Scald Burn-COVID-19 Fallout by Vijay YB

Abstract
The COVID-19 pandemic has created a worldwide scenario of fear and anxiety. People are turning to unverified home remedies for prevention of the disease. Steam inhalation is used by the masses for prevention as well as treatment of COVID-19 without any proven data. Studies have shown steam inhalation to provide symptomatic relief in case of upper respiratory infection but has no significant effect on the viral load. Vessel containing hot water is commonly used for steam inhalation in developing countries, toppling off this vessel results in scald burn sometimes. During the period of lockdown, our hospital has treated an increase number of cases of scald burn due to steam inhalation. Our study focuses on evaluation of the scald burns occurring as complication of steam inhalation and its relation to increased fear of corona infection. 16 patients were treated during 6 months lockdown period from March-August 2020. Demographic data, mechanism of injury, treatment methods were evaluated. Patients with superficial second-degree burns were treated with regular dressing while those with deep second degree burns required collagen application. General awareness regarding the electrical steamers should be propagated to prevent scald burns cases associated with steam inhalation.

Introduction
The Corona Virus Disease (COVID-19) pandemic has affected communities worldwide resulting in widespread emotion of trepidation among the masses of catching the disease. There are profound misconceptions regarding the preventive measures circulated by the social media and unverified home-made tutorial video, one such misconception is of steam inhalation. Any global disease outbreaks in past has shown people suffering from pandemic related anxiety that elevates levels of stress [1], corona virus related anxiety has been referred to as ‘coronaphobia’ by many [2]. Steam inhalation is a common home remedy for the upper respiratory tract infections. It has been recommended by many general practioners and popularised by media. A survey has shown that 80% of general practioners recommend steam inhalation as a home remedy for common cold [3]. However, there is no concrete proof that the steam inhalation helps in coronavirus infection. A Cochrane review for use of steam inhalation concluded that although there was evidence that symptoms were relieved, there was no objective improvement in the outcome measures like viral shedding or viral titres in nasal washing [4]. Vessel containing boiling water with or without added herbal concoction is most commonly used for steam inhalation, however many a times toppling of this vessels results in scald burns over the face, torso, perineum and lower limbs. Other safe methods are electrical steamer, respirator and rhinotherm. Paediatric and geriatric population are at a greater risk. With the increasing fear of COVID-19, increase in number of steam inhalation has resulted in increased number of scald burn cases. Our study focuses on evaluation of the scald burns occurring as complication of steam inhalation and its relation to increased fear of corona infection.
Material and Methods
A retrospective study of patients coming to our hospital was conducted from March 2020 to August 2020. Details of the 16 patients admitted or treated on the outpatient basis for scald injury due to steam inhalation was obtained from the database. Informed consent was obtained from the participants as per the guidelines of the ethical committee of our hospital. Data assessed includes: Demographics, mechanism of burns, total burn surface area % (TBSA %), modality of treatment, need for surgery, duration of hospital stay, outpatient follow-up. Patients with superficial second-degree burns were treated with regular dressing done on alternate days with paraffin gauze dressing and saline. Patient with deep second degree burns were treated with dry collagen sheet of bovine origin covered with non-adherent and absorbent dressing. All patients were given broad-spectrum antibiotic coverage.
Results
