Wound Care
Ok so, take this with a BIG grain of salt, because I may be a medical doctor BUT you need to know how much wound care training we get in medical school: none. Zip. Zilch. There may be medical schools where you do, but mine wasn't a bozo factory and there was NO wound care training. Everything I know I learned from one of several sources: an intensive 2-day wound care course I did in residency (highly recommend), the local Home Health wound care nurse (highly recommend), a completely batshit insane old white male doctor who started our learning sessions by yelling Vietnam War stories at me (do not recommend), a hospital wound care nurse (highly recommend), and experience (oh god do not recommend).
The first thing you need to know is that wound healing varies dramatically across the course of a lifespan. Kids? Kids will heal. If they don't, get their ass to a pediatrician because there's something genetic going on. Young adults will heal. Middle-aged adults will heal. You know who doesn't heal for shit? The elderly, and people with severe illnesses, and people with uncontrolled type II diabetes.
Your body needs several things in order to heal. It needs macronutrients, so you need to be able to EAT protein, fat, and carbs. If you are on total parenteral nutrition, aka TPN, aka IV nutrition, you are going to be worse at healing. If you are starving yourself, you are going to be worse at healing. If your body is desperately funneling all the calories you take in to surviving your COPD or cancer, you are going to be worse at healing.
It also needs micronutrients. If your diet sucks, you won't heal. Take a multivitamin once in a while.
There are two CRITICAL skin components to healing: collagen and elastin. Guess what we stop making as we age. Promoting collagen isn't just good for "anti-aging," it's good for NOT ripping your skin apart. Taking oral collagen is probably bullshit because your body is going to have to disassemble it to get it across the intestinal membranes to absorb, but it's also harmless, and if your diet REALLY sucks, who knows. Give it a try. Collagen is made of amino acids; think protein.
Another absolutely crucial component is blood flow. As people age, they start to develop cholesterol plaques lining arteries that eventually pick up calcium deposits. This makes blood vessels less elastic, which is a problem, but eventually also blocks them off, which is a much bigger problem. If someone has the major blood flow to their feet decreased by 90% by arterial stenosis, they are not going to heal for shit AND their foot's gonna hurt.
One component of blood flow I hadn't thought about before going into medicine is fluid retention. The way your body works, blood exits the heart at a very high velocity, but slows to a crawl by the time it gets into capillaries, the smallest blood vessels in the body. Water is a very small molecule and can leave the blood vessel, especially if there aren't big, negatively-charged molecules like proteins like albumin in the blood vessels to hold the water there. And we're built for this--some water is supposed to leak out of our blood vessels when it gets to real little vessels. It gets taken back up by the lymphatic system and eventually dumped back into the bloodstream at the inferior vena cava. But if you aren't making albumin--for instance, in liver failure--you may leak a LOT of fluid into the tissue, so much that your legs get swollen, tight, the skin feeling woody and strange. This isn't fixable by drainage because the fluid is everywhere, not in a single pocket we can drain. And because it puts so much pressure on the tissues of the skin, it often results in ulcers. Congestive heart failure, liver failure, kidney failure--these are all common causes of severe edema, aka swelling due to fluid in the tissues. And they're a real bitch when it comes to wound care, because we have such limited resources for getting the fluid back out, which is a necessary first step to healing.
Pressure is another common cause of wounds. Pressure forces blood out of those little capillaries, so you starve the cells normally fed by those capillaries, and they die. It's called pressure necrosis. Very sick people who can't turn themselves over--people in the ICU, people in nursing homes--are especially prone to these wounds, as are people with limited sensation; pressure wounds are common in wheelchair users who have lost some feeling in the parts of their bodies that rub against those surfaces, or diabetics who don't notice a rock in their shoe.
So, if you're trying to treat wounds, the questions to ask are these:
Why did this wound happen?
-Was it pressure? If it's pressure, you have to offload the source of the pressure or else that wound will not heal. End of story. You can put the tears of a unicorn on that thing, if you don't offload the pressure it won't heal.
