Alex let the warm water run over him as he got ready for the day. It had been a quiet morning and Jo was slowly getting up as they got ready for work. He was enjoying a few more minutes of alone time in the shower. He also secretly used his wife's favorite soap and loofah.
“Incoming and I don't have a lot of time!” Jo said as she ran into the bathroom in her favorite fluffy robe. He had gotten this one for her last Christmas after her last robe, the one she had stolen from the hotel at Bailey's wedding, had finally fallen apart. 
“Do you want to use one of our other three bathrooms to vomit?” Alex asked, turning around in the shower to look at her, although she smiled and didn't seem nauseous. 
“Shh,” Jo shushed him, turning away from him as she grabbed the hairbrush. “I feel decent for the first time in weeks, so don't jinx it! Oh and don't forget we have our first OB appointment today.”
“Yeah, I thought it was after work?” Alex asked, turning to look back at her again.
“It is.” 
“So what's the rush?”
“Well Luna is still asleep, my charts are done, and I've got just enough time for a quick shower,” Jo said, putting her hairbrush down and turning around to him. Alex opened the shower door as she untied her robe and he stared down at her body.
“How quick?” Alex asked with a smirk. It had been a while since she felt up to this and he was determined to enjoy it.
“If it's too quick, I'll kill you?” Jo said, clearly thinking the same thing as she stepped into the shower, putting a hand on his shoulders and closing the door behind her. 
Alex smiled and pulled her in with a hand around her waist. The water fell over her as he leaned in, their lips meeting as they kissed. However, Jo's hands moved faster as she grabbed his hips and pulled him into her. The hot water wasn't the only thing that caused the shower to steam up that morning.

“Hey I'm sorry I'm sorry,” Jo said as she ran down the stairs past the nurses station to where he was waiting. 
Alex had already changed out of his scrubs and into a change of clothes. He leaned against the counter on his phone, although he didn't look too pissed at her. The last she had talked to him was at lunch before she had left to study. It was supposed to be an easy day for both of them, but it never was. 
“The appointment was at 5:00 right?” Alex asked, looking at her with a raised eyebrow. 
“I know, I just got so busy with patients, and then Yasuda found this amazing surgery to save her sister's fertility and that took a little longer than expected. And I completely forgot,” Jo said, gesturing to her invisible patients with her hands before she shook her head and looked down.
“Hey, you have nothing to apologize for,” Alex said with a little shrug. “I waited for you at the clinic, sitting there like an idiot, being the only guy there.”
“I'm sure,” Jo rolled her eyes as he smiled, clearly teasing her. 
“But DeLuca had to leave.” 
“We were going to see the baby,” Jo wined with a pout that looked all too cute.
“We'll just reschedule,” Alex said, although he couldn't hide his disappointment.
“Wait, come with me,” Jo said with a little smile and a giggle. She realized they were both doctors and had access, and the training, to perform an ultrasound as she dragged him away.

Jo lay on the bed as Alex moved the probe around on her stomach. They had borrowed an ultrasound machine and retreated to one of the on-call rooms where they would have some privacy. They could hardly stop from smiling and Jo kept giggling. Luckily they didn't run into anybody on their secret adventure, lest they both spill their secret immediately. 
“I don't see anything, do you?” Alex asked as he moved the wander around and narrowed his eyes at the screen.
“Just my intestine, stop moving it around,” Jo said with a laugh as she grabbed his hand. She moved the probe down towards her uterus before looking back at the screen. 
“How long has it been since you've done one of these?” Jo asked as she tried to find a good angle. 
“Probably not since my second year.” 
“As an attending?”
“As a resident,” Alex said as they both laughed.
“Well, that was ages ago old man, no wonder you confused my uterus with my intestines. I thought you'd be well educated with my anatomy by now?” Jo said with a smirk.
“I am well educated, I did get you pregnant didn't I?” Alex said as they just laughed again and she tried to focus on finding their baby.
“I still don't see anything,” Alex said as he pressed something, and suddenly their baby was on the screen. “Wait, what is that?”
“Yeah, there's our baby,” Jo gasped as she saw them for the first time and the sound of their heartbeat filled her ears. Jo smiled so wide, her cheeks hurt. “And there's the heartbeat!”
There they were. The baby they had wanted for so long and didn't think they would ever have. Before they had adopted Luna they had been trying intermittently and then officially after the pandemic ended. However, Alex's sperm count was low and things would be difficult. Then Luna got diagnosed with hearing loss and they decided just to focus on her. She would be enough for them. 
Then she fainted in Alex's arms and Levi dropped the bombshell on them that she was pregnant. They were so overjoyed, but she was secretly worried that it was wrong. That it was just a false positive and a chemical pregnancy or something. But this, seeing their baby on the screen made everything so real. While Jo didn't couldn't stop smiling, Alex looked at the screen suspiciously. 
“You don't see it do you?” Jo said as she leaned up and pointed at the screen where their baby’s heart was. “You see it right there?”
However, Alex moved the doppler around and pressed a few buttons searching for a better image. “Oh, and also… there.” 
 Jo instantly sat up and started the screen as they saw another heartbeat, another baby. “Oh my God!”
“Twins!” They both said at the same time, staring at the screen in disbelief as if two babies had magically appeared. 
They looked back at each other, both with wide eyes and dropped jaws. Alex stared back at the screen and Jo sighed as she flopped back onto the bed, putting her hand over her eyes. Alex fumbled and mumbled like he wanted to say something but the words couldn't quite get out. 
“Twins, you knocked me up with twins, Alexander!” Jo yelled at him knowing it was half her fault, but still in disbelief. 
“Shush, Jo the whole hospital is going to hear you!” Alex said looking between her and the screen the disbelief still written across his face. 
“I don't care! You knock me up with twins! That's like twice the diapers and twice the screaming, and crying, and, and, bottles, and formula or breastfeeding, and diapers!! Alex the diapers!”
“I know!” Alex said in disbelief, but there was a little sliver of a smile across his face before Jo reached up and punched his shoulder.
“Twins Alex! Are you serious? How did this even happen?” 