During the period of study, 16 patients with scald burns due to steam inhalation were treated at our centre, of which 7 were male 43.7% and 9 were female 56.2%. Out of 16 patients, 5 were less than 16 years old Table 1. Mean age was 25.5 years (range 2-52 years). The most common area involved in pediatric age group was abdomen while in adults lower limbs were most commonly involved. The monthly incidence of scald burns due to steam inhalation during the covid-19 lockdown period of 6 months is an average of 2.7/month. Of the 16 patients, mean TBSA was 4.5% (range 1-9 %). 10 patients had superficial second degree burns while 6 patients had mixed superficial and deep second degree burns. The duration of treatment ranged from 10-22 days, 6 patients were treated as in-patient while 105 patients took treatment on outpatient basis. No difference in the burn wound in terms of depth, extent or treatment was noticed depending on the mode of injury. 10 patients having superficial burns were treated with regular dressing with paraffin gauze and saline; while 6 patients having mixed second degree burns underwent collagen application Figure 1. Blister formation required sterile rupture of blister followed by application of non-adherent dressing. Complete epithelization was demonstrated in all patients. Figure 2 Out of 16 patients, 13 patients were using steam inhalation as a prophylaxis for COVID-19 infection while 3 patient was using it as a part of the treatment as suggested by their doctor. None of the patients had any permanent scarring or contracture formation, 4 patients had hypopigmentation while 2 patients had hyperpigmentation over the healed burn area.
Discussion
Burn injuries are complex and needs meticulous management. Amidst the lockdown, coronaphobia [2] has led to an increase in number of scald burns cases as a side effect. More and more people are turning to home remedies for preventive methods. Although steam inhalation is not proven to be a protective mechanism against COVID-19 infection, it has been used by the population and suggested by many doctors for symptomatic relief for common cold [5]. In a developing country like ours the most common method of steam inhalation comprises of a vessel containing boiling or hot water, the patient sits beside the vessel and breathes in the vapor with a cloth overhead to contain these vapors. Toppling off this vessel sometimes results in scald burns. There is a general misconception in population that heat destroys the corona virus, breathing in the vapors more deeply will be more beneficial 6 and due to this misbelief steam is used for inhalation, the temperature of which is around 100 degrees Celsius. Scald burns comprise the most common type of burns in pediatric age group [6]. 3 out of 9 patients 33.33% belonged to pediatric age group in this series. The most common mechanism of injury was falling of the vessel while removing the overhead cloth or while carrying the vessel. The most common area involved was abdomen in paediatric age group and lower limbs in adults. Other involved sites were chest anterior, upper limb and perineum. In a study conducted by Sarah et al [3] the most common area involved was abdomen followed by perineum. 5 patients with superficial second degree burns required regular dressings while; 4 patients with mixed second degree burns required collagen application. Figure 2 Superficial burns epithelise on its own with regular dressing. The dressing should be non-adherent to protect the delicate epithelium. Collagen acts as a scaffold for fibroblast and neutralizes collagenase, it delivers collagen-1 to the wound bed which becomes a template for new tissue growth [7]. The average healing period was of 14.5 days. Since medical treatment in government-funded institute is free of cost in our country, the cost of treatment could not be assessed. However, it can be said that the total cost of treatment of these burn wounds is substantial as these injuries could be easily avoided. Our hospital treated an increased number of scald burns occurring due to steam inhalation this year compared to the same months in previous years, being a tropical country scald burns are rare during march-September. In England, half of the burn centers responding to a survey by Brewster et al [8] reported an increase in scald cases related to steam inhalation during COVID-19 pandemic. The traditional method of steam inhalation by leaning over a vessel is dangerous; however, the use of advanced equipment like mechanical steamer, rhinotherm can reduce the number of burns cases. Due to the COVID-19 lockdown people are turning to the traditional method of steam inhalation due their ignorance, fear and anxiety coupled with 7 unavailability of modern equipment [9] Many case reports and studies [9] have already tried to draw attention to the hazards of steam inhalation however this practice still persist.

