The conversion therapists met last November at the south end of the Las Vegas Strip. Behind the closed doors and drawn blinds of a Hampton Inn conference room, a middle-aged woman wearing white stockings and a Virgin Mary blue dress issued a call to arms to the 20-some people in attendance. âIn our current culture, in which children are being indoctrinated with transgender belief from the moment theyâre out of the womb, if we are confronted with a gender-confused child, you must help,â declared Michelle Cretella, a board member of the Alliance for Therapeutic Choice and Scientific Integrity. âWe must do something.â Cretella was delivering a keynote speech at the first in-person conference in four years of the Alliance, which describes itself as a âprofessional and scientific organizationâ with âJudeo-Christian values.â Its purpose: to defend and promote the practice of conversion therapy by licensed counselors.
Not that theyâd call what they do âconversion therapy.â That term lacks a precise definition, but it is used colloquially to describe attempts to shift a personâs sexual orientation or gender identity. In the 1960s, some psychologists tried to make gay men straight by pairing aversive stimuli, like electric shocks or chemically induced nausea, with images of gay pornâtechniques that ran the risk of causing serious psychological damage even as they failed to change participantsâ sexual orientation, researchers eventually concluded. Today, âconversion therapyâ generally takes the form of verbal counseling. Participants are typically conservative Christians who engage voluntarilyâmotivated by internalized stigma, family pressure, and the belief that their feelings are incompatible with their faith. Others are children, brought into therapy by their parents.
The American Psychological Association (APA) has concluded that conversion therapy lacks âsufficient bases in scientific principlesâ and that people who have undergone it are âsignificantly more likely to experience suicidality and depression.â Similarly, the Substance Abuse and Mental Health Services Administration (SAMHSA), part of the Department of Health and Human Services, published a report concluding that ânone of the existing research supports the premise that mental or behavioral health interventions can alter gender identity or sexual orientation. Interventions aimed at a fixed outcome, such as gender conformity or heterosexual orientationâŚare coercive, can be harmful, and should not be part of behavioral health treatment.â
Accordingly, the Alliance and the ideas it promotes have been relegated to the scientific and political fringes. In the 2010s, as acceptance of gay rights grew rapidly, 18 states and dozens of local governments passed laws forbidding mental health professionals from attempting conversion therapy on minors. Yet by 2020, a new front had opened in the war against LGBTQ people. Republican state legislatures started passing laws targeting transgender and nonbinary children at schoolârestricting their access to bathrooms, barring them from participating in sports, and stopping educators from teaching about sexual orientation or gender identity. The most intense attacks have banned doctors from providing the treatments for gender dysphoria backed by all major US medical associations. Nearly 114,000 trans youth live in states where access to puberty blockers and hormone therapy has been wiped out.
Last year, I received leaked emails illustrating how these laws are crafted and pushed by a network of anti-trans activists and powerful Christian-right organizations. The Alliance is deeply enmeshed in this constellation of actors. Although small, with an annual budget of under $200,000, it provides both unsubstantiated arguments suggesting LGBTQ identities are changeable and a network of licensed counselors to lend their credibility to these efforts. Among the collaborators were David Pickup, the Allianceâs president-elect; Laura Haynes, an Alliance advocate; and Cretella, the former executive director of an anti-trans pediatrics group who described gender-affirming medical care at the Las Vegas conference as âevilâ and part of a âNew World Order.â (âIâm not a conspiracy theorist,â she assured attendees. âIâm just someone who has been in the battle of the culture of life versus the culture of death long enough to see the big picture.â) All three have testified before state legislatures against gender-affirming care. When a US senator introduced a pair of bills to restrict trans youth health care in 2021, his press release quoted Cretella calling gender-affirming treatments âeugenics.â

If the Las Vegas conference made one thing clear, itâs that conversion therapy is alive and well, even in places where itâs been banned. One counselor told me he makes it a habit not to document his treatment plans in writing to avoid getting in trouble and simply treats âfamily dynamicsâ in states with conversion therapy bans. In a 2015 survey of more than 27,000 trans adults, nearly 1 in 7 said that a professional, such as a therapist, doctor, or religious adviser, had tried to make them not transgender; about half of respondents said they were minors at the time. By applying this rate to population estimates, the Williams Institute at UCLA projects that more than 135,000 trans adults nationwide have experienced some form of conversion therapy.
Despite the data, lawmakers frequently donât believe that conversion therapy is still happening in their community, says Casey Pick, director of law and policy at the Trevor Project, the LGBTQ suicide prevention group. âWeâre constantly running up against this misconception that this is an artifact of the past,â she says. So, five years ago, the Trevor Project began scouring psychologistsâ websites and books, records of public testimony, and known conversion therapy referral services, looking for counselors who said they could alter someoneâs gender identity or sexual orientation. As the research stretched on, Pick noticed webpages being revised to reflect changing times. âWe saw many folks who seemed to leave the industry entirely,â she says. âBut others changed their website, changed their keywords, [from] talking about creating ex-gays to talking about ex-trans.â Last December, Pickâs team published their report documenting active conversion therapists. They found more than 600 were licensed health care professionals and an additional 716 were clergy, lay ministers, or other unlicensed religious counselors.
