When patients visit the clinic, they come in so that someone competent and trusted can help them with their complaints. In that conversation, listening comprises the key step in establishing their trust in you, and builds up their perception of your competence as the attending surgeon. The reality is that even if there are proper indications to operate, patients still refuse surgery for a multitude of valid reasons. This patient with symptomatic flatfoot, midfoot arthritis, and toe deformities did not want surgery in any form. Swipe! In my practice, it is not uncommon to see patients presenting with multiple bumps and lesions. Involving many joints, these boggy masses are usually from tuberculosis and do very well managed medically. Here, we have a patient with multiple lesions on the leg presenting with night pain, allodynia, and there is neuralgia from mass effect as well as motor weakness in the anterior leg. There are enchondromas present elsewhere in the body but not shown here. (A) Hemangiomas are seen on coronal and sagittal MRI cuts, labelled 1, 2 and 3. The ultrasound slide is included showing tumor vascularity. Came to the surgery one month after. ------------------------------------------ Alonso-Llames (paratricipital) approach Provisory reduction and fixation with kw (a lot of them).. looks like a hedgehog.. Medial and longer plate and posterolateral plate. -------------- (Didn't like those 3 small distal and medial 2,5mm screws! The guide were aiming them posteriorly direct to the joint! 😤) -------------- The last pic shows the final fluoroscopic view, and in the end i took out the bullet (now he is safe!! AOTrauma Course—Advanced Principles of Fracture Management

Online precourse self-assessment prepares participants for the course and allows the faculty to tailor the course to the needs of the participants.

Before attending the course, participants are expected to complete an online module on fracture reduction.

The course will be taught in a modular format. Each module consists of several evidence-based lectures, which will cover the key information required. In practical exercises participants will be trained in the application of various techniques. Discussing cases in small groups will help participants to understand decision-making and management skills.

After the course, an online postcourse self-assessment will provide participants an opportunity to review the important topics from the course. In order to support self-directed learning, a range of additional online resources will be offered.

Apply reduction techniques in fracture management with attention to soft tissues

Assess and treat complex diaphyseal and (peri)articular fractures using advanced application techniques

Demonstrate strategies for assessing and treating open fractures and soft-tissue injuries

Hospital de Lomas Verdes ofrece curso a residentes sobre innovadoras técnicas de traumatología y ortopedia

Por séptimo año consecutivo, el Hospital de Traumatología y Ortopedia de Lomas Verdes (HTOLV) del Instituto Mexicano del Seguro Social (IMSS), ubicado en el Estado de México, es sede del Curso Internacional Tratamiento Quirúrgico de la Fractura, promovido por la Fundación AOTrauma, institución que desde 1958 agrupa a los mejores cirujanos especialistas en traumatología y ortopedia de todo el mundo.

El doctor Gilberto Meza Reyes, Director del Hospital, señaló que este curso --con duración de cuatro días-- está dirigido a 48 médicos residentes de segundo año y 12 enfermeros y enfermeras quirúrgicos provenientes de hospitales del IMSS de San Luis Potosí, Puebla, Querétaro, Guadalajara y Monterrey.

Son 21 médicos especialistas quienes imparten el curso, con el objetivo de estimular el razonamiento clínico de los participantes para el abordaje de las fracturas a través del reforzamiento teórico, el análisis de casos reales y la práctica de destrezas en talleres con modelos plásticos.

Por su parte, el titular de la División de Educación en el HTOLV y Chair Person para AOTrauma Latinoamérica, Ricardo Cienfuegos Monroy, señaló que los participantes realizan ejercicios con modelos plásticos, hueso, placas y tornillos proporcionados por AOTrauma, que son los mismos que se usan en todo el mundo.

La Fundación ha conseguido alianzas con fabricantes de implantes (huesos, placas y tornillos) que destinan una parte de lo que producen para educación en todo el mundo.

El equipo, cuyo propósito es la educación, viaja por todo el mundo; son más de 200 toneladas de material identificado y armado en paquetes, cada uno con un costo superior a los 400 mil pesos. El instrumental y los modelos óseos son una réplica exacta de los que se emplean en la práctica diaria.

La Coordinadora Clínica de Jornada Acumulada del HTOLV y enlace del Consejo AOTrauma Latinoamérica, Sara Gabriela Bernal Lastiri, indicó que este curso se prepara a lo largo del año, es de reconocimiento internacional y no tiene costo para los participantes. “Es parte de la tendencias de educación, va más allá del currículum del programa de residencia y acercamos a nuestros médicos a las técnicas más novedosas que se emplean a nivel mundial”, destacó.

“Es un trabajo bastante arduo, muy largo, pero vale la pena que los participantes se preparen para mejorar la destreza en el quirófano durante una cirugía real. Ahí es donde deberán demostrar la habilidad y capacidad para resolver todas las fracturas complejas que vemos aquí en el hospital”, expresó el doctor Luis Gutiérrez, médico adscrito al Servicio de Reemplazo Articular y director del curso actual.

Por último, algunos de los residentes que asistieron a este curso destacaron la importancia de retomar las bases científicas de los procedimientos quirúrgicos y aplicarlas en el hospital donde hacen su residencia, ya que sin duda les ayudará a mejorar el proceso de una cirugía.