-Was it fluid? If it's fluid, you have get the fluid out of the issues or else it won't heal. You can sometimes do that with diuretics, medications that cause the body to dump water through the kidneys, but that's always threading a needle because you have to get someone to a state where they still have juuuuust enough fluid inside their blood vessels to keep their organs happy, while maintaining a very slight state of dehydration so the blood vessels suck water back in from the tissues. You can use compression stockings to squeeze fluid back into the vessels, but if they have arterial insufficiency and not just venous insufficiency, you can accidentally then cause pressure injury. The safest option is using gravity: prop the feet up above the level of the heart, wherever the heart is at, at that moment, and gravity will pull fluid back down out of the legs. Super boring though. Patients hate it. Not as much as they hate compression stockings.
-Was it a skin tear because the skin is very fragile? This is extremely common in the elderly, because they're not making collagen and elastin, necessary to repairing skin. If this is the case, make sure they're actually getting enough nutrition--as people get into their 80s and 90s, their appetites often change and diminish, especially if they're struggling with dementia. And think about just wrapping them in bubble wrap. Remove things with sharp edges from their environments. I have seen the WORST skin tears from solid wood or metal furniture with sharp edges. Get rid of throw rugs and other tripping hazards. I had somebody last week who tried to a clear a baby gate and damn near destroyed their artificial hip.
The next critical question: why isn't it healing?
-Are you getting enough nutrients? Both macro and micro?
-Are you elderly?
-Are you ill?
-Do you have a genetic disorder of collagen formation?
Fix why it's not healing and almost anything will heal. If you're diabetic, find a medication regimen that improves your sugars and stick to it. If you're anorexic, get treatment for your eating disorder. If you have congestive heart failure, work with your doctor on your fluid balance. Wear the damn pressure stockings. Prop up your feet.
If, after those two unskippable questions are done, you want to do something to the wound--apply a dressing, do a treatment--that's a whole other kettle of fish. I'll write that later. The dryer just sang me its little song and I need to put away the laundry.

there's a certain kind of article on the internet that pisses me off so badly. it's written by a person with a surface-level understanding of the topic and little-to-no personal experience, who collates a few sources and then litters the information highway with yet another piece of trash
today's trash article is a discussion of bone broth.
my position on bone broth is that it has personally done a lot for my physical health, energy levels hair, skin and nails. but it matters a lot HOW i've been doing it. first, i've been cooking it myself, from organic bones, and with alcohol and certain kinds of vegetables, which increases the breakdown of the bones and release of nutrients in the water. second, I have been eating Mass Quantities, that is 1-2 bowls a day, with seaweed and other vegetable nutrients added, for almost 2 years now. the level of healing i've experienced is directly. proportional. to the amount and quality of the broth.
so it can help you. but you have to use quality ingredients, and invest time and energy into cooking it right, and you have to eat A LOT. and yes, many people don't have the time, resources and money etc, of course! so of course this can't help people who don't have the time, resources, money etc. this is not a cure all.
what makes me mad is the stupid black and white thinking the internet is prey to. like, if not everyone can benefit, then NO ONE can benefit. if vegans and vegetarians cannot benefit, then NO ONE can benefit. if poor and under-resourced people cannot benefit, then NO ONE can benefit. (the biggest of eye rolls)
and like, yes. people who don't beleive in eating animals exist. people with lack of access to resources exist. I feel a lot of compassion for both of these groups, but I can't! do anything! about their concerns! does that mean I am not supposed to share a method that CAN work for SOME people?
I myself am a former vegetarian. I've discovered that vegetarianism, combined with overwork and a lack of sleep, fucked my health right up. Do you know what I've found that fixed that? fucking bone broth. Is abandoning vegetarianism right for you??? maybe not! is bone broth right for you?? maybe not!! and that's okay!
so but, along comes this article which apparently is a gotcha about that bone broth is useless. which, if it were actually sourced from actual studies about actual bone broth, I might consider its arguments.
but you know what it actually argues?
that a) collagen supplements provide more and better collagen than bone broth, and b) you need the minerals from vegetables to make them available. Okay, but where do you think the fcuking collagen in supplements comes from? Fucking industrially produced fucking bone broth. just, y'know in giant factories, not in your kitchen, and reduced water content so it's a gell or a pill and not a soup. so what they're actually arguing is that industrially produced bone broth is better. which...okay, but that's bone broth. and it's true about the vegetables by the way, which is why you cook the fucking bones with vegetables when you make the broth, to make the bones more bioavailabile. So the recommendation here is actually: don't make bone broth yourself, take our pills and eat your salad like a good little girl.
