Percutaneous Nephrolithotomy (PCNL)
 Percutaneous nephrolithotomy (PCNL) is a surgical procedure to remove kidney stones that are too large to pass naturally or by other non-invasive treatments. In this procedure, a small incision is made in the back to create a tunnel through the skin and tissue to access the kidney. Then an endoscope is inserted through the incision to find and remove kidney stones.
  PCNL is usually performed under general anesthesia and requires a short hospital stay of 1-3 days. This procedure has a high success rate and is considered safe and effective in treating large or complex kidney stones. This procedure is recommended for patients with kidney stones larger than 2 cm or located in areas of the kidney that are difficult to access with other non-invasive treatments. PCNL is also used in patients with other medical conditions that make other treatments less safe or effective.
 Prior to the procedure, the patient may need to have imaging tests, such as a CT scan or ultrasound, to locate kidney stones and determine the best method for their removal. Patients are also advised to stop taking blood thinners such as aspirin and warfarin before surgery to reduce the risk of bleeding.
 During surgery, the patient lies on their back and a small incision is made in their back.  A guidewire is then inserted into the kidney through the incision and tunneled with a series of dilators. A nephroscope,  a thin tubular instrument with a camera and light source, is inserted through a tunnel to locate and remove kidney stones. After the stone is removed, a tube called a nephrostomy tube is inserted into the kidney to drain any remaining fluid and debris. 
 After the procedure, patients may experience pain or discomfort in the back or abdomen and may be given pain relievers to manage these symptoms. Patients are also advised to drink plenty of fluids to wash away any remaining stone debris. In some cases, patients may need to undergo additional procedures to completely remove all kidney stones.
  Overall, PCNL is a safe and effective surgical procedure for treating large or complex kidney stones. Patients should discuss the risks and benefits of this procedure with their doctor to determine if this is the best treatment option for their individual needs.

MIT engineers have developed a magnetically steerable, thread-like robot that can actively glide through narrow, winding pathways, such as the labrynthine vasculature of the brain.
In this new paper, the researchers combined their work in hydrogels and in magnetic actuation, to produce a magnetically steerable, hydrogel-coated robotic thread, or guidewire, which they were able to make thin enough to magnetically guide through a life-size silicone replica of the brain’s blood vessels.
The core of the robotic thread is made from nickel-titanium alloy, or “nitinol,” a material that is both bendy and springy. Unlike a clothes hanger, which would retain its shape when bent, a nitinol wire would return to its original shape, giving it more flexibility in winding through tight, tortuous vessels. The team coated the wire’s core in a rubbery paste, or ink, which they embedded throughout with magnetic particles.
Finally, they used a chemical process they developed previously, to coat and bond the magnetic covering with hydrogel — a material that does not affect the responsiveness of the underlying magnetic particles and yet provides the wire with a smooth, friction-free, biocompatible surface.
They demonstrated the robotic thread’s precision and activation by using a large magnet, much like the strings of a marionette, to steer the thread through an obstacle course of small rings, reminiscent of a thread working its way through the eye of a needle.
The researchers also tested the thread in a life-size silicone replica of the brain’s major blood vessels, including clots and aneurysms, modeled after the CT scans of an actual patient’s brain. The team filled the silicone vessels with a liquid simulating the viscosity of blood, then manually manipulated a large magnet around the model to steer the robot through the vessels’ winding, narrow paths.

Atlantic Canada’s first hybrid operating room opens at Halifax hospital
A new, specialized operating room is now open at the Halifax Infirmary site of the QEII Health Sciences Centre.
The hybrid operating room, which has been described as “unique” by the Nova Scotia government, includes advanced diagnostic imaging equipment and is the first of its kind in Atlantic Canada.
The province says patients having cardiac or vascular surgery will be able to undergo diagnostic testing in the operating room if complications arise, which is safer for patients. Surgical teams can also use the room for minimally invasive procedures like inserting catheters, guidewires or balloons through small incisions. It can also be used for more complex, open surgeries.
“We’re excited to see the completion of this important infrastructure project that will greatly improve the delivery of health-care services,” MLA Colton LeBlanc said in a news release.
The Stewart E. Allen Hybrid Operating Room is named after the Truro-based business leader and philanthropist who donated $1 million to the project. It opened in October.
For Eileen MacGibbon, the vice-president of operations for Nova Scotia Health’s Central Zone, hybrid operating rooms are “the way of the future.”
“Most everything needed for the patient can be provided in one room; emergencies or complications can be handled in one location without needing to transport the patient between radiology and operating suites,” MacGibbon said in the release. “As well, a hybrid OR provides enormous flexibility in what surgeries can be performed.”
The opening of the hybrid operating room comes after the QEII Foundation raised $4 million to support medical equipment purchases.
