When do the fissures occur?

The procedure of Kshara Sutra:
In the Kshar Sutra treatment, a surgical linen string gauge number 20 is encased 11 times with Snuhi spandex (Euphorbia neriifolia), 7 times with Apamarga Kshara (Achyranthes Aspera), and 3x with Snuhi latex and Haridra churna (turmeric powder).
In Fistula-in-ano patients, Kshara Sutra is administered by anesthetizing the patient and inserting a malleable device with the Kshara Sutra in its slot through the external urethral orifice of the fistula to the intrinsic one, then carefully removing the probe with the Kshara Sutra intact. Then, a week later, both ends of the Khara Sutra are tied together and replaced by the new one. The track is eventually cut and healed in this manner, expelling unhealthy tissue, dictating microbial infection, and facilitating drainage of pus in the tract to aid in healing

Anal cancer types
There are numerous types of anal cancer, each characterized by the malignant tumor that develops. A tumor is a type of cancerous growth in the body. Tumors are classified as benign or malignant. If left untreated, malignant tumors can spread to other parts of the body.
Tumors include the following:
Benign tumors. Noncancerous tumors are benign tumors. Polyps, skin tags, granular cell tumors, and genital warts are examples of the anus (condylomas).
Precancerous conditions. This term refers to benign tumors that can develop into malignant tumors over time, which is common in anal intraepithelial neoplasia (AIN) and anal squamous intraepithelial neoplasia (ASIN) (ASIL).
Squamous cell carcinoma In the United States, squamous cell cancer is the most common type of Bowen’s disease This condition, also known as squamous cell carcinoma in situ, is distinguished by abnormal cells on the anal surface tissue that have not spread to deeper layers.
Cancer of the basal cell. Basal cell carcinoma is a type of skin cancer that primarily affects sun-exposed skin. As a result, it is a very rare form of anal cancer.
Adenocarcinoma. This is a rare type of cancer that develops from the glands that surround anal cancer.
Symptoms to look out for:
Bleeding is the most common symptom associated with anal cancer.
Anal itching can be a symptom of cancer, many people mistake their bleeding and itching for haemorrhoids. This can cause anal cancer to be misdiagnosed.
Anal discomfort or pressure.
Discharges from the anus are unusual.

Fistulotomy – Fistulectomy
AbstractAim : Anal fistula is usually treated by either fistulotomy or fistulectomy. We described the routine use of setons to treat anal fistula without any surgery.
Method : Forty-seven consecutive patients with diagnosed anal fistulae were treated using setons alone.
Results : The median age of the patients was 41 (range: 18-70). Of the 47 patients, 15 had surgery previously for fistula and perianal abscess. At least two setons were inserted through each fistula. One was tied tightly to function as a cutting seton and this was sequentially tightened by the patient and another was tied loosely for drainage. Of the 47 patients, 33 (70%) had the placement of setons in the clinic without any anaesthesia. The remaining 14 patients had the setons inserted in the operating room, with one patient having a complex anal fistula and 13 patients having perianal abscess requiring drainage at the same time. There were no post procedure complications in the series. Forty-one patients had completed follow up at clinic within a median duration of 15 weeks (range: two to 67 weeks). The fistula was completely healed by this method in 37 patients (78%). The median healing time was nine weeks (range: four to 62 weeks). One patient developed recurrent fistula and was healed after another seton placement. No patient developed any faecal incontinence and all patients were satisfied with this treatment.

Anal stenosis is the most common type of outflow obstruction and is usually mistaken for constipation. It is the second most frequently missed anal diagnosis. This is unfortunate because it can be treated very easily in the office under local anesthetic. The symptoms are difficulty pushing a firm stool through a narrowed Anal canal.
