In advance of the Big Announcement this Thursday, I made a vlogbrothers video today on how we end TB–with the comprehensive care plan often known as S-T-P, which is short for “Search, Treat, and Prevent.” But one thing I didn’t discuss in that video is the downstream benefits of comprehensive TB care.

Once you’ve hired community health workers to screen for TB, it becomes much easier to screen for other illnesses like diabetes, high blood pressure, and non-TB lung issues (especially lung cancer). TB is notoriously a disease of vicious cycles–a disease of malnutrition that makes malnutrition worse, a disease of poverty that makes poverty worse, and so on–but addressing TB can be a story of virtuous cycles: TB survivors become TB advocates, as I’ve seen with my friend Henry in Sierra Leone. More effective TB treatment leads to less stigmatization of the disease, as communities come to see the disease as curable and survivable rather than terrifying and deadly. And better access to TB care leads to a stronger overall healthcare system, because more community health workers are better connected to more primary healthcare clinics, which allows communities to better address all kinds of health problems.

Mycobacterium tuberculosis is not the only bacteria of its family that causes a lot of human suffering; there is a closely related species called mycobacterium leprae that causes the disease known as Hansen's Disease, or more commonly leprosy. There are still around 200,000 cases of leprosy diagnosed each year around the world, and while the disease is curable, it also remains��especially if not caught and treated early–a significant driver of suffering and disability in our world. 

There are many connections between TB and leprosy: Not only are the bacteria that cause these illnesses very similar, but patients have often expressed similarities in experience. TB patients who were encouraged or forced to live in sanitariums often compared themselves to lepers. One disheartening parallel between the diseases is that in both cases, those living with these illnesses are often abandoned by their families and must make new social connections within the new community of “leper” or “consumptive.” Also, both Hansen’s Disease and TB continue to exist largely because of systemic failures rather than due to a lack of knowledge or technology.

Last link from me today: I’ve been thinking a lot about the complex intersection between public and private investment (for reasons that will be clear on Thursday!) and I keep coming back to one infographic in an excellent paper (https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0256883) about the public money that was poured into the creation of the GeneXpert Machine, which can quickly and accurately test for TB. The GeneXpert machine has created a lot of profit for Danaher’s shareholders, and it has also created some societal benefit, but it could create a lot more societal benefit if it created less profit for Danaher’s shareholders. This tension seems to me one of the defining features of 21st century life. Anyway, here is the infographic:

That’s the money–over $250,000,000 of it–that came from taxpayers (mostly in the U.S. and Europe) to fund the creation of the GeneXpert Machine. And yet, this tech largely funded by the public is controlled entirely by private enterprise. I’m troubled by that model of value allocation, even if I still believe that private money and private enterprise have important roles to play in fueling innovation. But taking a quarter billion dollars of public money and then claiming total ownership over a technology, and using that ownership to deny the technology to the world’s poorest people, seems like a deeply flawed system of resource distribution to me.

Excellent addition to the series ~ Couldn’t put it down!

What I liked:

* Audrey: dowager duchess of Fournier, has been traveling in Europe for five months with her sister-in-law Cassie, in love with Hugh, enjoys solving mysteries, intelligent, impulsive, possesses paranormal psychometric abilities, is accused of a murder she did not commit, her life is in danger

* Hugh Marsden, Viscount Neath: ex-principal Bow Street officer, scandal in his past, good friend of Dr. Thornton, well-educated and eloquent, protective, intelligent, accepting, intriguing, in love with Audrey and plans to marry her when she is finished with the mourning period

* Sir: born with the name Davy Given, a young man from the streets befriended and employed by Hugh as his assistant, savvy, capable, resourceful, intelligent, growing up quickly, stays away from his abusive father

* Thornton: physician, Hugh’s friend, compassionate, capable, supportive, widow that loved his wife, intriguing, might be interested in Cassie * Cassie: Audrey’s sister-in-law, had a misfortunate situation to deal with in a previous book that has left her less than willing to trust men, looking forward to seeing what her future holds in store for her, she may be interested in Thornton

Other information you might like to know (that isn't decently covered in the reference article)! This list is based off my own knowledge and common mistakes/misinterpretations I have come across in fics which you might also have come across, not realising it is inaccurate! We love being obsessive about such a niche topic as carriages.

Horses would be changed roughly every 12 miles (no more than 15 unless it really couldn't be helped, and really it was just not worth it to change it if only 10 or less miles had been traversed)

2. Horses would generally be changed at a stage stop, also known as a stage station. These were generally established specifically for the purpose of individual riders changing horses or coaches/carriages doing so. They do not necessarily coincide with towns/villages or inns.

3. Most carriages with hard bodies (thus generally more accurately a coach) are designed to be used at night as well as in the day. They would have usually been oil lamps, at the time, and they were surprisingly big (generally 40cm-50cm tall in total, or about 16"-20"). Of course, you would likely not find it as comfortable (given you do not have enough light within the carriage to do anything), and it would make the roads more difficult to navigate and thus more dangerous, but they would be used. If you only have one or two hours left of the journey, it does make sense. In summary - for the wealthy, travelling was not restricted to the daytime.

4. Almost every single carriage ever has had a step built into the framework. They are not that difficult to enter or exit, and being helped into a carriage is mostly a status or respect marker (depending on whether you are helped in by a servant or someone of your own status).

5. If you were wealthy and had a carriage, it would have your family crest painted onto it. Details are important, especially status symbols. This is not necessarily as important as the other points, but it interesting nonetheless.

As a quick aside about this last one, in Persuasion, if Mr. Elliot had not left Lyme in a curricle but a coach of some sort, he would have had the crest on the side, and thus been identifiable to the party espying him from within without them having to ask after his identity. Of course, he likely could not afford that at the time, but they would not have known that.

I could not discuss carriages and not mention, in length, the travelling chariot. Serious fanfic writers NEED to know it. So, without further ado.

At the time, the main private carriage used for long journeys by the (upper) gentry was a travelling chariot. It would not be used for short trips (i.e. Longbourn to Netherfield) but for longer trips, even those which could very easily be done in a day (i.e. Longbourn to London). I cannot emphasise to you how key this carriage was. It has a hard body, space for luggage under the seats and atop the carriage, as well as room for two people on the front outside (one of whom would be the coachman, aka the driver, and the other of whom would generally be a footman. Also regarding footmen, they would almost always be the servant helping anyone from or into any sort of carriage) as well as space for two other servants on the rumble seat at the back, which is also exposed. Usually a valet and a maid would sit here. Many also had a boot of sorts by the rumble seat, for even more luggage, and spikes on the framework that extended beyond the rumble seat to prevent people grabbing on to the carriage and hitching a ride.

Why so much storage?? People would literally tour Europe in these. They are the ultimate comfort travel carriage of the time. Because they would be heavy, they would be drawn by four horses, making them expensive to maintain. For a vague idea: the Darcys would definitely have one, the Bennets definitely would not have one, and the Bingleys likely would not have one, but might hire one for their travels instead.
