This scene right here, after Agatha lashes out in anger and says she never wants to see Rio again. There's a brief moment as she watches her walk away that you can see how hurt she is. Agatha's expecting Rio to put up more of a fight. I don't think she ever wanted her to agree and that's what shocked her the most. Because she knew that once Rio gave up on them it's really over. Kathryn Hahn's acting right here, the expressiveness of her eyes says all we need to know.

It’s heartbreaking that Agatha and Rio broke up due to the simple grief of losing a child to natural causes. Rio actually did give her the gift of time with him, but Agatha’s grief kept her from seeing that.
Even worse, Agatha blames herself for not saving him. She would rather people believe that she killed him rather than admitting that she couldn’t save him.

I cannot emphasise enough that Rio lets Billy go. Agatha, a person who is going to die someday, dying in the place of someone who is taking a second life doesn’t even out the balance. Not really. Rio needed something to use as a reason to let this boy, who reminds her love so much of their son, live.
She lets him go after Agatha dies. She doesn’t take back what is rightly hers and she lets him go find another who has broken the rules. She loves Agatha so much she gives her more time again.

Rio didn’t even get a chance to mourn her son or comfort her lover because Agatha was too busy blaming her to give her the chance, then in the end when it seems like they might be able to have some sort of understanding or resolution, Agatha says she never wants to see her face again. Of course Rio snapped I would too
I'm just thinking about Agatha telling Rio she didn't want to see her face again and now we know why. Nicky is Rio's literal twin and he's a perfect blend of them both. Rio really had to reap her own sons soul and afterwards she was rejected by her love. It wouldn't have mattered if Rio granted Agatha 6 years or 16. His death was always going to stand in between them. Let that sink in.
