Chỉ định:

Co thắt trong: xơ cứng rải rác; tổn thương tuỷ sống khác (u tuỷ sống, rỗng tuỷ sống, bệnh thần kinh vận động, viêm tuỷ ngang, chấn thương tuỷ sống); đột quỵ mạch máu não; liệt do não; viêm màng não; chấn thương đầu.

Liều lượng – Cách dùng

– Người lớn: 3 ngày đầu: 5 mg x 3 lần/ngày, 3 ngày kế: 10 mg x 3 lần/ngày, 3 ngày sau: 15 mg x 3 lần/ngày, 3 ngày tiếp: 20 mg x 3 lần/ngày. Tối đa 100 mg/ngày. – Trẻ em: 0,75 – 2 mg/kg/ngày. – Trẻ em: 12 tháng – 2 tuổi: 10 – 20 mg/ngày. – Trẻ em: 2 – 6 tuổi: 20 – 30 mg/ngày. – Trẻ em: 6 – 10 tuổi: 30 – 60 mg/ngày. – Trẻ > 10 tuôỉ tối đa 2,5 mg/kg/ngày.

Chống chỉ định:

Mẫn cảm với baclofen. Loét dạ dày.

Tương tác thuốc:

Rượu, thuốc ức chế thần kinh trung ương, chống trầm cảm, hạ áp. Thuốc độc thận. Fentanyl. Lithium. Levodopa, carbidopa.

Tác dụng phụ:

– Chóng mặt, buồn nôn, lẫn lộn, đau đầu, mất ngủ, ngái ngủ, mệt mỏi. – Hiếm: kích động, trầm cảm, ảo giác, tê nửa thân dưới, đau cơ, mất điều hoà, khô miệng, run, nôn, loạn điều tiết & ác mộng. Hạ huyết áp, khó thở, bí tiểu.

Chú ý đề phòng:

Suy thận. Ðột quỵ, suy hô hấp, hoặc suy gan. Tâm thần, co giật & lú lẫn. Tăng co thắt bàng quang. Ðang dùng thuốc hạ áp, chống co giật. Người duy trì thế đứng & cân bằng khó khăn. Người cao tuổi. Có thai & cho con bú. Tránh lái xe & vận hành máy. Không nên ngừng thuốc đột ngột.

Thông tin thành phần Baclofen

Dược lực:

Baclofen là thuốc giãn cơ. Về mặt hóa học, baclofen có liên quan đến acid gamma-amino butyric (GABA), một chất dẫn truyền thần kinh trong não. GABA do một số dây thần kinh tiết ra, làm giảm hoạt động điện của các dây thần kinh khác.

Tác dụng :

Giống như GABA, Baclofen phong bế các dây thần kinh trong việc hình thành tổ chức lưới của não, các dây thần kinh này kiểm soát các cơ. Baclofen, một chất có cấu trúc giống GABA có tác dụng hoạt hóa một chất dẫn truyền thần kinh ức chế, từ đó ngăn chặn được các kích thích nấc. Đây là một trong những thuốc hiệu quả nhất trong điều trị các trường hợp nấc mạn tính do nhiều nguyên nhân khác nhau như các bệnh lý ở dạ dày – thực quản, tổn thương thân não hoặc nấc vô căn, kể cả những trường hợp không đáp ứng với nhiều loại thuốc khác. Thuốc giãn cơ ngăn chận hoạt động thần kinh trong tuỷ sống. Baclofen giảm co thắt cơ và cứng khớp do chấn thương sọ não và tuỷ sống, do đột quỵ hay do các chấn động thần kinh như xơ cứng rải rác

Chỉ định :

Liều lượng – cách dùng:

Người lớn: 3 ngày đầu: 5mg x 3 lần/ngày; 3 ngày kế: 10mg x 3 lần/ngày; 3 ngày sau: 15mg x 3 lần/ngày; 3 ngày tiếp: 20mg x 3 lần/ngày. Tối đa 100mg/ngày. Trẻ em: 0,75-2mg/kg/ngày, 12 tháng-2 tuổi: 10-20mg/ngày; 2-6 tuổi: 20-30mg/ngày; 6-10 tuổi: 30-60mg/ngày. Trẻ > 10 tuổi tối đa 2,5mg/kg/ngày.

And, sometimes when you ask for help, you don’t receive it. Like when I asked my mom for help with my brother, when he was out of control, while I was caring for my aunt just out of the hospital, and after coming down and not able to deal with it herself, her response was, “Grow up and take responsibility! He’s your brother!” And left and went back to her carefree life. Or, you have no one left to turn to...

