Split Pleura Sign of Empyema. At least two empyemas (E) are seen in the right hemithorax. The empyemas insinuate themselves between the visceral (white arrows) and parietal (yellow arrows) pleurae. There is associated compression atelectasis for the largest empyema (red arrow).
Today, the fellow I'm working with was telling me that atelectasis draws the structures towards the side of atelectasis, whereas pleural effusion pushes the structures away from the side with the pleural effusion. So If you are looking at a CXR after you drain a pleural effusion that was on the left side and the trachea is still positioned more to the right side, it can be because there is atelectasis in the right lung, pulling to the right. Or if you drain a pleural effusion on the right and there is still opacity on that side, it can be beacuse there is atelectasis there as well.
Pleural effusions can be simple or complex. Complex pleural effusions have septa. Empyema is even worse than pleural effusion. You will either see pus when you drain it or you will get gram positive bugs when you culture the fluid from it.

SUMMARY AND RECOMMENDATIONS
●A parapneumonic effusion is a pleural effusion that forms in the pleural space adjacent to a pneumonia. When bacteria or other pathogens infect the pleural space, a complicated parapneumonic effusion or empyema may result. Parapneumonic effusions and empyema are common complications of pneumonia but an empyema can also develop without the presence of an adjacent pneumonia. The incidence is approximately two to three percent of all pneumonias but appears to be rising.
●Parapneumonic effusion and empyema may be part of a developmental spectrum that includes three stages (table 2) [stage 1 = small, free-flowing fluid which resolves with antibiotics; stage 2 = organization and loculations; stage 3 = thickened fibrinous coating]. Stage 1 is typically an early stage where fluid is free-flowing and small, such that resolution occurs with antibiotics alone. In some cases, patients will progress to stage 2, a fibrinopurulent stage where infection precipitates organization and loculations. Stage 3 is characterized by a thickened fibrinous coating that may limit lung expansion.
●The microbiology of complicated pleural effusions and empyema vary with source of infection, route of acquisition, local epidemiology and patient-specific risk factors (table 3). Parapneumonic pleural space infections typically result as complications of community acquired pneumonia (CAP) or aspiration pneumonia. Those associated with CAP tend to be caused by the pyogenic bacteria that cause CAP (eg, S. pneumoniae and S. aureus) whereas those associated with aspiration pneumonia tend to be polymicrobial and caused by oral streptococci and anaerobes. The list of pathogens associated with nonparapneumonic pleural space infections is more extensive and varies by source.
●The clinical findings of a parapneumonic effusion and empyema are nonspecific and, with the exception of decreased fremitus, overlap with those of pneumonia. Among patients with pneumonia, the presence of risk factors (eg, aspiration, poor dental hygiene, alcohol or intravenous drug abuse, immunosuppression, young or old age, partially-treated pneumonia, and gastroesophageal reflux) and the development of a persistent or new fever despite appropriate antibiotics should heighten the suspicion for the development of a parapneumonic effusion or an empyema.
●In all patients with pneumonia, the imaging modality that defined the pneumonia should be re-examined for evidence of pleural fluid. Once a pleural effusion is suspected on chest radiography, ultrasonography is typically performed at the bedside to evaluate the nature of the effusion and feasibility of sampling or drainage. Computed tomography is generally additionally performed when complications (eg, loculations) are suspected, complex interventions are planned, and/or more detail of the underlying anatomy is expected to help with management.
We have an elderly pt today who has a pleural effusion, which we looked at with bedside ultrasound. It was a complex pleural effusion because we saw septations in it. Because of the septations, thoracentesis may not be able to get the fluid.
●Most cases of suspected parapneumonic effusion or empyema should be sampled under ultrasound guidance unless the effusion is too small or sampling is deemed unsafe. In some cases, therapeutic thoracentesis is simultaneously performed when symptomatic relief is needed or when drainage is indicated (eg, frank pus is observed). Fluid should be sent for cell count and differential, chemistries (protein and lactate dehydrogenase [LDH]), Gram stain and culture with additional inoculation of fluid into blood culture bottles (aerobic and anaerobic), pH (drawn directly into an arterial blood gas syringe and analyzed within one hour), and cytology. Special stains and cultures should be requested when unusual organisms or organisms that require special culture conditions are suspected.
●Several conditions need to be entertained when evaluating a suspected parapneumonic effusion or empyema including other causes of an exudative effusion (table 4), pleural fluid acidosis (eg, malignancy, tuberculosis, rheumatoid pleurisy, lupus pleuritis, urinothorax, or infused saline), and pleural masses or lung abscesses.
