Private equity plunderers want to buy Simon & Schuster

Last November, publishing got some excellent news: the planned merger of Penguin Random House (the largest publisher in the history of human civilization) with its immediate competitor Simon & Schuster would not be permitted, thanks to the DOJ's deftly argued case against the deal:

When I was a baby writer, there were dozens of large NY publishers. Today, there are five - and it was almost four. A publishing sector with five giant companies is bad news for writers (as Stephen King said at the trial, the idea that PRH and S&S would bid against each other for books was as absurd as the idea that he and his wife would bid against each other for their next family home).
But it's also bad news for publishing workers, a historically exploited and undervalued workforce whose labor conditions have only declined as the number of employers in the sector dwindled, leading to mass resignations:

It should go without saying that workers in sectors with few employers get worse deals from their bosses (see, e.g., the writers' strike and actors' strike). And yup, right on time, PRH, a wildly profitable publisher, fired a bunch of its most senior (and therefore hardest to push around) workers:

But publishing's contraction into a five-company cartel didn't occur in a vacuum. It was a normal response to monopolization elsewhere in its supply chain. First it was bookselling collapsing into two major chains. Then it was distribution going from 300 companies to three. Today, it's Amazon, a monopolist with unlimited access to the capital markets and a track record of treating publishers "the way a cheetah would pursue a sickly gazelle":

Monopolies are like Pringles (owned by the consumer packaged goods monopolist Procter & Gamble): you can't have just one. As soon as you get a monopoly in one part of the supply chain, every other part of that chain has to monopolize in self-defense.
Think of healthcare. Consolidation in pharma lead to price-gouging, where hospitals were suddenly paying 1,000% more for routine drugs. Hospitals formed regional monopolies and boycotted pharma companies unless they lowered their prices - and then turned around and screwed insurers, jacking up the price of care. Health insurers gobbled each other up in an orgy of mergers and fought the hospitals.
Now the health care system is composed of a series of gigantic, abusive monopolists - pharma, hospitals, medical equipment, pharmacy benefit managers, insurers - and they all conspire to wreck the lives of only two parts of the system who can't fight back: patients and health care workers. Patients pay more for worse care, and medical workers get paid less for worse working conditions.
So while there was no question that a PRH takeover of Simon & Schuster would be bad for writers and readers, it was also clear that S&S - and indeed, all of the Big Five publishers - would be under pressure from the monopolies in their own supply chain. What's more, it was clear that S&S couldn't remain tethered to Paramount, its current owner.
Last week, Paramount announced that it was going to flip S&S to KKR, one of the world's most notorious private equity companies. KKR has a long, long track record of ghastly behavior, and its portfolio currently includes other publishing industry firms, including one rotten monopolist, raising similar concerns to the ones that scuttled the PRH takeover last year:

Let's review a little of KKR's track record, shall we? Most spectacularly, they are known for buying and destroying Toys R Us in a deal that saw them extract $200m from the company, leaving it bankrupt, with lifetime employees getting $0 in severance even as its executives paid themselves tens of millions in "performance bonuses":

The pillaging of Toys R Us isn't the worst thing KKR did, but it was the most brazen. KKR lit a beloved national chain on fire and then walked away, hands in pockets, whistling. They didn't even bother to clear their former employees' sensitive personnel records out of the unlocked filing cabinets before they scarpered:

But as flashy as the Toys R Us caper was, it wasn't the worst. Private equity funds specialize in buying up businesses, loading them with debts, paying themselves, and then leaving them to collapse. They're sometimes called vulture capitalists, but they're really vampire capitalists:

Given a choice, PE companies don't want to prey on sick businesses - they preferentially drain off value from thriving ones, preferably ones that we must use, which is why PE - and KKR in particular - loves to buy health care companies.
Heard of the "surprise billing epidemic"? That's where you go to a hospital that's covered by your insurer, only to discover - after the fact - that the emergency room is operated by a separate, PE-backed company that charges you thousands for junk fees. KKR and Blackstone invented this scam, then funneled millions into fighting the No Surprises Act, which more-or-less killed it:

