Maybe it isn't that I actually hate medical professionals? They just suck and are weird sometimes, and a lot of them shouldn't be practicing, but I don't hate them as a group, like, personally.

What I hate is their ability to make my life harder in ways that are often completely opaque to me, and a lot of the crap things they do are not really possible to challenge. And I hate the fact that holding them responsible fort dogshit behavior in any way that will actually benefit me is almost always impossible.

And I also hate the fact that they have to do stupid things sometimes because that's how the system is set up, and those things sometimes mean patients actually get harmed. They aren't fond of that part either! They don't want the system to be the way it is! But they don't have a choice, so sometimes people like me get forced by bureaucracy into doing things that are re-traumatizing. And I can't imagine that feels good for them at all, knowing that their patients are sometimes only "consenting" because that bureaucracy will not let them be helped in any other way. Which isn't consent at all. I imagine that must be pretty traumatizing for them, too, sometimes.

If it were easier to actually access medical care without tremendous delays in this country right now I would have much less trouble finding providers who are good at what they do and are not horrible people, and who have clinic staff who can do their fucking job.

Oh and I also don't appreciate how evasive and unwilling to commit they are out of fear of being held to an answer that turns out to be inaccurate, but I can't make an informed decision about my own care unless they give me at least some information about probabilities and trajectories and typicalities. Genuinely, how the fuck am I supposed to navigate that shit. I get that some patients are really fucking difficult, but I should be able to get a special stamp on my file or something that says I understand that sometimes medicine isn't an exact science and the best answers that my doctors can give may not always prove to be accurate in the long term. I know they don't like being in that situation either.

A lot of medical professionals are fucking assholes, and unfortunately the ones who are not are still hamstrung by a system set up to actively prevent people from getting care.

I miss my old doctor. He gave no shits about anything that wasn't the patient. He prescribed scheduled meds based on what the patient needed and not based on fear of consequences potentially being imposed on him by the punitive patient-hostile drugs-are-bad moral panic machine developed to force suffering people into buying more dangerous drugs off the street in order to prevent far fewer people from maybe getting high off of drugs that at least weren't laced with lethal substances. (The purpose of a system is what it does.) Did he get sanctioned and become locally unhireable? Unfortunately yes he did. Does he now provide concierge care to rich people? Yes he does. He found a way to make it work, God bless him.

Everything about the medical system in this country is fucked. Hospitals, doctors, nurses, pharmacies, pharmacists, pharmacy techs, phlebotomists, clinic administrative staff, insurance companies, medical schools and schooling, licensing boards, drug advertising to both providers and patients, pharmaceutical reps, researchers, research, publishing, medical trials, pharmaceutical companies, manufacturers and distributors, medical equipment, charting software, billing and billing codes, diagnostic criteria, charity and low income services, accessible transportation, home care, the lack of independent individual patient advocates, dietitians and nutritionists, access to physical and occupational therapy and physical and occupational therapists, the massive bigotry of every kind rampant in every corner of the medical field, social work, senior care and assisted living, deprioritization of informed consent and harm reduction, disability applications, inaccessibility of medical records, especially psychiatric notes which are specifically allowed to be withheld from patients, lack of continuity of care for disadvantaged people, care that is equitably accessible to disabled people, telemedicine, patient portals, phone systems, clinic hours, every single aspect of inpatient and outpatient psychiatry, facility security, all sorts of things going on with therapists who are nevertheless probably the least malicious group of people in this entire charade, aaaaaand patients themselves.

Also hospital toilets that are too tall and make it literally physically impossible for me to poop while I'm there waiting for somebody to come out of surgery. I just needed to take a crap, guys. You didn't need to make the toilets so tall that my feet didn't even touch the floor. It is very clean but there is no shitting for short people at St Francis.

my coping methods for people dying are ridiculous.

Volunteering for food programs and elderly services so I can feel literal life giving with every moment of ingredient preparation, cooking, serving, waitressing, and dish washing. The face someone makes when they first bite into something that soothes them. The content face one makes when they are warm.

performing concierge services and emotional counseling for people that can only afford it for free, that like to have their hands held. I cut their nails, file them, and polish them. I put together a meal for us and pour their tea and coffee.

