Their second lead asset is a wholly owned monoclonal antibody Pamrevlumab (FG-3019) directed against CTGF, a growth factor involved in fibrogenesis. Neutralizing this factor has shown promise in multiple preclinical models of fibrosis and in an open label Ph1 study in patients with IPF, a type of fibrosis in the lung. In that clinical study, patients saw a decline in the rate of progression (as measured by Forced Vital Capacity and High Resolution Computed Tomography) on par with both Nintedanib and Pirfenidone, the two competitors on the market. Moreover, in ~25% of patients there was an improvement in lung function, arguably making it the first drug capable of reversing fibrosis in humans. IPF affects around 100K people in the US and sales of IPF drugs are projected to exceed $1B by 2020. FibroGen has now fully enrolled a placebo-controlled Ph2 study with expected results in summer 2017. They will also be including a 6-month sub-study evaluating the efficacy of FG-3019 as a companion product with the standard of care. Data for this study is also expected in summer 2017.

The FDA approval of pirfenidone and nintedanib in October 2014 as the first-ever treatments for idiopathic pulmonary fibrosis (IPF) represented the culmination of decades of research into the mechanisms underlying this devastating disorder. The understanding of this disease as an epithelial-cell-driven aberrant wound healing process orchestrated by transforming growth factor beta (TGF-β) not only led researchers to study these two agents, but opened the door for a number of other therapies. Many of the agents under investigation have the exciting potential to not only slow fibrosis, but halt and even reverse the fibroproliferative cycle in IPF. A number of lines of evidence suggest that no single agent is responsible for the lung injury seen in IPF, but that individuals with IPF have intrinsic epithelial cell defects that render the epithelial cell susceptible to repetitive injury from any number of environmental agents (e.g. cigarette smoke, acid reflux, viral infections). This injury leads to aberrant signaling between the lung epithelium and mesenchyme, resulting in a persistent, abnormal wound healing response.

The respiratory system is a vital part of the human body for it supplies oxygen to the different parts of the body which is necessary for survival. As the body’s breathing mechanism, it is composed of the nose, mouth, trachea ( or wind pipe ) which collectively serve as the airway, a pair of lungs and a muscle for respiration, the diaphragm. The respiratory system begins with the nose where air is inhaled and then carried through the nasal cavity into the pharynx. Once air passes through the pharynx, it goes into the trachea, and from there, enters into the lungs. H/T - Pieter van den Assum

Nanoparticle Exposure Can Awaken Dormant Viruses in the Lungs Nanoparticles from combustion engines can activate viruses that are dormant in in lung tissue cells. This is the result of a study by researchers of Helmholtz Zentrum München, a partner in the German Center for Lung Research (DZL), which has now been published in the journal ‘Particle and Fibre Toxicology’. To evade the immune system, some viruses hide in cells of their host and persist there. In medical terminology, this state is referred to as a latent infection. If the immune system becomes weakened or if certain conditions change, the viruses become active again, begin to proliferate and destroy the host cell. A team of scientists led by Dr. Tobias Stöger of the Institute of Lung Biology and Prof. Dr. Heiko Adler, deputy head of the research unit Lung Repair and Regeneration at Helmholtz Zentrum München, now report that nanoparticles can also trigger this process.
