No formal risk assessment was done on a man who beat a fellow care home resident to death, a review has found.
Alexander Rawson attacked 93-year-old Eileen Dean with a metal walking stick at a care home in south-east London.
Mrs Dean suffered catastrophic injuries to her head and body and died later in hospital.
A review found Fieldside Care Home in Catford did not provide the specialist mental health services that Rawson - who had a history of violence - needed.
Last year, Rawson was given an indefinite hospital order by a judge at the Old Bailey after it was decided he was mentally unfit to stand trial.
Rawson, who had a history of mental health problems caused by alcoholism, was 62 when he was placed in the home a few days before Christmas 2020.
He was put in the room next to Mrs Dean and, in the first week of 2021, he went into her room at night and attacked her.
In a review published on Friday, the Lewisham Safeguarding Adults Board said Rawson had been moved into the home after being an inpatient at a psychiatric unit run by the South London and Maudsley NHS Foundation Trust.
The care home was the only place that agreed to take him after his discharge from hospital.
The report described how Rawson was moved between four hospital wards and a care home in three locations and between two NHS trusts with information not gathered, recorded correctly or passed on.
In the months before he was moved into the care home, Rawson was involved in at least 34 recorded incidents of violence or threats to patients and health staff, including a threat to kill.
He had threatened to use a bread knife, scissors and dinner trays as weapons, had thrown jugs of water and cups and smashed equipment including computers, a radio and a crash trolley.
Before he was placed in the home, no attempts were made to find out whether Rawson had come into contact with the criminal justice system over his behaviour, the report found.
It states that the care home had asked about the risks Rawson posed before they took him and had been reassured by a social worker and medical staff.
The review also found that when the care home agreed to take Rawson, it had not told the watchdog, the Care Quality Commission (CQC), that it was housing people under the age of 65 with certain mental health needs.
A CQC spokesperson said: "The circumstances of Eileen Dean's death are appalling and our thoughts are with everyone affected.
"Health and social care providers must ensure they adequately assess the needs of people receiving their services, and collaborate to ensure appropriate placements are made.
"This includes sharing all relevant information when a transfer is being considered, so a person's new service can assess whether it can meet their needs while ensuring the safety of other people using the service."
Mrs Dean's daughter Georgina Hampshaw said in a statement: "It is with great sadness and regret that the published report confirms our worst fears of the many failings by the various agencies and trusts charged with managing the care and placement of the person responsible for the death of my mother Eileen Dean.
"The decision to place this person at the care home where my mother resided could not have been more flawed and resulted directly in the horrific murder of my gentle mother.
"Numerous opportunities were missed by so many agencies that may have prevented the horrific incident.
"I sincerely hope that the recommendations detailed within the report are adopted expeditiously and in full.
"No other family should have to suffer the loss of a loved one in such tragic circumstances."
Nathalie Zacharias, director of therapies at South London and Maudsley NHS Foundation Trust, said: "We welcome the report's findings and recommendations and have conducted our own full investigation which identified where we can make improvements to the care we provide.
"This includes putting robust checks in place to ensure all known risks, including those which take place off our premises, are fully recorded and documented and ensuring people in our communities continue to receive the care and treatment most suitable for their needs."

Families have been left feeling distressed and anxious after witnessing the video of 88-year-old Ann King being tormented and abused by those meant to be caring for her. This is a realisation of the worst nightmares of all those who have loved ones in care.
This sadly, is not a case of a few bad apples, but indicative of a broken system. The sector is facing a staffing crisis due to low pay and poor conditions, and caring has become a profession of last resort with many homes being forced to recruit those with little or no other options. As a result, conditions in care homes have become untenable.
The Care Quality Commission is toothless to take on any issues that arise in private care homes. The campaign group Rights for Residents, of which I am a cofounder, has raised issues with the CQC on many occasions, as have individual families, and this is a typical response from them: “If you have experienced or seen poor care, you have a right to complain to the organisation that provided or paid for the care. We cannot make these complaints for you or take them up on your behalf because we do not have powers to investigate or resolve them.” There seem to be no consequences for those who provide poor care and the only people paying the price are our loved ones.
Prior to Covid, many residents and their families felt shut out from decision-making processes. This dynamic was further exacerbated during the pandemic, when residents were separated from their relatives and unable to advocate together. Many residents remain subject to unacceptable visiting restrictions, despite the government stating there should be none. The presence of relatives is the most important safeguarding mechanism, as they are often the first to spot when anything is wrong. Limiting family contact with residents has allowed the development of closed cultures that are a breeding ground for neglect and abuse.
How can this be solved? To start, staff shortages can be solved with better pay and conditions. Caring requires a level of empathy and skill that is not common to us all and should be properly rewarded. Investment in training is also critical. Those without the most basic level of educational attainment and training cannot shoulder such important work.
Residents and family members should be involved in developing future social policy, which includes discussions with ministers and policymakers. In addition, each care home should have regular meetings with residents and their families.
Finally, the government and the CQC need to come clean if the powers and resources don’t exist to investigate serious concerns reported to them. An urgent overhaul or complete reset is needed if the dignity and human rights of residents is to be safeguarded.
Social care needs to be properly funded by the government, and private care home companies need to reinvest profits into quality care, rather than pocketing large dividends at the expense of residents.
Unless there is urgent action, it’s inevitable that we’ll hear of more tragic cases like that of Ann King.

