Measuring Tritium in Water

Measuring tritium in water and tritium in seawater continuously on a real-time basis is an exceptionally difficult task due to tritium’s low beta emission energy (18.6 keV maximum, 5.6 keV mean) and the shielding properties of water. Traditionally, liquid scintillation counting has been used to determine low level tritium concentration in water, although this method is typically performed by manually taking a sample and analyzing it later.

Overhoff has developed several novel solutions for measuring ultra-low levels of tritium in water on a real-time, continuous basis. Applications include: measuring heavy-water leaks in nuclear power plants (especially of the CANDU type) as well as monitoring tritium concentration in groundwater, drinking water, seawater, and liquid effluents.

Model 1925

The Model 1925 was originally designed in 2004 working in conjunction with AECL and installed at Cernavoda NPP in Romania. The Model 1925 is a real-time, continuous tritium water monitor utilizing an automated liquid scintillation counting system. The primary use of the Model 1925 is to detect the leak of heavy water (tritium) in nuclear power plants, but it can also be used to monitor tritium concentration in groundwater, drinking water, rivers, lakes, or the ocean.

The Model 1925 detects tritium decay via liquid scintillation counting using dual high-quality photomultiplier tubes (PMTs) working in coincidence counting mode. Liquid scintillator and the sample water are automatically mixed at the sample cell and measured. The liquid scintillator is connected to the unit externally and is stored inside a polyurethane drum of 23 liters, which is sufficient for 60 days of continuous 24/7 operation.

The use of highly effective PMTs, a specially designed sampling cell to minimize cosmic radiation and Cherenkov effects, and 1” lead shielding provides for low background noise of only one count per second with a phenomenal counting efficiency of 30%. MDA is 3.7 kBq/L in 9 minutes or less with a 95% confidence level and a resolution of 1.0 kBq/L.

By working in ‘coincidence counting’ mode, the 1925 only counts radiation from actual tritium decay events that hit both PMTs at the same time, rejecting background noise to improve sensitivity and accuracy. Furthermore, pulse shaping and discrimination selects only recognizable tritium decay events, rejecting large amplitude and long-duration gamma effects and system noise.

The Model 1925 is an unparalleled automated liquid scintillation counting system capable of measuring tritium in water or seawater down to 3.7 kBq/L or 100,000 pCi/L in 9 minutes or less with 95% confidence.

The Model 1925-PR is an automated, real-time low-level tritium in water monitor that utilizes a novel electrolysis method to separate the hydrogen (tritium) from oxygen and measure it using an ultra-sensitive gas proportional detector. The main advantage of the 1925-PR is that it does not require liquid scintillation counting cocktail nor does it produce hazardous mixed waste that must be carefully stored, transported, and disposed of.

An electrolytic hydrogen gas separator is used to separate the input water into hydrogen gas (tritium) and oxygen. The oxygen is diluted and safely released while the hydrogen (tritium) is mixed with P-10 gas and pumped through an ultra-sensitive proportional counting detector. By utilizing proportional counting technology, the user only has to acquire a compact standard steel tank of proportional gas (8.5 m3 or 300 ft3), which will last for up to two months of continuous operation and is available from a variety of suppliers.

Measurement via this method achieves same or better low-end sensitivity, without having to deal with LSC fluid and waste. A second, sealed proportional counter eliminates the effects from external gamma fields and 1” lead shielding around the counter tubes further reduces background gamma and improves the statistical signal to noise ratio to enhance sensitivity.

Model Trimaran-H20

The Model Trimaran-H20 is our most sensitive automated, continuous tritium water monitor. It is based on the 1925-PR but includes a sample enrichment loop to iteratively concentrate the tritium for the lowest MDA possible.

After the input water is electrolytically separated, the oxygen is diluted and safely released while the hydrogen gas containing tritium is cycled iteratively through a proprietary enrichment loop which increases the concentration of tritium (HT and T2). After sufficient enrichment, the tritium sample is mixed and diluted with P-10 gas and then measured by an ultra-sensitive gas proportional counter. The low concentration of hydrogen is dealt with safely through dilution with the P-10 gas and having the instrument enclosures under negative pressure.

The sensitivity of the Trimaran-H20 is 14,000 pCi/L (500 Bq/L) in 8 hours or less, which is below the US EPA’s tritium drinking water limit of 20,000 pCi/L (740 Bq/L).

The Models 1925, 1925-PR, and Trimaran-H20 are all capable of measuring both freshwater and seawater and include an appropriate prefiltration system.

For measuring low to high concentrations of tritium in water, several other real-time, continuous tritium water monitors are also available. No liquid scintillant is required for these systems, allowing you to save money on operating costs.

The Model NEX-Tritium utilizes a unique flow-through detector assembly consisting of a crushed bed of our custom-made anthracene scintillation crystals. By using custom crushed scintillation crystals, the input water is intimately mixed with the scintillator material in order to provide better sensitivity in a flow-through, real-time system. The light-tight detector assembly is viewed by a pair of matched photomultiplier tubes (two 5” diameter PMTs, totaling 200cm2 sensitive area) working in coincidence mode. The output from the photomultiplier tubes is analyzed by pulse height and coincidence, thereby permitting the system to eliminate counting most background (noise) counts. Optional: The input water is chilled by a cooling unit and the detection assembly is mounted inside of a refrigeration unit to provide long-term accuracy, stability, and enhanced sensitivity.

