In Battle Mode in Mario Kart DS, it is possible for CPUs to become stuck in a loop of circling a banana in the Twilight House battle course.
This is theorized to be a result of the course's bottleneck room structure requiring the CPUs to align themselves with a path to change rooms, and the banana avoidance routine interfering with this.

In Super Mario 3D World, the way Prince Bully is defeated is by forcing him inside a Clear Pipe, whereupon he will emerge on the other side in a shape deformed to fit the pipe.
Due to this being a brief segment, it may not be easy to see the details of his design during this. Above is Prince Bully's deformed pipe form, extracted from the game's files.

In the ending of Super Mario World (left), the Empty Blocks that the Sumo Bro is standing on aren't actually background tiles, which is how Empty Blocks are implemented everywhere else in the game, but sprites instead. Disabling the sprites shows that they are displayed over a row of background Turn Blocks instead (right).
It is possible that the original intent was to make the Sumo Bro stand on the Turn Blocks before the sprite artist realized that this never happens in-game, and covered them up with Empty Blocks instead of going back to the background artist and requesting the change be made to the background itself, though this is merely speculation based on the most plausible interpretation of this scenario.

In Super Mario 64, stepping on the first elevator platform inside the volcano in Lethal Lava Land after letting it despawn and respawn will cause Mario to slide sideways on it a short distance, as seen in the footage.
This is due to the platform actually still undergoing its rotation from the moment it despawned, being applied to Mario's position even though the newly respawned platform is stationary, due to an oversight in the code.

In Donkey Kong Bananza, the Trustytone at the Getaways asks Donkey Kong for a small tip of 10 gold after each business transaction. As gold is extremely easy to come by, Donkey Kong is virtually guaranteed to have at least 10 gold on him at all times.
However, if the player either very carefully spends all gold or loses it by repeatedly dying and then does not collect any before trying to buy something from the Trustytone again, this obscure dialog will occur instead when the tip would normally be requested.