With African swine fever wiping out a quarter of the world’s pigs, primarily in China, doctors and drugmakers around the world are sounding the alarm about a possible prolonged shortage of heparin, a critically important blood thinner. The drug, derived from pig intestines, is widely used to treat heart attacks and prevent deadly blood clots. China has been the primary source of the medicine, and the crisis there highlights a need to develop alternate supplies.
1. What is heparin?
Heparin is a naturally derived, sugar-based molecule that’s administered by injection or infusion to prevent blood from coagulating and causing vessel-blocking clots that can starve organs of critical oxygen. Discovered more than a century ago, today some 10 million to 12 million people in the U.S. alone are treated with heparin each year, with the drug’s global sales exceeding 200 tons, or $5 billion, annually. A dose though is inexpensive, costing less than a packet of bandages.
2. Why is it important?
It’s routinely prescribed for heart attack patients with clogged arteries, patients undergoing major orthopedic and cardiac surgery, and patients with a central venous catheter or receiving a blood transfusion. Recipients of certain life-saving treatments such as blood-cleaning kidney dialysis or mechanical heart and lung support receive it, as do patients who are immobile for long periods. Heparin is often used to coat materials such as coronary stents that are in contact with blood. It’s the preferred anticoagulant for hospitalized patients because it works rapidly, can be carefully administered and there’s an effective antidote for it: protamine sulfate. A study by the Royal Children’s Hospital in Melbourne found as many as 15% of patients are exposed to heparin in some form on a daily basis.
3. What’s the concern?
Researchers are warning of an “imminent risk” of a global shortage because of the unprecedented scale of the cull. Most heparin-based drugs are sourced from the intestinal lining of slaughtered pigs. About 60% to 80% of the active key ingredient is prepared by companies in China and formulated into ready-to-use drugs by pharmaceutical companies around the world. China, home to half the planet’s pigs, is the only country that can meet the global demand for the raw material. The World Health Organization recommends governments stockpile heparin as an essential medicine.

4. What’s the supply like now?
A U.S. congressional committee asked the Food and Drug Administration in July to monitor heparin supplies, noting a six-to-nine-month lag time before a shortage could affect the U.S. market. The agency said in October that there had been no impact. That might be because the outbreak caused a pig-killing frenzy that initially yielded plenty of heparin. However, as the number of hogs slaughtered in China falls, so too will the volume of the raw material. Drugmakers including Germany’s Fresenius SE and South Africa’s Aspen Pharmacare Holdings Ltd. have mentioned shortages linked to rising prices. Fresenius’s Kabi unit told its U.S. customers in July that it put heparin on a protective allocation list due to a potential shortage. Massachusetts General Hospital warned in August of an impending global shortage; the chief of its emergency preparedness team said stock levels were so low at one point that the Boston center was two weeks away from having to cancel lifesaving cardiac surgery.
5. Are there other sources of heparin?
Yes. Heparin can be extracted from other farm animals and was historically prepared from cow lung and sheep intestines. The outbreak of mad cow disease in Europe in the 1990s made pigs a preferred source in many countries. The FDA has encouraged the development of bovine-sourced heparin as an alternate to pigs, though products from other sources may require rigorous testing to obtain regulatory approval, which could result in delays in their availability. Alternative blood-thinners aren’t typically as safe, effective or cheap.
6. Has this happened before?
Yes, though the current African swine fever outbreak is on a much bigger scale, heightening quality and supply concerns. The most recent shortages reported by the FDA involved Baxter International Inc. and Hospira Inc., now part of Pfizer Inc., and date to 2017, when Hurricane Maria disrupted manufacturing in Puerto Rico. In Australia, Pfizer advised authorities in November of a temporary shortage of two lines of heparin owing to manufacturing issues unrelated to the swine fever in China. A 2006-2007 outbreak in China of porcine reproductive and respiratory syndrome, or blue-ear disease, slashed heparin supplies and allegedly prompted some adulteration of the raw material in China. U.S. and European regulatory officials determined a cheaper substance derived from shark cartilage was added to mimic heparin’s anticoagulant properties. That tainted product made its way into the global supply chain, killing 149 people in 11 countries, including at least 81 in the U.S. Baxter recalled its heparin injection products in the U.S. in 2008 but faced more than 1,000 related lawsuits. China never formally accepted responsibility. In October 2018, the Italian Medicines Agency found serious manufacturing violations during an inspection of a heparin plant in Sichuan, China, and recommended the facility be banned from supplying the drug ingredient to the European Union.