KKR took one of the nation's largest healthcare providers, Envision, hostage to surprise billing, making it dependent on these fraudulent payments. When Congress finally acted to end this scam, KKR was able to take to the nation's editorial pages and damn Congress for recklessly endangering all the patients who relied on it:

Like any smart vampire, KKR doesn't drain its victim in one go. They find all kinds of ways to stretch out the blood supply. During the pandemic, KKR was front of the line to get massive bailouts for its health-care holdings, even as it fired health-care workers, increasing the workload and decreasing the pay of the survivors of its indiscriminate cuts:

It's not just emergency rooms. KKR bought and looted homes for people with disabilities, slashed wages, cut staff, and then feigned surprise at the deaths, abuse and misery that followed:

Workers' wages went down to $8/hour, and they were given 36 hour shifts, and then KKR threatened to have any worker who walked off the job criminally charged with patient abandonment:

For KKR, people with disabilities and patients make great victims - disempowered and atomized, unable to fight back. No surprise, then, that so many of KKR's scams target poor people - another group that struggles to get justice when wronged. KKR took over Dollar General in 2007 and embarked on a nationwide expansion campaign, using abusive preferential distributor contracts and targeting community-owned grocers to trap poor people into buying the most heavily processed, least nutritious, most profitable food available:

94.5% of the Paycheck Protection Program - designed to help small businesses keep their workers payrolled during lockdown - went to giant businesses, fraudulently siphoned off by companies like Longview Power, 40% owned by KKR:

KKR also helped engineer a loophole in the Trump tax cuts, convincing Justin Muzinich to carve out taxes for C-Corporations, which let KKR save billions in taxes:

KKR sinks its fangs in every part of the economy, thanks to the vast fortunes it amassed from its investors, ripped off from its customers, and fraudulently obtained from the public purse. After the pandemic, KKR scooped up hundreds of companies at firesale prices:

And now KKR has come for Simon & Schuster. The buyout was trumpeted to the press as a done deal, but it's far from a fait accompli. Before the deal can close, the FTC will have to bless it. That blessing is far from a foregone conclusion. KKR also owns Overdrive, the monopoly supplier of e-lending software to libraries.
Overdrive has a host of predatory practices, loathed by both libraries and publishers (indeed, much of the publishing sector's outrage at library e-lending is really displaced anger at Overdrive). There's a plausible case that the merger of one of the Big Five publishers with the e-lending monopoly will present competition issues every bit as deal-breaking as the PRH/S&S merger posed.

Mom, Dad, I think I’m going to change my major.
Unlike when you were an Undergrad, colleges today offer plenty of majors that are designed to maximize fun with a minimum of effort.
If tuition money isn't an issue for your family, or if Tommy Boy is next in line to run the brake factory upon graduation, please skip to the adorable kitten blog.
Still here? Good. If your child is headed for a professional or graduate school, please help them by encouraging they stay within the traditional, rigorous disciplines, of the real arts and sciences. Help your kid steer clear of majors created by activism rather than academic inquiry. Specifically, the market for African American Studies, Environmental “Science”, and Womyn’s Studies graduates is slim outside the academy or government.
Should they have the goal of joining the workforce upon graduation, a sound business management education is a good choice. My business management degree may have taught me skills that weren't in high demand for 21 year olds at the time, but my solid performance in an average State University’s undergraduate business school landed me a sales position with a Fortune 50 company two weeks after graduation in the midst of a deep recession.
Beware the rigor adjacent majors, those that sound difficult but aren’t. Think of anything with “management" appended to something fun.
Arts management.
Music industry management.
Parks and recreation management.
Sports management.
And also beware of majors for fields that require little beyond being skinny and attractive like Communications, Fashion, and Textiles. There is no need for a degree in the rag trade.
Do you have friends in the dying media world who will bring your child aboard as an unpaid intern? Because, standing on the sidelines asking the football coach what his team needs to do differently in the second half isn’t a high demand gig. And, surely not one requiring a degree.
A cousin whose family had means attended a finishing school in the 50’s. She was a social secretary to a DC Democratic fundraising doyenne. She arranged seating depending on who was in or out that week. These skills have resurfaced in universities as Event Management. Throwing a party and hiring entertainment on a budget isn’t a four year degree pursuit, the way waiting tables doesn’t demand a degree in Hospitality Management.
Journalism. Oy vey. Try English literature, Biology, Philosophy, something with an established canon and rigor. Develop critical thinking skills. Nobody would care about Woodward and Bernstein if they’d been played in the movie by Charlton Heston and Walter Matthau.
Your child may be a young adult, but they still need your guidance.