●In most patients, the diagnosis of a parapneumonic effusion and empyema is made by a constellation of clinical signs and symptoms with confirmatory chemical and microbiologic data from pleural fluid (table 2). It is rare that pleural biopsy is required. In general, we and others categorize parapneumonic effusions as the following (see 'Diagnosis' above):
•A parapneumonic effusion is considered uncomplicated (simple) when the effusion is free-flowing and too small to sample or when a small free-flowing effusion has a neutrophilic exudate (an elevated protein level greater than 0.5 that in serum and/or a LDH level greater than 0.6 that in serum) with normal pH and glucose levels and does not contain micro-organisms. These effusions typically resolve with antibiotics although the threshold to reimage should be low to look for disease progression.
•A parapneumonic effusion is considered complicated when an exudative effusion has any one or more of the following characteristics: has a pH less than 7.2 (or a low glucose level less than 40 mg/L in the absence of pH data), and/ or contains evidence of micro-organism invasion by culture or Gram stain. It is typically large, loculated, and has associated pleural thickening. An empyema is considered present when frank pus is seen on pleural fluid sampling.
From "Management and prognosis of parapneumonic pleural effusion and empyema in adults:"
●All patients with suspected (or diagnosed) parapneumonic effusion or empyema should be treated with antibiotics. Antibiotic therapy should be administered promptly and not delayed for sampling or drainage procedures.
•Empiric antibiotic selection varies based upon the site of acquisition (ie, community versus hospital-acquired), severity of illness, local epidemiology, and patient risk factors for drug-resistant pathogens or infection with other specific organisms. For complicated parapneumonic effusions and empyema, empiric regimens should generally include antibiotics that target anaerobes (table 1) and other likely pathogens (eg, streptococci if community-acquired; methicillin-resistant Staphylococcus aureus [MRSA] and Pseudomonas if hospital-acquired).
•For infections in the pleural space that are acquired in the community, we suggest use of a third-generation cephalosporin (eg, ceftriaxone or cefotaxime [3rd]) plus metronidazole or single agent therapy with a beta-lactam/beta-lactamase inhibitor combination (eg, ampicillin-sulbactam) (Grade 2C). For those who are penicillin allergic who cannot tolerate cephalosporins, we use single agent therapy with a carbapenem (eg, imipenem, meropenem) or combine metronidazole with either a fluoroquinolone (eg, levofloxacin) or a monobactam (eg, aztreonam).
•For hospital-acquired infections (eg, healthcare-associated pneumonia or post procedural empyema), we suggest combining vancomycin with metronidazole and an antipseudomonal cephalosporin (eg, cefepime, ceftazidime) or combining vancomycin with a beta-lactam/beta-lactamase inhibitor (eg, piperacillin-tazobactam, ticarcillin-clavulanate [limited supply]) (Grade 2C). For those who are penicillin-allergic who cannot tolerate cephalosporins, we suggest combining vancomycin with metronidazole and an antipseudomonal quinolone (eg, ciprofloxacin); alternatively, combining vancomycin with an antipseudomonal carbapenem (eg, imipenem or meropenem) is appropriate.
•The duration of antibiotic therapy for parapneumonic effusion and empyema is individualized. For most patients, we suggest continuing antibiotic therapy until there is clinical and radiographic improvement. In general, for self-resolving parapneumonic effusions, therapy may last one to two weeks, while therapy for complicated parapneumonic effusions and empyema are often longer (eg, two to three weeks for a complicated parapneumonic effusion and four to six weeks for empyema).
●Patients with uncomplicated parapneumonic effusions (ie, typically small or moderate sized, free-flowing effusion (table 3)), do not generally require drainage and are treated with antibiotics alone (algorithm 2 and algorithm 1). This approach is based upon the rationale that most of these effusions do not have direct micro-organism invasion of the pleural space and resolve with antibiotics alone. Should patients or the effusion not improve, repeat imaging with chest computed tomography (CT) should be performed to evaluate for the development of a complicated parapneumonic effusion that may need to undergo sampling and drainage.
●For patients with a complicated parapneumonic effusion, we suggest prompt drainage of pleural fluid in addition to antibiotic therapy (Grade 2C). A complicated parapneumonic effusion is one that has gram microbiologic or biochemical evidence of infection (algorithm 2). Although high quality data are lacking to guide selection of patients for drainage, such patients appear to be at risk for poorer outcomes including an increased requirement for more than one procedure, eventual need for surgery, and longer hospitalization. For patients with empyema, we recommend antibiotics and prompt drainage together with antibiotic treatment (Grade 1C), based upon the rationale that among those with parapneumonic effusion, those with empyema have the highest mortality.