In Coronavirus, China Weighs Benefits of Buffalo Horn and Other Remedies

As it races to treat patients infected with the new coronavirus, the Chinese government is seeing potential in a cocktail of antiviral drugs. It is also recommending the Peaceful Palace Bovine Pill, a traditional Chinese medicine made with the gallstone of cattle, buffalo horn, jasmine and pearl. There is no known cure for the coronavirus that has sickened more than 28,000 people and killed 563 in China. The country’s National Health Commission says doctors should try treating patients mainly with a combination of Western drugs used to treat HIV and fight viruses, depending on the severity of illness. But the government is also looking at ways to supplement the treatment with remedies that are integral to its national identity — traditional Chinese medicine. It has its supporters. “I think it is the correct approach,” said Cheng Yung-chi, a professor of pharmacology at Yale University School of Medicine. “The evidence is going to come and we have to give it the benefit of the doubt.” There is no clinical proof that the roots of various plants, licorice, and the Peaceful Palace Bovine Pill can help combat the deadly disease. Practitioners say the regimen could help ease symptoms such as swelling in the lungs, with fewer side effects. Critics say the use of such concoctions could raise concerns about patient safety.

The use of these ancient Chinese remedies dovetails with a push by Xi Jinping, China’s top leader, to harness them as a source of national pride. He has said that officials should place as much importance on traditional Chinese medicines as they do on Western medicines. His government has called for the remedies to be promoted in its “Belt and Road” trade route, China’s push to build ports, rail lines and other infrastructure around the world. In turning to traditional medicine, China is relying on past experience. During the severe acute respiratory syndrome, or SARS, outbreak in 2002 and 2003, doctors found that steroids prescribed to reduce inflammation had harmful side effects such as bone destruction. Chinese medicine, they said, would mitigate some of these adverse reactions. In its treatment plan for the coronavirus released on Wednesday, the National Health Commission recommended traditional Chinese medicine remedies that could be used with antiretroviral H.I.V. drugs like Lopinavir and Ritonavir. The national health department suggested trying the Peaceful Palace Bovine Pill for severe symptoms such as wheezing and respiratory distress. Some hospitals are already using a combination of Western and Chinese medicines. In recent weeks, Beijing’s health department reported that two patients who were discharged had been treated with traditional Chinese medicines together with other unspecified drugs. And in Guangzhou, a major city in the south, health officials said 50 patients reported having no more fever and half of them said their coughs went away after using traditional Chinese medicines and other drugs.

Kwok for The New York Times Doctors are conducting clinical trials to test the efficacy of traditional Chinese medicine in Wuhan, the epicenter of the coronavirus outbreak, said Dr. Cheng, the expert at Yale, who is also chairman of the Consortium for the Globalization of Chinese Medicine, a group of academics in the field. Jiang Xianfeng, a traditional Chinese medicine practitioner at United Family Health, a top hospital for the affluent in Beijing, said these medicines are safe, effective and easy to get. “Western medicine does not have better answers to this virus,” Mr. Jiang said. “The Chinese people have experienced these sort of plagues many times in our thousands of years of history. If traditional Chinese medicine was not effective, the Chinese people would already be destroyed.” After the SARS outbreak, the World Health Organization studied the use of traditional Chinese medicine during that period and determined that they were safe and showed some potential in relieving symptoms such as fatigue and shortness of breath. Researchers from the United States and Taiwan found that certain herbs could suppress the virus if prescribed at specific concentration levels, while other studies said their findings were inconclusive. That scientific uncertainty is not stopping the Chinese government. Since the second version of its treatment plan, the health commission has added more traditional Chinese medicines to the mix. The authorities in Wuhan say coronavirus patients with light or moderate symptoms should be treated with traditional Chinese medicine, the state-run Beijing News reported. Zhu Mao, a representative of a traditional Chinese medicine manufacturer in Hubei, said he was producing more than 20,000 prescriptions per day at the request of the government. On Jan. 25, the State Administration of Traditional Chinese medicine dispatched 25 teams of Chinese healers to Wuhan.

