It has been 2 months since I started working as an intern in San Lazaro which is one of the prestigious hospitals in our country. And in that sheer span of time I could say that I learned more than I ever expected and I’m still looking forward to learn more as we go along in our internship.
Over the past few months, during my regular duty in TB and SACCL Section I feel like I have gained so much experience and real world wisdom. And I am so grateful for this experience in a somewhat sheltered environment. Because the environment of the workplace of these two sections is way different from Microbiology which was my recent post. My working experience in these two special laboratories has allowed myself to work to a higher standard but to be honest, the first few days of my duty in TB and SACCL were hectic and sweat filled as I tried to cope up with some of the laboratory works, intricacies and tasks to be done. Tb Section doesn’t seems to have a bustle works to do that’s why I was culture shocked at first as I entered this section since I came from Microbiology which is a very busy laboratory and I’m already adapted to it. But once I got into a routine and learned the jargon of the laboratory I quickly began to enjoy my work day.
Experiencing full regular work days and all the bumps and turns in these two special laboratories is something that other hospital can never replicate since it is the only National Reference laboratory for STD/AIDS and TB. In the TB Section I developed a key skill that I’ve never get in the classroom and that is coiling of sputum and the day to day interactions with the patients. TB laboratory works are not really hands on experience, the thing we are only allowed to do was just receiving and coiling/staining the specimen for DSSM yet I am still thankful that we were able to get the opportunity to work at DSSM. Direct Sputum Smear Microscopy is the most widely used means for the diagnosis of pulmonary tuberculosis (TB). In this section, we were taught how to receive specimen for DSSM and how to coil and stain sputum specimen. We were also given lectures about TB and the gold standard they are using in the laboratory. We were also allowed to observe the staff doing the GeneXpert MTB, it is a cartridge based nucleic acid amplification test, an automated diagnostic test that can identify Mycobacterium tuberculosis (MTB) DNA. It uses a sputum sample and can give a result in less than 3 hours. They are also doing the TB Culture, it is a test that involves studying bacteria by growing the bacteria on different substances. The medium they are currently using is called Ogawa, it is a semi-synthetic selective medium applied for culture BCG Bacteria. It has green-coloured slant obtained from egg yolk mass combined with glycerol and malachite green. Working in Tb, gives us also a background about (NTP) The National TB Control Program, it is one of the public health programs being managed and coordinated by the Infectious Diseases for Prevention and Control Division (IDPCD) of the Disease Prevention and Control Bureau (DPCB) of the DOH. By observing and learning from them, I managed to pick up tips and learned different types of skills. So far, my 20-day duty in TB especially the hands on, often pressure filled, work that I experienced will be a great advantage to me in my future work setting.

The next post I had this month was the SACCL, San Lazaro Hospital STD/AIDS Cooperative Central Laboratory. It is known as the National Reference Laboratory for AIDS and other STI with the following mandates: Confirmatory Services, Training, External Quality Assurance Program, Kit Evaluation and Surveillance. One of the more rewarding things about this experience has been the ability to apply things that I learned in some of my subjects especially immunology and serology. My experience in SACCL might not be really that hands-on but at least I’ve learned a lot through observing all the things they are doing inside the laboratory. I gained an in-depth understanding of the STD/AIDS and their corresponding tests and also the awareness and prevention of acquiring this kind of infections.
Some of the laboratory tests available in SACCL are thoroughly run by staff that are HIV proficient that’s why we are only allowed to observe what they are doing. They have 3 different test field including Serology, Molecular Diagnostics and Microbiology. Under the Serology testing these includes; HIV Screening Test (Immunoassay & Rapid), HIV Confirmatory Test (Western Blot), HBV/HCV Screening Test (Immunoassay), HBV Markers, HBV/HCV Confirmatory Test (Neutralization Assay), Syphilis Screening (RPR) and Confirmatory Test (TPPA or TPHA) . Under the Molecular Diagnostics these test includes; HBV/HCV Viral Load (RT-PCR) and HIV Viral Load (RT-PCR). And lastly the Microbiology these includes; gram stain, wet mount, KOH Mount and N.gonorrhea Culture and Sensitivity.
One of the strongest and most apparent skills that I learned during my 9 day - duty in SACCL is the open communication and interaction with the various outpatients I encountered and also the staff I am working with. At first I was intimidated and nervous to ask too many questions as I was observing them working but as time goes by I realized being inquisitive is not really bad at all in fact asking questions is the only way to learn what you are really supposed to be doing. In this section I’ve encountered so many outpatients who are HIV positive and some who has STI’s. We all know that HIV infection is not socially acceptable in most countries especially in our country and thus discrimination is really rampant. Those most at risk to HIV continue to face stigma and discrimination based on their health status. This one actually has to stop, that’s why every outpatients that came to the laboratory to get their results of their CD4 monitoring we make sure that we interact with them with constant kindness to the best of our ability just to make them feel that they are not different and thus makes them feel the total comfort that they want every time they interact with people.

In my 2 months working in this institution I never had late despite the temptation to sleep in, I woke up every morning and made it at the hospital on time. But there comes a time I was stuck in traffic one morning that’s why I already had one late but thankfully it only serves as a warning so I still don’t have a make up to do. But it will eventually get reset by the next month.
