency in the figures they picked.

“The numbers were all over the map,” Dr. Yates said. And 42 percent who picked a body mass index cutoff said they had done so because they were worried about their performance score or that of their hospital.

“It’s very common to pick an arbitrary B.M.I. number and say, ‘That is the number we won’t go above,’” Dr. Yates said. Yet a person with an index of, say, 41 might be healthy and active, he said, but in terrible pain from arthritis. A knee replacement could be life transforming.

“It’s a zero-sum game, with everyone trying to have the lowest-risk patient,” Dr. Yates said. “Patients who may be at a marginally higher risk may be treated as a class instead of individuals. That is the definition of discrimination.”

Surgery involves anesthesia, of course, giving rise to another issue.

There are no requirements for drug makers to figure out appropriate doses for obese patients. Only a few medical experts, like Dr. Hendrikus Lemmens, a professor of anesthesiology at Stanford University, have tried to provide answers.

His group looked at several drugs: propofol, which puts people to sleep before they get general anesthesia; succinylcholine, used to relax muscles in the windpipe when a breathing tube must be inserted; and anesthetic gases.

Propofol doses, Dr. Lemmens found, should be based on lean body weight — the weight of the body minus its fat. Using total body weight, as is routine for normal-weight people, would result in an overdose for obese patients, he said. But succinylcholine doses should be based on total body weight, he determined, and the dosing of anesthetic gases is not significantly affected by obesity.

As for regional anesthetics, he said, “There are very few data, but they probably should be dosed according to lean body weight.”

“Bad outcomes because of inappropriate dosing do occur,” said Dr. Lemmens, who added that 20 to 30 percent of all obese patients in intensive care after surgery were there because of anesthetic complications. Given the uncertainties about anesthetic doses for the obese, Dr. Lemmens said, he suspects that a significant number of them had inappropriate dosing.

Yet for many fat people, the questions about appropriate medical care are beside the point because they stay away from doctors.

“I have avoided going to a doctor at all,” said Sarai Walker, the author of “Dietland,” a novel. “That is very common with fat people. No matter what the problem is, the doctor will blame it on fat and will tell you to lose weight.”

The nominations for the first ever Young-Howze Theatre Awards are finally here! We have been working hard since March of 2020 reviewing over a hundred and forty digital productions. We have been poring over them all since January to bring you these categories and the shows vying for them.

Let’s remind everyone what our criteria were: Because of the pandemic this year every show that we have reviewed was nominated for at least one category. We picked winners by a very specific set of criteria: What was this show attempting to do? Was it successful? Was it the most successful of all the shows that tried to do this? We apologize to anyone who thought that we would be announcing winners today. We decided that we wanted to save all of the spoilers for the live show on February 21st at 8PM EST. Hi! I took a look at your blog, and it seems like we agree on a lot of topics concerning animal welfare. I thought you deserved a proper response from me as to why I don’t agree on this matter.

I don’t believe that SeaWorld tortures orcas. Far from it. There was a time I bought into the narrative presented in Blackfish, but that view was chipped away as I earned my degree in zoology, entered vet school, and had the chance to learn from and hold conversations with veterinarians, zookeepers, and marine mammal trainers who had worked at or with SeaWorld and similar facilities. All three SeaWorld parks are AZA-accredited. AZA accreditation is only granted to about 10% of North American zoos and aquariums for adherence to rigorous standards in a variety of categories, including animal health and welfare. That’s not to say all AZA facilities are without fault, or that all zoos without AZA accreditation are automatically roadside circuses, but it’s safe to assume that animals at an AZA zoo are well cared for. When orcas were first held in captivity back in the 60s/70s/80s, frankly no one knew what they were doing. Heartbreakingly, a lot of animals paid the price and died far too young. It’s true that orcas in captivity do not swim great distances every day like their wild counterparts. But they don’t have to. Orcas, like all animals, spend a vast majority of their energy in search of food, which in their case happens to be ever-moving prey. Wild orcas can go up to three weeks without eating thanks to their impressive blubber reserves. In human care, they are fed every day. They don’t have the need to swim these tremendous distances. Training sessions are not just to entertain audiences. They serve as vital enrichment and exercise to keep their minds and bodies occupied. Each whale has his or her own trainer team whose only job is to make sure they are kept as content as possible, with games, puzzles, massages, and environmental enrichment. There are hours of amateur video on YouTube showing SeaWorld orcas interacting with enrichment and even performing trained behaviors on their own. They are tended to by world-class veterinary staff and are active participants in their own husbandry and healthcare, which in turn benefits wild orcas. If he could never be assimilated into another group of orcas, why does Visser want to locate an adoptive pod for him if his original pod cannot be found? Why has Morgan thrived as second-most dominant in her adoptive pod at Loro Parque, even bearing a calf? Why have studies found that orcas in captivity communicate just fine despite different “cultural” backgrounds? Additionally, Loro Parque has experience successfully bottle-rearing orca calves and has offered their assistance. Visser does not. Despite her obvious disdain for them, she should allow a team with relevant experience to lend aid if she wants Toa to survive.