According to Pick, some conversion therapists have embraced a new label for what they do: âgender exploratory therapy.â Itâs a term that Cretella used to describe the approach she recommended, and unlike the other euphemisms thrown around at the conference, this has gained traction. In 2021, a group of therapists, who ranged from conflicted about medical interventions for kids with gender dysphoria to skeptical of the very concept of transgender identity, formed the Gender Exploratory Therapy Association (GETA) to promote an approach they characterize as neither conversion nor affirmation.
Some current and former leaders of the group, which claims a membership of 300 mental health providers, have been involved in influential organizations lobbying against gender-affirming care across the world, such as the Ireland-based Genspect and the Society for Evidence-Based Gender Medicine, a nonprofit registered in Idaho. Theyâve notched some big wins: In November 2023, the UK Council for Psychotherapyâthe nationâs top professional associationâdeclared that it was fine for counselors to take GETAâs âexploratoryâ approach to gender. This April, a long-awaited review of gender-related care for youth in Englandâs National Health Service endorsed exploratory therapy, according to Alex Keuroghlian, an associate psychiatry professor at Harvard Medical School. And in the United States, in cases in which families of trans children have sued states for banning gender-affirming care, the state often calls expert witnesses who endorse âexploratoryâ psychotherapy as their preferred alternative treatment.
After all, the idea of âexploringâ oneâs gender identity sounds benign. The World Professional Association for Transgender Health, which issues guidelines on gender-Âaffirming treatment, recommends that clinicians working with teens âfacilitate the exploration and expression of gender openly and respectfully so that no one particular identity is favored.â Yet, as with mindfulness, âthat term has now been hijacked by folks on the other side,â says Judith Glassgold, a clinical psychologist who chaired the APA task force that in 2009 documented the lack of science behind conversion therapy.
GETAâs guidelines instruct therapists to dig deep into âthe entire landscape of the young personâs life and subjective experience,â probing all possible reasons they might identify as transgender. The catch, says Glassgold, is that âexplorationâ means âtrying to find negative reasons why someoneâs diverse.â Last year, SAMHSA issued a report saying that âapproaches that discourage youth from identifying as transgender or gender-diverse, and/or from expressing their gender identityâ are sometimes âmisleadingly referred to as âexploratory therapy.ââ These approaches are âharmful and never appropriate,â the report concluded.

Are Supplements Really Useless?
Of course not, but thatâs what the media would have you believe in the latest hit job on dietary supplements. Action Alert!
A recent meta-analysisâa review of over 100 different randomized controlled trialsâfound that multivitamins, vitamin D, calcium, and vitamin C showed no benefits in the prevention of cardiovascular disease, heart attack, stroke, or premature death but did find benefits for B-vitamins in preventing stroke. Despite glaring issues with the analysis, and positive findings regarding the benefits of supplements notwithstanding, (not only in this meta-analysis but in a number of others) the media is replete with headlines such as âYet Another Study Says Vitamin Supplements Are Worthless.â This reporting evidences the mediaâs penchant to twist the facts to fit a particular narrative about supplementsâone which benefits drug companies that spend billions on advertising each year.
First, the actual findings of the study are being widely misreported. Far from proving there are no health benefits from supplements, the study found that B-vitamins such as B12 and folic acid were associated with a 20% reduction in risk of stroke. As our Scientific Director Dr. Rob Verkerk has pointed out, had the 20% reduction in risk of stroke found for folic acid been found for a drug, Pharma companies would have been shouting the result from the rooftops. To put this in perspective, the number needed to treat to protect against one case of stroke with B-vitamins was 176, while that for statins found by a meta-analysis carried out by the Cochrane Collaboration was 196.
The study itself also suffered from a number of limitations, very few of which are discussed in the articles claiming that supplements are âuseless.â First, many of the studies included in the meta-analysis used Pfizerâs Centrum multivitamin. A look at Centrumâs ingredients shows that many of the vitamins are synthetic and are present in the least absorbable form. For example, Centrum contains synthetic vitamin E rather than the full range of vitamin E compounds that optimize its beneficial functions. Many mainstream vitamins contain folic acid and cyanocobalamin (B12). This is important because a substantial portion of the populationâaround 30%âis unable to metabolize these forms of vitamin B and require the methylated versions (folate and methylcobalamin). (Note that even though folate may be superior to folic acid for many people, the meta-analysis showed that supplementing with folic acid still imparts benefitsâif folate were used in the studies, the benefit would likely have been even greater.) Centrum is also full of other chemicals, including preservatives with known negative health effects such as sodium benzoate and butylated hydroxyanisole.