The current service director, who is Polly Carmichael, will “have a place on the GIDS Interim Management Board.” Jenkins states other members of the current leadership team will be assigned “roles in workstreams and projects relevant to their skills and experience.” These include current medical director Dr Dinesh Sinha, whose internal review of the service (pdf) in March 2019 found “no immediate issues in relation to patient safety or failings in the overall approach taken by the service.”
The interim team will start work on 1st February and oversee the service for 6-12 months. The shake-up at the top follows a period of mounting scrutiny and criticism of GIDS, which has seen a surge in referrals of children questioning their gender from 77 in 2009 to 2590 in 2018-19. In December 2020 judges in the High Court found the Tavistock was unable provide evidence of the long-term effects of prescribing puberty blockers to children and ruled that court approval must be sought for children to consent to such medical interventions. GIDS has suspended prescriptions of puberty blockers pending an appeal of that ruling. In June 2020 BBC Newsnight reported staff concerns being shut down at the clinic. Sonia Appleby, lead safeguarding officer at GIDS claims she is being penalising by her employers after she raised concerns and has mounted a whistle-blowing legal challenge. In September 2020 the NHS announced an independent inquiry into children’s services at the Tavistock.

Obviously, these conditions are not suitable for any human, especially those that are already fragile psychologically. Additionally, facilities would deem many women incurable despite their symptoms being treatable or preventable with proper care. Many viewed the mentally ill as less than human and treated them as such. While Dorthea Dix made great strides among others to reform mental health institutions, there is still a huge prevalence of abuse today.

Care home prosecuted after woman raped by fellow resident
There were almost 80 sexual and violent incidents involving the man before victim was attacked
A care home provider has been fined £363,000 after failing to protect a woman who was raped by another resident despite almost 80 sexual and violent incidents being reported.
The firm, which cannot be named to protect the identity of the victim, was also ordered to pay £12,441 in prosecution costs at Nottingham Magistrates' Court on Wednesday.
The Care Quality Commission prosecuted the company after what was described in court as its failure to respond effectively to a series of incidents involving a man who was eventually convicted of eight sexual offences at Nottingham Crown Court in 2019.
Sentencing the company, which admitted two charges under the 2008 Health and Social Care Act relating to a care home in Nottinghamshire, District Judge Leo Pyle said there had been a "marked change" within the firm since the failings.
The judge told the court: "The court should ensure that the effect of the fines is proportionate to the gravity of the offence.
"This case in my view involves high culpability.
"Staff were raising concerns, management were not acting. In my view it was a systemic failure within the organisation. There is no minimising the harm caused by the male resident – that is reflected in the convictions at the Crown Court.
"His behaviour affected residents and staff. It should have been addressed earlier.
"During that period it would have been obvious... that there was a high likelihood of harm."
The charges stated that the firm failed to protect the woman who was raped "from abuse and improper treatment resulting in avoidable harm" and had failed "to have systems and processes in place and operating effectively" to avoid harm to service users.
During the opening of the case by the CQC's barrister, it was stated that there had been 79 incidents of violence and sexualised behaviour involving the male resident.
Prior to sentence, defence lawyer Mark Ruffell, offering mitigation, said what had happened in the care home, amid a "geyser" of difficulties from many different residents on multiple occasions, was unique.
Mr Ruffell said the company accepted that staff should have "joined the dots" but stressed that eight or nine safeguarding referrals made to social services had been rejected.
Meanwhile, police had been called out several times but either did not attend or "matters just dissipated," Mr Ruffell told the judge.
The court also heard that the perpetrator had initially been bailed by police, and that social services had then wanted the suspect to return to the same home.
Speaking after the court case, Mary Cridge, CQC deputy chief inspector for adult social care, said: “People have a right to safe care and treatment, delivered in an environment where threats to their wellbeing – including abuse – are well managed. Sadly, this care provider did not meet these fundamental standards for residents at the home where this incident took place.
“The company’s failure to protect a vulnerable woman in its care from a resident who was known to present a sexual threat is appalling. The crime she was subjected to was avoidable.
“Similarly, the company’s lack of safeguards to protect all residents at the home from the risk of abuse was unacceptable.
“The majority of care providers do an excellent job. However, when a provider puts people in its care at risk of harm, we take action to hold it to account and protect people.
“I hope this prosecution reminds care providers they must always take all reasonable steps to manage risks to people’s safety, including ensuring people are safeguarded from abuse.”
