Nation's healthcare shines in global spotlight
Vast market catches the eye of drug and medical device makers
It was a significant moment for world healthcare when two global pharmaceutical companies looked to China as the first country for approval of a new drug-bypassing the United States and European nations.
In December, the National Medical Products Administration approved Roxadustat, a drug developed by AstraZeneca and FibroGen, as a new treatment for anemia caused by chronic kidney disease in patients dependent on dialysis.
Chris Chung, managing director of FibroGen China, said in a statement: "Almost 10 years ago, Fibro-Gen made the decision to develop Roxadustat in China as a domestic Class 1 innovative drug. This arose from a commitment to give Chinese patients accelerated access to critically-needed innovative medicines. We are grateful to the patients and physicians in China who participated in our clinical studies."
The approval was supported by a phase III clinical trial (the final stage of a drug's development) in China, and the two companies, who expect to launch Roxadustat by the second half of this year, are working to get it approved in other markets.
The decision to launch the drug first in China reflects the newfound importance of the country's healthcare market and its emergence as a priority for global drug and medical device makers.
The healthcare system has undergone various revamps in recent decades, with much being achieved through State-sponsored care programs and multiple national health campaigns.
In October 2009, the country said it intended to provide universal healthcare by 2020 through the Healthy China 2020 initiative.
Helen Chen, Greater China managing partner at L.E.K. Consulting, said changes made that year were a turning point for the country's healthcare system.
"Healthcare reform in 2009 significantly strengthened the healthcare infrastructure, such as having a Level 2 hospital of 100 beds or more in every county, and also provided near-universal insurance coverage," Chen said.
"Essential drugs were introduced, and broader funding meant that mandatory vaccines became available nationwide. These substantially eased problems such as difficulty in securing a doctor's appointment, and expensive treatment, which were affecting the public."
By 2012, some 95 percent of the population had health insurance coverage. The program also included a focus on disease prevention through the promotion of healthy living.
In 2016, Margaret Chan Fung Fu-chun, then-director-general of the World Health Organization, said in a statement: "It's time to put people first. This means strengthening services at all levels, from the community through hospital care back to the community, and linking them more effectively, so people get the care they need every step of the way.
"It's also time to work more efficiently across sectors to protect people from risks such as smoking and unhealthy diets, and prepare them to age healthily."
Sakshi Sikka, a pharmaceuticals and healthcare analyst at Fitch Solutions, said China's universal healthcare system has evolved to cover more of its huge population.
"Over the years, the Chinese government has launched ambitious healthcare reforms with the aim (of achieving) equitable access to healthcare services. The government developed a system of universal medical insurance with three major programs," she said.
"The first was the Urban Employee Basic Medical Insurance Program, which was launched in 1999, covering individuals who are employed in cities. The second was the Urban Resident Basic Medical Insurance Program, which was launched in 2007, for urban residents who are unemployed.
"Then there is the New Rural Cooperative Medical Insurance Scheme for rural residents, which was also launched in 2007."
These programs are meeting a large part of the country's pharmaceutical costs.
Sikka added, "They were followed by a round of healthcare reforms in 2009 when China substantially increased investment to expand the health infrastructure, strengthen the primary care system and reduce the number of people being left out-of-pocket.
"In 2016, the central government reshaped health reform through the 13th Five-Year Plan (2016-20) and the 2030 Healthy China campaign," she added.
"Finally, in 2018, the government promised to further reform the healthcare and elderly care systems, raise insurance benefits and increase per capita government subsidies for public health services. Over the years, these reforms have enabled a significant improvement in healthcare access and led to an increased demand for medicines, providing opportunities for healthcare providers and pharmaceutical companies."
Sikka said the universal healthcare system has two key benefits.
"China's expanded medical insurance coverage at each stage significantly boosted drug sales growth, with accelerated expansion following the introduction of government-sponsored medical insurance plans," she said.
"Moreover, out-of-pocket spending significantly declined, from 52 percent in 2005 to 28 percent in 2016."
