I don’t think there was an immediate romantic interest from either of them, at most they find each other appealing.

Later as they learn more about each other Heavy appreciates Medics genius is a different way and finds his blood lust on the battlefield extremely arousing.

Medic finds himself very attracted to Heavy’s openness to his weird and off the wall ideas and learns that the big guy is actually very caring and very intelligent, only hindered by his limited vocabulary.
