The creeks around East Palestine, Ohio, were so badly contaminated by last year’s disastrous Norfolk Southern derailment that some workers became sick during the cleanup. Workers who reported headaches and nausea — while shooting compressed air into the creek bed, which releases chemicals from the sediment and water — were sent back to their hotels to rest, according to a report obtained by The Associated Press about their illnesses. The findings were not released to the public last spring, despite residents’ concerns about the potential health effects of exposure to the long list of chemicals that spilled and burned after the disaster. The workers’ symptoms, as described in the report, are consistent with what Centers for Disease Control and Prevention workers going door-to-door in town had reported shortly after the Feb. 3, 2023, derailment. Since then some residents have also reported unexplained rashes, asthma and other respiratory problems, and serious diseases including male breast cancer. Researchers are still determining how many of those health problems can be linked to the derailment and how the disaster will impact the long-term health of residents in the area near the Ohio-Pennsylvania border.

Origins
Charles Dennis Buchinsky was born on November 3, 1921 in Ehrenfeld, Pennsylvania, to parents who immigrated from Lithuania and was the eleventh of fifteen children, all of whom grew up in extreme poverty. In early childhood, Charles did not speak English, but he had to catch up quickly

Early fatigue
When Charles was ten, his father died and the boy followed in his footsteps by getting a job in the mine, like many of his peers, risking his life every day for about a dollar a week. Not too surprising that at that age the future stone face of American cinema was already a smoker.

After becoming the first of the Buchinsky family to graduate, in 1943 Charles enlisted in the United States Army and fought in World War II as a pilot in twenty-five aerial missions.

The name
At the beginning of his film career, Buchinsky had to stop for a moment to think about his own name. In the 1950s Senator Joe McCarthy launched a hunt for the alleged Communist who massacred intellectuals and artists across the country on charges of treason and collusion with the Soviet Union. Charles' Lithuanian surname might have made him an easy target, so the aspiring actor took on the all-American name of Bronson.

For his film debut in 1951's "Commander Johnny" Bronson resorted to a particular skill that no other of the suitors for the role of him had: that of burping on command.

European success
Before becoming an icon of genre cinema in America with "Death Wish" in 1974, Charles had difficulty breaking through in Hollywood, but by the late 1960s he was already a beloved face in Europe, thanks to western films such as “The Guns of San Sebastian” by Henri Verneuil and “Once Upon a Time in the West” by Sergio Leone.

The death
Charles Bronson died on August 30, 2003 at Cedars-Sinai Medical Center in Los Angeles after a long hospital stay. The actor had been in failing health since the late 1990s, when he was hospitalized for hip surgery. In the following years, he was also diagnosed with lung cancer and Alzheimer's. His grave is in Brownsville Cemetery in the town of West Windsor, Vermont.

