What is the ISCCM Critical Care Fellowship?

Indian Society of Critical Care Medicine is a professional body and non-profit organization that represents medical professionals in the field of Critical Care Medicine in India. It was established on the 9th of October 1993. This organization aims to increase awareness, education, and research in the field of Critical Care. It also recognizes fellowships at various hospitals and institutes in India.
ISCCM recognizes five fellowships for doctors and one fellowship for nurses. There are also TWO fellowships available for paramedics & allied healthcare professionals. These fellowships were recognized by 239 hospitals/institutes across India as of December 2002.
ISCCM offers two online fellowships for doctors. These fellowships are open to candidates who have been admitted and confirmed as members of ISCCM.
List of fellowships for Doctors:
1. Indian Diploma in Critical Care Medicine (IDCCM)
The IDCCM fellowship is designed for postgraduate medical professionals who are interested in working in Critical Care.
Eligibility Criteria:
MD Medicine/Chest/Anaesthesia
DNB Medicine/Chest/Anaesthesia
MD/DNB Emergency Medicine
Diploma in Anaesthesia/Diploma in Chest diseases
CTCCM
MRCEM (If approved by National Medical Commission (NMC)
FCPS Medicine
Duration:  
ONE year for MD/DNB candidates and TWO years for Diploma candidates.
Academic Session: January (last 15 Feb) and July (last 15 Aug)
Stipend: Usually between ₹50,000/- to ₹70,000/- (depending on hospital policy).

2. Certificate Training of Critical Care Medicine (CTCCM)
The CTCCM fellowship is designed for medical graduates who are interested in working in Critical Care.
Eligibility Criteria: MBBS or equivalent allopathic medical degree with valid MCI registration.
Duration: TWO years
Academic Session: January (last 15 Feb) and July (last 15 Aug)
Stipend: Usually between ₹40,000/- to ₹50,000/- (depending on hospital policy).
Fees: ₹23,600/- plus ₹5,154 Associate Membership Fee
3. Indian Fellow of Critical Care Medicine (IFCCM)
IFCCM Fellowship is for IDCCM Graduates who are pursuing an advanced level in Critical Care.
Eligibility Criteria:
Candidates who have completed IDCCM
Those who have passed FNB/ DM critical Care are eligible to take the IFCCM examination
Candidates with a foreign qualification for Critical Care may apply directly for the IFCCM examination two years after they return to India. However, such candidates must be employed in an ISCCM-recognized institution
Post MD candidates can register for IFCCM directly. However, such candidates must pass IDCCM before appearing for IFCCM. The course duration for directly registered candidates is 2 years.  
Duration: ONE year
Academic Session:  April (last 15th May) & October (last 31st Oct)  
Stipend: sually between ₹70,000/- to ₹80,000/- (depending on hospital policy)
Fees: ₹11,800/- plus ₹5,900/-
4. Fellowship in Neurocritical Care (FNCC)
The FNC is a postdoctoral fellowship for neurocritical care. The program is designed to train candidates in the multi-system care of critically ill patients suffering from neurosurgical and neurological insults. It also teaches the principles of Neurocritical Care. This program has been recognized by the Society of Neurocritical Care India (SNCC India), and ISCCM.
Eligibility Criteria
This fellowship program is open to Neurologists, Anesthesiologists and General Physicians.
Candidates should have a MD/MS or equivalent degree (DNB), from an MCI-recognized institute and at least six months experience in critical care.
Candidates who have a diploma from an MCI-recognized institute are eligible to apply for a post-graduation two-year experience that includes a minimum of six months of experience in critical care.
The personal interview with the candidate at each institution will determine the candidate's eligibility for selection.
Each candidate must be a member at least one of the ISCCM or SNCC.
Duration: ONE year (with compulsory two-month rotation in General Critical Care).
Academic Session: January and July of every year
Stipend: The remuneration of the candidate depends on hospital policy.
