Does Commoragh have a sky? Is there an up and down?
Commorragh has the equivalent of sky (skies, to be exact), yeah. It's a truly gigantic dimension populated by the intersection of other gigantic dimensions across space, all of it lit with suns the Drukhari nicked. So, y'know, there's altitude, there's atmosphere. It's not organic or consistent, per se, but it's there. The concept of "the sky" is very much a thing.
As for "up and down", yes, there's an up and down. There are a lot of ups and downs, in many directions. What you personally perceive as up and down will depend on where you are.
Say you have an enormous tower in Commorragh that is connected to it via one of many satellite realms. Scourges live up at the top of the tower, where it's drafty and they can look down on people and do aerial crimes. At the base of the tower, under the ground, there's a haemonculi coven doing incredibly unlicensed medical work. To them, and to the people that go there or live there, the top of the tower is "up" and the base of the tower is "down". However, if you were to pull back and look at its spatial position in Commorragh as a whole, it could be sitting at a 45 degree angle from the top of the greater dimensional space like a weird stalactite.
Does this matter to the Drukhari that go there or live there? No. They've made it so that up and down are what they say it is, where they say it is, and if you don't like it you can hand over all your skin.
I've attached two of the more famous Commorragh arts pieces as a visual aid also
