Targeted Therapy and Immunotherapy for Gallbladder Cancer
As researchers learn more about the changes in cells that cause gallbladder cancer, they’ve developed drugs to target some of these changes. These targeted drugs work differently from standard chemotherapy (chemo) drugs. They sometimes work when standard chemo drugs don’t, and they often have different side effects.
FGFR2 inhibitors:
FGFRs (fibroblast growth factor receptors) are proteins on cells that help them grow and divide normally. A small number of people with gallbladder cancer have changes in the genes that make FGFRs, which result in abnormal FGFR proteins that cause cells to grow out of control and turn into cancer.
Pemigatinib (Pemazyre) and futibatinib (Lytgobi) are FGFR2 inhibitors. They block the abnormal FGFR2 protein in gallbladder cancer cells and keep them from growing and spreading to other places.
These drugs can be used to treat some advanced gallbladder cancers that cannot be removed by surgery or have spread to distant areas after at least one previous chemotherapy treatment. For these drugs to work, your cancer must have an abnormal FGFR2 gene, so your cancer will be tested before starting any of these drugs.
These medicines are taken by mouth as tablets, typically once a day.
IDH1 inhibitor:
In some people with gallbladder cancer, the cancer cells have a change (mutation) in the IDH1 gene, which normally helps cells make the IDH1 protein. Mutations in this gene can lead to an abnormal IDH1 protein, which can stop cells from maturing the way they normally would.
Ivosidenib (Tibsovo) is an IDH1 inhibitor. It blocks the abnormal IDH1 protein, which seems to help the cancer cells mature into more normal cells. This drug can be used in people with advanced, previously treated gallbladder cancer, if the cancer cells are found to have an IDH1 mutation. Your doctor can test your cancer cells to see if they have an IDH1 mutation.
This drug is taken by mouth, once a day.
Less common but more serious side effects can include changes in heart rhythm, pneumonia, and jaundice (yellowing of the eyes and skin).
NTRK inhibitor:
A very small number of gallbladder cancers have changes in one of the NTRK genes, called NTRK gene fusions. Cells with these gene changes make abnormal TRK proteins, which can lead to abnormal cell growth and cancer.
Larotrectinib (Vitrakyi) or entrectinib (Rozlytrek) are NTRK inhibitors. TRK inhibitors target and disable the proteins made by the NTRK genes. This drug can be used in people with advanced gallbladder cancer, who have not received prior systemic therapy.
These drugs are taken as pills, once or twice daily.
Possible side effects of NTRK inhibitors:
Common side effects can include abnormal liver tests; decreased white blood cell and red blood cells; muscle and joint pain; tiredness; diarrhea or constipation; nausea and vomiting; and stomach pain.
Less common but more serious side effects can include mental changes, such as confusion, changes in mood, changes in sleep; liver damage; changes in heart rhythm and/or function; vision changes; harm to a fetus.
BRAF inhibitor:
In some gallbladder cancers, the cells have changes in the BRAF gene. Cells with these changes make an altered BRAF protein that helps them grow. Some drugs target this and related proteins. A combination of BRAF inhibitor and a MEK inhibitor is often given together to treat advanced cancer with the BRAF V600E mutation.
Dabrafenib (Tafinlar) is a BRAF inhibitor. Trametinib (Mekinist) is a MEK inhibitor. This drug combination can be used in people with advanced, previously treated gallbladder cancer, if the cancer cells are found to have an BRAF V600E mutation.
These drugs are taken as pills or capsules each day.
Possible side effects of BRAF inhibitor:
Common side effects can include skin thickening, rash, itching, sensitivity to the sun, headache, fever, joint pain, tiredness, hair loss, nausea, and diarrhea.
Less common but more serious side effects can include bleeding, heart rhythm problems, liver or kidney problems, lung problems, severe allergic reactions, severe skin or eye problems, increased blood sugar levels, and squamous cell skin cancer.
RET inhibitor:
In a small percentage of gallbladder cancers, the tumor cells have a rearrangement in the RET gene that causes them to make an abnormal form of the RET protein. This abnormal protein helps the tumor cells grow.
Selpercatinib (Retevmo) or pralsetinib (Gayreto) are RET inhibitors and can be used to treat advanced gallbladder cancers with the RET rearrangement.
These drugs are taken by mouth as capsules, typically once or twice a day.
Possible side effects of RET inhibitors:
Common side effects can include dry mouth, diarrhea or constipation, high blood pressure, tiredness, swelling in hands and/or feet, skin rash, muscle and joint pain, low blood cell counts, or changes in other blood tests.