5. Fellow of Indian College of Critical Care Medicine (FICCM)
The FICCM is an honorary Fellowship awarded to medical professionals who have made significant contributions to the field.
Eligibility Criteria:
Candidate should have been Life Members of ISCCM at least for the last 5 years.
Candidates must have a subspecialty certificate in one of the following: Anesthesia, Chest Medicine (Internal Medicine), Pulmonary Medicine and Surgery, Critical Care Medicine, Pediatrics, or Anesthesia. You may also be eligible for Diplomas in the same subject areas.
At the time of applying, candidates must have spent at least half of their practice time in Critical Care Medicine. The candidate must sign a statement regarding this at the end the application form.
Candidates must demonstrate a commitment to the practice of Critical Care through education, research, and publication.
Candidates must demonstrate leadership and dedication in the practice of Critical Care Medicine.
Also, at LEAST ONE of these criteria must be met:
The Candidate has passed the Indian Fellowship in Critical Care Medicine (IFCCM), Fellowship of National Board in Critical Care (FNB) in Critical Care, or Indian Diploma in Critical Care Medicine.
American board certified in Critical Care Medicine, the candidate.
The Candidate has successfully completed the European Diploma in Critical Care Medicine or FJICCM in Australia.
After MRCP, FRCA, or FRCS in the United Kingdom, the Candidate has received a Certificate of Completion for Specialist Training (CCST /CST).
After receiving a postgraduate degree from the Medical Council of India, the candidate has been practicing critical-care medicine in India for at most 10 years.
Selection Process:
Applications will be accepted in August and September each year. The last date for submissions is September 30, 2018.
Each application for Fellowship would be reviewed by the Credentials Committee.
The Credentials Committee will consider factors such as: length of experience in critical medicine, teaching experience in critical care (including examiner for IDCCM/IFCCM), publications and memberships in editorial boards for critical-care journals, contributions to the ISCCM at National/Regional levels (including conference organization and participation in socially relevant programmes etc.) and the applicant's Fellowships and Awards.
The College Board would then approve the Credential Committee's decision.
The decision will be made by December month.
The Credentials Committee and College Board will make the final decision. Appeals from applicants will be denied.
Fellowships will be presented at the Annual Conference by ISCCM's College Convocation.
The award certificate must be collected at one of the following national conferences if the candidate is not able to attend the convocation.
6. Fellowship of Comprehensive Respiratory Support (FCRS)
The FCRS fellowship is an online program created & validated by ISCCM and ICCM for people interested in learning the nuances of respiratory support.
Eligibility Criteria:   MD/DNB/ Diploma in Internal Medicine, Anesthesia, Emergency Medicine, Pulmonology, Respiratory Diseases  
CTCCM candidates Duration: 6 Months
Training Schedule:

The “Sick Character Goes To Rivendell” Trope
There was a scene in the recent Rings of Power episode in which an injured character "needs elvish medicine" and must be taken to the nearest elvish settlement, which would be hundreds of miles away. Many viewers are criticizing this plot point, questioning the logic of having a critically injured character traveling for hundreds of miles for medical treatment, when it would be far more logical for them to find a closer healer.
I have encountered this often annoying and problematic trope many times in the nineteen years that I have been involved in Middle-earth-related activities. I have seen sooo many roleplay storylines involving injured characters being taken to Rivendell for EMERGENCY healing, even when the characters were NOWHERE near the hidden elven village.
I remember one particular storyline in which a character wandered into Shelob's Lair and got a nasty spider bite, and Glorfindel randomly appeared to take her to Rivendell for healing. Rivendell is almost 800 MILES from Shelob's Lair, and the player who came up with this idea expected everyone else to believe that the injured character would survive such a long and arduous journey. In the real world, most likely she would either die from spider poison or recover on her own long before she ever reached Rivendell. The more logical choice would have been for Glorfindel to take this character to the Houses of Healing, as Minas Tirith is only around 45 miles from Shelob's Lair.