Don’t people need bosses and experts?
How can anarchists organize themselves in the workplace and coordinate production and distribution across an entire economy without bosses and managers? In fact, a great deal of resources are lost through competition and middlemen. Ultimately it is the workers who carry out all the production and distribution, and they know how to coordinate their own work in the absence of bosses.
In and around Turin, Italy, 500,000 workers participated in a factory takeover movement after World War I. Communists, anarchists, and other workers who were pissed off at their exploitation launched wildcat strikes, many of them eventually gaining control of their factories and setting up Factory Councils to coordinate their activities. They were able to run the factories themselves, without bosses. Eventually, the Councils were legalized and legislated out of existence — in part co-opted and absorbed into the labor unions, whose institutional existence was threatened by autonomous workers’ power no less than the owners were.
In December 2001, a long-brewing economic crisis in Argentina matured into a run on the banks which precipitated a major popular rebellion. Argentina had been the poster child of neoliberal institutions such as the International Monetary Fund, but the policies that enriched foreign investors and gave middle class Argentinians a First World lifestyle created an acute poverty for much of the country. Anti-capitalist resistance was already widely developed among the unemployed, and after the middle class lost all its savings, millions of people took to the streets, rejecting all the false solutions and excuses offered by politicians, economists, and the media, declaring instead: “Que se vayan todos! ” They all must go! Dozens were killed by police, but people fought back, shaking off the terror left over from the military dictatorship that ruled Argentina in the ’70s and ’80s.
Hundreds of factories abandoned by their owners were occupied by workers, who resumed production so they could continue to feed their families. The more radical of these worker-occupied factories equalized wages and shared managerial duties among all workers. They made decisions in open meetings, and some workers taught themselves tasks such as accounting. To ensure that a new managerial class did not arise, some factories rotated managerial tasks, or required that people in managerial roles still work on the factory floor and perform the accounting, marketing, and other tasks after hours. As of this writing, several of these occupied workplaces have been able to expand their workforce and hire additional workers from Argentina’s huge unemployed population. In some cases, occupied factories trade needed supplies and products with one another, creating a shadow economy in a spirit of solidarity.
One of the most famous, the Zanon ceramics factory located in southern Argentina, was shut down by the owner in 2001 and occupied by its workers the following January. They began running the factory with an open assembly and commissions made up of workers to manage Sales, Administration, Planning, Security, Hygiene and Sanitation, Purchasing, Production, Diffusion, and Press. Following the occupation, they rehired workers who had been fired before the closing. As of 2004, they numbered 270 workers and produced at 50% of the production rate before the factory was closed. Bringing doctors and psychologists on site, they provided themselves with healthcare. The workers found that they could pay their workforce with just two days of production, so they lowered prices 60% and organized a network of young vendors, many previously unemployed, to market the ceramic tiles throughout the city. In addition to producing tiles, the Zanon factory involves itself with social movements, donating money to hospitals and schools, selling tiles at cost to poor people, hosting films, performances, and art shows, and carrying out solidarity actions with other struggles. They also support the Mapuche struggle for autonomy; and when their clay supplier stopped doing business with them for political reasons, the Mapuche began supplying clay. As of April 2003, the factory had faced four attempted evictions by the police, with the support of the trade unions. All were forcefully resisted by the workers, assisted by neighbors, piqueteros, and others.
In July 2001, the workers of the El Tigre supermarket in Rosario, Argentina, occupied their workplace. The owner had shut it down two months earlier and declared bankruptcy, still owing his employees months in wages. After fruitless protesting, the workers opened El Tigre and began running it themselves through an assembly that allowed all workers a part in decision-making. In a spirit of solidarity they lowered prices and began selling fruit and vegetables from a local farmers’ cooperative and products made in other occupied factories. They also used part of their space to open a cultural center for the neighborhood, housing political talks, student groups, theater and yoga workshops, puppet shows, a café, and a library. In 2003, El Tigre’s cultural center held the national meeting of reclaimed businesses, attended by 1,500 people. Maria, one collective member, said of her experience: “Three years ago, if someone had told me we’d be able to run this place I’d never have believed them... I believed we needed bosses to tell us what to do, now I realize that together we can do it better than them.” [32]