In addition to a reduction in spending, more treatments are being approved in the country at a faster rate.
Chen, at L.E.K. Consulting, said, "Most recently, the innovation drive (supported by the National Medical Products Administration-the former China Food and Drug Administration) has streamlined the drug registration process, and reimbursement for newer, more innovative and higher-value therapies is now taking place. These are all exciting for the industry."
She said clarification on the use of global data in clinical submissions in China is a particularly important step.
"We also see new entrants to the broader healthcare system, with technology companies like Tencent and Alibaba, or insurance companies such as Ping An, introducing different ways to look at the business and potentially create disruptions," Chen said.
Increased healthcare research and development is bound to see the country advance in certain fields, Chen said, adding, "China is quite active in a number of new research areas, such as AI (artificial intelligence) for image reading and CAR-T (cell therapy) research."
This improved research and development, along with stronger intellectual property protection, could increase innovation in the healthcare sector.
Lewis Ho, a partner in the life sciences practice at law firm Loeb & Loeb, said: "The intellectual property protection system is getting better. Most companies would acknowledge it has generally improved.
"With Chinese companies now generating many patents, there is more reason than ever to improve the system, as they have their own patents to protect."
Chen praised the streamlining of the drug registration process but said it could be improved for medical devices. "Unfortunately, the medical devices registration process continues to be confusing and subject to individual reviewer interpretation."
She also said other areas of the healthcare system could be improved.
"Some fundamental issues in the system, such as the low salaries of physicians and nurses, remain. This is a complex issue that will require significant coordination and further structural changes," Chen said. "Another issue is that healthcare service fees do not fully cover the cost of provision."
She added that some local governments had attempted to address this by raising fees, but were met with protests from patients, who took to the streets to demonstrate.
The aging population could also prove taxing for the universal healthcare system. In a report in 2016, the World Bank Group said there were 140 million people in China age 65 and older. The number was expected to rise to 230 million by 2030.
It also noted a surge in noncommunicable diseases such as cancer, diabetes and heart disease.
According to a two-year study by the World Bank Group, the World Health Organization and several Chinese ministries, the country has lifted more than 600 million people out of poverty in the past three decades and achieved noteworthy successes in health reform.
The World Bank Group's 2016 report urged China to maintain its goals and direction of healthcare reform. This could be achieved by continuing the shift from a "hospital-centric" model that focuses on volume and sales to one that is centered on primary care.
Jim Yong Kim, who was president of the World Bank Group until Feb 1, said in a statement, "Decades ago, China's innovations in health such as the introduction of barefoot doctors (village physicians) and cooperative healthcare showed the world it was possible to improve health and greatly increase the life expectancy for hundreds of millions of people.
"Today, China can once again lead the way with cutting-edge primary healthcare reform that puts the patient first and moves away from expensive hospital care that often does little to improve people's health," Kim said.

“Enabling Technolo rugged  gy” Helps Transform Carbon Dioxide into Acrylate
www.inhandnetworks.com
In the presence of nickel and other metal catalysts, CO2 and ethylene gas form an acrylate precursor configured in a five-membered ring. The challenge has been to crack that ring open, allowing a carbon-carbon double bond to form, creating acrylate. Lewis acids do the trick. Credit: Berkskoetter lab/Brown University
In a newly published study, researchers have demonstrated a new “enabling technology” that uses a class of chemicals called Lewis acids to help transform carbon dioxide into acrylate, a valuab Vending Telemeter  le commodity chemical used to make many everyday items.
Providence, Rhode Island (Brown University) — A key advance, newly reported by chemists from Brown and Yale Universities, could lead to a cheaper and more sustainable way to make acrylate, an important commodity chemical used to make materials from polyester fabrics to diapers.
Chemical companies churn out billions of tons of acrylate each year, usually by heating propylene, a compound derived from crude oil. “What we’re interested in is enhancing both the economics and the sustainability of how acrylate is made,” said Wesley Bernskoetter, assistant professor of chemistry at Brown, who led the research. “Right now, everything that goes into making it is from relatively expensive, nonrenewable carbon sources.”