Cochlear Endoscopy in Cochlear Implantation of a X-Linked Stapes Gusher Syndrome

Abstract

A 12-year-old boy with a five-year history of bilateral sensorineural hearing loss and X-linked stapes gusher syndrome developed progressive left-sided hearing loss. The pre-operative computed tomography of the temporal bone showed a bulbous internal auditory canal with a dysplastic cochlea and no apparent modiolus. A 1.3mm salivary endoscope was placed at the cochlear entrance to assess the intracochlear anatomy. This revealed membranous structures of the cochlea without direct communication to the internal auditory canal. We advocate for the use of cochlear endoscopy to better delineate inner ear anatomy, which will influence the implant selection and potentially hearing outcomes in patients.

Introduction

X-linked stapes gusher syndrome (otherwise known as X-linked deafness type 3 or Nance deafness) is a rare form of sensorineural hearing loss (SNHL) syndrome. Inherited in a sex-linked recessive manner, it is believed to be the consequence of a loss-of-function mutation on the X-chromosome in the POU3F4 gene at the DFN3 locus [1,2]. Males tend to present greater phenotypic severity than females, who present less frequently [3-5].

Affected patients have an abnormal configuration of the lamina cribrosa and internal auditory canal (IAC) [1,3,4,6,7]. This malformation leads to increased perilymphatic pressure and to stapes’ footplate fixation, giving rise to conductive hearing loss and progressive cochlear nerve incompetence. This is relevant for surgeons, as it results in an increased risk of perilymph gusher with surgical manipulation [1,3,4,6,7]. This may lead to other complications, such as otorrhea, rhinorrhea, and recurrent meningitis [8]. Meningitis complicating cochlear implantation (CI), occurs at a higher rate in patients with inner ear (IE) abnormalities, including X-linked gusher syndrome [3,9].

Traditional approaches to imaging for CI utilize computed tomography of the temporal bone (CT TB) and/or magnetic resonance imaging. We report a case whereby intra-operative otoendoscopic visualization allowed for real time visualization of the IE anatomy, which allowed us to optimize our electrode choice for CI. In this case, such visualization was valuable, as it indicated the presence of the membranous portions of the IE to evaluate if the modiolus was present, which we believe implied that a directional electrode was the most appropriate choice.

Case Report

A 12-year-old boy with a five year history of bilateral SNHL and X-linked stapes gusher syndrome presented with progressively worsening left-sided hearing loss. He was initially performing well with bilateral traditional hearing aids but developed progressive mixed loss and worsening performance in his left ear. His preoperative audiogram can be seen in Figure 1. He was sent for CI candidacy assessment and was deemed to be suitable.

The pre-operative computed tomography (CT) of the temporal bone was assessed. This showed a bulbous IAC with a dysplastic cochlea and no apparent modiolus (Figure 2). A CI24RE (ST) implant was initially selected by the implant team due to the uncertainty regarding the location of the spiral ganglion cells.

A 1mm cochleostomy was made anteroinferior to the round window (RW). Upon entering the cochlea, a moderate perilymphatic gusher was encountered. A 1.3mm rigid salivary endoscope (Karl Storz, Germany) with a 3-chip full HD camera head (Image1 S H3-Z) was approached trans-tympannically and was placed at the edge of the 1mm cochleostomy to assess the intracochlear anatomy.

To avoid injury to the inner ear, the CSF was not suctioned. The same view was not achieved with the microscope as the endoscope provided a more magnified and higher definition view of the modiolus. There was evidence of membranous structures of the cochlea without direct communication to the IAC. Full insertion was achieved on the first attempt without any difficulty and the gusher was controlled with packing of periosteal tissue at the cochleostomy site. An intra-operative plain X-ray confirmed the electrode placement.

Discussion

When the cochlear modiolus and osseous spiral lamina are deficient, the absence of a bony septum creates a common space seen on the imaging study [10] (Figure 2); there is an abnormal communication between the IAC and the vestibule responsible for the periphymphatic gusher. Patients with IE anomalies may have both atypical positions of their spiral ganglion cells (SGC) and have a higher likelihood of having fewer spiral ganglion cells [11]. This has functional implications because at least 10,000 functional SGNs are necessary for effective speech discrimination 12]. Also this number can be further reduced by surgical trauma [12]. Therefore, choosing the proper electrode device, namely, a directional or a full banded electrode can help reduce the number of lost SGNs. The fully banded CI electrode may be more useful in the absence of a modiolus to allow for full and multi-directional stimulation, whereas the pre-curved directional electrode may be more appropriate if there is a modiolus present for precise stimulation [13].

The proper electrode choice may have important hearing outcome implications for patients with IE anomalies following CI due to the challenges of reaching an optimal level of cochlear stimulation, decreased dynamic range, a wider pulse width, and weakened neural synchrony [7]. The functionality of CI is correlated to the number of SGCs and their distances from the stimulating electrode [14].

