Pediatric Continuous Renal Replacement Therapy (PCRRT) Expert Committee Recommendation On Prescribing Prolonged Intermittent Renal Replacement Therapy (PIRRT) In Critically Ill Children
Introduction
Recently, prolonged intermittent renal replacement therapies (PIRRT) have emerged as cost‐effective alternatives to conventional CRRT and their use in the pediatric population has started to become more prominent. However, there is a lack of consensus guidelines on the use of PIRRT in pediatric patients in an intensive care setting.
Methods
A literature search was performed on PubMed/Medline, Embase, and Google Scholar in conjunction with medical librarians from both India and the Cleveland Clinic hospital system to find relevant articles. The Pediatric Continuous Renal Replacement Therapy workgroup analyzed all articles for relevancy, proposed recommendations, and graded each recommendation for their strength of evidence.
Results
Of the 60 studies eligible for review, the workgroup considered data from 37 studies to formulate guidelines for the use of PIRRT in children. The guidelines focused on the definition, indications, machines, and prescription of PIRRT.
Conclusion
Although the literature on the use of PIRRT in children is limited, the current studies give credence to their benefits and these expert recommendations are a valuable first step in the continued study of PIRRT in the pediatric population.

Testing For And Diagnosing Nephrotic Syndrome In Children
Nephrotic syndrome explained:
Nephrotic syndrome is a condition which affects the kidneys and the miniscule filtering system within it, called the nephrons. There is damage to these structures as a result of various reasons, and mostly in children the cause remains unknown, also known as idiopathic nephrotic syndrome. As a result of this damage, the kidney starts to filter less of the substances that it usually helps our body get rid of, and it also starts to filter in excess certain substances which are useful to the body. Therefore an affected child usually presents with certain characteristic symptoms, including, swelling of the tissues, especially in the face and abdomen.
Diagnosing the condition:
It is when a child presents with this characteristic symptom of swelling in most areas of the body, that your doctor/pediatrician will suspect nephrotic syndrome. It is very important that the diagnosis is made correctly, because only then can prompt treatment be started in order to effectively manage the condition. Nephrotic syndrome is a condition which can be accurately diagnosed using a few tests, because the condition has some telltale signs which can easily be detected.
The investigations used to diagnose nephrotic syndrome:
The diagnostic tests which are carried out when a doctor suspects nephrotic syndrome in your child include:
Urine test – to look for the levels of protein in the urine, because nephrotic syndrome results in increased leakage of protein form the kidney and high levels of protein in the urine. The urine is also tested under the microscope in order to look for signs of inflammation of the kidney and kidney damage, which may indicate the type of nephrotic syndrome your child is suffering from.
Blood test – in order to look for the level of protein in the blood, because the increased leakage of protein in the urine, will result in reduced levels of protein in the blood, which is the reason why the child develops swelling within the body tissues.
Renal biopsy – Children usually do not require kidney biopsy at the onset. Which is a test where a small sample of kidney tissue is drawn out using a needle, and viewed under a microscope, in order to see the changes which have occurred in the kidney tissue. Some children may require additional immuno suppression if they frequently relapse or are steroid dependent.
Nephrotic syndrome can present in two forms. The first known as Minimal Change Disease is the most common form to affect children, and is a form that doesn’t show a lot of damage or change in the urine test, and even in the biopsy, there will not be any identifiable damage to the filtering system of the kidney. The second form known as Focal Glomerular Sclerosis, is a more aggressive form of the disease, and shows extensive damage in the urine test as well as renal biopsy.
On certain occasions, if your child is very symptomatic, your doctor will begin treatment straight away without waiting for the tests to confirm which form of nephrotic syndrome your child is suffering from. The treatment for nephrotic syndrome is a course of steroids, which are administered daily, till your child becomes symptom free. Nephrotic Syndrome
Most people have two kidneys, one on either side of the body just beneath the ribcage. Healthy kidneys filter the blood and allow small particles of waste products and water to be excreted as urine. Kidneys also play important role in the control of blood pressure, maintenance of bone health and formation of red blood cells.
