Taken For A Ride: Doctor Injured In ATV Crash Gets $56,603 Bill For Air Ambulance Trip

It was the first — and only — time Dr. Naveed Khan, a 35-year-old radiologist, ever rode in an all-terrain vehicle.
Khan took the wheel from his friend and drove circles in the sand, on a trail along the Red River in Texas.
“As soon as I turned to the side where my body weight was, this two-seater vehicle … just tilted toward the side and toppled,” Khan recalled. It landed on his left arm.
“I had about a 6-inch-wide exposed flesh gap that I could see below, on my forearm,” he said. “And I could see muscle. I could see the fat. I could see the skin. The blood was pooling around it.”
Khan, feeling lightheaded, tied his jacket around his arm like a tourniquet. He and his friend managed to right the ATV, drive back toward the street and call 911.
When an ambulance delivered him to the emergency room at United Regional Health Care System in Wichita Falls, Khan was surprised to hear a doctor murmur that it was the worst arm injury he’d ever seen.
Khan needed immediate helicopter transport to a trauma center for surgery in Fort Worth, if there was any hope of saving the arm.
Groggy from painkillers, Khan managed to ask the doctors how much the flight would cost and whether it would be covered by his insurer. “I think they told my friend, ‘He needs to stop asking questions. He needs to get on that helicopter. He doesn’t realize how serious this injury is,’ ” Khan recalled.
Flown 108 miles to John Peter Smith Hospital in Fort Worth, the closest Level I trauma center, Khan was whisked into surgery to clean out the wound, repair his shattered bones and get blood flowing to the tissue.
He had a total of eight operations to try to save his left forearm before he finally gave up. After weeks in the hospital, he asked the doctors to amputate, so he could get on with his life.
And then the bill came.

Total bill: $56,603 for an air ambulance flight. Blue Cross of Texas, Khan’s insurer, paid $11,972, after initially refusing altogether; the medevac company billed Khan for the remaining $44,631.
Service provider: Air Evac Lifeteam, an air ambulance company that operates 130 bases in 15 states. It’s owned by Air Medical Group Holdings, a holding company that owns four other air ambulance companies and one ground ambulance company. Air Medical, in turn, is owned by the giant private equity firm Kohlberg Kravis Roberts, or KKR.
Medical service: Khan was flown from the United Regional Health Care System in Wichita Falls, Texas, to the John Peter Smith Hospital in Fort Worth.
What gives: Khan got his first call from Air Evac Lifeteam just three days after the accident, while he was still lying in the hospital. A company representative told him the helicopter ride would likely cost more than $50,000 and asked him how he planned to pay.

For Khan, rapid transportation to the trauma center was essential since the blood supply to his arm had been cut off, said Dr. Rajesh Gandhi, the medical director for trauma services at JPS Hospital.
“If there’s no blood going that means there’s no oxygen,” he said. “It there’s no oxygen, that means those cells are going to die.” Minutes are precious and the helicopter can get from Wichita Falls to Fort Worth in an hour or less, half the time it takes by ground ambulance, he said.
But complaints about sky-high bills to patients for air ambulance services are common. Since launching the “Bill of the Month” series in February, NPR and Kaiser Health News have received more than a dozen bills from patients like Khan who were charged tens of thousands of dollars for an air ambulance ride even after insurers’ payments.

Rick Sherlock, president of the Association of Air Medical Services, a trade group, said air ambulances require a more highly trained crew than a ground ambulance, because only the sickest or most seriously injured patients need air transport.
AAMS commissioned a study to determine the actual cost of a medevac ride. The report found it takes about $2.9 million a year to run a single helicopter base. Each base handles about 300 transports a year, and the rides cost about $11,000 each, according to the report.
A spokeswoman for Air Evac Lifeteam said the company bills people so much because it is trying to make up for what she said are meager payments from Medicare and Medicaid.
“Our real cost per flight is the $10,200 plus the unreimbursed cost on each flight for Medicare, Medicaid and patients without any coverage,” wrote Shelly Schneider, the company spokeswoman.
The Centers for Medicare & Medicaid Services said it pays an average of $4,624 per ride, plus $31.67 a mile, which works out to an average Medicare reimbursement of $6,556 for helicopter ambulance rides for seniors. Medicaid in most states pays less.

