I was just thinking about this not even 2 days ago. What he's saying is definitely not matching how he truly feels because in the bath later that day, he insists again on calling Izuku by his true name. Aside from putting to rest their negative history and how "Deku" was first formed, to help relieve any extra pressure Izuku might still suffer from it, we KNOW that's also his way of being and feeling CLOSER to Izuku. He was telling him that he is now somebody Izuku can trust, and honestly? Kacchan was loving him quite earnestly at this point. :')

Lmao "he didn't expect them to change" and yet, he wasn't going to relent on being allowed to feel closer to Izuku, no matter what. He had definitely been struggling with feelings of not knowing how to change their dynamic to what he knew should have been 'close'. The people around them often commented on them being "childhood friends" despite them not being emotionally close (I'd say what they had was a spiritual closeness), and he would watch as Izuku formed these close friendships with others with a kind of... longing? that things could be different between them.

He needed their relationship to match the softness that had formed, the love he now felt for Izuku with no outlet to go that was building deep inside his heart (sigh), and ch 322+327 was him finally being honest to Izuku and HIMSELF about it and giving priority to his own emotional needs maybe for the first time in his life.