Since the 1980s researchers have been looking into the possibility of making acrylate by combining carbon dioxide with a gas called ethylene in the presence of nickel and other metal catalysts. CO2 is essentially free and something the planet currently has in overabundance. Ethylene is cheaper than propylene and can be made from plant biomass.
There has been a persistent obstacle to the approach, however. Instead of forming the acrylate molecule, CO2 and ethylene tend to form a precursor molecule with a five-membered ring made of oxygen, nickel, and three carbon atoms. In order to finish the conversion to acrylate, that ring needs to be cracked open to allow the formation of a carbon-carbon double bond, a process called elimination.
That step had proved elusive. But the research by Bernskoetter and his colleagues, publis cost effective  hed in the journal Organometallics, shows that a class of chemicals called Lewis acids can easily break open that five-membered ring, allowing the molecule to eliminate and form acrylate.
Lewis acids are basically electron acceptors. In this case, the acid steals away electrons that make up the bond between nickel and oxygen in the ring. That weakens the bond and opens the ring.
“We thought that if we could find a way to cut the ring chemically, then we would be able to eliminate very quickly and form acrylate,” Bernskoetter said. “And that turns out to be true.”
He calls the finding an “enabling technology” that could eventually be incorporated in a full catalytic process for making acrylate on a mass scale. “We can now basically do all the steps required,” he said.
From here, the team needs to tweak the strength of the Lewis acid used. To prove the concept, they used the strongest acid that was easily available, one derived from boron. But that acid is too strong to use in a repeatable catalytic pro 4g  cess because it bonds too strongly to the acrylate product to allow additional reactions with the nickel catalyst.
“In developing and testing the idea, we hit it with the biggest hammer we could,” Bernskoetter said. “So what we have to do now is dial back and find one that makes it more practical.”
There’s quite a spectrum of Lewis acid strengths, so Bernskoetter is confident that there’s one that will work. “We think it’s possible,” he said. “Organic chemists do this kind of reaction with Lewis acids all the time.”
The ongoing research is part of a collaboration between Brown and Yale supported by the National Science Foundation’s Centers for Chemical Innovation program. The work is aimed at activating CO2 for use in making all kinds of commodity chemicals, and acrylate is a good place to start.
“It’s around a $2 billion-a-year industry,” Bernskoetter said. “If we can find a way to make acrylate more cheaply, we think the industry will be interested.”
Other authors on the paper were Dong Jin and Paul Willard of Brown and Nilay Hazari and Timothy Schmeier of Yale.

Enabling or Crippling? The Pitfalls of State-by-State Blockchain Legal guidelines

Enabling or Crippling? The Pitfalls of State-by-State Blockchain Legal guidelines
Andrea Tinianow is the director of World Delaware. Joshua Ashley Klayman chairs the Wall Street Blockchain Alliance’s Authorized Working Team.
In this impression piece, Tianiow and Klayman take into consideration no matter whether so-called enabling laws passed by a handful U.S. states is needed for blockchain technologies to prosper – and if so, some probable approaches.
The sights expressed are solely their own. The authors thank Lewis Cohen, Matt O’Toole and Aaron Wright for their contributions.
When governments announce the adoption of blockchain laws, even self-professed blockchain skeptics get recognize. But are blockchain-enabling statutes definitely needed in numerous situations, and could their passage direct to some unintended success?
To day, to our understanding, four U.S. states have passed formal blockchain laws. These statutes authorize the use of blockchain technologies for 1 or additional capabilities, for instance: to carry out commerce (Arizona and Nevada) to introduce facts into proof (Vermont) and to retain corporate data (Delaware). No question other states also are contemplating and, in some situations, actively doing the job to draft and go laws authorizing the use of blockchain technologies for some of the similar – and probably, myriad other – capabilities.
Do we want permission?