that because proponents of bone broth tell you that glucosamine is one of the components of bone broth, and that glucosamine supplements don't work, therefore bone broth doesn't work. but we're not talking about fucking glucosamine supplements? we're talking about bone broth? which you haven't shown a study about the uselessness of?
then they're like, "okay, there's supposed to be protein and calcium in bone broth, but you can get more protein from other sources, and the calcium is actually very little, so you see, it's not very useful". you know what's very high in calcium? milk. do you know how bioavailable the calcium in milk is? Not Very! because you need magnesium to put calcium in the right places in your body. without magnesium, calcium ends up in your joints, causing stiffness and arthritic symptoms. magnesium signals calcium to go to the right places, your bones. If you cook bone broth right, with vegetables, then you have the magnesium to do the job right. but also, you have to drink a lot, on the daily. it's not like taking concentrated vitamins, it's broth, it's just soup, it's going to take greater quantities. Also the amount of daily calcium you need is not as much as milk ads have made people think.
here, all they say is they have no evidence to support that bone broth helps leaky gut syndrome. how and why does this merit its own number on their list? "Uh.....we got nothing, but we're going to make it a bullet point" okay there player
aaaaand this is the only point I agree with, because I'm also sick to fucking death of all of the claims of detoxing around. you can't detox your body. your liver does that shit. you can maintain your liver and kidney health to HELP them do that shit, but you can't eat stuff to do their job FOR them.
and so the article's grand conclusion? what they actually write is that maybe bone broth is okay, it's neutral. SO their main point is actually that bone broth is okay? Even though the tone of the article is trying to lead you to believe that it's a big scam, even though they have maybe one point that supports that?
so how is this article actually informing you in any way about the supposed failures of bone broth? all they're saying is that collagen supplements--industrially produced bone broth--is better than home-made, that glucosamine supplements are not bone broth, that bone broth doesn't have 'enough' calcium without discussing bioavailability and actual studies of the amount of calcium needed and the actual amount of bone broth required, they can't even back up their argument about gut health and they have to concede that it's still actually nutritious.
okay so that's your gotcha article.
Listen I understand that bone broth has become trendy, and i'm also sick to death of food trends that take off because they've been passed around the internet looking for the next big thing to promote. but articles like this are also fucking shady, because they're using false equivalencies and ad hominem and faint praise to do their takedown, instead of actual fucking facts and studies. it's just as lazy as all the next-food-trend promoters.
I just want the internet to actually be the accessible central repository of all human knowledge that we all think it is, is that too much to ask?

BALITANG REHIYONAL: An Illicit Skin Lightening Products from Pakistan found Mercury-contaminated cosmetics in Tacloban City, says Toxics and Environmental Watchdog Group

TACLOBAN, LEYTE -- Environmental watchdog group 'BAN Toxics' recently exposed the unlawful sale of three (3) variants of 'Goree' skin lightening products (SLPs) which previously banned by the Philippines' Food and Drug Administration (FDA) in the late mid-October 2017 due to health damaging mercury concentrations.
In an exclusive market surveillance conducted by the BT Patrollers, the said group discovered the localized over-the-counter (OTC) sale of Goree Beauty Cream with Lycopene Avocado & Aloe Vera, Goree Day & Night Beauty Cream Oil: Free Total Fairness, and Goree Gold 24K Beauty Cream, pricing at PHP300 to PHP350 (roughly between U$D5.15 to U$D6.00 or PKR1,425.76 to PKR1,663.39) in beauty and wellness shops along downtown area within this week by the end of May 2024 in Tacloban City, Leyte.
Using a chemical XRF (X-Ray Fluorescence) analyzer to determine mercury concentration, the group found that 'Goree Beauty Cream' with Lycopene Avocado & Aloe Vera had 21,700 parts per million (ppm) of mercury, 'Goree Day & Night Beauty Cream Oil: Free Total Fairness System' had 22,900 ppm; and 'Goree Gold 24K Beauty Cream' had 21,800 ppm. All of these unsuspected skin creams from a manufacturer source out of Pakistan have existing public health warnings against their use and sale due to toxic mercury content.