“I'm pretty sure you know how this happened,” Alex said with a chuckle as he smiled although shrank away when she reached up to slap him again. “Ow, It's more likely that somewhere along our lines, someone must've had twins and we just didn't know. I mean we still don't know anything about your biological parents, beyond your mom's immediate family. And it's not like my family history is that well documented either.” 
“Yeah,” Jo said in a whisper, staring at the screen as they both fell quiet.
“Jo,” Alex whispered too, looking over at her with concern.
“I'm serious though, how are we going to handle twins?” Jo said, staring at the screen and shaking her head as tears appeared in her eyes as she blinked them away, although they fell onto her cheeks. 
“Hey, hey,” Alex said, seeing her tears and dropping the doppler as he reached down to hug her. 
She sat up and buried her face in his chest as he held her. Alex rubbed her back and Jo tried to breathe and calm down. She didn't want to cry, but she did. She was confused, and surprised, and scared, and yet so full of love for her two babies. All these emotions just overwhelmed her and spilled over in tears. 
“Hey, it's going to be okay,” Alex whispered to her, pressing his lips to the side of her head. “If anybody is more prepared to have twins it's us. The best pediatric surgeon and the best OB resident in the Northwest. If we can handle Luna being in the NICU and everything that we’ve had to go through with her, then having twins is going to be like a walk in the park.” 
Jo laughed as she pulled back, sitting up and wiping away her tears as she looked up at his smile. “If you say that, you're going to jinx us.” 
“I don't care, we're having two babies Jo, twins!” Alex said, letting the excitement fill his eyes as they twinkled with joy. 
“Yeah, we are!” Jo said as she smiled and let the excitement fill her as she reached forward and kissed him pressing her lips to his as the excitement sparkled on their lips. 
“Okay, now lay back. I want to see them again!” Alex said, grabbing the doppler as Jo laid back. Although, she reached out for his hand and he held it, giving him a squeeze. 
It was easier for him to find the babies this time. They both smiled at their babies on the screen and listened to their heartbeats. They should’ve been taking measurements and checking their organs, but they were both healthy and they looked at their twins as their parents. They stared at the images of their babies, just enjoying seeing them on the screen. 
“We're having twins!” Jo said with excitement as she giggled and Alex looked it over at her. 
“Yeah, Karev twins!” Alex said, with a chuckle as he leaned down to kiss her before they stared back at the screen. 
“Oh my God, I'm pregnant with two big headed Karev babies!” Jo said, the smile falling from her face and her eyes went wide as she stared at him.
But Alex just roared with a laugh, as he leaned back, but Jo shook her head in disbelief. “Don't worry, chances are with twins you'll have to have a C-section anyway.”
“No,” Jo shook her head as she glared at him. “It's safer to have a vaginal delivery!”
“Yeah, but with twins they’ll be smaller!” 
“Shut up, no they will not. I've seen the size of your head,” Jo said, although Alex just laughed as he avoided another one of her attempts to smack him. “I am so going to kill you.”
“I don't care,” Alex said, his smile wide on his cheeks as he just laughed. “We're having twins!”
“Yeah,” Jo said in a breath as she smiled, still in disbelief as she said it again. “We’re having twins!”

AN: I thought I should also feature our favorite canon couples set in this episode because so much horrible stuff happened that I thought some rewriting might brighten it up. Like and reblog below and let me know what you think.
Summary: Alex visits Jo and Luna in the NICU who tells him Val died. He is saddened by this and proposes to Jo that they adopt Luna and raise her as their daughter.

April 30th, 2020
Jo and Alex push Luna in the incubator down the hall to take her to see Val before taking both of them into surgery. Their hearts break seeing this woman who wanted to be a mom for so long not being able to hold her child. Along with that they both have to have surgeries back-to-back due to complications that come from a fetus growing in a liver.
It took some convincing but Alex was able to get Hayes on board with allowing Val to see Luna before they take her to surgery. He wanted to give Val one chance to see her child in person instead of a screen because they both need joy right now. Seeing his baby sister break down over her ex-boyfriend being in surgery has Alex fight extra hard for this poor family because he can’t do it for Amber right now.
“Hey.” Alex sees April Kepner behind them as they keep pushing the incubator, “I just spoke to Hunt, DeLuca is gonna be okay. They had to do a splenectomy, but the injuries weren’t that serious.”
Alex sighs in relief, “Thank god.”
“How’s Amber doing?” Jo feels bad for not being there for her sister-in-law during this difficult time.
“I left her with Jackson so I could get her a coffee. Hunt and Altman were on their way to tell her and Carina right now.”
“Tell her I’ll check on her later I just have to operate on Luna.” Alex tells April who nods before leaving them and Luna, “I needed at least one piece of good news today.”
“You and everyone else honey.” Jo tells her husband before leaning down to talk to Luna, “But good news for you Luna, your gonna see your mom before the mean doctors take you to treat you.”
Alex grins at that comment, “Your mommy is gonna freak out when she sees you, yes she is.”
The sweet talk is interrupted by the PA system announcing Code Blue before the couple look up to find nurses and Schmitt going into Val’s room with a crash cart. Alex and Jo frown at Val coding when they were ten feet away from the door with her daughter ready for her to see Luna in person.
“Val’s coding! Hurry!”
Jo snaps out of it from Schmitt’s command, but her worries are visible, “No. No, no, no.” Jo leaves Alex and Luna out in the hall to go to Val. Alex swallows at the sight but knows he needs to keep moving to give Luna the surgery she needs to live. He knows he has to keep his promise to Val and make sure Luna gets the care she needs to walk out of this building alive.
That Night
Jo stands in the NICU is grief waiting for someone to come in. The young surgeon tried everything she could to bring Val back but in the end it was futile. Jo was forced to call time of death on Luna’s mother when it became clear Val wasn’t gonna make it. Now Jo waits in the NICU hoping that while Val is dead her daughter is still very much alive after her surgery.
Alex enters the NICU wheeling in Luna’s incubator to Jo’s relief. Still her relief is only mild as she acknowledges that with Val dead, Luna is gonna have to go into foster care. The thought of this little girl who has already been through so much in her short life having to go into a broken system that she and Alex know too well won’t give Luna the care she needs breaks her heart. Her face remains impassive as Alex parks Luna causing Jo to look down at the tiny baby sleeping in her blanket.