Hormone Therapy for Gender Dysphoria May Raise Cardiovascular Risks - American College of Cardiology
WASHINGTON (Feb 23, 2023) -
People with gender dysphoria taking hormone replacements as part of gender affirmation therapy face a substantially increased risk of serious cardiac events, including stroke, heart attack and pulmonary embolism, according to a study presented at the American College of Cardiology's Annual Scientific Session Together With the World Congress of Cardiology.
Gender dysphoria occurs when a person's gender identity conflicts with the sex they were assigned at birth. Gender affirmation therapy, part of a process known as transitioning, includes a variety of medical, psychological and behavioral interventions to help individuals, many of whom identify as transgender, acquire secondary sex characteristics that align with their gender identity. Recent data suggest about 1 million to 1.6 million people identify as transgender in the U.S. and that hormone therapy use is rising rapidly, especially among teens and young adults.
The current study is the largest to date examining the cardiovascular risks of gender affirmation therapy in this historically understudied population. Previous research on hormone-modulating medications has primarily focused on younger women using hormone-based birth control or on older women following a hysterectomy or during menopause. In these populations, long-term hormone replacement therapy has been associated with an increased risk of breast cancer, stroke and blood clots.
Researchers retrospectively examined rates of cardiovascular events in over 21,000 people with gender dysphoria from a national database of hospital records, of whom 1,675 had used hormone replacement therapy. Typically, people assigned male at birth receive estrogen and people assigned female at birth receive testosterone. Overall results found hormone replacements were associated with higher rates of cardiac events, mostly related to dangerous blood clots, but were not associated with higher rates of death.
"It's all about risks and benefits. Starting transitioning is a big part of a person's life and helping them feel more themselves, but hormone replacement therapy also has a lot of side effects—it's not a risk-free endeavor," said Ibrahim Ahmed, MD, a third-year resident at Mercy Catholic Medical Center in Darby, Pennsylvania and the study's lead author.
In the study, people with gender dysphoria who had ever used hormone replacements saw nearly seven times the risk of ischemic stroke (a blockage in a vessel supplying blood to the brain), nearly six times the risk of ST elevation myocardial infarction (the most serious type of heart attack) and nearly five times the risk of pulmonary embolism (a blockage in an artery in the lung), compared with people with gender dysphoria who had never used hormone replacements. Hormone replacement therapy was not associated with any increase in deaths from any cause or with increased rates of atrial fibrillation, diabetes, hypertension, hemorrhagic stroke or heart failure.
Both estrogen and testosterone are known to increase the clotting activity of blood, which could explain the increase in clotting-related cardiovascular events, researchers said. Those taking hormone replacement therapy also had higher rates of substance use disorder and hypothyroidism.
"Looking at a person's medical and family history should definitely be part of the screening protocol before they even start hormone replacement therapy," Ahmed said. "It is also important that people considering this therapy are made aware of all the risks."
One limitation of the study is that it only accounted for whether individuals had ever used any type of hormone replacement therapy. To better inform clinical decisions, researchers said it would be helpful to assess whether the duration of treatment, the age at which it is initiated or the type of hormone therapy used affects the risks.
"I'm curious to see if the method of administration alters the outcomes," Ahmed said. "Is one way of giving hormone replacement therapy better or associated with a lower risk of cardiovascular outcomes? If so, then that should be the focus for how we give these patients their hormone replacement therapy going forward."
In addition to considering ways to mitigate potential cardiovascular risks before starting hormone replacement therapy for individual patients, researchers said it will be important to continue to study potential long-term cardiovascular and other health effects of gender affirmation therapies as the use of these therapies become more common.