The industry has been advocating hard to get Medicare to boost its reimbursements, Sherlock said. There are bills pending in both the House and Senate that would do so, but there hasn’t been much movement on them.
But others say the industry’s cost estimates are inflated by profit-driven expansion of a lucrative industry. Ground ambulances often carry critically ill patients, too.
Too many air ambulances sit idle much of the time, said Dr. Ira Blumen, a professor of emergency medicine at the University of Chicago and medical director of the university’s Aeromedical Network.
Blumen said the industry — which is dominated by a few companies owned by private equity firms — expanded dramatically the last time Medicare boosted its payments in 2002. And now there are too many helicopters — 908 as of last year — fighting for patients and profits at the same time.
“The number of helicopters is outrageous for the continental United States,” he said. In the 1990s, most helicopters ran more than 500 flights per year on average. At that rate, the cost per flight today would be less than $6,000.
A Blue Cross of Texas spokesman said the insurer does have a contracted rate with an in-network air ambulance company, but it is not Air Evac Lifeteam. After initially refusing to pay anything for an out-of-network claim, it agreed to the $11,972 payment.
But in some sense, the reason ambulance companies charge so much is simply because they can: Air ambulances are largely regulated not as health care but as part of the aviation industry. Federal laws prevent states from limiting aviation rates, routes and services.

So many people have been hit with shockingly high air ambulance bills that members of Congress on both sides of the aisle are trying to do something about it. Legislation to reauthorize funding for the Federal Aviation Administration that is moving through Congress now would set up a council of industry experts to address balance billing and other issues, and set up a complaint line for consumers.
Resolution: Khan has allowed Air Evac Lifeteam to negotiate with Blue Cross of Texas over the remaining $44,000 air ambulance bill. The company has asked him to appeal to the state’s Department of Insurance, and though he first balked at the suggestion, he’s now considering doing so. Khan says he doesn’t understand why the helicopter flight, which was an integral part of the emergency medical care he received, is treated differently than his surgeries, nursing care and physical therapy.
“I thought that this was another piece of that puzzle,” he said. “It turns out that this was glaringly different.”
He is waiting for resolution as he gets accustomed to life with his disability. Holding his baby son, he asked in frustration: “How do I hold him while he’s crying and at the same time heat up his bottle?”
Khan, who has had to fight with his insurance company to get coverage for a prosthetic arm, is frustrated when he learns that the air ambulance company expects him to pay far more than the actual cost of his flight.
“It’s unfair,” he says. “It’s random, it’s arbitrary. It’s whatever price they want to set. And to put that onto a person who’s already been through what I’ve been through, I hate to say it, but it’s cruel.”
The takeaway: Most people with health problems serious enough to require a helicopter flight are in no position to ask if the medevac company is in-network or if there’s a choice. But if you or a family member has time to ask, it could pay off.

First, let your insurer’s process play out. Blue Cross Blue Shield of Texas first denied Khan’s claim altogether. But he looked closely at his policy and saw that the threat of loss of limb was explicitly covered. He appealed, and that’s when the insurer paid $11,972.
Second, negotiate! The air ambulance company might be willing to negotiate a settlement for a fraction of the bill to avoid turning to debt collectors, who would pay them pennies on the dollar.
Both Sherlock of the Association of Air Medical Services and Schneider of Air Evac Lifeteam said companies will try to determine what a patient can afford. So people with high incomes may find it hard to obtain a substantial reduction for their bill. Still, if patients know the true cost of the service they received, they may be better equipped to negotiate a discount.
Many air ambulance companies offer membership plans that can cost less than $100 a year, and guarantee that the company will accept whatever payment an insurance company makes without billing the patient for the rest. But buyer, beware: When someone needs an air ambulance, they are often not in a position to choose which company will respond to the call.