“Surgery went well.” Alex informs his wife, “No respiratory distress, chest tube is clear. This little one is a fighter.”
“She’s gonna need to be.” Jo says in sadness.
Alex can recognize the look in Jo’s face and sighs sharing her grief as well, “Damn it, no.”
“I ran the code for an hour.” Jo looks down at Luna with regret feeling like she is the reason Luna is alone now, “She has no idea her mom didn’t make it.”
Alex wishes he could hug his wife right now but he settles for words while keeping a safe distance, “You did everything you could.”
“I know.” Jo says still sad, “But it makes it that much harder looking at Luna right now. We can all do everything we can to help people who need us and make sure they’re here to see another day and it still isn’t enough.”
Alex looks at Jo in worry, “Do you want to clock out and call your therapist?”
“I will, I just want to be here and make sure she’s okay.” Jo sniffles, “Her mom died, her father doesn’t want anything to do with her and there’s no telling if she’ll have long term effects from being born at 26 weeks. What kind of a childhood is she gonna have? I just…I just don’t know what’s gonna happen to this little girl when she goes into the system. I was in it, you were in it and we both know the chances of a premie like her getting a good family are very low. I want her to have what Val was supposed to give her. I want her to have a family, a real family with people who love her and will take care of her. She deserves better than what she’s been given.”
Alex sees his wife sniffling and it makes him sad. He looks down at Luna with her tiny face scrunched up as she sleeps while she’s wrapped in a blanket. The thought of Luna being discharged to a random foster family kills him.
He remembers the first day he met Val. He remembers how happy she was to find out she was pregnant after years of trying. He remembers her crying in joy at the sight of Luna on a screen. He also remembers promising Val that he would take care of Luna if anything happened to her. He made it as a surgeon but more than that he made it as another human being who wants a baby more than anything. It’s that memory that has him make a decision that will change his life and Jo’s. It’s that memory that has him looking at his wife imploringly before making his proposal.
“We can give it to her.” Alex states to Jo who looks up with confused eyes, “Let’s tell CPS we can foster to adopt her as soon as she’s discharged.”
“Really?” That one worded question is asked in elation rather than fear.
“Yeah.” Alex says with a grin, “She’s gonna need extra love and attention, we’re both doctors, we want to adopt so let’s adopt her.”
Jo begins to grin at this, “You really want to?”
“I promised Val the day she had Luna that I would take care of her baby if something happened.” Alex recaps to Jo before looking down at Luna, “And seeing this little girl right now in need of people who can love her and raise her like their own. It feels right, it feels like Val would have wanted us as her parents instead.”
“It does. Okay let’s do it, let’s be her parents.” Jo smiles with tears in her eyes and Alex also smiles at his wife on board.
Jo takes her gloves off, sanitizes her hands and opens the tiny door in the incubator. She puts her hand inside and touches Luna’s tiny hand with her index finger smiling down at the sleeping baby. The sight of it makes Alex see the family he has wanted for so long, causing him to stand by Jo as they smile down at Luna already being her parents.
“Hi Luna.” Jo greets her future daughter in a low voice, “It’s Jo remember me? And this is Alex you know him. We’re gonna be your mommy and daddy, yes we are.”
Alex chuckles in joy before greeting Luna as well, “Yeah that’s right Little Moon, I’m gonna be your dad and I already love you honey. We love you.” Alex lays his gloved palm on top of the incubator cherishing the contact he has right now. Jo places her hand on top of his smiling down at their future daughter, Luna Karev.

Powdered Donuts, Princess Crowns, and Bubbles
In which Jo announces something to Alex (unintentionally with the help of Luna); Inspired by the powdered donut scene in the closet.
A/N: This is my first published fanfic, so pleeease be nice. That being said, enjoy, and comments, prompt suggestions, and reviews are always welcome in the comments or in the 'ask me anything' box.

Side note: I'm partially ignoring canon with this fic, bc Jolex is my OTP and Grey's writers damaged me with Alex's exit.
"Here. I'm sorry for how I've been with you lately." Jo said apologetically to Alex, her voice fragile as she handed him an envelope with a box attached.
She'd been mad at Alex for reasons he didn't understand, and to be perfectly honest, she didn't understand them either...until she found out about this.
"Hey, don't worry about it. It's okay. What's this?" His voice was soft, and curious, and his eyes were gentle.
"Open it." Her face flickered with a ghost of a smile, but her eyes were nervous.
Alex's face lit up as he opened the box to find powdered donuts, but that expression quickly turned into a confused one, then finally one of worry. "Babe, did I miss an anniversary or something?"
"No, Alex, I promise you didn't miss anything. Just open the envelope," she said, sipping Alex's coffee and biting a donut.
Before Alex makes a move to steal a bite from Jo, Luna waltzes in, wearing a princess crown and blowing bubbles. "Moonshine, why're you wearing a crown?"
"Because, Daddy. Mommy's your princess. And that means I'm both of your princesses." Luna responds, very matter-of-factly, putting the bubble wand back into the container.
"Yes, you are, Luna-bug," Jo says, scooping her up, making Luna giggle.
"Mommy, that tickles!" Luna squeals before stopping to point at the envelope. "What's that, mama?"
"I don't know, Luna, why don't we let Daddy tell us?" She winked to Luna before smiling at Alex.
He opens the envelope, his face puzzled before morphing into one of pure joy. The envelope read, "Congrats!" signed by Mer, Bailey, Webber, April, Maggie, Amelia, Jackson, Owen, Arizona, Callie, Cristina, Aaron, Amber, and Helen.
"Jo, what is this?" Alex asks, in awe of what it might turn out to be.
"Well, how would you feel if I told you we're having another baby?"
"Jo, you better not be joking. Are you pregnant?"
She smiles, her eyes sparkling with unshed tears as she nods.
"Alex Karev, we're having a baby," she holds up a positive pregnancy test, the two pink lines obvious and almost too dark.