I beg the men to tell me about life through war and they always end up telling a story of how they met a woman who’s love prevailed against all the fear and loathing in the world. I beg them to tell me the secrets of fatherhood, as the evidence of their love lays around them in the form of devoted and grown adult-children.

I beg the women to tell me their secrets of loving and raising good people throughout the years. So they always end up tell me stories of binding and tending wounds, how to doctor and barber and sew, how they can tell love from manipulation, to explain empathy and compassion from performative sympathy, and how to theoretically grow anything from an empty space.

My transition plan is set and approved. I tell my team next Thursday. My boss said “you and KG (my colleague who they’ll report into) have thought so much about my experience, I really appreciate that. I’m not used to it.” I replied, “Well, we love you. You’ve helped us a lot. Your experience at work matters”, and I meant it. She’s a lot of things - I’m not sure what’s real or not real in her but I’m also not sure what’s real or not real in me most of the time. So why not just decide to believe she’s worthy of kindness and love and support.

I put a number of things in motion for next week. It’s going to be a massive lift between now and summer but I’m up for it.

Alki is really trucking along. I asked the contractor to hire an electrician just to check everything and make sure it was wired correctly given I’ve had a couple of power surges and the power seems to keep going out on one side of the house. It’s good that I did, they discovered that the initial contractor who also did the electrical wiring did it incorrectly and it could’ve caused a fire. So it’s all fixed, and now I’m going to do the upper unit as well. I’m not really sure why I’m feeling so calm about the expense, I just know I need to get all of this done to make sure it’s safe.

My sister is incredible, I am paying her to help me that it’s a lot less than what I would pay to a project manager. She’s finding the stone that needs to go around the fireplace, creating budgets and just making sure everything is on track. We think so similarly and she really knows what I like which makes it so much easier.

I’m also hiring this concierge service for medical stuff. I have these great benefits that I’ve hardly used in 15 years and I’ve just decided to invest in my physical health. I’ve always focused on my mental health and I’m glad that I did but now it’s really important to get over this phobia of doctors. I need a baseline of where I’m at, help with scheduling a mammogram, blood work, cancer prescreening, and getting on a program where I can lose some weight. This is the age where stuff starts going wrong and I’ve been really lucky that I’ve been pretty healthy – at least I think so anyway – and the service is meant to make it really easy for you to get started. I signed up today.

It’s thunder and lightning outside, I just wrapped up work and now I’m watching season two of The Bear over again. How is it that I have previously loved almost every cast member before they starred in the show? I think this will be in my top five for the rest of my life, a show I watch over and over again.

The Concierge’s Concern (Part Three)

The Manager is a woman of impeccable grace; savage and elegant all at once. Hair curled into a perfect coif, piercing eyes drifting up from the computer to look up at you as you enter the room with a knock.

“My Heart,” she purrs, beckoning you with a hand. “Come. What concerns you?”

Five steps bring you from the elevator to her desk, where you stand with your hands loosely folded over your belly. “Mister Sans has returned again.”

Her eyes narrow, darting over your face as though looking for something. She finds it with a sigh and rises from her chair to look out the window over the sprawling cityscape. “Did he come with any associates, this time?” There is a slight lilt in her voice, an unidentifiable accent. 

“No, ma’am.”

“But you’re expecting him to.”

Your eyes flick to a run down warehouse at the corner of the city, dark smoke rising from the chimneys. “They were previously unaware of our other services, but Mister Sans requested a doctor. I am expecting his brother to come knocking soon.”

A tilt of the Manager’s head summons you to her side. “Normally this would hardly phase you,” she looks at you sidelong with interest and a small smile on her darkly painted lips. “And yet...”

You flick your gaze down to your hand, raising it to your chest. Your fingers relax, then fist into a ball weakly. The strained, deeply scarred muscles protest at the movement, until you force past the ache. “They seek a seat at the High Table.”

“And you are sure?” The Manager’s face turns serious. 

Turning your body to face her, you nod with the slightest incline of your head, murmuring, “I suspect it. I have heard that they are making a play to gain the attention of the other Families. Familiarising themselves with the Continental Hotel is but one of the ways they are slinking into this world.” After all, Mister Sans has become a familiar face. Along with the rest of his...gang.

“I trust your suspicion. They do not know what they seek,” the Manager sighs. Crosses her arms. “This will shake the High Table - it will tip that precarious balance.” She looks back at the window then, tracing along her jaw with a manicured finger. “But I have the feeling that that’s not what you’re concerned about.”

