The patient's facial features are assessed for typical facial stigmata of aging involving; lentigines, erythema, fine wrinkling, loss of skin laxity, eyebrow and eyelid ptosis (or chronic eyebrow elevation to correct latent brow or lid ptosis), mid-facial ptosis, jowling, loss of facial volume, and deep dynamic wrinkles in the glabellar, perioral (around the mouth), and periocular (around the eyes) regions. It is particularly important to view the patient before concentrating on the periocular region, as the goal of any treatment is to create a harmonious facial appearance. An observer's eye is drawn to asymmetry more so than rhytides (wrinkles or lines). That is, the creation of a "smooth island in a sea of wrinkles produces a less natural appearance than if the patient were not treated at all. The patient is then evaluated from a frontal perspective. Particular attention is paid to the presence of rhytides (wrinkles) at rest, eyebrow contour and position, horizontal forehead wrinkles or lines that may be indicative of chronic brow elevation, the extent of photoaging, presence of dermatochalasis (excess skin) in the upper eyelid. If white sclera is visible above the superior limbus or below the inferior corneoscleral limbus, added ophthalmic evaluation is warranted. The presence of horizontal forehead lines may either show overactive use of the muscles or facial expression or a compensatory response to eyebrow ptosis. One must make this distinction, as weakening the frontalis muscle in the following setting will uncover previously latent eyebrow ptosis. That is, patients who must elevate their eyebrows continuously to prevent impairment of their superior visual field will no longer be able to do so if their frontalis muscle is weakened. The examiner can find the patient with latent brow ptosis (dropping) by studying the patient when the frontalis muscle is completely relaxed (instruct the patient to relax their forehead). If the eyebrows assume a lower position when the frontalis muscle relaxes, Botox should not be injected into the forehead. These patients need eyebrow and/or eyelid surgery to correct their underlying problems. Next, the patient is asked to gently (not forcibly) close the eyes to ensure complete apposition of the upper and lower lids. Some patients who have had prior surgery, trauma, or thyroid disease may be able to forcibly close their eyes, but have an incomplete blink that leaves them highly vulnerable to symptomatic dry eyes if the orbicularis oculi muscle is weakened. A careful injection record should be kept, and first-time patients are usually followed up two weeks after their injection. At the follow-up visit, the treatment goals should be reviewed and compared to the clinical results. If added doses are needed to achieve the desired goals, these should be administered at the follow-up visit. If the patient has not reached the best possible outcome, careful notes should be made about adjustments that need to be made in dosage, placement, or both. The benefit of the treatment of frontalis muscle should be carefully evaluated. For some patients, the treatment of the upper part of this muscle is the best choice, because the treatment of the entire frontal region causes significant loss of facial expression (masked appearance). Moreover, total paralysis of the frontalis muscle can cause brow ptosis. The frontal region should always be treated in association with the glabellar area for more satisfactory results, avoiding the excessive use of glabella's muscles, which are depressors. The injections are typically given 2.5–5 units per injection sites, depending on the extent of the area to be treated, whether totally or partially. Thus, the clinician should be conservative and allow some functional areas to remain intact to enable brow elevation. Patients with a narrow brow should receive fewer injections (four sites, compared to five) and lower doses than patients with broader brows In the glabella, the aim of treatment is to reduce the vertical as well as the horizontal lines. Contractions of the corrugators and to a lesser degree the medial orbicularis oculus and the depressor supercilii produce vertical lines between the eyebrow. The contraction of the procerus produces horizontal lines over the bridge of the nose. Five injections, with two needles into each corrugator and one into procerus, have been recommended for the cosmetic outcome. Clinical techniques should consider the variation in location, size, and use of the muscles among individuals. Injection of botulinum toxin into certain areas of the face that carry a high risk of dropping (danger zone) needs caution in the application. An example is a lateral forehead, particularly the lower part. Recently, however, injection of BTX-A through the temporal hairline to correct supraciliary wrinkles seems to be safe and effective Injection of BTX-A for glabellar frown lines can cause mild medial brow ptosis, a lateral brow elevation, and a more pleasing contour to the eyebrow. She’d introduced herself as Sandra, probably to make the whole thing feel that much less formal. To make it feel like her compassion didn’t have an Upper East Side price tag with a hidden confidentiality clause. “That’s a, um... Good one. Existential, you know? I feel like I should be wearing a pair of fancy spectacles. Or maybe a monocle. Speaking of, I’d totally hook up with the Monopoly man. He looks like he’s secretly a dark horse. I bet he was the inspiration for Katy Perry’s single.”