"Baby!" Luna cheers, blowing bubbles from Jo's arms as Alex leans in to sweetly kiss Jo on her lips before grabbing the donut in Jo's hand and bit into it.
"Baby, I love you, but I think I love donuts more," Alex said playfully as Jo softly swatted his hand away.
"Hey! What about me?" Luna said, both Jo and Alex laughing at her reaction.
"We love you too, princess. Now come on, we have donuts to finish!" Jo says, excitement filling Luna's eyes.
"Donuts!" Luna's remark had shown them just how ready Jo and Alex were to give their little girl a baby sister of brother.
"Do you think we're ready for it?" Alex asked as he saw his daughter's face light with happiness at the donuts.
"Mhmm, they'll love you just as much as you love them and Luna." Jo nodded, planting a kiss on Alex's lips, letting herself linger.
"And powdered donuts," he teases, kissing her back.
"Speaking of which, your daughter got into. You're putting her to bed tonight." Jo giggled, a sound that filled the room and Alex's heart.
"Always, baby. Always." He said, his voice filled with a conviction that resonated in both of them.

“How was your day?” Alex asked her as Jo gave him a smile. 
He met her in the hallway as she wrapped her arms around him, and avoided his question with a long kiss. Alex smiled and melted into her lips as they reunited, letting their lips greet each other with passion and eager romance. His tongue begged for hers as she moaned. He wondered if she would let him pull her into an on-call room before they picked up the kids. However, Jo pulled away before his tongue could dance in her mouth.
“My day was long. I had six laboring moms and three C-sections and ER consults in between that. I was running around all day. How about you?”
“It wasn't so bad, I did my first couple of surgeries and then mostly rounded and helped out the interns. Are you ready for tonight though?” Alex said with a smile as they walked towards the door. “I'm going to make you an offer you can't refuse.”
Jo looked at him with her eyebrows raised. “What?”
“It's movie night,” Alex said as Jo shook her head. 
Friday movie night was a long standing tradition in the Karev household, dating back to when they first started dating. Jo usually loved movie nights but these days she was too tired to stay up late with him.
“I’m on call, we should just sleep,” Jo said, she wanted to close her eyes right now, but she had to amp herself up for the energy of their toddlers. Once they got to the car, she could take a quick nap on the fifteen minute drive home. 
“How come you're only tired on my movie nights?” Alex asked, giving her a look that was a half-hearted joke. 
Jo rolled her eyes. “That's not true, I sat through that one wrestling movie you picked out the other week.”
“That was like a month and a half ago, Jo,” Alex said as he chuckled at her and they approached the desk to check out the girls. 
“No it wasn't, it was like 4 weeks ago or something,” Jo said with a shake of her head as he logged into the tablet.
“No, that was six weeks ago. Remember because we watched your cheesy rom-com. Then we let the girls stay up and watch the Barbie movie, then I chose Jason Bourne and then you wanted to watch Scream. We watched Trolls with the girls last week, so it's my turn again, 6 weeks.”
Alex casually smiled at her, but Jo’s heart stopped. He was right, it had been six weeks.
Her movie had been this Western romantic drama. She clearly remembered because she had been on her period and Alex had helped her relieve some pain with a couple of orgasms. Then two weeks after that, they watched his Jason Bourne movie and had sex on the couch, which would have been when she was ovulating. 
“I'm late,” Jo said, staring at him as he looked up at her. 
“No, we're right on time,” Alex said as he checked the girls out and set the tablet back down. 
“No Alex,” Jo whispered to him. “My period is late.”
“Are you saying you could be pregnant again?” Alex asked, his jaw dropping open as he stared at her.
Before Jo could respond though her phone beeped and she looked down at her notification. “I’m being paged, it's the ER.”
“Shit, is it urgent?” Alex said as Jo just glared at him telling him it was. “Should I get the girls or do you want me to follow you?” 
“Stay with the girls, but check them back in and have dinner with them, then you can go get something to eat and I'll text you when I'm done. Then we can take a test together,” Jo said, sure of her plan as she turned to leave, but Alex grabbed her hand. 
He pulled her in, giving her a quick hug, and kissed her forehead before he let her go. When she looked back up at him, he had the biggest smile across his face. “Hey, we'll be okay, regardless of the results.” 
She knew he was giddy at the prospect of potentially knocking her up. When they found out she was pregnant with Eden, he was so happy that he walked on air for weeks. Everyone knew before they told them, and he told everyone how excited he was. However, she didn't return his smile and just nodded. Jo loved Eden and she loved being pregnant with her, but the first trimester was rough and Jo labored for hours. She wasn't sure she was ready for that again.

Nearly half an hour later Jo finally paged him to come to meet her in the ER’s stock room. He wasn't sure how Jo was feeling but had settled into the idea of having another baby. He had gotten time to think about it but he had also gotten time to analyze Jo’s expression. Her eyebrows were timorously married and her lips were parted in a frown. He wasn't sure if she would want this baby. Although he had gotten excited and browsed online for baby onesies. Still, he waited to purchase one until she had taken a test, then they would know for sure.
“Hey, did you find one?” Alex asked as he found her rifling through the packages, trying to find a pregnancy test. 
“Not yet,” Jo said, her shoulders tense as she went to another shelf and ransacked the supplies. “This is the worst possible time to be having a baby! You're working all the time as the co-chef with Bailey, Meredith and I have our Mini-Liver final publishing paper due. My oral exams are coming up, Luna is starting preschool, and Eden is going to be moving up to the two-year-old room. We’ll have three kids in diapers. Oh my God, Alex the diapers. It’s like seven diapers per kid every day, that's like a thousand diapers a week.”
“Your math isn’t quite right on that one,” Alex said, shaking his head as he chuckled. 
Jo turned around to glare at him and he schooled his features and crossed his arms. “Okay, but Luna’s going to do great in preschool and Eden is doing amazing too. You're going to ace your exams and you love writing papers, plus you can get the interns to help you write it! And I can step down from the chief position for a while until things get easier, but let's try not to panic until you take the test.”
“How could this happen?” Jo was still going on with her rant as she moved to another section. “I'm an OBGYN, half of my job is to not get women pregnant and we've only had like one pregnancy scare before, but then again, it only took us one try to get pregnant with Eden. Oh my God, here it is!” 
