Percutaneous Nephrolithotomy (PCNL)
Percutaneous nephrolithotomy (PCNL) is a surgical procedure to remove kidney stones that are too large to pass naturally or by other non-invasive treatments. In this procedure, a small incision is made in the back to create a tunnel through the skin and tissue to access the kidney. Then an endoscope is inserted through the incision to find and remove kidney stones.
PCNL is usually performed under general anesthesia and requires a short hospital stay of 1-3 days. This procedure has a high success rate and is considered safe and effective in treating large or complex kidney stones. This procedure is recommended for patients with kidney stones larger than 2 cm or located in areas of the kidney that are difficult to access with other non-invasive treatments. PCNL is also used in patients with other medical conditions that make other treatments less safe or effective.
Prior to the procedure, the patient may need to have imaging tests, such as a CT scan or ultrasound, to locate kidney stones and determine the best method for their removal. Patients are also advised to stop taking blood thinners such as aspirin and warfarin before surgery to reduce the risk of bleeding.
During surgery, the patient lies on their back and a small incision is made in their back.  A guidewire is then inserted into the kidney through the incision and tunneled with a series of dilators. A nephroscope,  a thin tubular instrument with a camera and light source, is inserted through a tunnel to locate and remove kidney stones. After the stone is removed, a tube called a nephrostomy tube is inserted into the kidney to drain any remaining fluid and debris.
After the procedure, patients may experience pain or discomfort in the back or abdomen and may be given pain relievers to manage these symptoms. Patients are also advised to drink plenty of fluids to wash away any remaining stone debris. In some cases, patients may need to undergo additional procedures to completely remove all kidney stones.
Overall, PCNL is a safe and effective surgical procedure for treating large or complex kidney stones. Patients should discuss the risks and benefits of this procedure with their doctor to determine if this is the best treatment option for their individual needs.

Rotational Atherectomy Treatment in Chennai
Understanding Rotational Atherectomy Treatment: A Comprehensive Overview
Rotational Atherectomy, also known as Rotablation, is a minimally invasive procedure designed to treat heavily calcified and narrowed coronary arteries. It utilizes a high-speed, diamond-coated burr to carefully shave away calcified plaque, clearing the artery and improving blood flow to the heart. Often combined with balloon angioplasty and stenting, this procedure is ideal for patients with complex coronary artery disease who are unsuitable for traditional angioplasty. In Chennai, the procedure is performed through a catheter inserted into the femoral artery, offering a vital treatment option for severe cases.
What is Rotational Atherectomy?
Rotational Atherectomy is an advanced intervention used to address severe coronary artery disease, particularly in patients with calcified plaques that cannot be treated effectively with conventional methods like balloon angioplasty or stenting. The procedure involves a rotating diamond-tipped burr that pulverizes hardened plaque into microscopic particles, which are safely washed away through the bloodstream.
Why is Rotational Atherectomy Needed?
Rotational Atherectomy is essential for patients suffering from complex coronary artery disease, particularly those with rigid, calcified plaques that impede blood flow. Traditional treatments like balloon angioplasty often fail to clear these calcifications, making Rotablation an effective solution. The buildup of fatty substances, cholesterol, and other materials leads to hardened plaque over time, restricting blood flow to the heart and potentially resulting in chest pain (angina) or even a heart attack.
Tumblr media
Who Should Consider Rotational Atherectomy?
Rotational Atherectomy is best suited for patients with severe coronary artery disease who face challenges with traditional treatments. The procedure is especially effective when calcified plaques obstruct blood flow, and other options may not be sufficient. If you are dealing with severe blockages, especially in the coronary arteries, this procedure can offer a solution to restore proper blood flow.
Choosing the Right Doctor for Rotational Atherectomy
Dr. M. Kathiresan, a highly respected interventional cardiologist, is an expert in performing Rotational Atherectomy in Chennai. With years of experience in advanced heart care, he offers comprehensive treatment and ensures that patients receive the best care available. Dr. Kathiresan is known for his patient-centered approach and expertise in managing complex cases.
