I had a theory regarding Douma and how he got the upper moon 2 ranking before Akaza did. In fact, how did he even get that ranking in general? He wasn’t always upper moon 2 because the first time we see Douma is when he met Gyutaro and Daki, at that time he was upper moon 6.

Yet the next time we see him, he’s upper moon 2.

According to Douma himself, he surpassed Akaza in ranking even though Akaza has been a demon much longer than him. If there’s one thing Akaza loves more than sparring with Rengoku Strong people is being the strongest. Which is why we know he fought Douma and Kokushibo to take their rankings (which he failed) but how did Douma surpass him?

Well let’s take a look at a few things.

For one we know Douma’s diet mainly consists of women if not exclusively only women. Douma said himself women carry the most nutrients because they carry babies. Of course diet and excerise are very important but I doubt only eating women was how he suddenly jumped from upper moon 6 to 2. Kokushibo is impossible to defeat so it’s no wonder he never tried to go for that title as much as Akaza does. In fact, because Douma feels no emotions I doubt he even cared for the rankings so why did he go for upper moon 2 and surpass Akaza? Well there’s my theory for that. It all starts with Akaza and Douma’s contrast, what’s one thing Akaza will never do? Kill or eat women.

Given that, Akaza would not have gone for the upper moon 2 ranking had the previous holder of that title been a woman. We know from Kokushibo if a demon successfully defeats an upper moon, not only do they take their rank they also get to eat them. Which is a big no no for Akaza. Which means he would have refused to fought her. So wouldn’t it make sense for Douma to surpass Akaza in that sense had the upper moon 2 been a woman. Which brings up a previous point about Douma not caring about the ranks, we know for a fact because Douma will only eat a woman, wouldn’t it make sense for the previous upper moon he ate was a woman and not a man? He would have specifically went for the title because the previous holder was a woman not for the rank itself.

We know if a demon eats another demon, they get their powers which is why Muzan wanted to eat Nezuko because he wanted her sun conquering abilities.

And when Genya eats demons he temporarily gets their powers because he’s still human.

With that in mind, it adds onto the theory because whenever Douma uses his blood demon art, you see a woman made of ice, this might be due to his fascination of eating women but you only see the same one women not different kinds of women just that one, so who is she? Why is she the only one to appear in his blood demon art? Is it because she was previous holder? After all, if he killed and ate her, her powers probably went to him as well but she still appears in his blood demon art. Remember Douma says he eats his followers to save them and that they’re a part of him, so when he ate the previous upper moon 2, she appears in his blood demon art because she’s always “a part” of him. It makes a lot of sense as to why that woman is there and why Douma got the rank before Akaza. Akaza will not kill a women which is why he never fought for the upper moon 2 rank until Douma (a man) got it.

Overall, the theory is that Douma only managed to surpass Akaza is because the previous holder was a woman which Akaza cannot fight but because Douma will only eat women, he went for the title instead, defeated her, ate her and absorbed her which would explain who the ice woman is and how Akaza only went after his title when Douma got it. Is it a bit of a far reach? Yes but it makes sense, given his sinister and creepy nature.
