here’s that breakdown/rating of Vlad’s (major) official artists/designs i said i’d do. for reference’s sake this is counting renders not necessarily done by the actual artist if it’s still their specific design (ApoVlad in the anime is a Konoe Vlad, FGO Arcade Vlad is a Maeda Vlad, etc).

Vlad's original/FGO character design. what Maeda’s Vlad lacks in consistency he makes up for with almost everything else. generally has the best proportions and best overall design, although his Vlads aren’t really very expressive outside of FGO arcade. the fluffiest Vlad.
11/10 for putting Dracula in a skirt.

ApoVlad. an older-looking Vlad than Maeda’s, which is good too, both are good. is more consistent than Maeda’s. usually has the least ideal proportions, but has the iconic goth dad eyeshadow and some very good cat-like Vlads. also his Vlads have the best expressiveness. the smuggest and scariest Vlad.
loses a point for the anime Dracula Vlad design. 10/10.

manga ApoVlad. he got a lot better at drawing Vlad over time. the expressiveness of his Vlads never quite hit Konoe’s level but they’re still very good. his Vlads look older than Maeda’s but not as old as Konoe’s. i appreciate that he sometimes draws Vlad’s tattoos and sometimes gives him the aquiline nose that Maeda dropped. Maeda might have been the genius who put Dracula in a skirt but Ishida is the absolute madman who put Dracula in a miniskirt. the most legs Vlad (usually).
gets bonus points for drawing the hottest possible Dracula Vlad design, the best thing to happen since Vlad was created in the first place, for which i eternally owe Ishida my life. 11/10.

Honestly, it's kind of hard to say. We do know that a lot of the Apocrypha characters weren't actually conceptually by Ototsugu Konoe at all but were instead designed by someone else and then he adapted them into his own art style. That's also why some characters in FGO from Apoc like Vlad and Atalanta aren't drawn by Konoe, they gave them to their concept artists. As for why some like Semiramis and Jack weren't given to their original artist? Eh... Idk, honestly I think it's just because Konoe drew them better.
We have development art of Pako's initial concept art for him, with Wada Arco specifically talking about Karna during the development of CCC and how the "design by Pako" was too hard to transfer into 3D so she had to change it, so Pako did definitely do the original art and process of making most of his design... But it's also said that the original designer was Nasu and it was drawn by Pako. Of course, this could also mean anything. Using Jack again as an example, it's also said her designer was the author Yuuichirou Higashide with her artist in Unbirth being Taketo Sanada.
Nasu isn't much of an artist so as far as we know, they could have had the different writers come up with the backstory and general design concepts and simply handed it over to their assigned artist and just told them details to include, or maybe the artists had full control. Or perhaps the pairs worked on it together?

I don't really have much to contribute here besides speculation, but with how radically different Karna's beta design was to his final design, personally I'm just placing my bet on Nasu simply writing the backstory and some personality traits with Pako being the one to have actually make his design and was responsible for his skin color being the way it is, and somehow along the way the two decided to make him a good guy. Not totally sure tbh.
The type-moon wiki says Karna was designed by Nasu and then illustrated by Pako, which... idk what exactly that means responsibility-wise? Though I suppose I could imagine Nasu reading something about Karna often being portrayed/described as pale and telling Paki "ah yes. we must make the whitest man in existence"
yeah that’s where my confusion comes from bc ik his character was initially made by nasu for the rpg thing that became apocrypha but I’ve no idea if he was the one who came up with the initial design-not to mention karna’s first appearance was apocrypha where most of them share a designer and idk if the artist who designed most of the other apocrypha cast had a hand in it at all, but it could also be that as well
But fjsjakak yeah he could’ve been like ‘bring me the whitest man you can’ at pako we don’t know
