You suddenly wake up in the middle of a torrid summer night and you see your girlfriend sleeping next to you. Enlightened by the moonlight passing through the window you notice she’s sleeping in the perfect position to listen to her heartbeat. Without making any noise, moving slowly you find your Littmann stehoscope, you wear its eartips and after you’ve taken its drum you gently place it over her apex. The acoustic waves generated by her heart’s valves slamming underneath moves through the stethoscope’s rubber tubes till your ears and without any other noise the sound is perfectly clear and loud. Her steady and rhythmic beats fill your mind with an idea of health and peace, a flebile yet strong vibration is felt through your fingers while they move up and down following her breathing. You suddenly remember that cardiology lesson about coronary circulation and with the power of imagination you see through her skin and through her sternum directly to her heart where you see that vital hug that will provide to that loved heart all the necessary to keep going on beating from the calmest of the nights to the hardest of the efforts. It’s a very intense moment that you’d like it would last forever.

Which is the lowest heart rate to sustain consciousness? It’s really hard to find an answer… What creates blood pressure is clearly the systole and globally we can define blood pressure as: (cardiac output x periferal vascular resistance) with the first one as (heart rate x stroke volume). In the case the heart rate becomes too slow there will be an important BP drop between the two beats overcoming compensation mechanisms and this event will make the patient to loose consciousness. A normal heart rate for an average person is between 60 - 100 bpm so it’s reasonable to consider that under 60 bpm that average person should start to experience symptoms (at least on a standing or sitting position). I’ve been very busy with a new project in the real life these days so here’s the lag till my last post.
This is inspired by some videos I’ve seen in which a radiography is recorded in continuous and this enables to see the beating heart in live or as a “video radiography”. I don’t think it’s still utilized as a diagnostic tool, or its use is reduced to the minimum, considering the amount of radiations the subject would absorb.

I wanted to contextualize the idea of my previous post with this. Probably inspired by a movie or some sort of a mash-up between a well known diagnostic tool and a body scan, something like the ones used in some airports (an equipment that actually bring out some privacy concerns, for obvious reasons). Here’s a see trhough screen with a simple ecg. One day I’ll probably implement this with the sound.

“Draw me like one of your french…” ehm…. Here’s represented an echocardiogram, four chambers view with some extrasystoles. The heart is an “electrical machine” that works following a very precise order of electric discharges. The correct sequence of activation gives an effective beat. The cells inside of the heart (there are actually 3 different main groups of cells) charge and recharge spontaneously in an electro-chemical, automatic cycle. In some cases, for small groups of cells, it becomes easyer to reach a point in which they can activate a new contraction so the charge is released excessively early and considering that all the cells in the heart are interconnected the wrong sequence will spread, the correct cycle is broken and the beat less effective (or non effective). Any condition that makes easyer for the cells to reach the action potential will also make easyer the arise of local, non organized automaticities like, as example: anxiety, drugs, structural abnormalities, damages, stress (pshysical or psycological), etc.

Lena Bradycardia Part 2
"Nothing, charge again, charge to 300". Another imaginary shock ran through Lena's body. He continued with the compression. "Come on Lena, Fight Lena" Julian said with a desperate tone in his voice.
She noticed how he took the Ambu Bag in his hand and gave her several breaths. Then his hands found their way between her sternum again and began chest compressions. This time he pushed even harder. Her belly bulged more, but she enjoyed how he slowly started to get desperate.
He ran the stethoscope over her chest, looking for a heartbeat. "She's in asystole, we'll give her epinephrine after that immediately proceed with chest compressions" he called into the room. He continued chest compressions for several minutes.
She enjoyed it as he pumped cold fresh air into her lungs with the ambu bag. Afterwards, he placed the stethoscope on her chest. " Still asystole, cardiac arrest for 40 minutes, administer another round of epi" he said in a firm voice.
After three more rounds of compressions and ventilations, Julian ran the stethoscope over her chest " Fuck ventricular fibrillation, paddles to 360." he said.
An imaginary shock ran through Lena's body. This was immediately followed by another shock.
He put the stethoscope on Lena's chest and listened to her. "We have her back, she has a sinus rhythm again," he said in a happy voice.

Lena Bradycardia Part 1
I had just put on my T-shirt after showering when I got the message from my boyfriend that he would be home in a few minutes. My heart was pumping with excitement. We had both had a rigorous week at work and were just looking forward to the evening. You see, we were both fascinated with CPR and role playing. He loved it when he could play doctor and I loved it when he tried to resuscitate me. I got out the little kit we had.
The door opened and my friend came in. His face was beaming when he saw that I had already prepared everything. He put on his stethoscope and the role play began. I rolled my eyes and pretended to faint. I felt my friend come over to me and shake me and check my vital signs. "Lena, can you hear me?" he asked. She noticed how he pulled up her t-shirt and started listening to her with the stethoscope. " Irregular breathing, slowed heartbeat, she's sliding into bradycardia" he said tensely.
Julian placed a breathing mask over her mouth and nose and began to help her breathe while he continued to listen to her heart with the stethoscope. "Shit, heart rate continues to drop" she heard him say. He now completely removed her t-shirt and began to give compression with one hand to support her heart.
After a few minutes, she gave him a sign that her heart was now completely stopped. "Fuck, she is in complete cardiac arrest, start resuscitation immediately" he said in a desperate voice. He began to ventilate her, she felt the cold air flowing in her lungs and her chest rising. He then began to give her deep compressions. She enjoyed how his warm hands were on her sternum giving her deep compression. With each compression, her belly bulged and air escaped her.
She wandered into her thoughts and imagined herself in the emergency room with Julian and a team desperately trying to get her heart beating. Her thoughts were interrupted when she heard him counting out loud. „95..96..97..98..99..100“. Then she felt him give her 2 breaths. "Come on Lena, you're too young to die." he said desperately while giving her deep compressions. "Asystole for 10 minutes, we'll give her a round of epi" he said a little louder.
After giving her epi, he continued the compression. To give her better compression, he cut open her bra. After three more cycles, she gave him a sign that her heart was going into ventricular fibrillation. He responded immediately and taped one pad over her right breast and the other pad below her left breast. As he delivered the imaginary shock, she imagined the surge going through her heart, her back arching before it fell crashing to the living room floor....