It is fascinating and, in numerous methods, laudable that Arizona, Delaware, Nevada and Vermont have taken management positions with regard to blockchain technologies. By passing blockchain enabling statutes, individuals states arguably have signaled to the community and the current market their self confidence in the technologies and, numerous consider, have specified their imprimatur to the technologies. These statutes show up to propose, “Go in advance and use blockchain technologies. We won’t penalize you for applying it. You have our permission.”
But is it needed? Do we want U.S. state governments to give their permission for the use of blockchain technologies? Have there been other periods at which we have called out a unique technologies and necessary laws to authorize its use? Why do we think we want to do it in this article? Will we have to go by this 1 additional time to authorize the use of synthetic intelligence?
And, if laws is passed in 1 jurisdiction, what does that say about individuals jurisdictions that decide on not to enact related legislation? Does that omission by a specific state somehow suggest that this kind of blockchain activity is invalid, unlawful or not permitted there?
Many jurisdictions likely are having difficulties with these quite issues, particularly, no matter whether unique laws is necessary to authorize the use of blockchain technologies and, if so, to what extent is this kind of laws necessary and with what diploma of specificity?
In other words, how do we persuade blockchain innovation in all 50 states and across numerous sectors, together with healthcare, schooling, banking, and so forth.? Do we detect every single one functionality for which blockchain technologies and smart contracts could be deployed, and then draft laws accordingly?
Hopefully not. What a lengthy and cumbersome approach that would be.
Possibility of balkanization
Evidently, a scattershot solution amid the 50 states (and metropolitan areas, and counties) is not suitable when it comes to enacting blockchain laws. Forethought and coordination amid jurisdictions is significant.
Normally, the final result could be a hodgepodge of laws covering distinctive capabilities, with distinctive benchmarks, and distinctive concentrations of specificity, most likely building nationwide blockchain alternatives unduly intricate and probably commercially impracticable.
Without the need of some diploma of harmony across jurisdictions, even well-intentioned firms that may possibly have functions or consumers situated in or spanning additional than 1 state may possibly be faced with compliance problems.
While it is real that there are numerous scenarios in which state legislation differ from 1 another and that firms may possibly be accustomed to complying with this kind of a tapestry of legislation, a scattershot solution may possibly final result in a lack of clarity regarding no matter whether the use of blockchain technologies in a specified state is permissible.
Even within a state, the passage of blockchain-enabling laws with regard to particular capabilities and sectors also could propose that individuals contemplating deploying blockchain technologies for other capabilities and sectors not explicitly sanctioned may possibly be prohibited.
Copying and pasting
In an effort to attain coordination, some U.S. jurisdictions may possibly take into consideration copying their blockchain enabling laws from another jurisdiction that already has passed it. This may possibly be much less than suitable for a range of causes.
For instance, there may possibly be lawful distinctions and nuances across states that may possibly not be apparent from a simple reading of their respective statutes. Even equivalent statutory language in two distinctive states may possibly have distinctive meanings, as that language may possibly have been interpreted in different ways by distinctive courts.
Simply grafting the similar language, copycat style, onto another state’s statute is not likely to be clever, absent significant assumed, investigation and preparing.
Also, individuals states that have enacted blockchain enabling statutes may possibly have encountered hurdles or complications that they experienced not foreseen at the time of passage. Centered on their post-passage encounters, individuals states may possibly recommend distinctive approaches or language modifications, or could have approached the blockchain enabling laws in different ways with the profit of hindsight.
Insights should really be sought from individuals states that have been leaders in passing blockchain technologies before merely copying their blockchain enabling statutes or approaches.
A rule of thumb
With the objective of allowing blockchain enabling laws to be certainly enabling and not inadvertently crippling or perplexing, we propose that states take into consideration enacting blockchain enabling legislation only in which the failure to enact this kind of laws would perpetuate current – or develop new – ambiguity about no matter whether the technology’s use is permissible, or in which the current state legislation preclude the deployment of the technologies altogether.