As a vet student, I certainly agree with you that there a plenty of situations in which euthanasia is far more humane than keeping an animal alive. I see them everyday as owners cling to a beloved dog dying a painful death from cancer or a 20-year-old cat withering away. I want nothing more than for these animals to be free of their pain. But Toa is a healthy calf. His death would not serve to feed or clothe humans or other animals or to end a painful illness. It would only prevent him from a safe and potentially long life in human care with the companionship of other orcas. So no, I cannot condone euthanizing him. I do not believe he is better off dead or that he would be tormented in captivity. I know I won’t change your mind, but at the very least I hope you understand why I think the way I do and why I will be incredibly disheartened if Toa is put to sleep.

Let’s make one thing clear. If Toa’s pod doesn’t miraculously return very shortly, he will die. Not because he’s ill or injured, but because his rescuers would rather see him dead than living a potentially long life in expert human care at a facility like SeaWorld or Loro Parque. These are the same people who fought to release Morgan—a deaf animal who cannot hunt—and unintentionally killed stranded calf Bob because they refuse to seek help from those who actually know how to take care of cetaceans (i.e. aquarium vets and trainers). This is radical animal rights activism at work. This is a world with “empty tanks.”

Inside the Empire: The True Power Behind the New York Yankees – Book Review

It would be a stretch to compare Inside the Empire: The True Power Behind the New York Yankees to a piece by Leni Riefenstahl mirroring content usually produced and presented on the Yankees’ state-sanctioned propaganda ministry, the YES Network. Were that the case, it would have been penned by Jack Curry, had twice the ingratiating obnoxiousness and a quarter of the skill.

Still, within the first 20 pages, the direction of the narrative was clear as the authors – Bob Klapisch and Paul Solortaroff – dumped on, in order, Derek Jeter, Joe Girardi and Joe Torre. Ranging from a Yankees icon to a Hall of Fame manager to a key role player and World Series winning manager, they had fallen out of favor in the Bronx for a variety of sins and were cast out to the purgatory not limited to estrangement, but to open hostility.

This is no coincidence as it occurs simultaneously to avoiding foreshadowing (or foreplay) or any other writerly (or sex-based) techniques and going straight into the borderline pornographic worship of Brian Cashman. Reading between the thinly veiled lines, Cashman could also be referred to as “The Man Who Could Do No Wrong.”

Part of the book’s disappointment and failure is not the story itself, but of the expectations that preceded the news that it was being written in the first place. For those who read baseball tell-alls like Ball Four, The Bronx Zoo and others, a yearlong case study followed by an autopsy regardless of the outcome holds tremendous allure. Unfortunately, the writers retreat to the safety of the current trend of “baseball business” books, most of which pale in comparison to the initial and admittedly interesting while incredibly flawed and misunderstood Moneyball. Post-Moneyball, The Extra 2% was the next and last of the immersing stories that had yet to be told. After that came the love letters to the Pittsburgh Pirates, the Chicago Cubs and Theo Epstein, the Houston Astros and Jeff Luhnow, and a few others, all of which were agenda-based, misleading and largely dull.

It’s tiresome not just because the stories have basically been told, albeit in different forms with a different cast of characters, but because the stories are so repetitive and devoid of criticism. This goes beyond the caveated individual mistakes that turned into learning experiences with substantial blame doled out on others since the main characters certainly couldn’t have been at fault as that would have sabotaged the entire goal of the story: to create a hero even if there wasn’t one.

The book doesn’t enter the realm of The Yankee Years by Torre and Tom Verducci where Torre aired his gripes, executed his vendettas and cemented his self-created and media-promoted visage of a combination Vito Corleone and the Pope, but Cashman and Torre’s perception of what occurred during that time and, by extension, how that impacted his replacement Girardi, are key parts of Inside the Empire.

The baseball business book model might be what publishers are looking for and what editors steer the narrative towards, but for those who want an insider’s perspective, it simply no longer works. We want meat, not cotton candy.

Perhaps Klapisch was scarred by what was, on the surface, a legitimate attempt at a tell-all with The Worst Team Money Could Buy about the disastrous 1992 New York Mets. The book itself was also a disappointment for those who hoped for a day-by-day diary of spending spree, a cast of compelling characters and a promising season that quickly devolved into a nightmare, but it was far better than this patched together mess, a book that tries to appeal to the Yankee fan and retain access while taking care not to offend anyone who is still closely affiliated with the club.