Jo grabbed the pregnancy test and turned around, staring at it with wide eyes. Alex took in a breath as he felt his heart beat faster and sweat beads on his neck. He knew could feel the butterflies in his stomach. He glanced at Jo to see that she was just as tense and was rubbing the package with her fingers as if it was covered in a dust, he knew was not there.
“Jo take a breath, you’re spiraling,” Alex said, stepping up to her and putting his hands on her shoulders. 
“How are you not?!” Jo said as her wild eyes stared back at him.
“Because I've had time to sit with this and you haven't,” Alex said, taking a breath as he reached up to hold her face and rub her cheek with his thumb. “Whatever happens, we can't handle it. And we’re lucky enough to live in a state where you can get an abortion if you want. I know that we're married and you’ve been pregnant before, but you said you didn't want another baby and I respect that.”
“If I'm pregnant with another baby, I think I want to keep it,” Jo said, letting out a breath as she stared at him. 
Yet, she closed her eyes as a few tears appeared in the corner of her eyes. She leaned in to kiss him before she pulled away. Then her phone rang and she pulled it out. 
“It's another patient in the ER with abdominal pain,” Jo said, handing him the pregnancy test and walking away.”
“You know I can't take this for you, right?” Alex asked, holding up the test, although she had already left the room. 
Alex looked down at the test and smiled again, then he pulled up the onesie on his phone and added it to the cart. He wouldn't buy it just yet, but the second it was positive, he would buy it. Alex had always wanted a house full of kids and he hoped that dream would come true.

It was another hour before Jo called him, saying that she was in the fourth floor bathroom. By then Alex said caught up on all his paperwork and done some more online shopping. Although he would never admit to it, he had started a registry and had saved some baby clothes he liked. 
He also thought about how they would turn the playroom/second bedroom into another nursery. They had always planned on only having two kids, Luna and Eden. But they had a three bedroom house, and the two girls shared a room for now. They hated to be separated and would refuse to sleep without each other. The plan was for them to eventually have their own bedrooms when they wanted. 
He had also done the calculations on how many diapers it would be. When the girls were newborns, they would go through diapers like crazy, so it would probably mean about an extra pack a week. They had done cloth diapering with Eden and Luna and they could do that with the new baby as well, which would cut down on the cost. Also, he was pretty sure he could get Eden potty trained by the time the new baby got here. By then she would be almost three and she was already eagerly sitting on the potty with a few successes here and there. 
He thought about all of this as he approached the fourth floor bathroom. However, he was brought back to reality when he was faced with the door to the woman's bathroom. He knocked on the door but didn't get a reply right away.
“Jo it's me,” Alex called out to her, but he didn't hear anything and knocked again, wiggling the lock. 
“I'll be out in a second,” Jo said as he heard the sound of the toilet flushing.
A few minutes later, Jo came out of the bathroom and handed him the test. “I took it 5 minutes ago, but I can't look at it.”
He knew she needed a moment of comfort and brought her in for a hug. He held her close as she melted into him and kissed her forehead. However, when he breathed in, Jo's hair smelled. He sniffed her hair and pulled back looking at her. 
“Have you thrown up?” Alex asked her, studying her for a glimpse of sickness in her face.
“It's been very stressful!” Jo said as she huffed and wiped her mouth.
“Or maybe it's just morning sickness,” Alex said with a half-hearted smile. 
“Just look at the test!” Jo demanded as she huffed and pulled away from him and she leaned against the wall.
Alex smiled as he took a breath and flipped over the stick. Staring at it he almost cried, his voice a shaky whisper. “It's negative.”
Jo let out a breath and buried her face in his shoulder. She hugged his waist with a tight grip and he felt his shirt get wet. Alex was surprised, as she didn't react the way he thought she would.
“Are you relieved?” 
“I don't know.” 
Jo sighed as she pulled back a bit but still held him looking up at him with watery eyes. “I thought I would be, I kind of am, and it's just been so overwhelming I just want to cry. But a part of me wanted another baby.”
“I know, and I did the math. It would be like 120 diapers a week give or take a blowout or two. Plus I think I could get Eden potty trained before the baby arrived.”
“There's no baby, but it'd still be good to potty train her though,” Jo said in agreement. 
“Jo, I love being a dad and I would love to do it all over again with you anytime you want, besides you know I love getting you pregnant,” Alex said, grabbing her hips and pulling her into him as he kissed her.
Jo giggled, but she pulled away from him. “But being a parent is so hard especially when they're babies.”
“I know, so if you say you didn't want a baby, then I don't want one either. You know I would have a million babies with you, but realistically I love you and I love our two girls and the three of you are enough for me.”
Jo smiled as she leaned in to kiss him again, before he gave the test to her as Jo's eyes widened. “Alex! It’s not negative!”
“What do you mean it's not negative? There's nothing there!” Alex said, staring at the test. 
“It's not positive either, Alex it's a dud!” Jo said, shoving his chest. “One line is negative, two 2 lines is positive.”
“What does that mean?” Alex asked with wide eyes. “Should we just get another test, maybe we should do a blood test? They're more accurate anyway?”
“We are not doing anything. I am the one getting stabbed and having to pee on sticks and…” Jo put a hand over her mouth before she ran for the toilet again and this time he followed her in. 
Alex locked the door behind them as she kneeled in front of the toilet and threw up. She heaved several times more as Alex kneeled next to her. Once she stopped he wiped her lips and they sat back together. Then, Jo had a burst of emotion and started to cry as Alex instantly wrapped his arms around her. 
“I want another baby.”
Alex nodded and just held her. “I know, so we’ll call Carina and then she can tell us for sure if you're pregnant and if not we can start trying again.”
Jo nodded her tears drying up as she did and he leaned in to give her a quick kiss. The gentle reassurance was enough for him as he kissed her back. He pulled her up off the floor and wrapped an arm around her, Jo gave him another quick kiss before she sighed and let him pull her over to Carina's office.

“Ow,” Jo winced and looked away as Carina put the needle in her arm. 