“Lana,” she broke the purse of her lips to carefully tread closer to the source of the issue, clicking idly at her pen all the while in a way that made Lana want to fidget. “In the twenty minutes we’ve been here already, you’ve already managed to divert every question I ask into some sort of... abstract comparison or metaphor, I suppose you could call it. You have quite the talent for it. Do you do this a lot? At home?”
Bundling her hands up in her lap before disentangling them again, Lana repeated the process over and over until Sandra’s eyes dropped to study them and, subsequently, they froze in place.
“What do you mean?”
“I mean,” Sandra started again, clearing her throat softly before politely excusing herself so she could continue, “when people ask you how you’re feeling at home, do you do the same? Divert the focus. Distract them from what your answer might be. Hope they forget they even asked in the first place -- I’m assuming that’s your intention, conscious of it or not.”
“Um,” slipped out under her breath, hazel eyes stuck down on her fingers as she aimlessly fiddled with the chipped, pastel yellow polish on her thumb. “No, I don’t. I, um... Sorry. I didn’t mean to.”
“That’s okay, Lana. I’m only hoping to get to know you a little better, understand why. I’m just wondering... Well, maybe you can help me. Why do you think that is? That you don’t do that at home, yet you do it here.”
She felt like a spider with all eight of its limbs being pried apart in slow motion. Like a bug under a burning microscope, wriggling as the heat became something verging on unbearable.
“Because I never get the chance to do it.”

“I don’t know,” she finally voiced out loud, shy smile wavering. “That doesn’t matter, though. I’ll answer it. The seven deadly sins thing, I mean. That’s what you, um... That’s what you asked, right? Hm,” she tacked on to a theatrical degree just to stall for more time, index finger tapping absently against a freckle cluster on her thigh as she avoided looking at the mousy brown locks of hair across from her. “Tough one. A thinker.”
Gaze shifting towards the open blinds on the window, Lana’s attention slid along the stretch of exposed window pane between every strut, eyes silently assessing the different colours of cars sat out in the car park behind the building.

It looked so bleak outside that any effort made within the room to make it feel homely had gone completely to waste. It had good intentions, she supposed. The faded mustard of both arm chair facing each other made her feel like she’d been cast on a game show rejected for air by prime time television, stuffing shyly peeking just beneath the back of her right knee and making her leg ache the longer she awkwardly rest it there. There was a box of Kleenex freshly manhandled on top of the coffee table, two chips marking the front leg on the left corner, which Lana had used as a conversation piece within her first two minutes there in order to say that they looked like nipples.
“Lana...? You--”
“--Greed.”
“Excuse me? Sorry, I didn’t catch that.”
“Greed,” she repeated firmly, eyes still stuck outside as she watched another therapist fling up his boot and rifle around inside a box of files he’d loaded there. “You probably, um... I mean, you probably asked that because you wanted me to say lust, I’m guessing. Because that’s what this is about. This whole... “therapy” thing,” came along with a nervous hint of a laugh, slight frown on Sandra’s face from Lana’s use of air quotes going amiss as she continued. “Because I fucked a whole bunch of people, right? I marched right in and I fucked them all. Just... fucked them. Bish, bash, bosh. Fucked. Sorry, I don’t know why I... repeated that. Felt like I had to. Dramatic emphasis and everything. And if you say something enough times it loses its meaning so I kind of... I guess that’d be nice, too. If it didn’t mean anything. If it wasn’t real.”