In Euskal Herria, the Basque country occupied by the states of Spain and France, a large complex of cooperative, worker-owned businesses has arisen, centered around the small city of Mondragón. Starting with 23 workers in one cooperative in 1956, the Mondragón cooperatives included 19,500 workers in over 100 cooperatives by 1986, surviving despite the heavy recession in Spain at the time and with a survival rate many times better than the average for capitalist firms.
Mondragón has had a rich experience over many years in manufacturing products as varied as furniture, kitchen equipment, machine tools, and electronic components and in printing, shipbuilding, and metal smelting. Mondragón has created hybrid cooperatives composed of both consumers and workers and of farmers and workers. The complex has developed its own social security cooperative and a cooperative bank that is growing more rapidly than any other bank in the Basque provinces. [33]
The highest authority in the Mondragón cooperatives is the general assembly, with each worker-member getting one vote; the specific management of the cooperative is carried out by an elected governing council, which is advised by a management council and a social council.
There are also many criticisms of the Mondragón complex. To anarchists it comes as no surprise that a democratic structure can house an elite group, and according to Mondragón’s critics this is exactly what has happened as the cooperative complex seeks — and achieves — success within a capitalist economy. Although their accomplishment is impressive and gives lie to the assumption that large industries must be organized hierarchically, the compulsion to be profitable and competitive has pushed the cooperatives to manage their own exploitation. For example, after decades of sticking by their egalitarian founding principles regarding pay scales, eventually the Mondragón cooperatives decided to increase the salaries of the managerial and technical experts relative to the manual workers. Their reason was that they had a hard time retaining people who could receive much higher pay for their skills in a corporation. This problem indicates a need to mix manual and intellectual tasks to avoid the professionalization of expertise (i.e. creating expertise as a quality restricted to an elite few); and to build an economy in which people are producing not for profit but for other members of the network, so money loses its importance and people work out of a sense of community and solidarity.
People in today’s high-tech societies are trained to believe that examples from the past or from the “under-developed” world have no value for our situation today. Many people who consider themselves educated sociologists and economists dismiss the Mondragón example by classifying Basque culture as exceptional. But there are other examples of the efficacy of egalitarian workplaces, even in the heart of capitalism.
Gore Associates, based in Delaware, is the billion dollar high-tech firm that produces waterproof Gore-Tex fabric, special insulation for computer cables, and parts for the medical, automobile, and semiconductor industries. Salaries are determined collectively, no one has titles, there is no formal management structure, and differentiation between employees is minimized. By all capitalist standards of performance — employee turnover, profitability, product reputation, lists of best companies to work for — Gore is a success.
An important factor in their success is adherence to what some academics call the Rule of 150. Based on the observation that hunter-gatherer groups around the world — as well as successful communities and intentional communes — seem to keep their size between 100 and 150 people, the theory is that the human brain is best equipped to navigate webs of personal relationships of up to 150. Maintaining intimate relationships, remembering names and social status and established codes of conduct and communication — all this takes up mental space; just as other primates tend to live in groups up to a certain size, human beings are probably best suited to keep up with a certain number of companions. All Gore factories keep their size below 150 employees, so each plant can be entirely self-managing, not just on the factory floor but also including the people responsible for marketing, research, and other tasks.[34]
Skeptics often dismiss the anarchistic example of small-scale “primitive” societies by arguing that it’s no longer possible to organize on such a small scale, given the huge population. But there is nothing to stop a large society for organizing itself in many smaller units. Small-scale organization is eminently possible. Even within a high-tech industry, Gore factories can coordinate with one another and with suppliers and consumers while maintaining their small scale organizational structure. Just as each unit is capable of organizing its internal relations, each is capable of organizing its external relations.