“I'm sorry, Bambina,” Carina said as she released the tourniquet and drew her blood into two vials. 
“I told you not to look,” Alex said as he held her hand pulling it up to kiss her knuckles.
“No, nobody else can know about this,” Jo glared at him as she flinched again when Carina withdrew and put a cotton swab over her wound.
“I still think we should do an exam,” Carina said, giving her a knowing look as Jo huffed.
“Fine,” Jo said as Carina just smiled and handed her a gown and a paper blanket before she went to the door.
“I'll be back in a few minutes with the results,” Carina said as she left with the blood.
Jo hopped off the table and handed the gown and paper blanket to Alex before she started to undress. 
“We should have just done a blood draw the first time around,” Jo complained as she slipped out of her jeans and tore off her shirt quickly. Then she turned around and reached for her bra clasp. “Can you get it?” 
Alex easily hooked the latch and Jo handed her clothes to him in exchange for the gown but found his eyes were not exactly following hers.
“Alex!” Jo said, turning around and forcefully taking the gown from him.
“I don't know Jo, your boobs are huge,” Alex said, a little smirk as he stared at them. 
Jo huffed and pushed him away forcefully, turning him around as she got dressed again. She finally hopped up on the table and put the paper blanket over her knees before she cleared her throat. When he turned around again, he still had that stupid smirk on his face but he sat down in the chair next to her and grabbed her hands. 
“Isn't it normal for pregnancy tests to malfunction when there's too much HCG levels?” Alex asks as Jo thought about it for a second. 
“I don't remember, I think so,” Jo said as she shrugged. “Why do you ask?”
“Well, what if you're pregnant with twins, Jo?” Alex whispered to her his smile even wider. 
“I would probably kill you, Alexander,” Jo said as she scowled, Alex chuckled, but his smile dropped when she continued to glare at him.
“Well we can talk about it when we get the results back then,” Alex nodded as she dropped her eyes and picked at the seam of her gown before he took her hands again and gave them a squeeze. 
There was a knock at the door as Carina came back in. “There is a back up at the lab due to a trauma in the ER. The results will take several hours. So we're going to find out the old fashioned way.”
Jo took a deep breath as she laid back, putting her feet in the strips as Carina sat in front of her and put a hand on her leg.
“Okay Jo you're going to feel my fingers enter,” Carina said as she gently pressed inside of her. 
Jo squeezed Alex’s hands and tried to relax her muscles but she remained tense. Carina reached up to feel the top of her uterus and pushed in a few places, holding her womb before she smiled.
“How long ago did you say your period was?” Carina asked as Jo floundered, knowing what Carina was feeling inside her. 
“I don't know?” 
“Six weeks ago,” Alex answered for her. 
“I'd say you're more like eight weeks along, so you must’ve had some implantation bleeding. You've definitely got a baby in there, Jo,” Carina said with a small smile. 
Jo took in a breath as she looked over at Alex. He had a smile but he quickly bit his lip as he looked over at her giving her hand a squeeze. He held his breath, but then Jo smiled too.
“We're having a baby!”
“Yeah, we are yeah we are!” Alex said as he smiled and leaned down to kiss her. She cried happy tears that fell down her cheeks to her ears, but she smiled. 
“Well I suppose, we should confirm it with an ultrasound?” Carina asked as she pulled back, smiling at them both.
“Yes,” they said together as they both nodded. A few minutes later they were looking at their new baby on the screen. There was just one, Jo made Carina double check, but they were ecstatic nonetheless. Jo hasn't stopped crying and even Alex shed a few tears as they stared at their baby.
“They're so beautiful,” Alex said in awe, his eyes never leaving the screen. 
“They're already measuring big too,” Jo said, finger tracing her fingers along the femur bone. “I think our original estimate of six weeks is correct.”
Carina nodded as she agreed with them. “Well keep an eye on it, but as you said, Karev babies tend to be on the bigger size, so I'm not worried.”
She finished up the ultrasound and gave Jo some paper towels to clean off, then left her to get dressed again. But not before pulling Jo into a hug. “Congratulations, Bambina.”
“Thank you,” Jo said, hugging her back tightly.
Alex smiled as she got dressed but when she pulled her shirt on, he pulled it back up and leaned down to kiss her belly, before looking back up at her.
“Hi baby,” Alex said as he stared at her belly. 
“What are we going to do with you?” Jo asked as she pulled him up and put her hand over his, intertwining their fingers. 
But Alex just smiled and said, “We are going to have an amazing family, you are going to be a great Mom, again.”
Jo smiled as she leaned in and pulled him down to kiss him again. Alex gave her several kisses as they lingered on each other's lips. There was such joy and happiness between them it felt like fireworks on her tongue. 
Then they pulled back and looked down at their baby, hidden under their hands. Alex hugged her shoulders and kissed her head as they smiled sharing a giggle between them. Despite how nervous she had been, Jo couldn't wait for this new baby to join their family.

AN: I will forever be angry at Shonda Rhimes for breaking up such a beautiful and loving couple in such an awful way that destroyed over 10 years of Alex’s character development. But I can correct that mistake even if it’s head canon here on Tumblr. I want to show our appreciation for the dads out there that put our wellbeing above their own and guide us through the roughest times.
Summary: Alex and Jo get a diagnosis from Jackson Avery who came at their request with April and is the only one outside the family who knows about Luna’s prognosis and supports them through this.

Alex paces the attendings lounge back and forth anxiously waiting for his special visitor to walk through that door.
The door opens and Alex is relieved to see the person he was expecting. Jackson Avery is in a business suit, looking more like a board member than a surgeon. He grins at the sight of his good friend outside in his casual clothes and steps out to greet him.
“Hey man.” Jackson keeps his tone neutral knowing his friend needs him more as his daughter’s doctor than as a fun friend. They shake hands, “It’s good to see you, I wish it was under better circumstances.”
“You and me both.” Alex shares tensely, “Jo and Luna are already in an exam room.”
“Good I called ahead, and I got a nurse to set up an audiometer and don’t worry outside of my mom and Richard nobody knows I’m here and as far as they are concerned, I’m here for medical pitches.” Jackson says to Alex’s relief, “I figured this private visit should stay private until we get a diagnosis.”