Ambiguity about the permissibility of blockchain technologies use may possibly exist, for case in point, when an current regulation contemplates that an enumerated functionality be executed by 1 or additional human beings (as opposed to by technologies).
This was the circumstance in Delaware. Before enactment of the “Blockchain Amendments” to the Delaware Standard Corporation Regulation, the statute experienced contemplated that a corporate officer (a human currently being) would have demand of a corporation’s inventory ledger. The amendments modified the corporate regulation to expressly permit general performance of the administrative functionality of retaining a corporation’s inventory ledger by use of a network of databases (i.e., a blockchain), with particular qualifying necessities.
In our look at, states that are exploring no matter whether and how to enact blockchain laws may possibly profit from thinking about which of their statutes contemplates or demands intervention by a human intermediary (like a corporate officer below the prior Delaware regulation), and then take into consideration amending the regulation accordingly.
In situations and for capabilities in which state legislation do not preclude the deployment of blockchain technologies or develop lawful ambiguity regarding the permissibility of, the adoption or use of blockchain technologies, we propose that the passage of unique blockchain enabling laws not only may possibly be unneeded, but also may possibly have unintended or perplexing consequences.

The Effects of Obama Care on Nursing
Health care reform has changed immensely over the years. It started with President Theodore Roosevelt in 1912, and continued by Franklin D. Roosevelt in the 1930’s (Toth-Lewis 5). After the Roosevelts, each president had their own change and tweak to healthcare reform. Truman introduced the right to medical care and good health care, as well as healthcare for the elderly, which led to the creation of Medicare and Medicaid. Next was Nixon with legislation that called for universal coverage, which was not successful. Following Nixon was Jimmy Carter and Bill Clinton, who both proposed new plans for Medicare and Medicaid, in which both were unsuccessful (Toth-Lewis 5). Then in 2010, President Barak Obama signed into law the Patient Protection and Affordable Care Act, commonly called Obamacare. The act required that beginning in 2014, all individuals would need to have health insurance and those who previously have insurance, increased access (Toth-Lewis 6), and this requirement to insurance created a demand for healthcare professionals, and more importantly, nurses.    The intention of President Obama with this act was to aid the millions of Americans who lacked insurance, or had poor plans, with affordable and quality health care insurance and rights. Although this act did not only change the way healthcare is distributed, it changed the nursing profession in general. But how did these changes in healthcare reform, especially in the last eight to ten years with the Patient Protection and Affordable Care Act, change nursing? This act led to more people being insured, obviously, which will lead to more jobs and opportunities for nurses, as well as a shift in the way nurses are taught (because the style of care changes), and changes in the role of nurses.    With the changes in healthcare came a few changes in the role of nursing, for example, the role of nurses strengthened. The PPACA encourages the growth of nurse-led clinics and it is planned that there will be more community centers and non-hospital setting places to receive care, which will be staffed by nurse practitioners (Carlisle 2). In the past, nurses have been used to just check patients into a doctor office and get the patient ready for the doctor, asking what your symptoms are and such. But with these transformations, nurses are more of the caregivers than the doctors or physician assistants. Also, nurse practitioners, which is a nurse who is qualified to treat certain medical conditions without the direct supervision of a doctor, in the past were not common, but now with the establishment of more clinics and community centers, they have become more common and a great opportunity for nurses to expand their careers.      Nurses will need to adapt to a style of work in which they will be “full partners with physicians and other healthcare professionals to develop healthcare strategies that will increase primary care services to meet the consumer need for safe, quality healthcare” (Lathrop 6). One of the main things Obamacare wanted to achieve was efficient, quality care, and by nurses bouncing ideas off other healthcare professionals, they can achieve this aspect of quality with efficiency.     