Proponents of traditional Chinese medicine say the herbs were misused. Still, Nature argued in a 2017 editorial: “Hundreds of years of use in clinics that don’t standardize or analyze the clinical data are no match for blinded, controlled studies.”

Chen Xi, an assistant professor of health policy and economics at the Yale School of Public Health, urged the Chinese news media to be more careful in its reporting, saying that the potential for traditional Chinese medicine to inhibit a virus is not equivalent to a form of prevention or treatment. “If incidents like Shuanghuanglian happen again and again, it will lose the public trust, cause more panic, and go against the authorities’ advice to avoid crowd gathering,” Mr. Chen wrote. Qin Xi, a manager of a pharmacy in Beijing, said sales of some of his traditional Chinese medicines, including “Shuanghuanglian,” have been off the charts. “As for whether it works, who knows?” he said.

In Coronavirus, China Weighs Benefits of Buffalo Horn and Other Remedies
As it races to treat patients infected with the new coronavirus, the Chinese government is seeing potential in a cocktail of antiviral drugs. It is also recommending the Peaceful Palace Bovine Pill, a traditional Chinese medicine made with the gallstone of cattle, buffalo horn, jasmine and pearl.
There is no known cure for the coronavirus that has sickened 24,324 people and killed 490 in China. The country’s National Health Commission says doctors should try treating patients mainly with a combination of Western drugs used to treat HIV and fight viruses, depending on the severity of illness.
But the government is also looking at ways to supplement the treatment with remedies that are integral to its national identity — traditional Chinese medicine. It has its supporters.
“I think it is the correct approach,” said Cheng Yung-chi, a professor of pharmacology at Yale University School of Medicine. “The evidence is going to come and we have to give it the benefit of the doubt.”
There is no clinical proof that the roots of various plants, licorice, and the Peaceful Palace Bovine Pill can help combat the deadly disease. Practitioners say the regimen could help ease symptoms such as swelling in the lungs, with fewer side effects. Critics say the use of such concoctions could raise concerns about patient safety.
The use of these ancient Chinese remedies dovetails with a push by Xi Jinping, China’s top leader, to harness them as a source of national pride. He has said that officials should place as much importance on traditional Chinese medicines as they do on Western medicines. His government has called for the remedies to be promoted in its “Belt and Road” trade route, China’s push to build ports, rail lines and other infrastructure around the world.
In turning to traditional medicine, China is relying on past experience. During the severe acute respiratory syndrome, or SARS, outbreak in 2002 and 2003, doctors found that steroids prescribed to reduce inflammation had harmful side effects such as bone destruction. Chinese medicine, they said, would mitigate some of these adverse reactions.
In its treatment plan for the coronavirus released on Wednesday, the National Health Commission recommended traditional Chinese medicine remedies that could be used with antiretroviral H.I.V. drugs like Lopinavir and Ritonavir. The national health department suggested trying the Peaceful Palace Bovine Pill for severe symptoms such as wheezing and respiratory distress.
Some hospitals are already using a combination of Western and Chinese medicines. In recent weeks, Beijing’s health department reported that two patients who were discharged had been treated with traditional Chinese medicines together with other unspecified drugs. And in Guangzhou, a major city in the south, health officials said 50 patients reported having no more fever and half of them said their coughs went away after using traditional Chinese medicines and other drugs.
Doctors are conducting clinical trials to test the efficacy of traditional Chinese medicine in Wuhan, the epicenter of the coronavirus outbreak, said Dr. Cheng, the expert at Yale, who is also chairman of the Consortium for the Globalization of Chinese Medicine, a group of academics in the field.
Jiang Xianfeng, a traditional Chinese medicine practitioner at United Family Health, a top hospital for the affluent in Beijing, said these medicines are safe, effective and easy to get.