What is nephrotic syndrome?
Nephrotic syndrome occurs when the kidneys leak large amounts of protein (especially albumin) into the urine. It is these proteins which is mainly responsible for holding water in the blood vessels, and when they are lost in urine, their level decreases in the blood which causes the water to come out of the blood vessels and cause swelling (edema).
What causes Nephrotic Syndrome?
In most cases, the exact cause of nephrotic syndrome is not known. There is no relationship with diet or socioeconomic status of the family. It is non-infectious and does not transfer to other family members
.
What are the symptoms?
The most common symptom is swelling (edema). It first appears on the face, especially around the eyes which is most prominent in the morning when the child gets up and decreases by the evening.
Other symptoms include: Frothy urine, weakness and tiredness, passing less urine than usual, recurrent infections, diarrhoea.
What is the treatment?
Prednisolone (steroid) is the drug of choice when the child is first diagnosed. Most children respond to this drug with disappearance of the protein in the urine and loss of swelling within 1-2 weeks (we call this REMISSION). Other drugs like diuretics, ACE inhibitors, etc may be required for symptomatic treatment.
Those who do not respond to steroids are given second line drugs like Cyclophosphamide, MMF, Levamisole and other immunomodulators as decided by your doctor according to the individual patient profile.
Duration of treatment: First episode is treated for 3 months and subsequent episodes are treated as decided by your physician.
What are the side effects of the treatment?
Common Side Effects of Steroids are:
Decreased resistance to infections such as cough and cold
Increase in appetite
Flushed, swollen cheeks and stretch marks on the skin
Rise in blood pressure
Behavioral problems e.g temper tantrums, or mood changes.
However most of the side effects are reversible and wean off once the steroid is stopped.
Long term supervision and course of nephrotic syndrome
In most cases the child with nephrotic syndrome becomes completely well with prednisolone treatment, and there are very few who require second line treatment which includes immunomodulators and other drugs.
The child may remain well for several months or longer. During this period, the child should be regarded as being normal, and should not be made to feel different from other children.
In majority of cases, however, nephrotic syndrome recurs. The recurrence is indicated by appearance of swelling around the eyes, which, if untreated, gradually increases to involve the face, feet, legs and abdomen, and in such conditions, doctor should be consulted immediately. Relapses are common in early childhood but decrease with age.
More than 90% of children with nephrotic syndrome go into adulthood with their kidneys functioning normally.

Decreased urine output
Urine protein 1+ or more for persistent 2-3 days
Any sign of infection like cough, cold, coryza.
What immunizations are necessary?
Children who are on steroid medications and other immunosuprressives can become very unwell if exposed to CHICKENPOX or MEASLES, therefore they should be immunized for the same and also receive pneumococcal vaccine.

Story of Cholesterol
Cholesterol is the most well known villain in the Medical world today. It is one of the most important items forming the walls in every cell of the body and is indispensable for existence of life. Lately the increased blood level of this very important organic chemical has been found to be one of the leading cause of the killer Heart Attacks. Thus, more and more people know it for its notorious role rather than useful role.
Cholesterol is a long chain fatty acid (different from triglycerides or oils or fats which has three fatty acids) – consisting of 27 carbon atoms, 46 hydrogen atoms and one oxygen atom. It is supposed to be “the highly decorated small molecule in Biology”. It has been so important in medical science that since 1986 a number of noble prices have been awarded to the scientists making breakthrough in the research of cholesterol. This tiny molecule forms a major role in the heart disease and in the long run it has indirectly proved more fatal than the most powerful atom bombs!
Cholesterol is found only in animals. Plants and vegetables contain related molecules, which are not absorbed by the human body. All animal products can predispose to high intake of cholesterol. 93% of the body’s cholesterol is located in the cells and only 7% which circulates in the blood, has been held responsible for the formation of blockage.
