The Unseen Drama of Maternal and Child Health in South Africa: A Tale of Stats, Sanity, and Survival
Maternal health is not just a personal concern but a societal one that has the potential to determine the future health of our communities. When I first thought about maternal health, I have to admit, I had a very vague view. My thoughts included the usual: moms-to-be, pregnancy cravings, baby bumps, and a whole lot of baby showers. But it turns out, maternal health is almost like the iceberg that sank the Titanic—there’s a whole lot more beneath the surface, and it's far more important than just deciding on baby reveals, baby shower theme’s, and baby names.

Maternal health is about the well-being of women during pregnancy, childbirth, and the postnatal period. (World Health Organization: WHO, 2019) That there is a small human being growing inside is entirely dependent on their mother for survival—no pressure, yeah, right? When you consider that South Africa registered over a million births in 2020 alone, it’s not just something you ponder on; it’s a huge deal! This was 2 years ago and just to even begin to imagine what was happening in the economy during that time, it is frightening to consider that, that many children came into our society, communities and homes. In 2022, nearly 70% of those babies were born to moms aged 20–34, with KwaZulu-Natal leading in the "teen mom" category. Yay us right? As if we have the man-power, capacity and resources to manage even a fraction of that. Now, I don’t know about you KZN aside, but these numbers made me wonder: Can we provide the healthcare that all these moms and their children? Spoiler alert: South Africa has made some strides, but it’s not all sunshine and rainbows.
There has been some progress in the last two decades. For younger women aged 15–19, the number of those getting at least four antenatal care visits went up from 66% in 1998 to 75% in 2016. But here’s the twist: older women aged 35–49 are going in the opposite direction—fewer of them are getting the care they need. I believe this speaks volumes to the need for awareness of maternal mental health which is right in our ballpark as advocates but hold that thought! While nearly all births (97%) have a skilled health provider on deck, postnatal check-ups are where things start tumbling down again. Western Cape and Free State are leading us as a country with a solid 91% of moms getting timely check-ups, but KwaZulu-Natal is making us look bad at 80%. (“Almost One Million Births Registered at DHA Offices in 2022,” 2023)
Looking at the above mentioned issues, the role of healthcare providers, especially occupational therapists, becomes even more important. When maternal and child health is not prioritised, it’s not just the families who suffer—the ripple effect hits entire communities. This is where occupational therapists (OTs) swoop in like the superheroes, cool right? At a community level, OTs are out there promoting health services, educating parents on child development, and advocating for better access to maternal and child health services. Sure, we’re not the pop stars of the medical world, but we’re like the best-supporting actors who keep the whole show running smoothly. Think of us as the backstage crew making sure the show goes on, even in underserved communities like Cato Manor. Occupational therapists work on the ground, providing prenatal education, supporting mental health, and ensuring that mothers have access to resources that can prevent complications during and after pregnancy
Take the weekly immunization and maternal and child health clinics at the Cato Crest Community Hall, for example. It’s a small but mighty initiative aimed at tackling the growing number of planned and unplanned pregnancies in the area. It is intended not only to equip them with relevant skills but also provide a stipend that will see them through the training period until they are established on their own. Objectives of the NDDC Youth Internship Scheme. The main objective of the NDDC Youth Internship Scheme is bridging the skills gap existing amongst the youths from the Niger Delta Region. It is aimed at providing the trainees with enough skills to stay relevant in the modern job market through practical training in specified areas. Promotion of Self-Sufficiency The program is designed to achieve self-sufficiency for its youth. The stipends that will be paid during this period of internship are to hold them up financially, so the participants would not be having financial stress as they further their learning and development.