“Thank you for that. I wouldn’t normally ask that of you but Jo is…” Alex sighs rubbing his tired eyes, “She’s going to worst case scenario after worst case scenario and I’m trying to be supportive. And as someone who’s been at my field for 10 years, I know too well how much a diagnosis can ruin a child and their family.”
“Okay I’m gonna be a doctor right now, not your friend and tell you to relax so that we take this one step at a time.” Alex exhales to relax, “First step is to have Luna take a BAER test and if hearing loss is indicated then we run more tests to confirm if it’s progressive hearing loss or an underlying condition that’s causing hearing loss.”
“Yeah, I know all of that it’s just…it’s hard being on the other side of this.”
“Believe me I understand that more than you think.” Jackson sympathizes before motioning to the door, “Shall we?”
Later
Jo bounces Luna against her hip as she walks around the tiny exam room trying to keep her mind from exploding. So far she is failing as her worry for her child grows with every second. It’s not Luna is going deaf it’s what could have caused it. Jo knows that it could be a malignant acoustic neuroma and the thought of her baby having cancer fills her with dread. Luna is the one person she loves more than Alex and she can’t bear to see her in pain.
The door opens and she pauses to see Jackson entering the room with Alex who goes to them with a smile at Luna. He strokes her fine blonde hair causing Jo to grin slightly before turning to Jackson who is clad in a business suit.
“Hi Jo.”
“Hi.” Jo gives a slight grin to Jackson grateful to have him here as Luna’s doctor, “I’m sorry if were pulling you away from foundation stuff but we wanted someone who’s a skilled ENT and make sure all of the bases are covered.”
Jackson shakes his head, “You don’t have to apologize, I’m more than happy to do this for Luna. But I have to warn you I’m gonna use my doctor voice instead of my good friend voice. So, Alex says there have been signs of hearing loss at daycare, have either of you noticed any at home recently?”
Alex steps in first, “I call out her name but she doesn’t respond. I have to wave my hand to get her attention. And she hasn’t said her first words yet.”
“Could this hearing loss be caused by an underlying condition like encephalitis or CMV?” Jo asks worriedly.
“Well, she hasn’t been vomiting or had a fever, has she?”
“No.” Alex confirms.
“Then it’s neither but we’ll run labs just to be sure. Until then we have to first confirm that she has hearing loss.” Jo inhales at this as Jackson continues, “We’ll do a BAER test that’s the-”
“Brainstem Auditory Evoked Response we know.” Alex informs him worried as well.
Jackson nods seeing that it’s best to do this quick and get the results fast, “Okay you two can be in the room to hold Luna while I do the test. I’ll get the equipment out now.”
Jo swallows before turning to Alex who rubs her back in comfort as they wait for Jackson to set up so they can get a diagnosis that could change their lives forever.
An Hour Later
Jackson looks at the BAER results on his tablet in agony as the results confirm that Luna Karev does indeed have progressive hearing loss. He paces the Imaging Room trying to figure out a way to give this devastating news to Alex and Jo and he can’t come up with one idea that ends good. But then again there’s no way to spin your only child going deaf. The door opens stopping his thoughts as he sees his wife, April Kepner, come in.
“Hey.” April greets him with a grin before closing the door, “Your mom took Harriet and Mark to Marios after her meeting. I swear those kids have a better wardrobe than me.”
Jackson grins at hearing how their daughter and baby son are spending their day with their grandma. He would take that over holding his good friend’s baby’s diagnosis in his hands right now. He sits in a chair while April looks at him inquisitively.
“Are you okay?” April stands in front of him with kind eyes that always make him feel safe to share his turmoil, “What’s wrong?”
Jackson sighs before handing her the tablet, “I just got Luna Karev’s BAER results.”
April’s face falls as she reads the chart, “Oh no, do they know?”
“Not yet I’m just…” Jackson pinches the bridge of his nose, “I’m trying to figure out how to tell them this piece of news. I swear this is so much easier when I don’t know the parents.”
April looks at Jackson in pity squeezing his shoulder, “I know.”
Jackson shakes his head at this, “I knew what I came here for but a part of me, a huge part of me was hoping that it was nothing that there was nothing to tell. But there is and now I have to give them the worst news of their lives.”
“Kids like Luna can be deaf and live their best lives.” April points out, “I mean that CODA movie we saw won three freaking Oscars and Tony Kotsur is actually deaf and he gave his winning speech in ASL. If it wasn’t for Will Smith slapping Chris that would have made the awards memorable.”
Jackson chuckles lightly at that couple’s night of theirs remembering how shocked they were and how funny they found that memeable moment.
“Thanks, I needed a laugh.”
April strokes Jackson’s cheek, “There’s not gonna be a nice way to tell them, you just tell them the way you normally would with a patients family and guide them to resources so they can learn how to live with this along with their child. It’s horrible but knowing Alex and Jo they will be fine as long as they know their daughter is alive and healthy.”
Jackson looks at his wife in awe before nodding, “Your right.”
“I usually am.” April smugly grins causing her husband to chuckle and kiss her before leaving the room. He walks down the hall as slowly as he can while he recites his diagnosis in his head, so he doesn’t fumble and mess up. He stands outside the exam room for a good five seconds before going in seeing Alex and Jo with Luna sitting on the exam table playing with her stuffed unicorn.
He closes the door behind him before standing in front of them with a stoic face as the couple look at him anxious for the diagnosis, “The BAER test confirms that Luna has progressive hearing loss, I’m sorry guys.”
Alex and Jo look at each other in worry before Jo turns to Jackson with heartbroken eyes, “…What’s next?”
“We need to do an OAE to get a more accurate auditory read. She’ll be asleep for that and you two will be with her while I run the test it’s noninvasive and she won’t feel any pain.”
Alex clears his throat to get the lump in his throat out as he processes this news, “What about an MRI? To rule out underlying conditions and tumors?”
“And a genetic test. She’s adopted we don’t know what’s in her blood, it could be a genetic cause.” Jo adds causing Alex to nod in agreement.