In February, attackers from the Russia-based BlackCat ransomware group hit a physician practice in Lackawanna County, Pennsylvania, that's part of the Lehigh Valley Health Network (LVHN). At the time, LVHN said that the attack “involved” a patient photo system related to radiation oncology treatment. The health care group said that BlackCat had issued a ransom demand, “but LVHN refused to pay this criminal enterprise.” 
After a couple of weeks, BlackCat threatened to publish data stolen from the system. “Our blog is followed by a lot of world media, the case will be widely publicized and will cause significant damage to your business,” BlackCat wrote on their dark-web extortion site. “Your time is running out. We are ready to unleash our full power on you!” The attackers then released three screenshots of cancer patients receiving radiation treatment and seven documents that included patient information.
The medical photos are graphic and intimate, depicting patients' naked breasts in various angles and positions. And while hospitals and health care facilities have long been a favorite target of ransomware gangs, researchers say the situation at LVHN may indicate a shift in attackers' desperation and willingness to go to ruthless extremes as ransomware targets increasingly refuse to pay.
 “As fewer victims pay the ransom, ransomware actors are getting more aggressive in their extortion techniques,” says Allan Liska, an analyst for the security firm Recorded Future who specializes in ransomware. “I think we’ll see more of that. It follows closely patterns in kidnapping cases, where when victims’ families refused to pay, the kidnappers might send an ear or other body part of the victim.”
Researchers say that another example of these brutal escalations came on Tuesday when the emerging ransomware gang Medusa published sample data stolen from Minneapolis Public Schools in a February attack that came with a $1 million ransom demand. The leaked screenshots include scans of handwritten notes that describe allegations of a sexual assault and the names of a male student and two female students involved in the incident.
“Please note, MPS has not paid a ransom,” the Minnesota school district said in a statement at the beginning of March. The school district enrolls more than 36,000 students, but the data apparently contains records related to students, staff, and parents dating back to 1995. Last week, Medusa posted a 50-minute-long video in which attackers appeared to scroll through and review all the data they stole from the school, an unusual technique for advertising exactly what information they currently hold. Medusa offers three buttons on its dark-web site, one for anyone to pay $1 million to buy the stolen MPS data, one for the school district itself to pay the ransom and have the stolen data deleted, and one to pay $50,000 to extend the ransom deadline by one day.
“What’s notable here, I think, is that in the past the gangs have always had to strike a balance between pressuring their victims into paying and not doing such heinous, terrible, evil things that victims don’t want to deal with them,” says Brett Callow, a threat analyst at the antivirus company Emsisoft. “But because targets are not paying as often, the gangs are now pushing harder. It's bad PR to have a ransomware attack, but not as terrible as it once was—and it's really bad PR to be seen paying an organization that does terrible, heinous things.”
The public pressure is certainly mounting. In response to the leaked patient photos this week, for example, LVHN said in a statement, “This unconscionable criminal act takes advantage of patients receiving cancer treatment, and LVHN condemns this despicable behavior.”
The FBI Internet Crime Complaint Center (IC3) said in its annual Internet Crime Report this week that it received 2,385 reports about ransomware attacks in 2022, totaling $34.3 million in losses. The numbers were down from 3,729 ransomware complaints and $49 million in total losses in 2021. “It has been challenging for the FBI to ascertain the true number of ransomware victims as many infections go unreported to law enforcement,” the report notes.
But the report specifically calls out evolving and more aggressive extortion behavior. “In 2022, the IC3 has seen an increase in an additional extortion tactic used to facilitate ransomware,” the FBI wrote. “The threat actors pressure victims to pay by threatening to publish the stolen data if they do not pay the ransom.”
In some ways, the change is a positive sign that efforts to combat ransomware are working. If enough organizations have the resources and tools to resist paying ransoms, attackers eventually may not be able to generate the revenue they want and, ideally, would abandon ransomware entirely. But that makes this shift toward more aggressive tactics a precarious moment. 
“We really haven’t seen things like this before. Groups have done unpleasant things, but it was adults that were targeted, it wasn’t sick cancer patients or school kids,” Emsisoft's Callow says. “I hope that these tactics will bite them in the butt and that companies will say no, we cannot be seen funding an organization that does these heinous things. That’s my hope anyway. Whether they will react that way remains to be seen.”