As much as Klapisch says those Mets players labeling him as someone who can’t be trusted did not affect him one way or the other; that he was not intimidated when Bobby Bonilla physically threatened him, for someone like him, who is and has for a long time been under the impression that he was not just a journalist who covers the team, but a peer who sees himself as a player, this is a scar that could have been reopened had he been completely honest about the 2018 Yankees and not diluted the tale so as not to “betray” anyone in whose confidence he was taken.

And therein lies the problem. The authors traded access for the lavishing of praise upon the characters who remained in the Yankee family.

Aaron Judge? Awesome player and human who everyone loves.

Didi Gregorius? Emerging leader whose good humor and affability masks an intense competitor.

CC Sabathia? The Yankees’ version of Yoda.

Aaron Boone? Wonderful guy whose even keeled demeanor was a marked departure from Girardi’s twitchy tightness.

It goes on and on.

At its end, there is an open question of Cashman’s blueprint of power above all else, ignoring situational hitting and strikeouts, wondering whether he would eventually look at the Red Sox and Astros and admit that perhaps adaptation needed to extend beyond the restructuring of the organization and adherence to cold numbers, accepting that the analysts didn’t know everything and there was nuance to the tactics by using the strategic single rather than every swing being for the fences.

The one remaining Yankee who did get criticized was Giancarlo Stanton, but even that was limited to a hand-wringing, halfhearted musing of his positives and negatives.

Gary Sanchez – the player who deserves to be slaughtered for his inattention, lack of fundamentals and bottom line laziness – is largely spared from a deserved lashing.

Boone is protected from criticism for inexplicable reasons that one can only surmise of him being a nice guy who is so completely devoid of any responsibility apart from following orders and providing monotonous platitudes that the team could have won 100 games if they stationed a mannequin in a uniform at the corner of the dugout and used a series of wires for him to perform “managerial functions.”

It all reverts to Cashman and his vision; his goal; his intent when masterfully taking charge of the organization and nudging Hal Steinbrenner into the direction he wanted.

The excuses are mind-numbing and fall into precisely what the late Boss, George Steinbrenner, would not have tolerated not because he was an unhinged, raving lunatic (he was), but because he would have been right not to want to hear that Sanchez's lackadaisical behavior was because he was injured; that Boone's absence of fire was a positive; that Stanton repeatedly striking out was part of the $300 million package. Nor would he have quietly acquiesced to the other explanations as to what went wrong as a team that won 100 games was discarded like irrelevant debris by its most hated rivals.

Cashman tried to assuage the concerns of fans and media members who were slowly coming to grips with the reality that this was no longer the Yankees of The Boss by proclaiming the organization a “fully operational Death Star,” implying that the so-called Evil Empire had gotten its payroll under control, rebuilt the farm system sans the Boss’s constant interference and template of preferring to trade young players for proven veterans while spending on exorbitant free agents, and was again prepared to combine tactical decisions with price being no object to return the Yankees to baseball’s pantheon not with a sole championship to break their decade-long drought, but with a team that was set to be the next dynasty.

In truth, it was unabashed hyperbole. Without Darth Vader, there is no Death Star. And The Boss was the organizational Darth Vader and proud of it. Instead, the Yankees’ ultimate weapon is more something out of Mel Brooks with Cashman as Rick Moranis’s “Dark Helmet,” someone who looks unimposing in person, sounds unimposing in practice, and is a technocrat who seized power piecemeal with an admittedly admirable Machiavellian efficiency and has decided to use that power to be like every other supposedly forward-thinking organization in sports and hope for a chance at a championship rather than winning the championship itself. The constant statements about accountability are nonexistent under this regime because no one is getting fired if they fail; players are unafraid of checking their names on social medial for a missive from the deranged Boss; and a Little League credo of “just try as hard as you can” is deemed sufficient.

And that’s not the Yankees that George Steinbrenner built.

The book could have been an exposé of what would otherwise have been a failed season for the Yankees, but was instead a borderline celebration of what they have become with the architects credited for its own sake. Had they ignored the fallout of telling truths that would have angered the organization, the book could have been excellent. Instead, it’s another generic tale about the baseball business, the kind we’ve seen too much of already to be memorable.

We have friends all over the world, some of whom we met through our infertility journey. She’s happy as long as she has plenty to watch and do, and she even took a three hour nap (score!).

We had a lovely time with our friends and are hopeful to visit Pittsburgh again soon.
