In your opinion, do you think doing omorashi holds has any possibility of weakening your bladder and making you have issues in day to day life? Everyone likes having some fun every now and then but Iโ€™m not trying to risk permanent damage to an internal organ and even though I like omorashi I do NOT want bladder leaks for real.

the short answer is "yes, but it depends on how you do it."
the longer answer is: it depends on whether you're holding until you actually lose control, it depends on how often you hold, it depends on what kind of shape you keep your pelvic floor in, it depends on how well you listen to your body's signals, it depends on a lot.
in my view, it's basically about risk management, because lots of things we do every day have the possibility of long-term consequences.
if you want to minimize any theoretical possibility that your bladder would weaken late in life, you wouldn't really want to hold much (if at all.) you'd also want to be doing preventative shit to be good for your bladder long-term.
if you're mostly concerned about whether you'd do something that could fuck up your bladder and lead to incontinence, you probably shouldn't hold all the way to the point of failure (i.e. significant leaking and/or wetting) all that often, and you probably shouldn't be intentionally holding more than a day or two in a row without giving your body time to recover.
i've certainly read accounts from people who've done much worse to their bladder on a regular basis who've had no issues with incontinence, but that's purely anecdotal.
for a healthy adult, you're probably not going to screw anything up as long as you don't usually hold past your limit (to the point of wetting) and don't hold more than once or twice a week and stop any time your body gives you a pain signal. again, this isn't medical advice, and if you find your body isn't working like it used to be, take a break for a while and let things return to normal!
personally, i find that the muscles involved in holding are a little tired the day after a really intense hold, but i also feel like my bladder actually works a little better when i'm in the habit of holding every now and again than it does when i never intentionally hold. i wouldn't base any assumptions off that, but i think it's kinda neat.
idk how i became the "but how safe is omo REALLY" blog, but i don't mind answering these questions. i just hope y'all keep in mind that, like a lot of kinks, 1. yes there is risk inherent to play and 2. you can typically navigate the kink in a way that minimizes the likelihood of damage. in my opinion omo isn't as inherently risky as like. choking. or suspension bondage. but it's definitely still got a risk component and it's good to remain aware of it!