Stern line in Sandra’s forehead softening slightly, she made an effort not to move a muscle. She imagined doing so would have a similar effect to startling away a grazing deer that had only just worked up the nerve to wander a little closer.
“But it’s only people that don’t get sex that would say that. Maybe you’d say that. I mean, not you. Sorry. Just... scalping your sex life. You as in the general you. Professionals, or whatever. You’d say sex is sex and that’s that. Sex is the problem. Hold up a crucifix or whatever the second you see a tit. Because, I mean, nobody here has ever actually been railed so hard they see god, have they? Just unthinkable, apparently. Taboo. If you have money you’re not allowed to put a dick in your mouth. Cum? Banished! Ten days of chastity and you’ll be levitating, shooting lasers out of your eyes, erect as a flagpole as the national anthe--”
“--Lana.”
“Sorry,” she sighed shortly in frustration, eyes rolling up to the ceiling as she restlessly squirmed her knees together like she was trying to crush something between them.
“That’s okay, Lana. Just try not to... be quite so vulgar. You may carry on. I’d really love to hear what you have to say, if that’s okay.”
“Okay... Well, um... It’s not sex. That’s just... short sighted. That’s stupid. I mean, I’m pretty fucking stupid and even I know that. It’s just the... It’s the being close, I think. You’re the only thing in the room when it’s like that, you know? You’re the only thing in the whole world that exists. It’s like...” she trailed off, wetting her lips briefly as the conversation matter left her throat inadvertently dry, “it’s like you’re... I mean, you’re actually...”
“You’re what, Lana?”
Hesitation became something tangible in the silent few seconds that followed, as oppressive and skin tight as a well bound layer of cling film around a freshly laid corpse. She could feel it sticking to her skin.
“Important, I guess. You matter.”
“And you like that feeling? You think you’re... You think that’s it? You’re greedy for it?”
“Yeah,” she answered simply, stare remaining trained on the glowing back lights of the car in the lot as he shifted gears into reverse and started backing out of his spot. “They can’t take their hands off you. Sometimes it even... It doesn’t matter if it’s bad. Sometimes it even hurts and you can’t stand them for it but it’s, um, it’s okay, because it’s you. You and them. You’re a part of them and they’re a part of you. And for half an hour, maybe, it’s just... that. Someone can actually see you,” slipped out in an embarrassed breath verging on laughter, eyes adamantly blinking back the tears that had bud up on her bottom lashes out of nowhere in the space of her last syllable. “I think that’s the nicest feeling in the whole entire world. Don’t you? I think I’d live in that feeling, if I could. I’d live in that feeling and never let it go.”

I have an 18-month-old Goldendoodle who is a very kind and loving dog. My family’s intent is for him to work as a therapy dog. The problem is I am a 14-year-old small girl who this dog doesn’t respect at all. I feel like I have tried everything. My family has hired behaviorists and trainers. I walk him 30 minutes a day — morning and night — and I train him for 10 minutes after each one of those walks. No matter what I do, he doesn’t respect me. However, he respects the rest of my family. All my dad has to do is walk in the room and he will drop whatever he has. It’s the same with my brothers, who are 11- and 16-years-old.
The next issue is that he has possession aggression. He is very fast. He grabs everything and does not let it go (at least for me). For example, he loves socks and will swallow them. Because of this he has had to get two surgeries and has had multiple visits to the vet to induce vomiting. We have buckets with lids and locks, but he can also open doors. When he gets something to chew on and I try to take it away, he growls — a lot. Then when I try to open his jaw, he bites me.
I need help. How can I make him respect me? Is it my fault or the dog’s?
Wow. It sounds like you have quite a few issues going on. From what you wrote, it appears you are doing some good work already. Maybe we just need to add a little fine tuning. I can share a few tips.
Let’s break your letter down into individual sections so you can focus on one thing at a time.