Of course, the example of a factory producing successfully within the capitalist system leaves much to be desired. Most anarchists would sooner see all factories burned to the ground than anti-authoritarian forms of organization used to sugarcoat capitalism. But this example should at least demonstrate that even within a large and complex society, self-organization works.
The example of Gore is still problematic because the workers do not own the factory, and also because formal management could be reimposed at any time by the company owners. Anarchists theorize that the problems of capitalism do not exist only in the relationship between workers and owners, but also between workers and managers, and that as long as the manager-worker relationship persists, capitalism can reemerge. This theory is certainly born out by the Mondragón example, where over time managers gained more pay and power and renewed the unequal, profit-focused dynamics typical of capitalism. Taking this into account, several anarchists have designed an outline for a “participatory economy,” or parecon, though no one has yet had the opportunity to set up such an economy on any considerable scale. Among other things, parecon emphasizes the importance of empowering all workers by mixing tasks that are creative and rote, mental and manual, thus creating “balanced job complexes” that will prevent the emergence of a managerial class.[35]
During the rebellion in Oaxaca in 2006, people without prior experience organized themselves to run occupied radio and television stations. They were motivated by the social need for free means of communication. The March of Pots and Pans, the legendary women’s march on August 1, 2006, culminated with thousands of women spontaneously taking over the state-run television station. Inspired by the sudden sense of power they had won by rebelling against a traditionally patriarchal society, they took over Channel 9, which continuously slandered the social movements while claiming to be the channel of the people. At first, they made the engineers help them run the station, but soon they were learning how to do it themselves. One woman recounted:
I went daily to the channel to stand guard and help out. The women were organized into different commissions: food, hygiene, production, and security. One thing I liked is that there were no individual leaders. For each task there was a group of several women in charge. We learned everything from the beginning. I remember somebody asking who could use a computer. Then many of the younger girls stepped forward, saying, “me, me, I can!” In Radio Universidad, they announced that we needed people with technical skills, and more people came to help. In the beginning, they were filming headless people, you know. But the experience at Channel 9 showed us that where there’s a will, there’s a way. Things got done, and they got done well. In the short time [three weeks] that Channel 9 was running, until Governor Ulises commanded that the antennas be destroyed, we managed to spread a lot of information. Movies and documentaries were shown that you could never have imagined seeing on TV otherwise. About different social movements, about the student massacre in Tlatelolco in Mexico City in 1968, the massacres in Aguas Blancas in Guerrero and Acteal in Chiapas, about guerrilla movements in Cuba and El Salvador. At this time, Channel 9 wasn’t just the women’s channel anymore. It was the channel of all the people. The ones participating made their own programs as well. There was a youth program and a program where people from the indigenous communities participated. There was a program of denouncements, where anyone could come and denounce how the government had treated them. A lot of people from the different neighborhoods and communities wanted to participate, there was hardly enough airtime for all of them.[36]
After the occupied television station was taken off the air, the movement responded by occupying all eleven commercial radio stations in Oaxaca. The homogeneity of commercial radio was replaced by myriad voices — a radio station for university students, one for the women’s groups, one radio station occupied by the anarchists from a punk squat — and there were more indigenous voices on the radio than ever before. Within a short time, people in the movement decided to return most of the radio stations to their self-styled owners, but kept control of two of them. Their goal was not to suppress the voices that opposed them, as artificial as commercial voices are, but to win themselves the means to communicate. The remaining radio stations operated successfully for months, until government repression shut them down. One university student involved in taking over, running, and defending the radio stations said:
After the takeover, I read an article that said that the intellectual and material authors of the takeovers of the radios weren’t Oaxacan, that they came from somewhere else, and that they received very specialized support. It said that it would have been impossible for anyone without previous training to operate the radios in such a short amount of time, because the equipment is too sophisticated for just anyone to use. They were wrong.[37]