Some crucial information concerning my future survival:

I was given a preliminary diagnosis of Multiple sclerosis; symptom presenting/observation upon a thorough examination, extensive rule-out blood work/testing. My use of MS meds is improving my condition, although I’m currently presenting in the second stage of Relapsing-remitting MS with neurological symptoms, which could develop into Secondary progressive MS, and later Primary progressive MS, especially if left untreated. Not to mention other debilitating conditions such as, PTSD, past heart attacks, age-related decline. This is not told to cause concern or switch the focus back to me… just chatting so it’s out in the open, and if you ever wonder what pills I’m taking; Baclofen 20mg (one of the meds prescribed).

I keep a small notebook of the times doses are taken and record symptoms. The pills were given to see if any improvement was made with use. It was. Significantly over time, too. But, I’m still symptomatic and at times the pain or symptoms are worse, what I guess they call flare-ups. I need to seek medical care again before my prescription runs out in May, if out of state, and possibly switch to a different medication or increase the dosage I’m on now, sooner. As far as I know, 20 mg is the highest pill dose, but I’ve read some people take two or 30 mg, more 10 mg pills per bottle. Whatever. I hope to work it out and to be able to control symptoms or slow progression so I can work at least part-time the next 5 years to survive (see below).

(For me and anyone): In order to qualify for SSDI (disability), you had to have worked at least 5 years out of the last 10 years. (It doesn’t matter to our government that I started working in 1980, since I was 16 years old, minus a few years.) Anyway, any savings or assets do not disqualify you. Median payment is $1100. Future work, even part-time work, will qualify you if earnings exceed around $6,000 annually, and you must earn a work credit each quarter within those 5 years. Self-employment will earn you work credits, as long as you pay the proper tax.

If you become disabled and don’t qualify for SSDI you can apply for SSI. But, in order to qualify for SSI, you have to meet low-income criteria. Basically, be dirt-poor, and have no more than $2,000 worth of assets total; however, you may own one home and one car (wildcards). Median payment is only $600 (that number sounds low, and could be off, but I know it basically leaves you poverty-stricken). You can work (if able), but anything earned over $755 per month is deducted from your SSI and in some cases different deductions are made in some formula. Therefore, to work while on SSI you’re only receiving a minimal amount more per month, and at what cost to your health and recovery if you have a qualifying disability?

In order to qualify for early retirement social security, you must be almost 63 years old (early retirement is 62, but you have to account for at least 6 months of that year, depending on the year you were born) and SS will be decreased by at least 25%. Median payment is still barely enough to survive on.

SO, in order to survive, I’d have to split rent with someone, or buy a cheap house (impossible) or condo while I can, with low HOA dues, because they will be raised over time and I will need to be able to cover the cost, including property taxes and homeowners insurance, with a low income, and still afford medication, food, utilities, (forget about cable/anything extra). If renting, I can waive pet rent for at least one cat if officially certified an ESA, depending on rentors’ flexibility on the issue. My desired destination’s cost of living now is lower than in CA. I’d benefit from moving out of state in the long run. If for medical reasons I can’t work at least part-time the next 5 years, or establish a home-based business to survive, where I can work my own hours as able when not in a type of MS flare-up vs. work performance suffering or too many call-outs possibly occurring to lose any outside job, I’d have no choice but to rely on SSI alone (poverty level assets only, to qualify & survive on). I was hoping to at least get us set up (T & Alexandria too), while I can financially (barely), to have a home to rely on with minimal costs vs. rent. I have to formulate a potential disability and retirement plan and it has to all be planned out efficiently or I have no real chance of above poverty survival on my own.

FYI, I think T & Alexandria are possibly making other plans without me now. T doesn’t fully grasp the concept of this nor really know of my diagnosis... just that it’s a possibility. Hopefully, it’s a manageable condition for me, because my family doesn’t care, and since they think I won’t be able to care for them anymore, I’m not wanted around here. Also, because they don’t want to have to care for me, if it ever comes to that. Yes, it was said to me, they don’t want to have to care for me. My mom’s words, “Tough. You’re an adult. You’ll figure it out.”

You’re right, mom. I’ll figure it out on my own, my only option.

Needless to say, I have a lot on my mind... and trying not to let my disbelief and hurt hit hard or give up on everything entirely.