First, respect for dogs can be confusing. Some of the factors may be related to your age and stature. Size matters to dogs. That said, one of my former apprentices has gone on to become a dog behaviorist and she is just over 5 feet tall. So it’s something you can get past.
While the walks are great, they may not be enough. Your average dog needs an hour of exercise every day, but some dogs may need even more. From what you wrote, I’d guess your dog falls into that category. This video on creative ways to exercise dogs can help you supplement your walks with some easy indoor exercise options.
I’d look for some ways to build in compliance before your dog gets what he wants in your day-to-day life. Petting with a purpose is a wonderful way to help your dog learn to respect you as a leader and help him practice asking for things instead of telling you what to do.
Do you enforce rules with your dog? Dogs often see those who enforce rules as the leader. Enforcing rules gives you an opportunity to demonstrate your leadership in small, subtle ways multiple times a day.
I’d suggest you also start developing a strong leave it command with your dog. Once established, you can pull out high-value items and leave them on the floor when you can supervise and give the dog the leave it command. The more you repeat this, the less the dog will try to take things you want them to leave alone.
Lastly, this video includes a really easy and sneaky way to condition your dog to come to you — even when you don’t call him to come over.
Remember, training and behavior are separate. While training is awesome, I’d suggest you work more on these structural changes and behavior exercises to help your dog learn that listening and respecting you cause good things to happen.
Good luck and remember — everything you do trains your dog. Only sometimes you mean it.

Boy! Those caveman days were rough! They felt like listening to deafening leopards roar a rushed version of hells bells, inxs of pulp fiction sung backwards by cold play, or a brutally nasty yet thankfully short youtube video of foo jimmy john, fighting beastie boys winking in the hood.
Loud quiet rioting !@#$ growls shook bats overhead when this grizzled papa bear disturbed (like a flock of sea gulls fighting angry birds over a candlebox), and forced to wake prematurely from hibernation set his seething animal anger to boil, and smoke to issue from his jack rabbit ass nine looking don Quixote ears.
Argh!
The gumption from that gap toothed, high browed, red-necked ursine spouse giving him one swift kick in the bony arse sent him flying like a twisted sister queen phish careening out into the frigid air.
He wished at that moment to become gratefully dead.
Upon immediate and most unwelcome exposure therapy to arctic blast, this mama’s and papa’s boy (by george) felt moody blue, and neither sought to tangle nor play footsie with Mother Nature.
He wanted to whip the hide of that pen heck king wife, when needles of miniature aeroplane shaped snow (white slippery buckshot elements of style) kissed, pierced and smashed against his face from those shoddily made flimsy animal clothes that barely kept him warm.

Without a shutter shutter flying shadow boxer of sprinting doubt, these goods (needle lee sewn) by snoop doggy dog gone hooligans, who cawed like sum Cajun gumbo baboons.

No way, no how does this domesticated simian relish expending any ounce of energy.
Without the need to leave the comfort of my warm bed, a click of the remote can provide immediate needs at these fingertips. Why dress (perhaps just a coat of armor), when breakfast, lunch or dinner delivered via robot. Bathe? This waterbed doubles up as a washbasin. Ah!
How in the name of judas priest could our ancestors enjoy feeling like a beast of burden? Who says you cannot always get what you want? Alice in chains? Cinderella? Eddie money? Jane’s Addiction? Pink Floyd? Yes! The entire motley crue! Yeah!
If not totally tubularly clear regarding trappings of a Culture Club bygone era, lemme spell out loud and clear, which stance will be obvious, I AINT NO LUDDITE period.
This creature of habit would never give up his pad (shaped like an oversize ipod) and forego any of his labor saving devices the only way to take away these cherished, idolized, prized possessions?
You would have to pry these buzzing, flying, whirring gizmos loose from my cold dead fingers! Don’t get your hopes up! I aint planning to cross the river Styx anytime soon. Maybe not even in this lifetime! Ha!
