Imo Istvan seems like quite an empathetic person despite his dog vs dog ideology which I think is a nice contrast and a much fresher character than a cold-blooded villain would be. The ways in which he compares Henry to himself more than once, then there's 'I almost feel sorry for Radzig's bastard'. The first words from Erik's mouth in KCD1 when prompted to describe Istvan are 'He's an orphan, same as me' which is an peculiar point to start with and makes me think it's something Istvan himself emphasized in his relationship w Erik

Of course most of the time Istvan uses this ability to know which buttons to push and where to hurt.

Apparently you can run into Father Nicodemus later in the game and he tells you he was scared of István because István was weird (because he wanted him to bless his armor and sword???) Also, apparently, Father Nicodemus believes Istvan committed suicide, so, he definitely considered him mad and unwell. Interesting.