In our patient, we assumed that there was no IAC communication due to the presence of modiolus. The moderate CSF gusher was presumed to be secondary to his X-lined stapes gusher syndrome and his enlarged IAC. Perilymph gusher during CI in patients with X-linked gusher syndrome is inevitable, but a thorough examination of the IE is still critical. CT of the temporal bone is important for surgical planning, and it is also useful to assess the likelihood of perilymphatic gusher [4]. Distinguishing features on imaging include an enlarged bulbous IAC, a widened cochlear aperture without the lamina cribrosa, cochlear hypoplasia with modiolar deficiency, and a broadening of the bony canal for the labyrinthine portion of the facial nerve [6]. Enlarged vestibular aqueducts may also appear in conjunction with modiolar deficiency [15].

The rarity of X-linked gusher syndrome may result in radiologists failing to recognize these signs, which may mislead surgeons to perform stapedectomies that are otherwise contraindicated. Thus, Incesulu et al. [3] advocated for high resolution CT of the temperal bone to assess for congenital dysplasia with ¬at least 1-mm thick slices [as opposed to 2mm thick slices], which should ideally be assessed by an experienced neuroradiologist [3]. Quan et al. [16] proposed that CT virtual endoscopy should be done to evaluate CT data through threedimensional reconstruction [16]. Limitations to both modes of assessment assume that the CT images correlate perfectly with the anatomy of the IE and that consistent radiological consultation will occur. Our case, however, demonstrated that intra-operative endoscopic findings do not always correlate with CT findings, indicating that cochlear endoscopy may be a useful tool to better delineate the intracochlear anatomy.

The otoendoscopic intracochlear view gave us accurate and real-time information on the anatomy of the IE, which confirmed the presence of a modiolus and the confidence that the CI would not be in the IAC. This has the potential to allow us to make a better-informed decision regarding the type of electrode to place.

Electrode choice is significant for patients with IE abnormalities, since the location of neural tissue may be abnormal. In patients that have an absent modiolus, a circumferentially stimulating electrode may be preferred over a full-banded electrode, which may risk adverse facial nerve stimulation [3]. One explanation for post-operative facial nerve stimulation in children with IE abnormalities is the close vicinity of the electrode to the nerve [14]. Therefore, to avoid injury, the proper choice in electrode should be made.

Conclusion

While CT temporal bone has served as the conventional approach to assessing the anatomy of the IE, fthe endoscope offers better resolution of the modiolus than the CT temporal bone, as the CT indicated that there was no modiolus. We advocate the use of intracochlear endoscopy in selected cases as it offers a better resolution than even high-resolution CT. With the potential to change electrode choices in CI, the customization of electrode choice based on the presence of membranous IE anatomy may change the hearing outcome of the patients with anomalous IE anatomy and patients with uncertain location of spiral ganglion cells.

It will just be huge blocks with no orientation points for the reader. Break it up and make it clear what each section of your page is all about with header tags.

Your Images Aren’t Optimized

It’s important that you pick beautiful, high-resolution images that catch the eye.

But you’re losing if you don’t then optimize your images.

Your images need to:

Contain relevant alt text

Failing to compress images is still a major issue for some websites. You can use an image compressor tool to reduce the size of your images. This is really important because images that are too big will slow down your site speed. If a website takes too long to load, users will exit in a hurry and your bounce rate will soar.

But they only work if you get them right. Sumo has carried out extensive research into pop-ups and what works and what doesn’t. They found – as many other marketers have found – that pop-ups which appear too soon are extremely off-putting to the end user.

Put the most important information above the fold.

Don’t push valuable content down with oversized hero images. Make the content they came for clearly visible above the fold. Make life as easy as possible for your site visitors and don’t make them wait – or look around – for the content they came for.

Given that the best interests of the child are at the heart of every Utah family law case, legislators and family law attorneys alike continually strive to promote conflict resolution and effective communication between parents and families. Consequently, state guidelines regarding child custody and visitation are sometimes created and/or amended to be more practical and effective. In fact, the state legislature is set to consider a proposed bill that would have a direct impact on families across the state.

The Judiciary Interim Committee of the Utah legislature recently considered a draft of a proposed piece of legislation intended to increase the amount of visitation time that noncustodial parents have with their children. The proposed bill was reportedly drafted in order to reduce divorce-related litigation by resolving some points of conflict between parents, according to one source. And while the committee had yet to make a decision on the proposed bill, it is expected to be considered by the legislature next year.

The proposed piece of legislation would allow a family law judge to decide whether a noncustodial parent is capable of maintaining longer periods of parenting times. Several factors would be taken into consideration in the matter, including the schedule flexibility of the parent and his or her ability to care for the child after school. The bill would also mandate that the noncustodial parent in question illustrate his or her ability to communicate effectively with the child’s custodial parent and be active in the child’s life.

Ultimately, the proposed measure would allow noncustodial parents to pick their children up from school on Friday and bring them directly to school on Monday, instead of conducting exchanges at the custodial parent’s house.