Less common but more serious side effects can include liver damage, lung damage, allergic reactions, changes in heart rhythm, bleeding easily, and problems with wound healing.
KRAS inhibitor:
In some gallbladder cancers, the cancer cells have changes in the KRAS gene called a KRAS G12C mutation.  This mutation makes an abnormal form of the KRAS protein, which helps the cancer cells grow and spread.  KRAS inhibitors attach to the KRAS G12C protein, which helps keep cancer cells from growing.
Adagrasib (Krazati) is a KRAS inhibitor and is used to treat advanced gallbladder cancer with the KRAS G12C mutation if you’ve already had at least one other type of drug treatment.
This drug is taken as a pill, typically twice a day.
Possible side effects of KRAS inhibitor:
Common side effects can include diarrhea, nausea and vomiting, muscle pain, fatigue, cough, decreased white blood cell and red blood cell counts, and changes in other blood tests.

Chemotherapy and Targeted Therapies: Advancements in Cervical Cancer Treatment
Introduction:
Welcome to the blog of Dr. Snita Sinukumar, where we explore the latest advancements in cervical cancer treatment. In this article, we will delve into the realm of chemotherapy and targeted therapies, highlighting their crucial roles in combating cervical cancer. Understanding these treatment modalities is essential for patients and healthcare providers alike. So, let's explore the fascinating world of chemotherapy and targeted therapies for cervical cancer.
Tumblr media
Chemotherapy: A Powerful Weapon Against Cervical Cancer:
Chemotherapy plays a pivotal role in the management of cervical cancer. It involves the administration of drugs that circulate throughout the body, attacking cancer cells wherever they may be. While chemotherapy affects both healthy and cancerous cells, its primary aim is to selectively kill rapidly dividing cancer cells. The choice of chemotherapy drugs and regimens depends on the stage and extent of the disease.
Neo-Adjuvant and Adjuvant Chemotherapy:
In certain cases, chemotherapy is administered before surgery (neo-adjuvant) to shrink the tumor and increase the likelihood of successful surgical removal. This approach may be beneficial for locally advanced cervical cancer. Conversely, adjuvant chemotherapy is given after surgery to eradicate any remaining cancer cells and reduce the risk of recurrence.
Combination Chemotherapy:
Combining different chemotherapy drugs has shown enhanced efficacy in treating cervical cancer. Commonly used drug combinations include cisplatin with paclitaxel or topotecan. The synergy between these drugs maximizes their effectiveness and improves patient outcomes.
Targeted Therapies: Precision Medicine for Cervical Cancer:
Targeted therapies are a revolutionary approach to cancer treatment. Unlike chemotherapy, which affects both healthy and cancerous cells, targeted therapies specifically target molecular alterations that drive cancer growth. Here are two notable targeted therapies being explored in cervical cancer treatment:
Immune Checkpoint Inhibitors:
Cervical cancer can evade the immune system by exploiting certain "checkpoints" that prevent immune cells from attacking cancer cells. Immune checkpoint inhibitors, such as pembrolizumab and nivolumab, help remove this barrier and unleash the immune system's full potential to fight cancer. These therapies have shown promising results in advanced cervical cancer, with some patients experiencing long-lasting remissions.
Angiogenesis Inhibitors:
Angiogenesis, the formation of new blood vessels, is crucial for tumor growth and metastasis. Angiogenesis inhibitors, such as bevacizumab, work by inhibiting the blood supply to tumors, thereby stunting their growth. When combined with chemotherapy, these targeted therapies have demonstrated improved overall survival rates in advanced cervical cancer.
The Future of Cervical Cancer Treatment:
The landscape of cervical cancer treatment is continually evolving. Ongoing research aims to identify additional molecular targets and develop novel targeted therapies. Clinical trials are investigating the effectiveness of combination therapies, immunotherapies, and personalized treatment approaches tailored to each patient's unique tumor characteristics.
Conclusion:
Chemotherapy and targeted therapies have revolutionized the treatment of cervical cancer, offering hope and improved outcomes for patients. As a cervical cancer specialist, Dr. Snita Sinukumar remains at the forefront of these advancements, striving to provide her patients with the most effective and personalized treatment plans.
If you or a loved one are facing cervical cancer, don't hesitate to consult with a qualified healthcare professional who can guide you through the available treatment options. Together, we can fight cervical cancer and improve the lives of those affected by this disease.

Most Common type of Cancer in Children
Cancer is one of the most rapidly rising non-communicable diseases, and it has not spared children. However, the cancers that affect youngsters differ from those that affect adults. Unlike lifestyle or environmental variables, early DNA changes are known to cause childhood cancers.