The story was even more illogical and ridiculous when one considers that the injured character - let's call her Starwen - was a CITIZEN OF MORDOR, and what Glorfindel did was kidnap her and take to a far away land. The whole time that Glorfindel was traveling with Starwen, Khamul was desperately trying to chase them down and rescue her. Actually, when Starwen had been kidnapped by Glorfindel, Khamul had been about to rush her to Minas Morgul - a short distance from Shelob's Lair - and get her the medical attention she desperately needed. But, no, Glorfindel's player insisted that Starwen be taken 800 MILES AWAY to an ENEMY SETTLEMENT for magical elven healing.
In Lord of the Rings Online, player characters are constantly going to Rivendell for random quests. The Epic Quest is notorious for doing this, having the player travel for what would be hundreds of miles to Rivendell to fetch or deliver an item or piece of information, and then travel hundreds of miles back to where they started from. Due to the extreme time dilation of the game, these journeys have no bearing upon the story itself. A player character could travel a journey of a thousand miles with little more than a day or two transpiring in the storyline.
The High Elf intro quest has the player character being wounded in Mordor. Later they wake up in Rivendell, where they have been lying comatose for most of the Third Age. The ol' Sick Character Goes To Rivendell For Healing trope at play again!
LOTRO has so many Journeys to Rivendell that I can't remember a specific case of an NPC being taken to Rivendell for healing, but I'm pretty sure there are at least one or two. I do know of a quest involving a NPC trying to bring hope and cheer to a severely depressed NPC by asking you to take the depressed NPC's sword to Rivendell for reforging. In order to complete this quest, the player character must travel from Angmar to Rivendell, and then back to Angmar. There is a lot of traveling involved!
The Sick Character Goes To Rivendell For Healing trope was devised by Tolkien himself, but it makes a lot more sense in the Fellowship of the Ring than in most adaptions/apocrypha! When Frodo was stabbed by the Morgul blade, he was ALREADY heading for Rivendell, so it was not as though he went out of his way to get there. Also, Tolkien pretty much states that it was a miracle that Frodo survived the journey at all. The way a lot of people write this trope, however, would be the equivalent of a person in New York City being flown across the Atlantic Ocean to London to treat a heart attack or hemorrhage.
I wish that writers and roleplayers would take into consideration distance when it comes to transporting critically injured characters, as well as the delicate condition of these sorts of injuries as well.

This week we will discuss Monkeypox with Dr. Tiffany Najberg.
 Dr. Tiffany Alexis Najberg Pronouns: She/her Physician licensed in the state of Louisiana Dr. Tiffany is a Board Certified Emergency Physician who has served in busy emergency departments and provided prehospital care as well as remote care since 2007. A transwoman currently transitioning herself, she is the first open trans woman Emergency Physician in Louisiana. She is a business owner who co owns her clinic, UrgentEMS, in Shreveport Louisiana. She practices urgent care, some primary care, and gender affirming care there and via telehealth throughout the state. Her experience in remote medical direction led her to begin practicing transgender medicine via telemedicine, as making it accessible to all, even in isolated locations, is something she cares deeply about. She received her medical degree at the University of North Texas in Fort Worth, completed her emergency medicine residency at LSU New Orleans (Charity Hospital), and her EMS fellowship with New Orleans EMS. She has been a teaching staff at Ochsner in New Orleans and St. Francis in Monroe, and has a passion for medical education. She still actively instructs medical students at her clinic. Her non medical interests include amateur storm chasing, writing, all things social, and she is a fierce advocate for public health, trans rights, women’s rights, and lgbtq+ issues through her sizable online platforms.

  Symptoms of monkeypox can include
Fever
Headache
Muscle aches and backache
Swollen lymph nodes
Chills
Exhaustion
Respiratory symptoms (e.g. sore throat, nasal congestion, or cough)
A rash that may be located on or near the genitals (penis, testicles, labia, and vagina) or anus (butthole) but could also be on other areas like the hands, feet, chest, face, or mouth.
