Dépassée par l’ampleur de l’épidémie, l’Italie compte sur la chloroquine... Le point de vue du Pr Christian Perronne, chef du service d’infectiologie à l’hôpital universitaire de Garches

En Italie, qui nous précède de seulement huit jours et ou la situation devient presque incontrôlable, une course de vitesse contre le Covid-19, vient d’être enclenchée avec pour arme clé l’hydroxychloroquine (Plaquenil®, forme la mieux tolérée de la chloroquine). Tout particulièrement dans le Lazio, région très touchée, où elle est désormais largement prescrite aux patients ayant des signes débutants, avec d'ores et déjà des résultats très encourageants en termes de protection des sujets traités. Dans le journal « Corriere della Sera » le Dr Pierluigi Bartoletti, vice président de la Fédération Italienne des médecins généralistes, explique que toute personne isolée, Covid-19 positive avec des signes débutants (toux, fièvre par exemple), est aujourd’hui traitée par l’antipaludique. Le produit est remboursé par l’assurance maladie italienne. Il peut être obtenu facilement sur prescription d’un médecin (généraliste notamment), dans les pharmacies hospitalières, qui transmettent toutes les données relatives au patient à l’Agence italienne du médicament (AIFA).

"Nous venons de comprendre, ajoute le Dr Bartoletti, que le virus a une évolution en deux phases et que c’est au cours de la seconde, après quelques jours (une semaine environ), que la situation peut brutalement, en 24 ou 48h, s’aggraver et conduire à une insuffisance respiratoire réclamant des soins intensifs. Les résultats que nous commençons à accumuler suggèrent que l’hydroxychloroquine administrée tôt, donne la possibilité d’éviter cette évolution à une majorité de patients et de désencombrer les salles de réanimation"

Le point de vue du Pr Christian Perronne, chef du service d’infectiologie à l’hôpital universitaire de Garches, interrogé par le Dr Philippe Gorny :

Paris Match. Participez-vous à l’Etude Européenne Discovery qui évalue des traitements antiviraux ? Pr Christian Perronne. J’ai refusé d’y participer car cette étude prévoit un groupe de patients sévèrement atteints qui ne seront traités que symptomatiquement et serviront de témoins-contrôles face à quatre autres groupes qui recevront des antiviraux. Ce n’est pas éthiquement acceptable à mes yeux. On pouvait parfaitement, dans la situation où nous sommes, évaluer ces traitements en appliquant un protocole différent. De surcroît, le groupe hydroxychloroquine (qui a été ajouté à cette étude à la dernière minute), devrait être remplacé par un groupe hydroxychloroquine plus azithromycine, traitement de référence actuel selon les données les plus récentes. Enfin, le modèle de protocole choisi ne fournira pas de résultats avant plusieurs semaines. Pendant ce temps là, l’épidémie galope. Nous sommes pressés, nous sommes en guerre, il faut des évaluations rapides.

Etes-vous pour où contre une prescription plus large de l’hydroxychloroquine que celle actuellement autorisée en France ? Même si les preuves irréfutables que donnent les études randomisées à grande échelle manquent encore, je suis en faveur d’une prescription large pour les raisons suivantes : 1. Nous disposons d’un nombre important de preuves montrant qu’in vitro l’hydroxychloroquine bloque le virus. Nous disposons également de plusieurs résultats cliniques indiquant que ce produit est bénéfique s’il est administré précocement et nous n’avons aucune mention qu’il nuit ou est dangereux dans cette infection (une seule étude, mal détaillée, chinoise, sur 30 patients avec groupe contrôle, n’a pas observé de bénéfices mais pas non plus d’effets délétères). Que risque-t-on en administrant la chloroquine d’emblée : rien ! 2. Ce médicament est très peu coûteux 3. Il est bien toléré en traitement longue durée (recul de plusieurs décennies sur des milliards de sujets). Personnellement, je l’utilise en clinique dans la forme chronique de la maladie de Lyme, avec succès depuis 30 ans à la dose de 200 mg voire 400 mg/jour. J’ai pu juger et des centaines d’autres médecins avec moi, de son excellente tolérance chez l’homme. Les principales contre-indications sont les maladies sévères de la rétine et cardiaques non équilibrées. Les évènements cardiovasculaires restent exceptionnels si on prend soin : de proscrire l’automédication - de vérifier chez les personnes âgées prenant beaucoup de médicaments qu’il n’existe pas d’interactions médicamenteuses (avec des diurétiques au long cours notamment) et que le taux de potassium sanguin est dans les normes. Hors ces précautions, les effets indésirables sont mineurs. Ils le sont d’autant plus que le traitement est court, ce qui est le cas contre Covid-19. Il serait donc sage de produire sans plus tarder de l’hydroxychloroquine en très grande quantité, pour la rendre facilement accessible aux personnes infectées, dès lors que son efficacité sera définitivement prouvée. A défaut il suffira d’arrêter la production. Je signale que l’Italie vient d’autoriser la large diffusion de l’hydroxychloroquine sur prescription médicale dès le début de l’infection et que d’autres pays s’apprêtent à faire de même. Qu’attendons-nous ? D’avoir davantage de morts ?