Also, new positions were in a sense created to help provide leadership in areas of primary care and prevention to achieve the goals of Obamacare as well. These positions include advanced practicing registered nurses (APRN) and Doctor of Nursing Practice (DNP). Those who acquire Doctorate of Nursing practice degrees provide both direct patient care and clinical leadership, as well as utilizing evidence-based practice and information technology skills to improve patient healthcare outcomes (Lathrop 4). The plan is that “DNP nurses can bridge this gap by applying public health models of prevention to clinical medicine and educating their healthcare colleagues to do the same” (Lathrop 7).     Not only does the Patient Protection and Affordable Care change nursing in the field but also the way it is taught. It has changed to being more collaborative. “To achieve this end, nursing education must not only identify the skills graduate nurses need to be successful, but also implement innovative ways of teaching. Future nurses will need to be prepared to practice with other healthcare professionals in a collaborative manner, take leadership in the transformation of healthcare reform, coordinate care of patients whose conditions are complex, achieve higher levels of learning and become life-long learners” (Toth-Lewis 7). As I have mentioned numerous times before, one of the main points and goals of PPACA, is to make it more efficient, but this is not how a lot of nurses that were practicing before the act was passed learn how to care for patients. Current nurses learned how to practice in an acute environment, which is complementary with the needs created by healthcare reform legislation to take care away from the acute care setting. This legislation encourages and increases focus on new models that produce a need for restructuring of primary care and provides opportunity for an increased number of registered nurses in these settings.     The new laws have created so many new opportunities for nurses. For example, nurses can now participate in improving the quality of nursing homes, conduct comparative effective research, and more openings for things like being a midwife or at home care professional (Mason 5). Basically, there have been many new doors opening for nurses with the greater population being able to receive health care. “The law includes various measures to promote primary care, chronic care management, transitional care, and care coordination – all services nurses will provide” (Mason 3). This expansion of health care is going to really rely on the use of nurses, which in turn creates more jobs and positions, along with additional positions. One of these roles include attending registered nurse (ARN), whose purpose is to “build a relationship with the patients and his or her caregiver, ensuring that all the members of the patients’ health care team follow a shared care plan” ("Nurses Take on New and Expanded Roles in Health Care" 1).     A lot of these new jobs give nurses options with things for example the work schedule. It makes I more varied. The main difference between attending registered nurses and other registered nurses is that they are designed and meant to promote continuity of care, as well as providing the worker at five-day, eight-hour work schedule rather than the three to four-day, twelve-hour work schedule most nurses work. This is good for nursing because it provides another work schedule that may be more do able, particularly for working moms. “These new roles are empowering nurses to play a greater role in improving patient experiences and population health and lowering costs. Nurses in new roles are doing that by reducing unnecessary and costly hospital readmissions” ("Nurses Take on New and Expanded Roles in Health Care" 1). In all, the recent changes in health care have helped nursing. Under the ACA, there are so many opportunities and direction that you can take with nursing. Although it did take collaboration and changes in the way it was taught.

That 'Open Letter to Steve Jobs' 10 Years Later - Milestones in Patient Innovation!
New Post has been published on http://type2diabetestreatment.net/diabetes-mellitus/that-open-letter-to-steve-jobs-10-years-later-milestones-in-patient-innovation/
That 'Open Letter to Steve Jobs' 10 Years Later - Milestones in Patient Innovation!
Ten years ago this month I penned an ‘Open Letter to Steve Jobs,’ calling on the gurus of consumer technology to help transform medical devices into something more Apple-esque. After all, it was 2007, and our glucose meters, insulin pumps and other medtech gadgets were still mostly hospital blah, clunky and difficult to use.
The iPhone didn’t exist yet… But Apple was celebrating the sale of its 100-millionth iPod, the tiny MP3 player that revolutionized the enjoyment of music. Where was our help in creating ‘life devices’ that look good and feel good for the people whose lives depend on these tools 24/7?
Thankfully, my letter went viral.
And 10 years later, indeed a LOT has changed!
Aside from the untimely death of Steve Jobs in 2011, we have a lot to celebrate these days. The medtech industry has awoken to the need for improved design and practicality, and there’s an exciting wave of patient-led innovation underway in the aftermath of our DiabetesMine Design Challenge (first-of-its-kind patient crowdsourcing competition kicked off in 2008).
