In-universe why do you think Malenia gives us Miquella's Needle and an Ancient Dragon Smithing Stone to us in exchange for the dew-soaked Unalloyed Gold Needle? We probably wouldn't think it odd if a flower just gave us nothing. Was it gratitude? Empathy in case we have a similar curse as her? General dislike of the Frenzy? A reflex in an unconscious state? A sign of respect that we're fit to be lord, as she tells us in her defeat dialogue, given that the stone is for making god-slaying weapons?
i'm not sure if i would exactly consider her conscious, and my first impulse is to consider at least the smithing stone a gameplay thing (they're usually just used as generic "you're done!" quest rewards - latenna also gives you one, for example)
but if she theoretically is conscious i guess it might be something like gratitude, a thank-you for returning the needle to her. the one she gives you is still unfinished, so perhaps she hopes that perhaps both the needle and the stone could either lead you to miquella or that you might find the means to finish it yourself, seeing as she now considers you worthy of lordship.