Etes-vous au courant d’études en cours, avec ce produit en France ou ailleurs ? Les résultats d’une 2e étude ouverte du Pr Didier Raoult, chez 80 patients Covid-positifs en PCR, ayant des signes pulmonaires cliniques et radiologiques, viennent de paraître. Ils confirment les effets spectaculaires de sa première étude (avec le traitement combinant l’hydroxychloroquine et l’azithromycine), ainsi que ceux de la première étude chinoise sur 100 patients. Son équipe a constaté qu’au 5e jour, 97.5 % des patients ne sont plus porteurs du virus! Un seul décès concernant un patient âgé de 86 ans est survenu ; un autre sujet âgé de 74 ans est en soins intensifs.

Aux USA le Dr Zev Zelenko qui travaille dans deux centres hospitaliers des communautés juives orthodoxes de la région de New-York (au sein desquelles le taux d’infection est élevé), a administré depuis le 19 mars, à 500 personnes, sans test PCR préalable (pour ne pas perdre de temps), la combinaison du Pr Raoult à laquelle il a ajouté du zinc qui freine la réplication virale. Les patients traités furent : les adultes de tous âges ayant des signes débutants (toux, fièvre, courbatures..) avec une comorbidité associée (diabète, obésité, maladie pulmonaire, cardiovasculaire, cancer, immunodépression) - les adultes de tous âges ayant des signes débutants, sans comorbidité - tous ceux, quelque soit l’âge, ayant des signes pulmonaires (essoufflement ou davantage). Seuls les sujets jeunes avec signes débutants mais aucune comorbidité, ne furent pas traités. Au 26 mars aucune des personnes ayant reçu cette thérapie n’a été hospitalisée, aucune n’a évoluée vers une forme grave nécessitant une réanimation, aucune n’est décédée. Ce n’est pas une étude certes, mais ces résultats ont une valeur indicative notable. En marge de cette initiative communautaire, des essais institutionnels préventifs (protection des gens exposés avant même l’apparition de signes débutants) avec l’hydroxychloroquine, vont débuter aux USA !

Dans quels délais pouvons-nous espérer avoir les premiers résultats de l’étude Discovery ? Dans plusieurs semaines selon moi. Mais il n’y aura rien à espérer de cette étude concernant l’hydroxychloroquine qui est administrée trop tard. Le protocole indique que le produit peut être donné seulement si la saturation en oxygène des patients est inférieure à 95% , c’est à dire à des malades justiciables d’un apport massif en oxygène ou qui doivent être mis sous ventilation artificielle. Ce n’est pas la bonne indication.

Quel est votre avis sur les traitements qui pourraient s’avérer les plus efficaces dans les formes sévères et graves? Des médicaments autres que les antiviraux et que l’hydroxychloroquine et qui peuvent lutter contre l’inflammation broncho-pulmonaire , c’est à dire des anti-inflammatoires (non stéroïdiens exclus car délétères) tels que : des anticorps monoclonaux contre les interleukines (IL6, IL1) substances qui stimulent l’inflammation, ou contre des facteurs de croissance vasculaire (VGEF) – peut-être aussi des corticoïdes comme par exemple la methylprednisolone ( nette réduction de la mortalité pour les formes très graves dans une étude chinoise). Toutes ces options sont en cours d’évaluation. Ce qu’il faut éviter, c’est d’arriver à ce stade !

Que pensez-vous de la pénurie des masques en France… et de son inimaginable persistance ? J’ai présidé pendant 15 ans la Commission des maladies transmissibles des instances officielles de conseil, dont le Haut Conseil de la Santé Publique. Nos groupes de travail ont recommandé des mesures concernant la production de masques notamment lors des épidémies dues au SARS. Cette pénurie est absolument navrante et incompréhensible. A Hong-Kong, une des villes les plus denses du monde, où le port des masques en épidémie est traditionnel pour ne pas dire culturel, on a compté, à la date du 17 mars, un total que quatre décès dus à Covid-19 ! Là-bas, contrairement à la France où la pénurie a contraint le Ministère de la Santé précédent à ne le recommander que pour les sujets malades, le port des masques concerne aussi les gens sains ! On mesure là toute l’importance de cette protection et l’impréparation choquante des autorités françaises en ce domaine…