•The initial procedure of choice is a single tube or catheter thoracostomy. Image-guided placement of a catheter(s) may be needed when pleural loculations prevent adequate drainage by a single tube. During the next 24 to 48 hours, patients should be followed clinically (signs and symptoms, laboratory values, and drainage volume) and radiologically with chest CT to assess the response to drainage (degree of evacuation and re-expansion).
•For those who clinically improve and in whom imaging reveals significant improvement or resolution of the effusion with adequate re-expansion of the lung, and the drainage rate has fallen to below 50 to 100 mL/day for two to three days, the chest tube or catheter can be removed, antibiotics continued, and the patient can be followed up as an outpatient in two weeks with repeat imaging.
•For patients who fail antibiotics and thoracostomy drainage from a well-placed chest tube (eg, the effusion persists or worsens, fever persists or new fever develops, persistent or worsening leukocytosis), attention should be paid to reassessing antibiotic coverage and need for additional drainage options.
-For patients with evidence of ongoing or worsening infection, options include re-culturing pleural fluid directly from the pleural space or undrained locule (not from the tube or catheter) and adjustment of antibiotic coverage ensuring adequate anaerobic coverage or coverage of resistant organisms. Consideration should also be given to the presence of underlying necrotic lung as a focus of infection.
-For patients who require additional drainage, options include nonsurgical approaches using intrapleural tissue plasminogen activator/deoxyribonuclease (tPA/DNase) and/or placement of an additional drainage tube(s), OR surgery, typically video-assisted thoracoscopic surgery (VATS). Choosing among the options is often provider-specific and recommendations vary among clinicians, institutions, and guideline committees. Factors that influence the decision include the clinician’s specialty, expertise availability, patient values, patient prognosis from comorbidities, and candidacy for select surgical procedures as well as the number and size of locules and degree of pleural thickening on chest CT.
-As an initial strategy, we suggest intrapleural administration of a combination of tPA 10 mg and DNase 5 mg, twice daily for three days rather than surgery (Grade 2C). The rationale for this approach is based upon data that report lower rates of referral for surgery (by 30 to 80 percent) when tPA/DNase are used. Drainage of residual locules is also appropriate, especially in those who fail tPA/DNase therapy. A repeat chest CT should be performed 24 to 48 hours after the chosen intervention(s) to evaluate the response.
-Patients who fail tPA/DNase and/or additional drainage procedures should be evaluated for VATS. (See 'Video-assisted thoracic surgery (VATS)' above.)
•In patients refractory to nonsurgical therapy who are not candidates for or decline VATS, options are limited but may include persisting with conservative care (antibiotics and multiple drains) and in some cases, an open-window thoracostomy can be performed.
●Patients with an infected pleural space following resective thoracic surgery (ie, lobectomy or pneumonectomy) require special attention since infection is often associated with a bronchopleural fistula that may also require surgical or endoscopic closure. Patients with malignant pleural effusion and an indwelling catheter who have an infected pleural space should be treated conservatively with continued indwelling catheter drainage and a prolonged course of antibiotics.
●Apart from complications associated with medical and surgical treatments, long-term complications of infection in the pleural space include residual pleural thickening on chest CT and rarely, when, untreated, fibrothorax and pleural calcification. The pleural changes usually resolve over three to six months and patients should not be considered for decortication unless they fail to improve and are symptomatic after six months. Rare complications include bronchopleural fistula formation, empyema necessitans (rupture of the empyema through to the chest wall), and pleural lymphoma.
●The long-term survival of patients with parapneumonic effusion and empyema is good, provided therapy is adequate and prompt. Mortality is highest in those with empyema as well as in those who require open surgery or decortication. Death is highest during the first 400 days and is often due to comorbid conditions or surgical complications rather than due to the empyema itself.

The choice of needles should be based on two factors such as the type of injection and the depth of penetration. In the above paragraph, patients are unable to tell the difference between 25-, 27,-, and 30-gauge needles. The needles that are larger gauge (25 or 27 gauge) are more resistant to deflection and breakage. This allows for greater precision in reaching the desired target. The larger lumen makes it easier to aspirate this is a benefit.
For all injections, the 25-gauge needle is the recommended. However it is strongly recommended for injections that have a high probability of positive aspiration, and/or require deep penetration. The needle of 27 gauge is reserved for other injections where depth of penetration and risk of positive aspiration are not significant. The 30-gauge needle is not suggested for blocks or supraperiosteal injections however, it can be beneficial to treat localized infiltration.
Needle bevel
The bevel is the slanted surface of a needle. It creates the tip and allows access to tissues that is non-traumatic. Manufacturers can use the term "bevel" in a variety of ways.