THIS PAST WEEK, news broke that Apple has a secret team now working on “the Holy Grail for diabetes.” Seriously, it’s my OPEN LETTER TO STEVE JOBS COME TRUE!
And just look at this list of exciting milestones in patient-centered innovation in diabetes over the past decade:
In 2011, the year of our very first DiabetesMine Innovation Summit event, the iBGStar was FDA approved as the first Apple iPhone/iPad plug-in glucose meter, created by AgaMatrix and marketed by Sanofi. OK, upgraded phone tech eventually killed this one, but it was the precursor of much to come.
In 2012, glucose meter tech made huge advances: LifeScan’s OneTouch Verio IQ became the first to ID glucose trends (very 1.0, but an important step); Telcare’s meter was the first to offer cellphone connection to automatically upload BG results to an online platform and mobile app; and the FreeStyle InsuLinx brought a touchscreen experience to the BG testing process.
Later that year, then-Bayer’s Contour Next Link meter introduced the era of glucose meters that wirelessly transmit BG values to an insulin pump or CGM.
At the 2012 DiabetesMine Innovation Summit, our focus was “A Call for an Open Model of Diabetes Care,” complete with a video drawing the parallel between the IT industry and the diabetes industry in the sense that open data and interoperability are crucial -- a patient rally cry ever since.
We were also fortunate to bring in FDA for the first time that year, forging a path for open, candid interaction between the regulatory agency and the patient community. The diatribe Foundation picked up the thread and organized the now-famous DOC Asks FDA virtual town hall meetings, the first of which was so well attended that our D-community crashed the FDA’s server, shutting down their whole system for a few hours at least.
A number of open innovation crowdsourcing competitions followed our Design Challenge, notably from JDRF and Sanofi, and they continue to thrive (see the latest Alexa Diabetes Challenge for voice-activated solutions).
After extensive user research, Tandem Diabetes Care introduced the first-ever Apple-esque touchscreen insulin pump in 2012.
At the inaugural DiabetesMine D-Data ExChange diabetes tech gathering in Fall 2013, the #WeAreNotWaiting movement was born –- leading to a whole international push for patient innovation!
The grassroots Nightscout Foundation was formed in 2014 to support the patient-created “CGM in the Cloud” solution; it’s now active in 30 countries, and details of this do-it-yourself mobile diabetes innovators group’s direct dealings with the FDA have been covered in the prestigious Journal of the American Medical Association (JAMA).
Prompted in part by the Nightscout effort, Dexcom got FDA approval in 2015 to connect its CGM system to the cell phone. This first “official” mobile CGM offering is spurring all sorts of innovation, and even a partnership with Google to develop ever-better consumer-friendly diabetes tools.
Thousands of smartphone apps for diabetes have appeared on the scene. While many languish, those from engaged patient-led developers like MySugr and One Drop are changing lives.
Tidepool introduced its disruptive open source platform for diabetes -- initiating a world of possibilities and pushing the industry to embrace open data-sharing. Their CEO, D-Dad Howard Look, was even honored by the White House as one of a select few “Health Change Makers” in 2016.
The homemade artificial pancreas system OpenAPS introduced in 2015 by DIYers Dana Lewis and Scott Leibrand is sweeping the country and world -- influencing how these closed-loop systems will be designed and come to market for the masses.
The annual Stanford Medicine X conference has grown into a hub for patient-led innovation, where healthcare providers and industry folk can connect and work side-by-side with patients on solutions to improve lives.
“Biohackers” are creating exciting innovations such as open source insulin, and a “nano-technology solution” for insulin therapy from this father-son-development team.
Inhalable insulin has become usable! After the market failure of Pfizer’s Exubera due to zero patient input, MannKind Corp. created and launched Afrezza, with a tiny whistle-sized inhaler device that uses color-coded dosing cartridges, i.e. patient-friendly design!
