Low Birth-Weight Baby Gets New Life At KIMS Hospital

In a remarkable collaboration and care by the Department of Neonatology, Cardiology and Cardiac Anaesthesia, the life of a New-born with critical congenital heart disease was saved. A 10 days old baby with birth weight of 2.2Kg was referred to KIMS with a suspicion of congenital heart disease due to persistence of oxygen requirement and low oxygen saturation since birth.

The baby, at receipt, was maintaining saturation of less than 70%on oxygen prongs and on prostaglandin infusion. The second heart sound was single and a continuous murmur was present. Chest x ray and ECHO revealed less flow of blood to both lungs due to a congenital heart disease called pulmonary atresia. Such patients can survive if there is any additional blood supply to the lungs such as the presence of a PDA. However, afterbirth the PDA tends to close spontaneously. Surgical options like BT shunt can be done but it has serious morbidity and mortality especially in small babies who are less than 2.5 kgs.

The Paediatric cardiology team led by Dr. Asish Ranjan Mohakud, in collaboration with the neonatology unit of KIMS and cardiac anaesthesiologist Dr. Ashok Badamali, evaluated the case properly and opted for an invasive yet highly effective solution in this baby i.e. dilating; stenting the naturally present but collapsing connection (ductus arteriosus) to lungs.

Post-intervention the infant’s oxygenation improved significantly (>90% SpO2), and the immediate threat to life was averted. The baby maintained oxygen saturation on room air, took the feeds and got hemodynamically stable. The procedure’s success also reduced the  need for more invasive surgeries, thereby minimizing the risks associated with prolonged anaesthesia and recovery in such a young patient.

The precise coordination between the neonatology team, which stabilized and prepared the infant, the cardiology team, which performed the intricate stenting procedure and the anaesthetic team exemplify the pinnacle of patient-centred care and advancement in the field of Paediatric cardiology at KIMS hospital.

In the face of a monumental medical crisis during her first pregnancy, Sahana Datta, an expectant mother, found herself in a dire situation that threatened both her life and that of her unborn baby girl. Rushed to the emergency department of Narayan Memorial Hospital, the situation was critical, it was spontaneous delivery of the baby of 27 weeks into the pregnancy, and the odds were heavily stacked against them.

With the odds of survival heavily against them in such cases with immature formation of all body organs due to 27 weeks, it was the expertise of Dr. Swastika Pal, Consultant Paediatrician, and a dedicated team of specialists including paediatric cardiologists, immune haematologists, advanced technology, skilled nurses, and paramedics, that turned the tide for growth of immature organs of premature baby which took almost 2 months to develop and survive independently. The newborn also developed ROP (retinopathy of prematurity) treated by Dr Pranab Das and Dr. Ritam Chakraborty, immunohematologist collaborated during the repeated blood transfusions the baby needed to combat severe anaemia due to prematurity. Through unwavering clinical knowledge and exceptional care, they managed to breathe life into the premature 800-gram baby girl, taking her from a fragile state to a healthier 1.51 kilograms within just two months.

Today, Sahana and her husband Pradipto revel in the joy of parenthood, a reality that once seemed distant and uncertain. This achievement is particularly significant given the staggering 70:30 mortality in countries like ours rate that plagues cases like these. The Narayan Memorial Hospital in Behala stood as the beacon of hope for this family, offering not only cutting-edge technology but also a compassionate clinical team and paramedical support crucial for such triumphs.

Detailing the critical journey, Dr. Swastika Pal explained, "The baby's silent entry into the world in the emergency room prompted immediate intervention. Bag mask ventilation, chest compressions, intubation and medication were swiftly administered, followed by meticulous care in the NICU to address issues ranging from septicaemia, apnoea, oral feeding to HIE (Hypoxic-ischemic encephalopathy). Gradually, the baby's weight stabilized at 1.5 kilograms, with all organ functions normalizing. Presently the baby is undergoing regular check-up at NMH OPD where she is found to grow steadily"