Cited at the end of October 2017 advisory as one former director general of Philippines' FDA named Ms. Nela Charade G. Puno: "Mercury salts in cosmetic products inhibit the formation of skin melanin which will result in lighter skin tone. Adverse health effects brought by high toxic mercury in cosmetics products include kidney damage, skin rashes, skin discoloration and scarring. Chronic use reduces the skin's normal resistance against bacterial and fungal infections. Other effects include anxiety, depression or psychosis and peripheral neuropathy", the advisory stated.
As you may really notice when you check and read the label first, if you are attempting to purchase an imported Pakistani-based skin lightening product of 'Goree', these cosmetic products won't show a list about "mercury" in the ingredients label after they finally come out from the factory and later imported globally.
Thony Dizon, whose a Toxics Campaigner of the group told local media reporters: "The proliferation of such prohibited beauty cosmetics in cities and provinces nationwide must end and should be addressed immediately by regulatory agencies. We call the attention of the FDA in Region 8 to conduct post-marketing surveillance and enforcement action to stop the sale and use of these banned toxic products", he added.
Around late-April 2024, 'BAN Toxics' launched a video documentary titled "The Dark Side of Skin-Lightening" - a 13-minute documentary film shedding light on the hidden dangers lurking within many SLPs due to their mercury content.
The watchdog group of 'BAN Toxics' continuously monitors and documents the illegal trade of banned and prohibited beauty products sold in the local market in the entire region of Leyte, and online (such as Shopee and Lazada) to warn the public about the dangers of toxic mercury exposure and to protect human health and the environment from mercury-added cosmetics.
The documentary reveals that many mercury-added SLPs continue to proliferate in the local market despite being banned by the FDA. It includes market monitoring conducted by the group in both online and physical stores, showing how easy it is to buy several illicit brands such as Goree, 88 Total White, Collagen Plus, Faiza and among others. This aims to raise public awareness, advocate for robust regulations & enforcement policies, and promote international collaboration as key measures to address the issue of mercury-added SLPs.
Because of the illegal cosmetic SLP imports including 'Goree' family of products to be unexpectedly go on sale in-store or online, the beauty faces of Filipinos who are allegedly using it are in for a nationwide public health concern that will save you money from your fashion disaster of life.

Pharmaceutics, Vol. 15, Pages 2793: A Novel Handrub Tablet Loaded with Pre- and Post-Biotic Solid Lipid Nanoparticles Combining Virucidal Activity and Maintenance of the Skin Barrier and Microbiome
Objective: This study aimed to develop a holobiont tablet with rapid dispersibility to provide regulation of the microbiota, virucidal activity, and skin barrier protection. Methods: A 23 factorial experiment was planned to define the best formulation for the development of the base tablet, using average weight, hardness, dimensions, swelling rate, and disintegration time as parameters to be analyzed. To produce holobiont tablets, the chosen base formulation was fabricated by direct compression of prebiotics, postbiotics, and excipients. The tablets also incorporated solid lipid nanoparticles containing postbiotics that were obtained by high-pressure homogenization and freeze-drying. The in vitro virucidal activity against alpha-coronavirus particles (CCoV-VR809) was determined in VERO cell culture. In vitro analysis, using monolayer cells and human equivalent skin, was performed by rRTq-PCR to determine the expression of interleukins 1, 6, 8, and 17, aquaporin-3, involucrin, filaggrin, FoxO3, and SIRT-1. Antioxidant activity and collagen-1 synthesis were also performed in fibroblast cells. Metagenomic analysis of the skin microbiome was determined in vivo before and after application of the holobiont tablet, during one week of continuous use, and compared to the use of alcohol gel. Samples were analyzed by sequencing the V3–V4 region of the 16S #rRNA gene. Results: A handrub tablet with rapid dispersibility was developed for topical use and rinse off. After being defined as safe, the virucidal activity was found to be equal to or greater than that of 70% alcohol, with a reduction in interleukins and maintenance or improvement of skin barrier gene markers, in addition to the reestablishment of the skin microbiota after use.