The rash will go through several stages, including scabs, before healing.
The rash can look like pimples or blisters and may be painful or itchy.
  You may experience all or only a few symptoms
Sometimes, people get a rash first, followed by other symptoms. Others only experience a rash.
Most people with monkeypox will get a rash.
Some people have developed a rash before (or without) other symptoms.
Monkeypox symptoms usually start within 3 weeks of exposure to the virus. If someone has flu-like symptoms, they will usually develop a rash 1-4 days later.
Monkeypox can be spread from the time symptoms start until the rash has healed, all scabs have fallen off, and a fresh layer of skin has formed. The illness typically lasts 2-4 weeks.
If You Have a New or Unexplained Rash or Other Symptoms...
Avoid close contact, including sex or being intimate with anyone, until you have been checked out by a healthcare provider.
If you don’t have a provider or health insurance, visit a public health clinic near you.
When you see a healthcare provider, wear a mask, and remind them that this virus is circulating in the area.
Monkeypox spreads in a few ways.
Monkeypox can spread to anyone through close, personal, often skin-to-skin contact, including:
Direct contact with monkeypox rash, scabs, or body fluids from a person with monkeypox.
Touching objects, fabrics (clothing, bedding, or towels), and surfaces that have been used by someone with monkeypox.
Contact with respiratory secretions.
This direct contact can happen during intimate contact, including:
Oral, anal, and vaginal sex or touching the genitals (penis, testicles, labia, and vagina) or anus (butthole) of a person with monkeypox.
Hugging, massage, and kissing.
Prolonged face-to-face contact.
Touching fabrics and objects during sex that were used by a person with monkeypox and that have not been disinfected, such as bedding, towels, fetish gear, and sex toys.
A pregnant person can spread the virus to their fetus through the placenta.
It’s also possible for people to get monkeypox from infected animals, either by being scratched or bitten by the animal or by preparing or eating meat or using products from an infected animal.
A person with monkeypox can spread it to others from the time symptoms start until the rash has fully healed and a fresh layer of skin has formed. The illness typically lasts 2-4 weeks.

Take the following steps to prevent getting monkeypox:
Avoid close, skin-to-skin contact with people who have a rash that looks like monkeypox.
Do not touch the rash or scabs of a person with monkeypox.
Do not kiss, hug, cuddle or have sex with someone with monkeypox.
Avoid contact with objects and materials that a person with monkeypox has used.
Do not share eating utensils or cups with a person with monkeypox.
Do not handle or touch the bedding, towels, or clothing of a person with monkeypox.
Wash your hands often with soap and water or use an alcohol-based hand sanitizer, especially before eating or touching your face and after you use the bathroom.
​In Central and West Africa, avoid contact with animals that can spread monkeypox virus, usually rodents and primates. Also, avoid sick or dead animals, as well as bedding or other materials they have touched.
CDC recommends vaccination for people who have been exposed to monkeypox and people who may be more likely to get monkeypox.
​People more likely to get monkeypox include:
People who have been identified by public health officials as a contact of someone with monkeypox
People who are aware that one of their sexual partners in the past 2 weeks has been diagnosed with monkeypox
People who had multiple sexual partners in the past 2 weeks in an area with known monkeypox
People whose jobs may expose them to orthopoxviruses, such as:
Laboratory workers who perform testing for orthopoxviruses
Laboratory workers who handle cultures or animals with orthopoxviruses
Some designated healthcare or public health workers
Treatment
There are no treatments specifically for monkeypox virus infections. However, monkeypox and smallpox viruses are genetically similar, which means that antiviral drugs and vaccines developed to protect against smallpox may be used to prevent and treat monkeypox virus infections.
Antivirals, such as tecovirimat (TPOXX), may be recommended for people who are more likely to get severely ill, like patients with weakened immune systems.
If you have symptoms of monkeypox, you should talk to your healthcare provider, even if you don’t think you had contact with someone who has monkeypox.