Babe's 1948 Lincoln Continental Coupe
This was Babe Ruth's last car. The '48 Lincoln Continental Coupe has a V-12 engine; the last V-12 ever produced by a U.S. auto manufacturer. Base retail price for the coupe was then listed at $2,727 about $57,000 today. This was the last time that the Continental name was used for almost a decade.
Then, after an eight-year hiatus, for the 1956 model year, the Continental nameplate made its return; to launch the vehicle, Ford created a namesake "Continental" Division, centered around its new flagship vehicle. To again highlight the European influence of the original Continental, Ford assigned the Mark II suffix to the new Continental (also in an effort to distinguish itself from the similar Bentley Continental. Slotted well above Lincoln-Mercury, Continentals would be marketed and serviced through the new Lincoln dealership network.
Now, the New York Yankees star had more than his share of fender benders over the years, and they apparently had no effect on his thrill for speed, since he continued to purchase customized, powerful automobiles, and then promptly wreck them, or get caught for speeding. He routinely ignored stoplights and traffic signals on the drive from the Astoria Hotel to Yankee Stadium, but police usually let him go with a smile. In July 1920, however, Ruth threw caution to the wind and was lucky to walk away with only an injured knee.
On a road trip to Philadelphia and Washington, Ruth was driving late one night just outside rural Wawa, Pennsylvania, with [his first wife] Helen, and three friends from the club. When he sped dangerously around a curve, the auto skidded out of control. As the machine went off the road, Helen and coach Charley O’Leary were thrown clear as it flipped over. “Crawling from the wreck, Ruth found his wife in fine health, but O’Leary was looking unconscious and dead.
"Oh God, bring Charley back. I didn’t mean it," Babe cried, and O’Leary did in fact turn out to be OK. The car was another story, and after walking to a nearby farm and spending the night, the group returned to the crash site accompanied by a mechanic. ‘Sell it,’ Ruth supposedly said of the wreck. "Take whatever you can get for it. I’m through with it."
Leaving the stunned mechanic and his free parts supply behind, Babe’s party proceeded on to Philadelphia. RUTH REPORTEDLY KILLED IN CAR CRASH read the headline found in one local paper, but Babe was supposedly well enough to purchase a new version of the identical car when he returned to New York. Newspaper accounts did not specify the year, make, and model of the car Ruth was driving; the Sacramento Union referred to it only as a “big touring car.”
The Great Bambino, who retired from baseball in 1935 with a then-record 714 home runs, died from throat cancer 76 years ago on August 16, 1948, nearly three decades after escaping serious injury from a crash while on that road trip.
Nearly three decades after that accident, the Sultan of Swat still loved touring cars. In his final days, he drove this 1948 Lincoln Continental that he received as a gift from Ford Motor Company in appreciation for his tireless support of Little League Baseball. It was the last car he owned. Before becoming too weak to travel, Ruth drove the Lincoln around the country giving speeches and hitting lessons to young baseball players. We know a lot about the beautiful Continental because it was featured in newspapers across the country twice—first when it was given to Ruth and many years later, in 2012, when Texas collector Lonnie Shelton purchased the pristine two-door hardtop coupe from the Texas Museum of Automotive History.

Inflammation Regulation in Lung Cancer Immunotherapy

A recent strategy for treating advanced lung cancer has shown great promise in a small clinical trial led by investigators at the Abramson Cancer Center at the University of Pennsylvania Perelman School of Medicine. A study published recently found that adding an anti-inflammatory drug to anti-PD1 (programmed death-1) checkpoint inhibitor immunotherapy may increase the effectiveness of immunotherapy against advanced lung cancer.
The study involved using a drug known as a JAK1 (Janus kinase 1) inhibitor to decrease chronic inflammation in 21 patients with metastatic NSCLC (non-small-cell lung cancer). The patients took the JAK1 inhibitor for six weeks following two doses of anti-PD1 immunotherapy. The overall response rate was 67 percent, and the median progression-free survival rate was nearly 24 months, both of which are high for advanced NSCLC. The researchers noted that this approach could assist in regulating inflammation and interferon (a protein involved in inflammation) before the levels become toxic. The co-senior authors of the study, Andy Minn and E. John Wherry, pointed out that the patients who benefited most from the trial were those who had low inflammation at the beginning of the study or high inflammation that was reduced by the JAK1 inhibitor.
This study indicates that incorporating an anti-inflammatory drug into immunotherapy is a promising approach to managing advanced lung cancer. The results are quite positive, and the researchers are now preparing for a larger clinical trial to confirm the findings. Such a strategy might potentially assist in managing inflammation and, therefore, enhance the prognosis of patients with metastatic NSCLC.

What is Reproductive Justice? In June 1994, twelve black women working in the reproductive health and rights movement birthed the concept of reproductive justice at a pro-choice conference on health care reform in Chicago. We created “reproductive justice” because we believed that true health care for women needed to include a full range of reproductive health services. While abortion is one primary health issue, we knew that abortion advocacy alone inadequately addressed the intersectional oppressions of white supremacy, misogyny, and neoliberalism. From the perspective of African American women, any health care plan must include coverage for abortions, contraceptives, well-woman preventive care, pre- and postnatal care, fibroids, infertility, cervical and breast cancer, infant and maternal morbidity and mortality, intimate partner violence, HIV/AIDS, and other sexually transmitted infections. In simplest terms, we spliced together the concept of reproductive rights and social justice to coin the neologism, “reproductive justice.”