Ieri sera, domenica 12 agosto 2018, a Seborga, si è svolta, con successo di pubblico, presso l’Agriturismo Monaci Templari di Emanuela Rebaudengo e Flavio Gorni, una serata culturale, musicale e degustativa per la promozione del Ponente Ligure. Alla presenza dell’Assessore al Turismo della Regione Liguria Gianni Berrino è stato svelato un busto del poeta Francesco Biamonti e sono state lette le sue poesie, simbolo dell’entroterra del ponente ligure. Il busto, opera dello scultore David Maria Mariani, è stato donato dal Comune di San Biagio della Cima. La serata è proseguita con la presentazione del libro dello scrittore imperiese Luca Barozzi “Oltre il sogno…il vento” e la presentazione del libro dello scrittore Ugo Moriano “Attacco dal Cielo, droni”. ll Consorzio ‘InRiviera’ ha presentato i prodotti tipici dell’Unione dei 18 comuni del Comprensorio, promuovendo le specialità della nostra ricca terra, come “La Cipolla Egiziana”, una cipolla che era molto apprezzata da Libereso Guglielmi, l’esclusivo olio di Lucinasco, i tarallucci di Seborga e la michetta di Dolceacqua. Ad allietare ancor di più l’evento, l’esibizione musicale al pianoforte della giovane e talentuosa di Bordighera, pianista e compositrice, Veronica Rudian.

Everything else had fallen into place for the Christian retreat he planned to build here. Just up the road was the “evangelical triangle” of Capernaum, Chorazin and Bethsaida, the villages where, according to the Gospels, Jesus mesmerized crowds with his miraculous acts and teachings. Across the modern two-lane highway was a small town Israelis still call Migdal, because it was the presumed site of Magdala, the ancient fishing city that was home to Mary Magdalene, one of Jesus’s most loyal followers.

Solana is an urbane, silver-haired priest with the Legionaries of Christ, a Catholic order founded in Mexico. By that summer of 2009, he’d already raised $20 million for his retreat, which he was calling the “Magdala Center.” He’d bought four adjoining parcels of waterfront land. He’d gotten building permits for a chapel and a guesthouse with more than 100 rooms. Just three months earlier, Pope Benedict XVI had personally blessed the cornerstone. All that remained now was an irksome bit of red tape: a “salvage excavation,” a routine dig by the Israeli government to ensure that no important ruins lay beneath the proposed building site.

The IAA archaeologists had mucked around on Solana’s 20 acres for a month and found little. “Almost done?” he’d ask, emerging in his clerical robes from a shipping container that served as a makeshift office. “I have a budget! I have a timetable!”

In truth, the archaeologists didn’t want to be there either. Summer temperatures had ticked into the 100s, and the site prickled with bees and mosquitoes. They’d say shalom, they assured the priest, as soon as they checked a final, remote corner of his land.

It was there, beneath a wing of the proposed guesthouse, that their picks clinked against the top of a buried wall.

Dina Avshalom-Gorni, an IAA official who oversaw digs in northern Israel, ordered all hands to this square of the excavation grid. The workers squatted in the mealy soil and dusted carefully with brushes. Soon, a series of rough-cut stone benches emerged around what looked like a sanctuary.

It can’t be, Avshalom-Gorni thought.

This guide is intended for those that are interested in Biblical history. I explore the coins from the lifetime of Jesus Christ, one known as the Widow’s Mite, the Tribute Penny, the 30 Pieces of Silver, Pontius Pilate and much more. A Roman coin of Nero is also shown to illustrate one of the early persecutors of the Christian faith.

We then move on to the coins of the spread of Christianity through the Roman empire. A coin of Nicaea is shown, which is from the specific city that the Council of Nicaea was held which made Christianity the way it is today. Moving forward I show you an ancient coin of Constantine the Great from the time of the Council of Nicaea. We explore the topic of “By this sign you shall conquer” and what the symbol looked and how it was placed on ancient Roman coins as the Chi-Rho, also known as the Christogram or Christ monogram. A coin of Saint Helena is shown, who did a lot for the spread of Christianity and also Constantine the Great’s mother.

From this point we move on to an example of the Christian cross on a small Roman coin. After which point, we see the depiction of Jesus Christ on bronze and gold coins of the Byzantine empire.

In the story a poor widow woman gives her last 2 mites, the least valuable coins of the time. And Jesus points out that even though she gave very little, she gave her all, where some had a lot more and gave very little in comparison to their blessings.

For more than a century, the production of Follis denomination Byzantine coins had religious Christian motifs which included included Jesus Christ, and even Virgin Mary. These coins were designed to honor Christ and recognize the subservient role of the Byzantine emperor, with many of the reverse inscriptions translating to “Jesus Christ King of Kings” and “May Jesus Christ Conquer”. The Follis denomination coins were the largest bronze denomination coins issued by the Byzantine empire, and their large size, along with the Christian motif make them a popular coin type for collectors. This series ran from the period of Byzantine emperors John I (969-976 A.D.) to Alexius I (1081-1118 A.D.). The accepted classification was originally devised by Miss Margaret Thompson with her study of these types of coins. World famous numismatic author, David R. Sear adopted this classification system for his book entitled, Byzantine Coins and Their Values.