The point is that Centrum and other vitamins of its kind, given these ingredients, are hardly the best examples of vitamins to use in a study looking into the potential health benefits of supplementsâno integrative doctor worth his or her salt would recommend them. Thereâs also the question of dosage. The dosages used in many of the studies the authors reviewed did not exceed the abysmally low recommended daily allowances (RDAs) set by public health bureaucrats: the RDA for vitamin D, for example, is a mere 600 IU/day, whereas the Vitamin D Council recommends 5,000 IU/day for adults. Low vitamin dosages are not likely to confer therapeutic benefits to those who take them, once again demonstrating weakness of the meta-analysis.
There are more problems with the meta-analysis and the reporting that surrounds it. Not only did the studies that the authors reviewed mostly look at Centrumâthey only looked at what effect a few nutrients had on a limited number of conditions: stroke, heart attack, and all-cause mortality. The media takes the results of this narrow study to make the extraordinary claim that vitamins donât work, period. This is irresponsible journalism that could lead to real harm: how many people will stop taking their vitamins as a result of this sloppy reporting?
By focusing so narrowly on the effect of a few nutrients from a low-quality multivitamin on a limited number of conditions, the authors seem to have stacked the deck for a negative resultâa result that has been irresponsibly reported by the media. Claiming that supplements are âuselessâ requires a willful ignorance of the scientific literature. What about the demonstrated benefits of magnesium or vitamin K2 on the development of cardiovascular diseaseâor CoQ10, fish oil, resveratrol, etc.? Or the proven benefits of supplements to support bone, brain, or immune health? Did the media care to bring up any of these studies when they claimed supplements were âuselessâ?
It seems fairly obvious why supplements are so consistently attacked in the media. Big Pharma spends about $3 billion each year marketing to drugs to consumers, about $90 million of which is print advertising. Media companies, ever reliant on ad dollars, would hate to see that money disappear by, say, reporting honestly about supplements, which are Big Pharmaâs competition.
Biased journalism that misinforms consumers about the benefits of supplements is a major threat to consumersâ ability to take control of their health without expensive and oftentimes dangerous drugs. But there are other dangers that would eliminate consumer choice altogether. Weâve been reporting on the FDAâs New Dietary Ingredient (NDI) guidance which, if implemented unchanged, would wipe thousands of supplements off the market by forcing products that have been on the market safely for years to go through a system akin to a pre-approval process. FDA Accepts the Biologics License Application for Avelumab for the Treatment of Metastatic Urothelial Carcinoma for Priority Review

Merck and Pfizer Inc. Â today announced that the US Food and Drug Administration (FDA) has accepted for Priority Review the Biologics License Application (BLA) for avelumab*, as a treatment for patients with locally advanced or metastatic urothelial carcinoma (mUC) with disease progression on or after platinum-based therapy. The BLA was submitted by EMD Serono, the biopharmaceutical business of Merck in the US and Canada. The FDA has set a Prescription Drug User Fee Act (PDUFA) target action date of August 27, 2017, for avelumab in this indication.
"Taken together with last year's filing for metastatic Merkel cell carcinoma, this BLA acceptance confirms our rapid and continued progress in the clinical development of avelumab," said Luciano Rossetti, M.D., Executive Vice President, Global Head of Research & Development at the biopharma business of Merck. "We continue to evaluate avelumab in cancers that have limited or suboptimal treatment choices, such as metastatic or locally advanced urothelial carcinoma, to hopefully be able to provide patients with new treatment options for fighting their disease."
Despite advances in the treatment of UC, the prognosis for patients remains poor, particularly when the disease has metastasized. Bladder cancer makes up approximately 90% of urothelial cancers and is the sixth most common cancer in the US.[1],[2] Â
"Advanced urothelial carcinoma remains a difficult-to-treat tumor, which is why we are developing a comprehensive clinical development program that involves Phase I and III trials designed to address this challenge," said Chris Boshoff, M.D., Ph.D., Senior Vice President and Head of Immuno-oncology, Early Development and Translational Oncology, Pfizer Global Product Development. "We're continuing to accelerate our urothelial carcinoma development program and look forward to continuing our dialogue with the FDA." Â
Avelumab is an investigational, fully human anti-PD-L1 antibody. The FDA's Priority Review status reduces the review time from 10 months to a goal of six months from the day of filing acceptance and is given to drugs that may offer major advances in treatment or may provide a treatment where no adequate therapy exists. In November 2016, the FDA accepted, and granted Priority Review status to, the BLA for avelumab for the treatment of patients with metastatic Merkel cell carcinoma.
The international clinical development program for avelumab, known as JAVELIN, involves at least 30 clinical programs, including nine Phase III trials, and more than 4,000 patients evaluated across more than 15 tumor types. In December 2015, Merck and Pfizer announced the initiation of a Phase III study (JAVELIN Bladder 100) of avelumab in the first-line setting as a maintenance treatment in patients with locally advanced or metastatic UC. This trial is currently enrolling patients.

Urothelial Carcinoma includes several tumors originating from the cells lining the bladder, renal pelvis and urethra.