Every year, more children are diagnosed with cancer. Nearly 50,000–60,000 children in India are thought to have the condition, and there are undoubtedly many more who go undiagnosed or are unable to receive timely treatment. Due to a variety of reasons, the numbers are only anticipated to increase in the upcoming years. Cancer is still an illness that can be effectively controlled or treated if detected early, just like for adults.
Are cancers in children curable?
Even though they are frightening, childhood malignancies are very treatable if detected early and correctly. Nearly 80% of pediatric malignancies are treatable. If cases are caught early, children can enjoy longer, simpler lives without cancer. Lack of knowledge about signs and symptoms is one of the most frequent causes of why many youngsters do not receive a proper diagnosis. The signs and symptoms of juvenile cancer can differ from those of adult tumors in certain ways.
The most common type of cancer in children:-
Leukemia, or blood cancer
Leukemia, which is defined as a condition of the bone marrow when the body produces unwanted white cells or lymphoblasts instead of blood, affects roughly 30% of children with cancer.
Both acute lymphoblastic leukemia (ALL) and acute myeloid leukemia (AML), which can both be effectively treated provided the diagnosis is obtained in time, are forms of leukemia.
Infections and changes in the look of the skin, which are brought on by the abnormal production of white blood cells, can be leukemia’s signs and symptoms. If you as a parent see any of the warning signs highlighted below consult a Pediatric Cancer Doctor:
-Pale (White/Yellow skin)
-Frail immunity
-Recurring fever
-Anaemia
-Lethargy, dullness
-Profuse bleeding, low platelet count.
-Bone pain, aches, joint swelling
Other, less typical leukemia symptoms include headaches, nausea, vomiting, and squinting of the eyes. Blood cancer can be identified with the aid of a bone marrow test, which can identify the existence of malignant cancer.

How a Cancer Specialist in Pune Can Help Manage Stage 4 Cancer
Are you dealing with a stage 4 cancer diagnosis? It’s completely understandable to feel overwhelmed and uncertain. If you’re seeking cancer treatment in Pune, you don’t have to face this challenge alone. Dr. Pratik Patil, a top cancer specialist in Pune, is committed to offering compassionate care and cutting-edge treatments to support you in your fight against the disease. Stage 4 cancer, or metastatic cancer, is the most advanced stage, where the cancer has spread to other parts of the body. Managing stage 4 cancer requires a complex, multidisciplinary approach. A Medical Oncologist in Pune, like Dr. Pratik Patil, is crucial in delivering comprehensive care, treatment, and support to patients and their families during this difficult time.
In this blog, Dr Pratik Patil discusses how a cancer specialist can assist in managing stage 4 cancer and the significance of personalized care in enhancing the quality of life for patients.
Understanding Stage 4 Cancer:
Stage 4 cancer occurs when cancer cells have spread from the primary site to other organs or distant parts of the body. This process is known as metastasis. Metastasis can occur in the liver, lungs, bones, or brain. At this stage, the cancer is generally considered incurable, but it can still be managed to control symptoms, slow its progression, and improve the patient’s quality of life. The treatment approach for stage 4 cancer is focused on palliative care, which aims to alleviate pain and other symptoms, rather than curative intent.
The Role of a Cancer Specialist in Stage 4 Cancer Management:
Managing stage 4 cancer involves a multi-faceted approach tailored to each patient’s needs. Dr. Pratik Patil, a leading Cancer Specialist in Pune, plays a crucial role in this process through the following:
Personalized Treatment Plan:
Stage 4 cancer management is challenging and developing a personalized treatment plan is the most important part of the process. The key factors that make personalization effective are the type of cancer, the extent to which it has metastasized, the patient’s overall health, their personal preferences, etc. Dr. Pratik Patil, an experienced cancer specialist in Pune, uses a person-centered method to adjust treatment plans to meet the specific needs of each patient. This may include a combination of therapies such as chemotherapy, targeted therapy, immunotherapy, and radiation therapy.
Chemotherapy: Chemotherapy is generally a significant aspect of the stage 4 cancer treatment process. It could involve using strong medicines that either kill or slow down the cancer cells. The main purpose of chemotherapy is to control the disease, but at the same time, possible side effects are the other side of the coin. Dr. Patil, a cancer specialist, meticulously observes patients during the chemotherapy procedure to control side effects and modify the treatment when necessary. The aim is to achieve the best therapeutic results and at the same time, not cause any suffering to the patient.