Cholesterol level in the blood is not safe when about 200mg/100 ml. For reversal of blockages it is recommended to keep this level at 130mg/100ml. On an average the intake of cholesterol by humans who eat meat is above 600 mg/day. Vegetarians who take a lot of milk and milk products have about 200-400 mg/day.
The maintenance level of blood cholesterol depends on (i) Intake in form of food (ii) Production of cholesterol inside liver and other tissues (iii) Use of cholesterol by the body.
The intake of cholesterol in food can be lowered by non-consumption of non-vegetarian food items and can be easily curtailed. As it may not be feasible to cut the production inside the body without harming the liver, SAAOL recommends a minimal intake. (10mg/day or less). However, in some people who are genetically conditioned to produce little more cholesterol, have to take some drugs, if they cannot control the blood level by diet alone. Exercise and physical activity leads to more breakdown of cholesterol. Stress or tension can additionally increase blood cholesterol level.
TRIGLYCERIDES THE EDIBLE OIL
We already know that Energy or Calories are derived in our body from only three kinds of food – Carbohydrates (Cereals, fruits, vegetables, Sugar and items made from them), Proteins (Pulses or dals, non vegetarian food) and fats. More than 99% of the consumed fats, by almost all of us, are Triglycerides. When the body breaks these triglycerides, each gram of Triglyceride will give us 9 calories of energy.
WHICH OIL IS GOOD?
It is a deep rooted belief in Indian house holds that one cannot prepare meals without oil but with taste intact. A variety of oils are available in the market. Many oil manufactures claim that consumption of their brand will help reduce the chances of heart disease. The slogan which is commonly used is “Heart friendly oil” which is not true. All oil manufacturing companies give one reason or the other in favour of their product, e.g. “Oil keeps your family active and fit”, “oil adds healthy goodness to your cooking”, “helps reduce cholesterol levels”, “brings out the true taste of good food while making it easy to digest”, “one feels active throughout the day… if you consume our oil” and “soy oil is healthy for heart as it is rich in polyunsaturates”….. and so on. All vegetable oils have one thing common in their composition i.e. zero percent cholesterol content.
The fact is that cholesterol is present only in animal products. And since all these oils come from plant products , there is no question of cholesterol being present in them. The oil companies highlight the “zero cholesterol “ slogan in order to promote their product and the consumer gets misleaded. No oil made from vegetable or plant origin contains cholesterol.
All oils, whether corn oil, soya oil, sun flower oil or Kardi oil, are full of triglycerides. Consumption of 10gm of any oil will provide 90 cal. What one must understand is that this is just a marketing strategy used by the promoters of the oil companies. Triglycerides i.e. Oil in any form whether saturated , monounsatured, polyunsaturated or mixed, promotes the process of blockaging and is hence Bad for the heart.
SHOULD OIL AND CHOLESTEROL BE RESTRICTED IN YOUR CHILD
No. Nutrition for children should promote growth and meet energy and nutrient needs without promoting overfatness. Parents can assist their children in making positive food choices. Habits formed in childhood are likely to carry into adult years. No food should be forbidden in a child’s diet. Children grow till the age of 30 years and in the growing years their body requires these fats. Keeping fat intake at moderate levels without going to extremes is the key. Parents who are too restrictive with fat intake will limit a child’s ability to eat sufficient calories to maintain growth. However screening is advocated for children and adolescents whose parents and grandparents develop heart disease or have high lipids.
HOW CHOLESTEROL LEVEL RUNS IN THE FAMILY?
Mr. Basu, 55, retired cannot sit on the breakfast table without a newspaper. Being an avid reader he has read in one of those articles that sometimes HIGH Cholesterol levels run in the family.

Primary can be because of genetic factors (i.e. genes passed on in a family) and also to add to it is the dietary and physical activity factors.