The Broken System
Why Talk of “NHS Reform” Needs to be Treated with Suspicion

TALK OF A “CRISIS” in healthcare in the United Kingdom has been such a feature of the British news cycle for so long, it can be difficult sometimes to take it seriously. The National Health Service is frequently being described as attempting to deal with “infinite demand”; that its model of service delivery is locked into a byegone world of the 1950s and, frequently on the political Right, that the £180bn system is “unaffordable”. The fact that “winter crises”, once predictable annual events of pressure on emergency services and capacity, have become a continual phenomenon within NHS hospitals; that obtaining GP appointments has become, in certain areas of the country, a virtual impossibility; that elective waiting lists have surged to over 18 months and that ambulance response times have deteriorated markedly, are all held as “proof” that the current model of delivery for the NHS is unsustainable and “reform” is required. There is little that the government of Rishi Sunak and Keir Starmer’s Labour opposition agree on, but there seems to be a growing consensus that “reform” is the solution to the perceived falling apart of our once cherished system of healthcare. All this is compelling stuff, but when politicians achieve an unexpected unanimity on an issue, the wise person should approach what masquerades as a debate, with caution, and even suspicion.
The fact is that these assessments of the state of the NHS by lazy journalists, self serving commentators and bewildered politicians are simply not correct. The lie that NHS demand is “infinite” goes back at least 40 years and is usually propagated by the political and academic Right, who dislike both the lack of profit mechanisms within the British health system, but also, and more strongly, the comprehensive nature of the system. Because the NHS seeks to provide the full range of health services from primary and preventative care all the way to highly specialist tertiary surgery to all its citizens, this all encompassing offer is often conflated with an alleged limitless demand. This is simply untrue. While new treatments and technologies are developed by the NHS, there nonetheless remains a quantum of elective and emergency need that has remained fundamentally unaltered in the course of the NHS’ history: the variable has been the generosity of its funding settlements, its bed capacity, the robustness of its workforce planning, and the resourcing and co-ordination of social and residential and nursing care. These variables have been subject since 1948, to political decisions. What has changed over the last twenty years is a growing elderly population, suffering from a variety of long term conditions. It has been the system’s inability, and the political and structural choices made by successive governments, to adapt to this altered demand, that has brought the NHS close to tipping point.
The notion that the NHS continues, in the 21st century, to run an inflexible model of delivery designed in 1948 and therefore remains a slow moving centrally directed bureaucracy, akin to the former nationalised industries, is another false trope. Since 1948, the NHS has been formally reorganised on at least four occasions (1974; 1980; 1989 and 2012) and implicitly reorganised on a further two occasions (the 1983 implementation of “general management” and 2000 NHS Plan which reintroduced the internal market) and this does not include countless changes to clinical guidance, financial regimes and local restructurings. The biggest break with the 1948 model was the introduction by then Health Secretary Ken Clark of the first experiment in internal markets in 1989 after the publication of a White Paper curiously entitled Working For Patients, which introduced the principles of “purchased” or commissioned healthcare provided by (mainly) hospital services competing for service contracts. After briefly being abolished in 1997, the internal market returned with a vengeance under uber Blairite Health Secretary Alan Milburn before descending into the utter chaos of Andrew Lansley’s Health and Social Care Act of 2012, which extended markets, further broke up health systems, and extended competition to an almost dysfunctional level. If the NHS is “over managed” as so many politicians and newspapers now claim, it is due to a thirty four year experiment in trying to run socialised medicine on market lines, and the resultant competitive and disaggregated structures that still scar the health and social care system. These structures, despite some recent amelioration, continue to maintain unnecessary transactional costs and present constant barriers to integrated clinical care.