And now MannKind has stated that it’s relying on patient comments on blogs and Twitter to drive sales (which is pretty darn DISRUPTIVE).
And of course commercial Artificial Pancreas systems, that are on the verge of really AUTOMATING glucose control for the first time in history, are making incredible strides. We’re watching closely the developments from Bigfoot Biomedical, Beta Bionics (which just got a huge boost from Novo Nordisk), and TypeZero Technologies.
Medtronic is leading the way with the recent introduction of its 670G “hybrid closed-loop” (pre Artificial Pancreas) system. While one could argue this isn’t necessarily “patient-led,” their customer-fueled drive to get improved tools to market fast is helping us all. Indeed, 2016 was a pivotal year for closed-loop / automated insulin delivery.
Several exciting joint development agreements have been announced with consumer technology giants, such as Google+Dexcom, Google+Sanofi creating Unduo, and Samsung+Medtronic.
Patient data innovators are partnering up to create broader, more powerful platforms, such as Glooko+Diasend.
New mobile technology is pairing diabetes tools with an array of other health tools, i.e. Apple HealthKit.
New applications for continuous glucose monitoring (CGM) sensors are poised to expand the user base enormously, i.e. Abbott FreeStyle Libre and that tiny sensor Google is developing with Dexcom for type 2 diabetes.
Powerful voice-activated diabetes solutions are emerging, great for both the vision impaired and all of us whose hands are busy with other things 🙂
Apple’s Secret Team Developing Diabetes Tech?
And then of course there’s last week’s breaking story about Apple’s secret diabetes project. As the American Journal for Managed Care notes, 10 years down the road from our Diabetes Open Letter, this may be evidence that “Jobs listened.”
We find ourselves in such a PERFECT STORM of innovation at the moment that it’s hard to keep up…
The #WeAreNotWaiting movement has become an international Unstoppable Force…
The industry is recognizing patients as customers and the value of good design as never before…
And at healthcare and policy events, established players are starting to talk for real about how to tap into patient experiences to create better tools…
Patient innovation groups emerging in this #MakerCulture (like the NightScout Foundation) are starting to have a real seat at the table in policy discussions and in designing research, led by organizations like the congressionally-funded Patient-Centered Outcomes Research Institute (PCORI).
By engaging with these grassroots patient groups, manufacturers and designers are able to tap into an incredible worldwide community of willing and knowledgeable patients who can tell them -– like no one else can -- what makes a medical tool valuable for a person living with their condition.
A shout-out goes to fellow diabetes advocate Anna McCollister-Slipp for her pioneering work on patient-participatory research and lobbying for patients’ needs within the FDA, and to former federal HHS Chief Technology Officer Susannah Fox, worked so tirelessly to put engaged and empowered individuals at the center of our healthcare system (!)
It’s incredible to think that when I got started with DiabetesMine back in 2005, I almost literally had to beg, borrow and steal to get a press pass to cover the American Diabetes Association’s big annual meeting. No one had ever heard of a “blogger,” and the idea of a (gasp) patient attending a physicians’ and industry conference was unheard of. Nowadays, myself and other patient advocates are being invited as speakers and panelists, to ADA’s Scientific Sessions, the annual meeting of the American Association of Diabetes Educators (AADE), the Diabetes Technology Society (DTS) and more.
We’ve come a long way, Baby!
Our patient community should be proud of how we’re collectively raising our voices and are literally “changing the game.”
Thankfully, the industry is playing along, and the big national advocacy organizations are getting on board, too.
Let’s face it, Friends. Our politicians are dysfunctional and not likely to help us with real healthcare challenges any time soon. So we need to step up the creation of our own workarounds/ hacks/ solutions with an inventive problem-solving approach -- and step up pressure on the industry to pay attention to our real needs.
It’s wonderful to see how far we’ve come already, and how more and more people living with health conditions are asking themselves and their peers: What’s the fix? And how can we build it?
Never stop innovating, my (Im)Patient Friends.