As we all know, Sonam Kapoor comes from a family with a rich heritage in the film industry. She is the daughter of Bollywood actor Anil Kapoor and Sunita Kapoor and is the niece to producer Boney Kapoor and actor Sanjay Kapoor. So, it is no surprise that Bollywood runs in her blood. Interestingly, Sonam was more interested in writing and direction than in acting earlier. It was during the filming of Black when Sonam Kapoor began to develop interest in acting. During the shoot of Black, Sanjay Leela Bhansali told Sonam that he wanted to cast her as the lead in his next film. Motivated by Bhansali’s words Sonam Kapoor ended up loosing 35 Kg’s within a span of two years.
While we all are admirers of Sonam Kapoor’s hot bod now, many might not know she had severe issues with her weight when she was younger. Sonam was diagnosed with insulin resistance and polycystic ovarian disease. This provoked her to spread awareness for diabetes, for which she shot a special commercial and also promoted the launch of Kalli Purie’s Confessions of a Serial Dieter. During the launch of the book, Sonam spoke about her condition and revealed that it was Sanjay Leela Bhansali who gave her the wake up call which motivated her to get in shape. She said, “I was 63 kilos at the age of 12, then I lost my weight because at 13 I started noticing boys and boys started noticing me and then when I was 15 and I had to start studying I put on a lot of weight again and I was 86 kilos till I was 19. Sanjay Leela Bhansali saw my face and said you have a beautiful face but your body is like an Ajanta Elora painting, which was happening like 100s and 100s of years ago, it’s not happening anymore! You have to wear a backless choli and you can’t have fat rolls sticking out from behind! That was a reality check.”
Sonam Kapoor later went on to study acting under Roshan Taneja, Jayati Bhatia and Feroz Abbas Khan. During an interview with a leading daily, she also revealed that her admiration of Nutan and Waheeda Rehman further inspired her for being an actor. She said, “I simply adore Waheeda Rehman. From her acting to her choice of films — it needed guts to choose movies like Pyaasa or Guide — she went ahead with it. I’m also very inspired by Nutan. I love her for her work in Bandini and Saraswatichandra. Both have looked good and done path-breaking films. I love their quality of doing different things. They stood out as actors who are not just beautiful faces.”
While Saawariya did not manage to impress the audience and the critics, Sonam’s work in the film received praise. Although, her character in the film wasn’t very likeable but she definitely impressed everyone with her charming screen presence. Saawariya also earned her a nomination for the Filmfare Award for Best Female Debut (Female).
In 2009, she was seen in Rakeysh Omprakash Mehra’s Delhi 6 which also starred Abhishek Bachchan Om Puri and Waheeda Rahman. Even though, this film failed to perform at the box-office again, Sonam’s character of a bubbly and full of life Delhi girl certainly grabbed many eyeballs.
Sonam’s first commercial success came in 2010, with Punit Malhotra’s I Hate Luv Storys, which also starred Imran Khan. I Hate Luv Storys was a film about how two completely opposite people end up together. Sonam played the character of Simran, a girl who believes that her life is just like a typical Bollywood romantic film. In fact, her life does (sort of) resemble that theory as well with her perfect job and a “Mr.Perfect” finance. On the other hand, we had Jay (Imran Khan), who didn’t want anything to do with love stories. His hate for Sonam’s obsession with romance causes them to fight a lot earlier but they eventually become good friends. Simran’s close bond with Jay brings problems in her love life to the point that she believes Jay not Raj is the right man for her. However, when she finds out that he doesn’t feel the same, she leaves town and does not inform him out of embarrassment. During Simran’s absence Jay realises that he too has fallen in love with her. In the end, Jay confesses his love to her and she reveals that she still feels the same. I Hate Luv Storys was Sonam’s first commercial success and the major reason for that was it was able to struck the right chord with its target audience. Although the story didn’t have anything extra ordinary, Sonam and Imran’s chemistry in the film made it a fun a watch. The lifetime collections of the film were âÂù72.5 crore.
In the same year, Sonam played the eponymous role in Aisha. Aisha was a romantic comedy based on Jane Austen’s novel Emma and was also produced by her younger sister Rhea Kapoor. Along with Sonam, Aisha also starred Abhay Deol, Ira Dubey and Amrita Puri. Even though the film wasn’t able to perform at the box office, Sonam’s performance received rave reviews from critics all over. Many believed that Sonam’s role in Aisha was tailor made for her personality which is why she was such a good fit.
After this Sonam Kapoor had a string of films which failed to live up on people expectations. Films like Thank You, Mausam and Players failed to impress the critics as well as the audience.
A turning point in her career came when she seen in Aanand L Rai’s Raanjhanaa in 2013. In Raanjhanaa, Sonam played the character of Zoya Haider, a student who later gets drawn into politics after the death of her Sikh lover. Since Sonam was playing the role of a student, she interacted with students, attended several workshops and also practised with the theatre groups from Jawaharlal Nehru University. She also revealed during an interview with a leading daily, that she drew inspirations from Jaya Bachchan’s character in the 1971 film Guddi for her role. She said, “The spunkiness that Jayaji’s character had in Guddi… We needed that. I needed an inspiration as in real life my schooling was conservative and different. So I needed a starting point… I did watch that film again. It was quite amazing to play this role. Jayaji’s performance inspired me to do better. The difference between investing and speculation (or: The problem with Bitcoin)
Hold onto your hats, folks. It’s rant time!
Based on what I’m hearing on Facebook, Twitter, and in real life, it’s time for a refresher course on the difference between investing and speculation. Although these two concepts share some commonalities, they’re very different things.
Let me start by telling a story, one I’ve told many times before. It’s the story of the worst “investor” I’ve ever known: me.