The claim that the NHS is “unaffordable” is perhaps the most pernicious of all. The U.K. is currently estimated to spend approximately a fifth less on healthcare per citizen than the EU average - exacerbated by over eleven years of austerity funding which saw NHS budgets rise by between 2% and 3% per annum, against an annual health service inflation increase of 5% every year, while social care budgets were cut by 40% between 2010 and 2020. If the health and social care system is “unaffordable” it is not for the want of trying by a Tory government to continually squeeze its real term resourcing. But despite this, the siren calls for “reform” from the Right get louder - with the suggestion of the introduction of fees for certain services or the establishment of an insurance model to replace the existing system of funding through direct taxation. The Labour response is equally confused and inadequate, focusing on symptoms rather than causes (Wes Streeting’s extolling the use of private sector capacity for elective care and Starmer’s musings about renegotiating the position of GPs as independent contractors to improve the availability of emergency appointments) and miss the point of just how broken health and social care has become after thirteen years of underfunding, witless restructuring and neglect by the Conservative government.
The fundamentals of how we got to the current crisis of provision are not hard to discern.
NHS vacancies currently stand at over 130,000, of which nearly 48,000 are nursing staff and 36,000 are GPs: this represents a vacancy rate of over 10%; in social care, the vacancy rate is approximately 100,000. One of the main reason that patients are waiting in A&E; are failing to be discharged from hospital; cannot obtain a GP appointment, and cannot obtain admission to a care home is that there are simply not enough people to see, treat and accommodate them. Austerity cuts to clinical training places, an effective abandonment of any meaningful workforce planning by the Cameron and May governments, the criminally negligent ending of nursing and other health professional bursaries and the idiocies of Brexit, which has choked off a reliable source of European clinical workers who were plugging the gap, are symptomatic of a Tory government that is incapable of running public services or foreseeing the consequences of its spending obsessed short term decisions. Health and social care austerity chickens have come home to roost and no amount of fee for service, insurance models or sacking of “pen pushers” will address this accumulated workforce deficit.
Similarly, the privatised nursing and care home model, introduced by the Thatcher governments, has finally run out of road. It has been accepted for some time that the demographic reality of an ageing and increasingly sick population is behind the pressures on acute hospital beds in terms of admissions, but the reason for gridlock in A&E and patients lying in the back of ambulances waiting to simply get into that overcrowded emergency department, is due to the inability of the hospital system to discharge patients who no longer require hospital care. Currently 12,000 patients occupying acute beds daily are in that category, almost 10% of acute capacity; just 43% of patients who could be discharged are being so each day. This is a result of the devastating cuts to social care budgets and the vacancy rates of social workers mentioned above, meaning discharge assessments and the allocation of social services support to patients, are continually delayed. As significant, however, is the impact that these same cuts have had on the ability of local authorities to pay care homes a sufficiently high tariff for them to be able to stay in business. The result has been the closure of between 12% and 16% of residential and nursing home beds between 2010 and 2021. This twin financial assault on social care and the care home sector for over a decade has resulted in the the logjam at the front door of every hospital in the land and the consequent disappearance of ambulances from our streets which is so terrifying the public. Reductions in staff and capacity can only go on for so long. In 2022 and 2023, the bottom has finally dropped out.
If the politicians and commentators are agreed that “reform” is now essential, here are a couple of suggestions as to where to start:
Expand training places for health and social care staff and introduce funded, serious workforce plans to meet the demographic changes facing society and implement them;
Nationalise the care home sector, invest in it, increase its capacity and introduce the National Care Service the cross party select committee recommended to the Labour government in 2009, and so ensure all citizens receive the long term care they need without financial disresss;
