Two weeks ago, popular vote loser Donald Trump said this: "I think we're going to have some of the big drug companies, in two weeks, they're going to announce because of what we did, they're going to announce voluntary massive drops in prices." The "what we did" was HHS Secretary Alex Azar promising as of yet unspecified "bold action" on drug prices. It's now been two weeks, which makes Tuesday's hearing at a Senate Health Committee hearing with Azar quite convenient. Sen. Elizabeth Warren (D-MA) wanted some clarification about that that promise from Trump. "He said there would be massive decreases in prices within two weeks," she told Azar. "It's been two weeks and there have been no decreases and an indication of increase." Why have there been no decreases, she wondered. Azar insisted that there are "actually several drug companies that are looking at substantial and material decreases in drug prices," but that the go-betweens in the system, the negotiators known as pharmacy benefit managers (PBMs) are actually discouraging these decreases because the PBMs then wouldn't get as much back in rebates. A response Warren wasn't satisfied with. "In other words," she said, "the president's promise that we would see massive decreases in two weeks hasn't happened and you don't have anyone lined up who's actually going to decrease drug prices. Pretty much. In fact, just last week Bayer announced massive hikes to two of its cancer drugs. Massive as in $1,000 a month increases to both Stivarga, for patients with colorectal, liver and gastrointestinal cancers and Nexavar, which is used in kidney, liver and thyroid cancers. Stivarga costs about $16,860 a month, and Nexavar about $18,670 a month. A month. It wasn't just Bayer and these two drugs, either. The Post reports that "Wells Fargo analyst David Maris found that although fewer drug-price increases occurred in May than in previous months, dozens of increases did occur." It's almost enough to make you think that drug companies aren't paying any attention to what Trump says and certainly aren't worried about any regulatory action his administration might take. You'd be right. Could be the whole "voluntary" part, but I'm just an old redneck in Mississippi.

Two weeks after Trump unveiled plan to lower drug prices, two cancer drugs got a $1,000-per-month price hike

Two weeks after President Trump in May unveiled a plan to lower drug prices for Americans, promising “it will start to take effect very soon,” the drug company Bayer hiked the list prices of two cancer drugs by more than $1,000 per month.

Bayer wasn’t alone. A research note by Wells Fargo analyst David Maris found that although fewer drug-price increases occurred in May than in previous months, dozens of increases did occur.

Many of the price increases Maris tracked--particularly the double-digit-percentage increases--involved older, generic drugs. There were also price reductions on a slew of older medicines. But the 8 percent price increases that Bayer instituted are notable because the drugs, Stivarga and Nexavar, have already raised some concerns because of their cost. It is the second price increase for the two drugs in six months.

Stivarga, which is used for patients with colorectal, liver and gastrointestinal cancers, now carries a list price of about $16,860 per month. Nexavar, used in kidney, liver and thyroid cancers, carries a list price of about $18,670 per month. The prices for both drugs are now both 13 percent higher than they were during 2017.

“That is an exceptionally bold move to increase prices by over $1,000 a month on an already expensive set of cancer treatments, given the political context,” Stacie Dusetzina, associate professor of cancer research at Vanderbilt University Medical Center, said in an email.

Bayer spokesman Christopher Loder said in an emailed statement that Bayer believes in the value of its medicines.

More than a fifth of the patients on these two drugs get them free through a patient assistance program, according to Bayer, which said it will not take additional price increases on those drugs this year.

Some players in the drug industry, eager to avoid government intervention, have been keeping drug-price increases to single-digit-percentages each year. Health and Human Services Secretary Alex Azar has hinted that drug companies that raise prices without justification could find themselves the subject of unwanted attention from Trump.

NICE okays Roche liver cancer immunotherapy, Sanofi’s rare disease drug

Previously-untreated patients with hepatocellular carcinoma (HCC), a common form of liver cancer, will have an immunotherapy-based treatment option after NICE gave the nod to NHS funding of Roche’s Tecentriq and Avastin combination therapy.

The decision by NICE comes shortly after Tecentriq (atezolizumab) and Avastin (bevacizumab) was approved by the European Commission as a first-line treatment for adults with advanced or inoperable HCC, becoming the first and only immunotherapy regimen approved in Europe for this patient group.

Approval was based on findings of the phase 3 IMbrave 150 study, which showed an improvement in overall survival compared with Bayer’s Nexavar (sorafenib), one of the standard treatments for previously-untreated liver cancer along with Eisai’s Lenvima (lenvatinib).

NICE’s final appraisal document notes that patients receiving the dual regimen “live longer and have longer before their disease progresses than people who have sorafenib” and it is a cost-effective use of NHS resources.

It is the first treatment to be approved in Europe for over a decade that has improved overall survival for people with previously untreated advanced or unresectable HCC.

HCC accounts for 90% of all liver cancer cases and there are around 5,900 new cases in the UK every year. The disease is also becoming more common – Roche says the incidence of HCC is projected to rise by 38% to 15 cases per 100,000 people by 2035.

“Tecentriq, in combination with Avastin, is the only cancer immunotherapy treatment, approved by NICE for this indication,” said Gemma Boni, head of liver cancer at Roche Products Ltd.

“We hope that it can help further improve overall survival rates, while having a generally manageable safety profile, compared to what is already achieved,” she added.

The Tecentriq/Avastin combination is crucial to Roche’s immuno-oncology franchise, and along with HCC has also been approved to treat non-squamous non-small cell lung cancer (NSCLC).

Sales of Tecentriq sales rose 64% to around $2.2 billion in the first nine months of this year, but Avastin has started to succumb to biosimilar competition, shrinking 22% to a little over $4 billion in the same period.

NICE’s recommendation that Sanofi’s Cablivi (caplacizumab) can be covered by the NHS, is also a big step forward for patients with acquired thrombotic thrombocytopenic purpura (aTTP), a rare blood-clotting disorder that can be fatal without prompt treatment.

Cablivi is the first treatment specifically approved for aTTP in 30 years, and the only therapy approved for people experiencing an acute aTTP episode. NICE has backed the drug after rejecting it earlier this year due to an “improved commercial arrangement” with Sanofi – in other words a price discount.

Cablivi was also recommended for NHS use by the Scottish Medicines Consortium (SMC) in September.

aTTP is a rare disease where blood clots form in small blood vessels throughout the body, limiting the flow of oxygen to organs, such as the brain, kidney and heart. NICE’s appraisal document says that Cablivi, combined with plasma exchange and immunosuppression, reduces time to bring blood platelet levels back to normal.

Sanofi welcomed the positive recommendation, but said access to Cablivi had been delayed by procedural issues at NICE.

As a treatment for an ultra-rare disease, Cablivi was initially supposed to be considered within the Highly Specialised Technology (HST) process, but was assessed under the Single Technology Appraisal (STA) process.

The STA does not consider the concerns around long-term follow-up and management, which are a well-known issue in evaluating medicines for ultra-rare diseases, says the company, which claims this delayed access to the drug.

“We now hope that the forthcoming NICE Methods Review and Innovative Drugs Fund consultation will fix the ongoing barriers for patients,” commented Deborah Lough, head of rare blood disorders UK & Ireland at Sanofi Genzyme.

NICE estimates that more than 100 people each year will be eligible for treatment with Cablivi.
