I think what makes the hospital scene so impactful is that it’s the first time we see Joel in a position where he can actually keep something bad from happening to her.
Think back to his conversation with Tommy and how he feels like he can’t stop failing her. They lost Tess, then got to Kansas City just to watch her take hit after hit. She shot the guy to save him then survived a hoard of infected just to lose Sam and Henry a few hours after that. Then of course what happened in David’s town that we’re not even sure he knows the full extent of.
So when he’s presented with this information that he’s about to lose her, he doesn’t hesitate. For once, he can be there in time. For ONCE, he can prevent the bad thing from happening. FOR ONCE he’s not going to fail her. He’s in a situation where he can take this little girl he’s grown to love so much and keep the unspeakable from happening to her. It’s not even a question in his mind. And he’ll be damned if anyone stands in his way. His quiet resolve in that whole sequence is chilling. He knows what he has to do to get to her and he doesn’t hold back.
He does it for Sarah, for Tess, for Sam and Henry, for Ellie. For every moment he was too slow or too deaf or too far. He saves her just like she continues to save him, both emotionally and physically.

also people scrambling to justify joel's actions with "well a cure probably wouldn't have worked anyway" or "the distribution necessary is not feasible" etc. are missing the point. because joel's justification is love and he never tries to pretend otherwise. love can be incredibly dangerous and incredibly selfish and people can and will do terrible things for it. but it is also incredibly human and that's the whole damn point.