Targeted Therapy: One of the latest developments in cancer therapy is targeted treatment, which is a method of directing cancer to specific molecules to eliminate cancer cell growth and survival. Chemotherapy is a method that is used to treat cancer both in healthy and cancerous cells, while targeted therapy seeks to disrupt the cancer cell’s ability to grow, divide, and spread. Dr. Pratik Patil employs targeted therapies when suitable because these medications can be more efficient and cause fewer side effects than traditional chemotherapy.
Radiation Therapy: Radiation therapy may be used to reduce the size of tumors, relieve pain, and manage other symptoms. Cancer specialists carefully schedule the radiation treatments in a way that will not cause any damage to the healthy tissues. For example, Dr. Patil, a cancer specialist, plans the radiation therapy very carefully and administers it so that it is as effective as possible and at the same time, the surrounding healthy tissues are not damaged.
Immunotherapy: This method has come from the contemporary world, it draws on the body’s immune system to combat the cancer cells. Cancer specialists evaluate the patients to decide whether it is advisable to use immunotherapy as a method besides other treatments.
Palliative Care and Symptom Management: Patil works with a holistic approach to improve the quality of life for his patients by relieving pain, exhaustion, and other symptoms, and thus giving them a holistic treatment.
Nutritional and Psychological Support: Cancer therapy can bring about both physical and emotional well-being problems. Cancer specialists along with nutritionists and psychologists work together to provide a complete treatment for these problems.
Coordination of Care: Patients who are in stage 4 of cancer usually need several healthcare providers. The cancer specialists then coordinate with the other providers to make sure the patients receive the most effective and immediate therapy.
Conclusion:
Managing stage 4 cancer is a challenging and multifaceted approach that demands both expertise and compassion. In Pune, Dr. Pratik Patil, one of the best hematologists in Pune, is committed to offering personalized and complete care for patients facing this severe diagnosis. By combining advanced treatments, palliative care, and emotional support, Dr. Patil strives to help patients control their signs, improve their quality of life, and navigate the complexities of stage 4 cancer. Whether through chemotherapy, targeted therapy, or clinical trials, Dr. Patil is committed to exploring every available option to provide the highest quality of care.

Nutrition and Cancer: Eating Habits That Can Help Prevent and Manage Cancer
Cancer is a complex disease that can be influenced by various factors, including genetics, lifestyle, and environmental factors. While there is no one-size-fits-all solution for preventing or managing cancer, adopting healthy eating habits can play a significant role in reducing your risk of developing cancer or managing the disease if you have already been diagnosed. In this blog, we will discuss some eating habits that can help prevent and manage cancer.
Eat a Balanced Diet
A balanced diet is essential for overall health and well-being, and it can also help prevent and manage cancer. A balanced diet includes a variety of nutrient-dense foods such as fruits, vegetables, whole grains, lean proteins, and healthy fats. These foods provide essential vitamins, minerals, and antioxidants that can help protect against cancer and other diseases. In addition, a balanced diet can help maintain a healthy weight, which is another important factor in preventing cancer.
Limit Processed and Red Meat
Processed and red meat have been linked to an increased risk of several types of cancer, including colorectal, pancreatic, and stomach cancer. While it is not necessary to eliminate meat completely from your diet, it is recommended to limit your intake of processed and red meat. Instead, try incorporating more plant-based proteins such as legumes, nuts, and seeds into your diet.
Choose Whole Foods Over Supplements
While there are many supplements on the market that claim to prevent or cure cancer, there is little scientific evidence to support these claims. In fact, some studies have shown that high doses of certain supplements can actually increase the risk of cancer. Instead of relying on supplements, it is recommended to focus on getting essential nutrients from whole foods. Whole foods provide a variety of nutrients and are generally safer than supplements.
Include Antioxidant-Rich Foods in Your Diet
Antioxidants are compounds that can help protect against cancer by neutralizing free radicals, which are harmful molecules that can damage cells. Foods that are high in antioxidants include berries, dark leafy greens, nuts, and whole grains. These foods provide essential vitamins and minerals, as well as fiber, which can help promote a healthy digestive system.
Drink Plenty of Water
Staying hydrated is important for overall health and can also help prevent cancer. Drinking plenty of water can help flush toxins out of your body and keep your cells healthy. In addition, water can help maintain a healthy weight, which is another important factor in preventing cancer.
In conclusion, eating a healthy and balanced diet can play a significant role in preventing and managing cancer. Adopting healthy eating habits such as including a variety of nutrient-dense foods, limiting processed and red meat, choosing whole foods over supplements, including antioxidant-rich foods in your diet, and drinking plenty of water can help reduce your risk of developing cancer or managing the disease if you have already been diagnosed.