“I will do both after the OAE. Now we can do the test today but the scan and bloodwork is gonna have to wait until tomorrow.” Jackson tells the Karev’s, “I will get someone to put a rush on the results but the latest we can get the full results back is tomorrow night. I’m sorry but this is the best I can do right now.”
Jo sniffles with her tears glistening her eyes, “Could this be…could it be something fatal?”
Jackson looks at the couple in pity before answering, “We don’t know, we won’t know anything until we go over all of our tests.”
Alex nods understanding, “You’ll cover all the bases, right?”
“I always do. Now I’m gonna give Luna a light sedation before we get started. You two can hold her while I give her the oral administration. We should start now so we can get the results much faster, are you ready Luna?” Jo and Alex nod before holding Luna still as Jackson fills a mouth syringe with the medicine for their baby.
Later That Night
Alex looks over the brochure he got from the auditory floor while walking out of the hospital with Jo who is pushing Luna forward in her stroller. The test Avery ran for Luna revealed a lack of OAE in both of her ears that further confirms isolated hearing loss.
Now they are looking to go home and try to sleep before Luna has her MRI and genetic tests tomorrow that will tell them if this is a result of an underlying condition or not. Alex can see the worry forming in Jo’s eyes and tries to comfort her once again.
“Avery is the best ENT we could ask for; he goes to the bat for all of his patients not just the ones related to his friends.” Jo nods but still has a frown, “Most of these cases it’s just progressive hearing loss with no genetic conditions or tumors. And technology has come a long way, hearing aids are state of the art or we can use a cochlear implant.”
Jo sighs, “Can you…can you just not?”
Alex can see he’s upsetting Jo and wants to mend his mistake, “I’m just trying to let you know that this isn’t as scary as it seems.”
“Yes it is Alex.” Jo stops to face him with a frustrated face and tears in her eyes, “It’s extremely scary!”
Alex is taken back by this and looks at Luna who is sleeping in her stroller absent of the yelling. He stands back up and inhales and exhales to keep his mind at ease over this recent diagnosis. He thought that Jo needed reassurance, but he can see now that all she wants is someone who understands her pain, and he is the only other person who can. So he decides to make it known instead of shoving it down for his family’s sake.
“Your right.” Jo sniffles at Alex’s confirmation wiping a tear away, “It is scary, this scares me as much as it scares you. I’m scared of what this diagnosis could be a result of and even if it’s just progressive hearing loss I’m scared of what will happen after. I’m scared of Luna becoming an outcast and not having the advantages we had growing up. I’m scared of screwing up sign language and being one of those parents that doesn’t communicate with their child. I’m…I’m scared that I won’t be able to help Luna through this transition in her life.”
Jo has another tear down her cheek and she moves to hug Alex who hugs her back so they can comfort each other. For a moment they stand there in the hospital entryway holding each other with their baby sleeping inside her stroller. They share the pain that is coming from this scary diagnosis from their child who they love more than anything. Jo pulls back and wipes her wet cheeks as Alex grips her upper arms to keep her from breaking down completely.
“But just because we’re scared of that happening doesn’t mean it’s going to happen.” Alex states positively knowing it’s what Jo needs to hear as well, “Is it gonna be hard? Yes. Are we gonna know everything we need to do from now on? No. Even if it is just progressive hearing loss it’s gonna be hard for all of us. I know that but I also know that we can get through this and come out stronger than ever. We did it when you got out of treatment and we did it with Luna after she was born. If we can do that then we can do this, I know we can you have to know that too.”
Jo sniffles looking at Alex with awe and love before kissing his cheek, “I do know that.”
Alex nods before putting his arm around his wife, “Let’s go home.” Jo pushes Luna forward in her stroller with Alex by her side as they leave to go home as a family.

After all my rants about Alex’s horrible departure I just want to make clear that what I’m most angry about is the complete destruction of Alex’s character. I already see this becoming a rant. Hopefully my last on this subject.
My last couple too long posts ranting about this focus a lot on Jo and Izzie. But at the end of the day it’s not that Alex and Jo aren’t together or that Alex and Izzie are. I like Jo and Alex together, but I could take it or leave it. They’ve had seasons where I was over them. I did always hate Izzie and Alex together I’ll admit that.
But the real issue isn’t about any ship it’s about Alex. And the focus goes to Jo because that’s the relationship that shows most how all his character development was destroyed. Alex is a good man and he loves Jo and he knows her past. Jo was abandoned when she was a week old and that gave her some trauma. Then add in Alex’s own trauma of being abandoned by his deadbeat alcoholic father. Being walked out on and abandoned by Izzie during their garbage marriage. And we you consider these things and the fact that Alex is a good person who cares about people and is honorable and has worked for years to not be a loser like his father, the idea that not only he would abandon Jo but that he would do it without saying a word. He would do it by lying and not calling. That he would leave Jo like Izzie left him, knowing how painful that is. That he would be too cowardly to come home and tell Jo to her face that he was leaving her. Goes against everything Alex has become over the last 16 seasons. Not limited to his relationship with Jo but with anyone.
I feel the same way about how he left Meredith. It was less an outright betrayal but it was still awful. Walking out on your best friend in the world without even saying goodbye? Alex was her person and she was his. He took that very seriously, they were family. To just drop her like he did is absurd. Alex would kill for Mer, do the writers expect me to believe he’d kill for he’d die for her but he can’t be bothered to say a proper goodbye. Let’s pretend that nonsense they wrote about him not saying goodbye bc he knew Mer could talk him out of moving to be with his kids, like she would ever do that. Alex left without saying goodbye to the kids. How can any of us ever believe Alex would leave without saying goodbye to Zola, Bailey, and Ellis. He loved those kids like they were his own. He took care of them like they were his own. Alex who loves kids and understands them more than anyone in the show is just going to abandon his first family without a word? I don’t think so. Obviously, he has to go and raise his kids but I don’t believe for a second Alex wouldn’t Sit down with Mer’s kids first and tell them how much he loves them and always will and how much he’s going to miss them and how they have cousins to visit.
So anyway long rant I didn’t plan over. I will never be okay with Alex’s exit.