A moment of hesitation. You did not think you’d have to explain yourself, but the Manager knows you too well. And you know her too well. 

“This is no life for those who have newly earned their freedom,” you say quietly, tapping the back of that weakened hand with the gloved fingers of the other. “It is impossible to escape once you are under the Table. You cannot buy your way out, bargain your way out...” 

“You only change masters,” the Manager completes your sentence for you with soft eyes. “I know. You see yourself in him.”

“Perhaps a bit too much.” The rueful smile on your face stretches the unused muscles, your impassive expression softening into something real. “And I know how that went.” Your weakened hands flex.

The Manager hums. Considers your words, considers her own thoughts. And then speaks. “Then go. Warn him. But do not expect anything to change.” She turns to sit back at her chair, her legs crossing primly. “You know their type.”

You close your eyes but for a moment, sighing through your nose. “All too well.” With those parting words, you pace back to the elevator, summoning it with a tap of a button. “Would you like to have tea brought up for you, ma’am?” Your voice is back to its measured monotone.

“Yes. Irish breakfast, if you please.”

“Of course.”

Ok, get ready. Because THIS is one of my favorite things to talk about when it comes to Gen…You’re not missing anything because “parasite cleansing,” as she’s referring to it, is not a real thing. In recent years, G has become obsessed with “gut health” alongside the rest of the wellness community. Part of that is a concern for parasites that may be upsetting your delicate GI balance. 🙃 More info on the trend, scam, and risks here.

Time will tell if she actually circles back around to talk about this more like she said she would in that video. I hope she tells us who suggested it to her. But overall, Gen’s hypochondria stems from a mix of generalized anxiety, perfectionism/the drive to be constantly improving her “health”, and an abundance of access to resources and “professionals” who validate her concerns. G has been on this health optimization kick for quite a while but she recently named one of her major influences.

In the podcast where she talked about her implants, she also mentioned her doctor by name; Dr. Sharon Hausman-Cohen of Resilient Health. This whole medical practice is set up for the rich and the anxious. A few things stand out to me upon reading the doc’s bio on their website: First, she is the lead scientist for IntelxxDNA, a genetic testing service that claims to be able to tell you what medical concerns you may be at risk for, and advise on next steps.

As I researched this service further, other resources would note that one risk of this kind of testing is an increase in stress and anxiety. And that makes sense if you find out your DNA makes you at risk for some super scary disease. Just ask Gen:

So if I’m reading between the lines here, it sounds like G got her DNA profile read at this practice and then has become, understandably, even more hyper-aware of her body and body sensations as a result. And then you have a doctor with an invested interest going, “oh no, let’s monitor how you’re feeling at all times.” This is a recipe for disaster when you’re anxious. You will see what you look for, without fail. No wonder she bites her cuticles.

Furthermore, the clinical validity and utility of these tests cannot be confirmed as of yet. (Source) Clinical utility refers to whether the test can provide helpful information about diagnosis, treatment, management, or prevention of a disease. So all this hyper-focus and making it rain on this doctor could all be for nothing.

You might saying, “what does she mean by making it rain on the doctor?” Well, finally there’s this part:

Private membership at the doctor’s office? If you’re unfamiliar with this form of boutique or concierge medicine, it may be because you aren’t in the right tax bracket. Here’s a fairly quick rundown that I like, but it’s basically paying a monthly out-of-pocket fee to a physician for 24/7 access to their care. For my friends in the US, our insurance plans could never. (And you still need health insurance or deep pockets on top of it for additional prescriptions, tests, etc…seriously, read the article above.)

I won’t go into the ethical implications of providing access to care to only the rich, but it’s definitely there imho. Plus check out the practice’s condescending phrasing: a membership is the ultimate self-care. Don’t you care about your health??! Inquire about giving us your money today!

So why am I going on and on about this? Because our dear Gen is stuck in an echo chamber of health anxiety and she wants you to be right there with her. There’s nothing inherently wrong with integrative medicine or holistic practices, but life is hard enough without allowing this broad to add to your stress. If you find yourself asking if you too need some crap she’s peddling, the answer is likely no. And she doesn’t need it either. She just has access to it...and she’s probably hoping you help foot her bill with a purchase through a monetized link.