Diagnosis: How is Rotational Atherectomy Determined?
Diagnosing the need for Rotational Atherectomy is typically done through a combination of tests, such as:
Coronary Angiogram: To visualize blood vessels.
Intravascular Ultrasound (IVUS): For detailed artery imaging.
CT Angiogram: To assess arterial blockages.
Stress Test: To evaluate heart function under physical stress.
These diagnostic tests help doctors determine whether Rotational Atherectomy is appropriate and identify which vessels require treatment.
What to Expect During the Procedure?
The Rotational Atherectomy procedure typically lasts 1-2 hours and is performed under local anesthesia and mild sedation. It is done in a hospital or outpatient setting. A catheter is inserted into the femoral or radial artery and guided to the site of the blockage. A rotating diamond burr is used to break up calcified plaque, with the debris safely washed away through the bloodstream. The procedure is monitored closely for any complications, and once complete, the catheter is removed, and the incision is closed. Most patients recover within a few days.
Key Steps of the Rotational Atherectomy Procedure:
Local Anesthesia: Anesthesia is administered at the insertion site (typically the femoral or radial artery).
Guidewire Insertion: A guidewire is threaded to the blocked artery, allowing the device to reach the plaque.
Rotational Atherectomy: A rotating burr grinds and modifies the calcified plaque, turning it into small particles.
Plaque Removal: The particles are flushed out via the bloodstream.
Success Rate of Rotational Atherectomy
The success rate of Rotational Atherectomy varies, but in skilled hands, it is highly effective in improving blood flow to the heart. The procedure enhances the chances of successful follow-up interventions like balloon angioplasty and stenting, leading to better long-term outcomes.
Benefits of Rotational Atherectomy:
Enhanced Treatment for Complex Cases: It is particularly beneficial for patients with complex coronary artery disease and calcified plaques.
Improved Long-Term Outcomes: Patients typically experience reduced chest pain, enhanced exercise tolerance, and overall improved quality of life.
Minimally Invasive: The procedure involves smaller incisions and reduced recovery time compared to open-heart surgery, making it suitable for high-risk patients.
Advanced Technology and Expertise: Chennai offers cutting-edge cardiac care with skilled professionals and state-of-the-art facilities for optimal outcomes.
Risks of Rotational Atherectomy:
While the procedure is generally safe, potential risks include:
Infection or bleeding
Damage to blood vessels or surrounding tissue
Heart attack or stroke during or after the procedure
Arrhythmias or irregular heartbeats
Allergic reactions to contrast dyes or medications
Cost of Rotational Atherectomy in Chennai
The cost of Rotational Atherectomy in Chennai can vary based on factors like hospital choice, procedure complexity, and the patient's needs. Leading hospitals with advanced cardiac care facilities may charge higher fees. However, the long-term benefits, such as reduced need for repeat procedures, make Rotablation a cost-effective option. Many health insurance plans cover the procedure, and financial assistance programs are available in some hospitals.
Rotational vs. Orbital Atherectomy: Choosing the Best Option
When considering atherectomy, it’s important to understand the differences between Rotational Atherectomy and Orbital Atherectomy:
Rotational Atherectomy: Uses a high-speed rotating burr to remove plaque and is particularly effective for calcified lesions.
Orbital Atherectomy: Uses an orbital motion to modify plaque, ideal for concentric lesions.
Both procedures are available at top cardiovascular centers in Chennai. Your cardiologist will help you determine which option is best suited to your condition.
Conclusion:
Rotational Atherectomy is a vital treatment for patients with severe coronary artery calcification, especially when traditional treatments are ineffective. Dr. Kathiresan's expertise, combined with advanced technology and precise diagnostic tools, ensures optimal treatment planning and execution.  For more details : https://drkathiresan.com/treatments/robotic-minimal-access-bypass-surgery-in-chennai