The Worst Investor I’ve Ever Known
Before my financial turnaround, I didn’t really understand what the stock market was for. I viewed it as a sort of casino, I guess. I believed investors gambled on individual stocks and hoped that they’d outperform the rest of the market.
So, that’s what I did. I treated the stock market as if it were a casino. I’d pick a stock, put all my money into it, and cross my fingers. I took risky gambles hoping to strike it rich.
Unsurprisingly, I lost a ton of money.
During the late 1990s, some friends and I formed an investment club. Each month, we contributed money and picked where to put it. We chose stupid, stupid stocks — whatever was riding high at the moment. When the tech bubble burst, so did our bankroll and our enthusiasm.
In 2000, enamored by PalmPilot, I bought stock in the company that made the devices. I paid close to $90 per share. Just over a year later, the stock had lost 90% of its value. Oops.
One of my friends worked for The Sharper Image. In 2007, the company was struggling and the stock was in the toilet. At dinner one night, my friend told me how management was trying to turn things around. Sounded promising, so I put my $3500 Roth IRA contribution into the company’s stock. The company soon went bankrupt and my 2007 IRA contribution is now worth nothing.
During the banking crisis, I invested in Countrywide Financial. “Countrywide is on your side,” right? Wrong. Yet another stock that went to zero.
I wasn’t investing; I was speculating. I was gambling. I was trying to pick winning cards at the casino. But that’s how I thought the stock market worked.
The High Risk of Risk
After writing at Get Rich Slowly for a while, my viewpoint changed. As I became better educated, I realized that the stock market is not a casino. It’s a marketplace. It’s a tool that allows people to buy shares of businesses. (This is obvious, but trust me: Most people don’t understand this.)
When I buy a piece of one business, I’m taking on the risk associated with that business. We hear all the time that most small businesses don’t survive seven years, right? Well, even big businesses go under. Even big businesses lose money. There’s always risk associated with owning a business.
In the world of investing, “risk” is the probability that you’ll lose money. (There are many types of investment risks, by the way.) The notion of “return” is fundamentally tied to the concept of “risk”. The greater the risk — the greater the chance you’ll lose money — the higher your potential returns (gains) are.
One difference between investment and speculation is the amount of risk involved. When you put your money into something with minimal risk, you’re investing. When you put your money into something with high risk, you’re speculating. Like I said at the start, there are plenty of commonalities in the two actions — but the element of risk is a huge differentiating factor.
One way to mitigate risk is to own pieces of several businesses. Owning many businesses is even better. This practice is known as diversification. Diversification reduces risk. It allows you to enjoy the profits and benefits without getting screwed when one business goes under. This is investing. Putting all of your money into one stock and hoping that it increases in value is speculation.
A Random Walk
It’s also speculation when you hop in and out of the market, buying and selling stocks in the short term like day traders do. Day traders aren’t investing; they’re gambling on short-term price fluctuations.
In the short term, stock prices follow what has come to be known as a “random walk”. They bounce up and down with little rhyme or reason. Over the long term, however, the stock market as a whole tends to increase in value.
Why are stocks said to follow a “random walk”? Let’s look at an analogy. When I take Tally for a walk, we always follow the same route. We take a 1.85 mile loop through the neighborhood and it generally takes us 51 minutes. (Seriously, it almost always takes 51 minutes.)
However, we never travel at a constant pace. Because Tally is a hound dog, we move in fits and starts.
Is there water in the ditch? Time to splash and sniff!
Are there deer in the pasture? We have to stop and bark!
Is that a new gopher hole? Let’s dig!
Wait, is that a squirrel? Let go of the goddamn leash, you jerk!
Each day is different. It might take us 20 minutes to walk the first 1/2 mile today, then we’ll travel the same distance in 10 minutes tomorrow. But when it’s all said and done, it takes us right at 51 minutes to go those 1.85 miles. (Sometimes we do it in 49 minutes. Sometimes it takes 54. But we average 51 minutes. And 51 minutes is the most common time to complete the walk.)
Our daily strolls through the neighborhood follow a “random walk”.
Over the short term, the stock market also follows a random walk. Both individual stocks and entire indexes fluctuate — sometimes wildly. I wouldn’t care to place a bet on what the S&P will do today, or tomorrow, or even next year. But if past performance is any indication — and past performance is the best info we have to go on — then I can be pretty confident that, on the whole, the stock market will show a steady increase over the next decade or three.
Another difference between investment and speculation is your time horizon. Investors are in it for the long haul; speculator are hope to make money off of short-term market fluctuations.
The Problem with Bitcoin
All of this leads me back to the folks I know who are so eager to speculate right now. I have several friends who are riding the Bitcoin bandwagon, and they think they’re savvy investors. They’re not. What they’re doing is speculating, the same as I used to do. They’re taking on high risk in the hopes of huge gains in the short term. This isn’t investing; it’s speculation.
If you want to speculate in Bitcoin, go for it. But don’t delude yourself that what you’re doing is investing.
I don’t remember the exact date I first heard about Bitcoin, but I remember the place. I’d recently started doing Crossfit, and I was using a foam roller to work out a kink in my quads on the floor of the gym. My buddy Dan was next to me, and he asked me what I thought about Bitcoin. (Dan knew I wrote about personal finance, so he always had money questions for me.)
I told him I didn’t know anything about Bitcoin, so he gave me a primer. If you need an explanation or refresher, here’s a Khan Academy video that provides an overview:

