Ultrafiltration: transport of water through the transmembrane pressure gradient with the passage of water from the higher side to the lower pressure.
Peritoneal dialysis
Peritoneal dialysis uses the peritoneum as a natural permeable membrane to balance water and solutes.
The technique is less physiologically stressful for the patient, does not require vascular access that can present numerous complications, it can be performed at home and, for this reason, allows patients greater flexibility in its performance.
Although, with so many advantages, the technique requires much more patient involvement than in a hemodialysis at dialysis centre in Delhi, it needs to be performed carefully and sterile maintenance is of great importance.
Peritoneal dialysis can be done manually or using an automatic device (periodic, overnight, or continuous), and in general, the dialysate is instilled through a catheter into the peritoneal space, remains for a period, and then drains. In the double bag technique, the patient drains the fluid instilled into the abdomen into one bag and then infuses fluid from another bag into the peritoneal cavity.

In a study, it was possible to observe that PD was more favorable to a better quality of life for the patient, as it presents three significant variables (work, patient satisfaction and less contact with the team – less likely to experience situations of stress), while HD presented two significant variables (emotional and physical function).
However, it should be taken into account that the two significant items in HD are more relevant to the daily quality of life of the patient outside the clinic.

Disparities In Dialysis In Children
HYPOTHESIS: Acute kidney injury (AKI) is a common cause of morbidity and mortality worldwide, with a pediatric incidence ranging from 19.3% to 24.1%. Treatment of pediatric AKI is a source of debate in varying geographical regions. Currently CRRT is the treatment for pediatric AKI, but limitations due to cost and accessibility force use of adult equipment and other therapeutic options such as peritoneal dialysis (PD) and hemodialysis (HD). It was hypothesized that more cost-effective measures would likely be used in developing countries due to lesser resource availability.
METHODS: A 26-question internet-based survey was distributed to 650 pediatric Nephrologists. There was a response rate of 34.3% (223 responses). The survey was distributed via pedneph and pcrrt email servers, inquiring about demographics, technology, resources, pediatric-specific supplies, and preference in renal replacement therapy (RRT) in pediatric AKI. The main method of analysis was to compare responses about treatments between nephrologists in developed countries and nephrologists in developing countries using difference-of-proportions tests.
RESULTS: PD was available in all centers surveyed, while HD was available in 85.1% and 54.1% (p = 0.00), CRRT was available in 60% and 33.3% (p = 0.001), and SLED was available in 20% and 25% (p = 0.45) centers of developed and developing world respectively. In developing countries, 68.5% (p = 0.000) of physicians preferred PD to costlier therapies, while in developed countries it was found that physicians favored HD (72%, p = 0.00) or CRRT (24%, p = 0.041) in infants.
CONCLUSIONS: Lack of availability of resources, trained physicians and funds often preclude standards of care in developing countries, and there is much development needed in terms of meeting higher global standards for treating pediatric AKI patients. PD remains the main modality of choice for treatment of AKI in infants in developing world.

Dialysis in Children

Dialysis in Delhi is a procedure whose purpose is to replace the kidneys when they are not able to perform their functions of purifying and cleaning the blood. It may be due to a chronic disease of the kidney, with progressive deterioration over time, or a serious affectation in a short period of time.
To do this, artificial machines are used to perform various functions:
Eliminate waste products that can be toxic from the body.
Maintain an adequate concentration of salts in the blood.
Contribute to maintain an adequate blood pressure, preventing water from accumulating in our body.
Types of dialysis techniques
There are two types of methods of artificial blood clearance, haemodialysis and peritoneal dialysis, the latter, in general, more used in children with chronic kidney diseases.
The haemodialysis requires a pump that drives the blood to pass through a membrane that filters it. It is connected to the organism through a localized fistula, usually in the forearm. The fistula is made by surgery, joining an artery and a vein (arteriovenous fistula). It takes a while since it is created until it can be used, it is usually lasting and with few complications. If the blood vessels of the forearm are not suitable, the surgeon can place a synthetic tube in place (graft). The blood that comes out of the vein arrives at the machine to be filtered and, once clean, it is returned to the organism through the artery. When this fistula cannot be performed or the haemodialysis is done urgently, The type of access is a tube (catheter) that is placed in thicker veins (usually in the neck or groin) and connected directly to the dialysis machine; It can be used immediately, but it has a higher risk of infection or obstruction. Therefore, it is used for a short time.
The peritoneal dialysis used as filter peritoneum, the membrane covering the intestines into the abdomen and consisting of abundant blood vessels. To do this, a simple catheter (tube) is inserted into the abdomen through a simple surgery, with an outlet close to the navel, through which the dialysis fluid is inserted. This remains a time inside the abdomen and is responsible for attracting waste substances, which are subsequently removed along with the liquid introduced through the same catheter. Each liquid inlet and outlet are called an exchange.
Time and place of dialysis
Haemodialysis is performed in the dialysis centre in Delhi, usually 3 times a week, although some children may need more days of treatment. The duration of each daily session is approximately four hours, although it varies depending on the weight of the child and the severity of the illness.
Peritoneal dialysis is usually performed at home, with the parents responsible for carrying it out, after receiving adequate training in the dialysis units of hospitals. The liquid exchanges are made manually, several times a day (usually 4 or 5) with intervals of several hours. The fluid that enters the peritoneum remains in the abdomen for several hours before it is removed. If you have an automatic machine that can be programmed, it can be done at night, without waking the child. In this case, the number of exchanges is greater and the residence time of the dialysis fluid in each exchange is less.
At times, transient dialysis may be required until the kidneys recover (if acute), or until a kidney transplant in Delhi is performed. Other times, dialysis is necessary throughout life.
Possible complications
The haemodialysis technique can have complications that, in general, do not interrupt the treatment:
Lowering of blood pressure during filtration. It is the most frequent. It treats diminishing the speed of the filtration and with extra contribution of intravenous serum.
Allergic reaction to any of the compounds of the dialysis membranes, which may present as itching or urticaria.
Headache, nausea, vomiting and shortness of breath transiently.
Gas embolism.
Infections or thrombosis of the vascular access.
Other complications due to the lack of production of some substances by the kidney, such as growth deficit, red blood cells or vitamins, can be treated with medications.
The possible complications of peritoneal dialysis are infection of the abdomen cavity (the child may have fever, abdominal pain and fluid that is removed being cloudy), the leakage of fluid between the skin and the tube, the displacement of the catheter and the infection of the exit orifice of the catheter. These complications require treatment in the hospital.
Special diet
