Deep inspiration breath hold level variability and deformation in locoregional breast irradiation

Smith PurposeThe purpose of this study was to evaluate the dosimetric effect of breath hold level variability and deformation on breast, chest wall, internal mammary chain (IMC) nodes, and heart.Methods and materialsLeft-sided post-lumpectomy (n = 12) and postmastectomy (n = 3) patients underwent deep inspiration breath hold (DIBH) and exhale breath hold (EBH) computed tomography (CT) scans. Forward-planned locoregional breast plans were created on the DIBH scan. Two effects were modeled assuming no setup uncertainties: residual motion within the gating window and systematically shallow breath hold levels (BHLs). Real-time position management (RPM) was used to monitor BHL at simulation and during treatment. The RPM data were scaled to simulate BHL variation within symmetric gating window widths of ±1, 3, 5, and 7 mm; the dosimetric impact of this motion was simulated in the treatment planning system. Systematically “shallow” BHL errors were modeled using deformable image registration to map the patient trajectory from DIBH to EBH (n = 12). The deformable vector fields were scaled to produce synthetic CT scans modeling patient position during breath holds 1, 3, 5, and 7 mm shallower than simulator BHL. The original treatment plans were applied to the synthetic CTs and dose was recalculated.ResultsAcceptable plan quality was maintained for most patients with motion within gating windows up to ±7 mm. Patients with shallow median BHLs experienced loss of coverage at simulated gating windows ±5 mm or larger. At systematic 3 mm shallow BHL error, 4/12 patients had clinical target volume IMC V80% < 99%; this increased to 11/12 patients at 5 mm. Change in heart dose from systematic BHL errors was negligible.ConclusionsMotion within gating windows has minimal dosimetric impact for most BHL variability; however, loss of IMC coverage can occur even for small gating windows when BHLs are systematically shallow. Look, I��m gonna rant big here so if you’re happy with what happened in this episode, I’m warning this is not a post for you.

In general lines was a great episode, I was, literally, at the edge of my seat during the whole thing.

And I really had a big hope that Tom would make it, even when I saw Liz a year later in that bed… Damn I hopped that Tom was out for a mission or something, but no! His dead cold body was in the morgue, so we could have no doubts that he was indeed DEAD!

Isn’t this fucking great!!??? Just when Liz was happy, the happy she never felt before, with someone she actually loved, they took that away from her.

And it hugely pissed me off, that some are opening champagne bottles, celebrating over Liz’s loss. She lost the man she loved the most, and there are people celebrating because now she can move on real quick into Red’s arms.

I’m pissed because Tom was a good character, he was, if you want, the lead's  “love interest” but he was so much deeper than that, he had a backstory, he had a good arc in the show, and I think that was so roughly cut off in this episode.

And I’m pissed because God! I wanted to see more of the son-in-law/father-in-law dysfunctional dynamic. That could have been really fun!

Talking about that, the silver lining here is, maybe, that Red wasn’t the one who killed him. If he would had done that, idk who the hell is in the suitcase, but it wouldn’t matter because Liz would never forgive her father for this.

Yes, I believe they are father and daughter, I knew it from day one, I knew his interest on her was going in that direction. I might been throw in other directions sometimes but this man saw Liz grow, and protected her even from himself. Now we had DNA, now we had scenes between them acting like father and daughter, don’t tell me now that they gonna say “mmmm no, wait, now i’m not your father, now I want to be your lover“ because that would be so… ugh, i don’t eve find the word! To me it would be the end of this.

About Liz romantic future, I wouldn’t mind if she find love again in certain blonde co worker, or idk, maybe a new character. Hey! I woudn’t mind if she end up alone, raising Agnes with her father, and her work/family.

Anyway Tom died knowing who’s in the suitcase, but we are still in the dark till January, at least.

The wait is gonna be loooooooong!!!

So, we have decided the other daughter is a holodaughter (Emergency Baby Hologram - EBH) named Elizabeth who is designed as the daughter primarily taking after Emmet, Steward and Lorian, but she's been programmed with traits from Emil, Ian and Enoch too, even though they're not able to be on Threll with them.