Based on the location and the people present, this must have been in early January 2012, when Bitcoin had reached a then-record high of $7.
“Would you buy Bitcoin?” Dan asked as we put away our foam rollers and moved to the weight room.
“I don’t know,” I said. “I’d have to learn more about it. I have a policy of never putting money into something I don’t understand.”
After that conversation, I did some reading about cryptocurrency. I decided I liked the idea, but as a store of value, not as an investment. To me, that’s a very big difference. I like the idea of a decentralized currency that’s not controlled by any one government. That’s kind of cool. But in 2012, I didn’t like the idea of sinking money into a Bitcoin as an investment.
In the past 5+ years, the value of Bitcoin has skyrocketed. Recently, it’s been trading at $7000! In other words, the value of Bitcoin has multiplied one thousand times since my conversation with Dan on the floor of the gym.
If I had moved all of my invested capital from index funds to Bitcoin in early 2012, today I would be a billionaire. If I had invested even just $1000 then, that $1000 would now be worth $1,000,000.
Did I screw up? I don’t think so. And given the same choice again, I’d do nothing differently.
That’s because I believe strongly that nobody is actually “investing” in Bitcoin; they’re merely speculating. So far, that speculation has paid off. And maybe it will continue to pay off. But I believe it won’t go on forever.
Be Fearful When Others Are Greedy
Another one of Warren Buffett’s most famous quotes is appropriate to this situation: “Be fearful when others are greedy and greedy only when others are fearful.”
Right now, the folks pouring money into Bitcoin (and other cryptocurrencies) are greedy. For that reason, I’m fearful. This looks exactly like so many situations I’ve seen in the past, including my own past “investment” mistakes. While I’m fine with the underlying concept of cryptocurrency, there’s no way in hell I’d put my own money into it now. Not a chance. The risk is just too high.
What if you want to put money into Bitcoin? Fine. Go ahead. But don’t you dare think of it as an investment, because it’s not. It’s just like playing blackjack at the casino.
And if you do want to put money into cryptocurrency, please do me a favor: Only use money that you can afford to lose.
If you’re struggling to make ends meet, steer clear.
If you’re in debt, steer clear.
If you haven’t funded your retirement accounts for the year, steer clear.
If you don’t have your financial shit together and absolutely must make the leap, then limit yourself to at most sinking five percent of your “savings” into Bitcoin. Any more is gambling with your future.
But if you’re debt-free, have some savings, and have set aside money for retirement, then take the risk if you want to. In my mind, it’s no different than putting your money into the Palm IPO like I did. (And I think the results are likely to be the same.)
Right now, it feels to me as if we’re in a Bitcoin bubble. Maybe I’m wrong, but that’s what it feels like. It feels just the same as the housing market in 2007, as the stock market in early 2000. And here’s the thing: There’s no risk in not putting money into Bitcoin. If its value jumps to $70,000, then all I’ve lost is some opportunity cost. I’m fine with that.

Final note I’m going to state this explicitly because otherwise the comments are likely to get derailed. I am not anti-Bitcoin. I kind of like the concept of cryptocurrencies, but as currencies, not as investment vehicles. What I’m worried about here is Bitcoin speculation.
This is the same concern I feel when I read about anyone who tries to get rich quickly. In 2006, for example, I was writing about folks like Casey Serin, the young man who thought he could get rich buying and flipping houses. Instead, he crashed and burned. I’m afraid the same thing is going to happen to many folks riding the Bitcoin bandwagon.
