He's named for his unique eyes, though he's not actually blind. He just has weird eyes. He also went prematurely grey from being around all the chemicals and stuff in Callax.

He died from a pretty preventable accident, having ended up in machinery that normally Fulgrim would have worked on. Fulgrim had never really complained since he just tended to regrow the lost limbs he got from it. However, Opal had been working on it instead, and had ended up bleeding out. Fulgrim has yet to forgive himself, even now as a daemon prince of Slaanesh for Opal's death.