Known as Familial Hypercholesterolemia there is an increase of LDL and Cholesterol levels in the blood. Either the LDL binding to the cells is poor or transfer inside the cells is poor leaving it suspended in the blood and subsequently harming the inside of wall of blood vessels. Normally the Cholesterol inside the body, but her this mechanism fails, hence excessive synthesis of Cholesterol by the Liver leading to elevated levels of Cholesterol and also LDL. The receptors for LDL in the cells may be markedly reduced or even altogether absent.

What Is Nephrotic Syndrome And What Causes It?
Nephrotic syndrome is disease of the kidneys where the filtering system made of functional units called the nephrons, gets damaged. This is the name given to the collection of findings which develop as a result of such a damage. When the filtering system gets damaged, the kidney tends to filter substances which it usually doesn’t, and filter less of substances which it usually does. All of these issues give rise to some characteristic findings, which include:
Large amounts of protein present in the urine which results from leakage of protein through the damaged nephrons which usually prevent protein leak. This is known as proteinuria in medical terms and is calculated according to your child’s age and body weight.
Low levels of protein in the blood, which results from the protein leaking out into the urine, and is called Hypoalbuminemia.
Edema or tissue swelling in all areas of the body, which is the result of reduced levels of protein in the blood. This swelling is especially evident around the eyes, in the face, and in the abdomen which is known as ascites. The swelling results because the protein in the blood actually acts like a sponge which helps to keep most of the fluid in the blood, and when there is a reduced amount of proteins in the blood, the water tends to leak out from the blood into the tissues, resulting in swelling.
Increased levels of cholesterol in the blood, which results from reduced levels of protein in the blood which is a signal for the body to increase its production of certain types of fat.
The symptoms and signs of nephrotic syndrome:The signs and symptoms that children often present with include:
Malaise and fatigue
Abdominal pain and swelling
Frothy urine
Increased body weight and facial edema
Loss of appetite
Paleness of the nail bed
Dull hair
Development of intolerance to certain food or allergies
The cartilage of the ears may feel less firm
The different types of nephrotic syndrome:There are different types of nephrotic syndromes in existence as a result of the different causes. Some of these causes include, diabetes, hepatitis B and C, medications such as corticosteroids, and conditions like systemic lupus erythematosis, and cancer of the kidneys. The most common type of nephrotic syndrome to affect children is one that is known as idiopathic nephrotic syndrome, where there is no identifiable underlying cause.
Nephrotic syndrome can also be categorized according to the way the kidney tissue appears when it is viewed under a microscope following a biopsy. According to this tow forms have been recognized, Minimal Change Disease (MCD) and Focal Glomerular Sclerosis (FSGS).
Minimal Change Disease (MSD):This is most common form to affect children and you will see that the renal cells look almost normal when seen under the microscope. It responds well to steroids, and has a good outcome in most cases. This form doesn’t have any lasting damage on kidney function, and relapses less and less frequently as the child ages, and is usually absent in adulthood. It is very rare for a child affected by minimal change disease to require dialysis or kidney transplant.
Focal Glomerular Sclerosis (FSGS):This form of disease is present only in about 10% of the children with nephrotic syndrome. It requires biopsies in order to be diagnosed and doesn’t usually respond to steroid therapy, and is an aggressive form, which more often than not requires the child to undergo dialysis or a kidney transplant.
What is congenital nephrotic syndrome?As the word suggests this form of nephrotic syndrome is present at birth, but it is most likely that the condition will go unnoticed at the time of birth and be discovered only when the baby is few months old. It is a very rare condition and is caused by a genetic predisposition where both parents have to be carriers of the condition. It is a very aggressive and severe form of nephrotic syndrome where the affected baby will require a kidney transplant as soon as possible, ideally within the first few years of life. And till a transplant becomes possible, the baby will require transfusions of blood protein almost every day, because the protein leaks out from the kidneys at a rapid rate. They will also require frequent boosting of their immune system and hormones in order to keep them healthy till the transplant can be done, and till they reach the appropriate weight with which a transplant becomes possible, which is about 15 pounds when the child is roughly around one year.