Corrupt VA in Prescott Arizona

Unbelievable treatment by the VA, needing to be known. This is a letter stating just some of the horrible treatment obtained at the Prescott, Arizona, VA Hospital. Allowing Veteran to suffer and fall out of bed in spasms, injuring 2 Vertebrea in ER with major surgery on spine almost paralyzing me after screaming for help for 5 hours. Attending Physician Assistant stated he didn’t feel comfortable treating me due to medication, even though I had been treated approximately 12 times prior with same spasticity with Valium when my spasms get this far out of control. Which he saw in computer. Due to Spasticity I fell out of bed Majorly Herniating 2 more Disks with impingements. He offered my regular dose of 240 mg’s Of Oxycodone which I denied, because that is for pain not Spasticity. Told him why is he in VA E.R. If he is scared to treat war injured Veterans. He stated I had a script for Valium which was only 5 mg’s, and that I should go home and take the dose I needed. This is unsafe due to me usually needing 20 mg’s IV to stop Spasticity, and that’s why I came in E.R. as usual when it got this bad. His nurses who have seen me in E.R. even told me they were trying to get a hold of other doctors in hospital and didn’t understand why he wouldn’t treat me. My nurse even moved me to opposite side of E.R., in hopes a different doctor would treat me. But after 5 hours of massive spasms, I pulled my IV out and signed myself out, to come home and take the dangerous level of 20mg’s Valium in conjunction with my night pills including my 240 mg’s Of Oxycodone not caring what happen to me any more. I ended up in bed for approximately 11 weeks, getting fed breakfast, lunch, and dinner in bed. When VA sent me to get MRI, it stated I had 2 new Herniated Disks with Impingements, needing major surgery, which they sent me out to have at Honor Hospital in Scottsdale, Arizona. After the surgery, the surgeon came out to say he new why I was unable to transfer or move around in bed. It was because the MRI didn’t even show the amount of damage done. He had taken a picture of how much they had removed from around my spine, next to a 12” ruler, which I have. It was just about a foot of bone, cartilage and nerve endings. I now suffer from neuropathy from my toes to the top of my legs, to go along with my 9 other herniations in my back from when injured in Desert Storm and MS which is service connected along with a host of other injuries and illnesses sustained there when my command sent me out to a Saudi hospital where I was infused with 3 different chemicals the navy doctors said were not FDA Approved, and thought to be just chemicals, as I was in an open dorm of burnt, dying, and dead Saudi Arabians for 6 days until wife found me. This hospital has become the worst VA after being one of the best approximately 10 years ago. This could have all been avoided if this VA management would not have denied an ARP Wave Machine, I have had 3 Trials with now and 4 Letters Of Medical Neccessity for. With my VA Neurologist even stating that by not obtaining this device for me, they are causing my Disability and Home life to worsen. As it was documented to stop my Spasticity, Pain, and allowed me to cut back on a major amount of my Oxycodone, get off my Valium, get off my Baclofen and other medications. Even the acting COS at the time at this Prescott, VA, wrote several letters stating that I have the documentation proving it works and should be ordered, which Dr. Piatt who I’ve had problems which before as the Acting Medical Director Denied. The aCOS Dr Johnson, even went as far as ordering it as documented in the Patient Advocate files. Stating he had ordered it and had Barbera the Patient Advocate contact me to let me know. Just to be disappointed again. My primary Care doctor even wrote a letter agreeing with my VA Neurologist, Dr Friedman, at the Phoenix, VA, stating it was a special case due to Opiates taken. With no help. I would rather die at home than go back to this Prescott, Arizona, VA Emergency. They lied so much it is ridiculous. Stating this Devise has not been used before on Veterans, when I have a letter from Matt Devine 703-664-5000 or 719-447-3473 out Of Fort Belvior, HPO. Who stated, he has been using this device on men and Women Veterans with Outstanding Results. Even sending the 13 page purchase order for it. This device is an FDA APPROVED CLASS II MEDICAL DEVICE for safety, yet they would state it is unsafe to use to chain of command. There are other Veterans who have had several trials with amazing results, like Andrew Liebig out of Alaska getting the same horrible treatment. I have all documentation on this and have reached out to the Chain Of Command up to the President with no response as others have as well. They have now black listed me forcing me to check in and out of the Police Department, every time I go in and out of VA, because the PA who refused to treat me stated I drove irrationally out of the E.R. and cursed at him. Which I am willing to take a lie detector on. And after talking to my nurse about the horrible treatment stated I drove irrationally at the same time. When it is virtually impossible to drive a chair while having major body spasms.