“Western medicine does not have better answers to this virus,” Mr. Jiang said. “The Chinese people have experienced these sort of plagues many times in our thousands of years of history. If traditional Chinese medicine was not effective, the Chinese people would already be destroyed.”
After the SARS outbreak, the World Health Organization studied the use of traditional Chinese medicine during that period and determined that they were safe and showed some potential in relieving symptoms such as fatigue and shortness of breath. Researchers from the United States and Taiwan found that certain herbs could suppress the virus if prescribed at specific concentration levels, while other studies said their findings were inconclusive.
That scientific uncertainty is not stopping the Chinese government. Since the second version of its treatment plan, the health commission has added more traditional Chinese medicines to the mix. The authorities in Wuhan say coronavirus patients with light or moderate symptoms should be treated with traditional Chinese medicine, the state-run Beijing News reported.
Zhu Mao, a representative of a traditional Chinese medicine manufacturer in Hubei, said he was producing more than 20,000 prescriptions per day at the request of the government. On Jan. 25, the State Administration of Traditional Chinese medicine dispatched 25 teams of Chinese healers to Wuhan.
Traditional Chinese medicine teaches that disease arises from imbalances in the body and that some people have “hot” constitutions, therefore making them vulnerable to fevers and inflammations. When someone gets a fever or a respiratory illness, it is not uncommon to have Chinese people say: “You are on fire.”
Practitioners have determined that the coronavirus is a “hot” disease, and therefore, medicines should be prescribed to “clear the heat.”
It is unclear how the science works behind these medicines. Compounding the problem: There is no standardization for these herbs and regulation is poor.
Traditional Chinese medicine has also been linked to the consumption of exotic wild meats, though it is unclear whether the eating of the meats is really based on the practice. But some people, particularly in the south, believe that the way to achieving a balanced body is through eating certain meats. Wildlife markets such as the Huanan Seafood Market in Wuhan, the site that is believed to be the origin of the coronavirus, proliferated because of the demand for such meats.
Laurie Garrett, a member of the World Economic Forum’s global health security advisory board who covered the SARS outbreak in China, said that using traditional Chinese medicine in the new coronavirus outbreak could be risky because the approach is not rigorously backed by research.
“There’s no such thing as dosing,” she said. “It’s like being a chef in a kitchen.”
The prestigious science journal Nature pointed out that dozens of women who had taken Chinese herbs as part of a weight-loss program ended up with kidney failure in the 1990s. Proponents of traditional Chinese medicine say the herbs were misused. Still, Nature argued in a 2017 editorial: “Hundreds of years of use in clinics that don’t standardize or analyze the clinical data are no match for blinded, controlled studies.”
Doctors in neighboring Hong Kong, a semiautonomous region of China, are not convinced that traditional Chinese medicines can help. “I am not trying to devalue their treatment, but this is not something we practice,” said Arisina Ma, the president of the Hong Kong Public Doctors Association.
In China, people have searched anxiously for ways to protect themselves. In many Chinese cities, some stood in line for hours to buy “Shuanghuanglian” — a herbal concoction that mixes flowers such as honeysuckle and forsythia, among other ingredients — after two government-backed research institutes published a study that said it was effective in preventing the coronavirus.
Chen Xi, an assistant professor of health policy and economics at the Yale School of Public Health, urged the Chinese news media to be more careful in its reporting, saying that the potential for traditional Chinese medicine to inhibit a virus is not equivalent to a form of prevention or treatment.
“If incidents like Shuanghuanglian happen again and again, it will lose the public trust, cause more panic, and go against the authorities’ advice to avoid crowd gathering,” Mr. Chen wrote.
Qin Xi, a manager of a pharmacy in Beijing, said sales of some of his traditional Chinese medicines, including “Shuanghuanglian,” have been off the charts. “As for whether it works, who knows?” he said.
