a pelvic ultrasound with hospital radiology (semi-urgent, doctor is theorising that my new menstrual cycle could be linked to the sudden, severe anemia)

A quick guide on what different titles mean in my posts
(Since education AND healthcare systems vary around the world).
Medical Student (4.5-5 years)
You can get into medical school straight out of high school. College degrees do exist, but they are not the norm, not for medicine, and not for any career, tbh.
You fist 2-3 years are mostly theory. Calculus, chemistry, biology, anatomy, histology, embriology, physiology, pathology, physiopathology, microbiology, pharmacology…. That period end with semiology, and you get a Bachelor’s Degree in Medical Science.
Then, for the next 2 years, you have your clinicals, in which you spend half of the day in the hospital, with patients, and half the day in class, but definitely more focused on patient care and management.
Med student in clinicals = baby of the team (most of the time).
When you finish, you get your Academic Degree, Licenciate in Medicine.
Medical Intern (1.5-2 years)
No longer a student, you are now in your professional practice. Although you are technically still in med school in your university, you can say goodbye to classes, since you’re now a worker.
Probably bottom of the food chain, and probably does all the paperwork that nobody wants to do, but it’s a period where you gain a lot of independence and knowledge through work.
When you finish, you get your Professional Title, Médico Cirujano, but also need to pass a national test (EUNACOM) in order to be able to work.
Once you are a Doctor, you can work with that, or you can specialize.
Resident Doctor
A doctor, who is both working and studying towards a specialty.
Staff
Doctor who is on charge of a team. Tends to be an specialist.
Other titles that may cause confusion:
CNA: I use CNA to refer to TENS (Técnico de Enfermería de Nivel Superior). Technical degree (2.5 years). Takes care of patient’s basic needs, vital signs, may administer non-prescription medications.
Scrub tech: An specialized TENS. Takes care of the surgical instrumental and the sterile field in the OR.
Other TENS specializations: (that aren’t shared with other workers) Ambulance paramedic, anesthesia tech, trauma tech (takes care of plasters).
Medical Technologist: University degree (5Y). In charge of handling the machines and advanced technology equipment. They have 5 sub-specialties: ENT, ophthalmology, morphophysiopathology, blood bank and radiology.
Kinesiologist: University degree (5Y). They encapsulate both Physical Therapy and Respiratory Therapy.
Midwife: University Degree (5Y). Kind of like L&D nurses. Also in charge of reproductive health (i.e inserts IUDs, tests for STIs). Can assist births without a doctor if uncomplicated.
Other professionals that may not need further explanation:
Nurse.
Nutritionist.
Speech therapy.
Occupational therapy.

Also, I have seen many suspecting that her problem could have been on the colon, but maybe it wasn’t there. Some think that maybe is in the ovary, and as far as we know, unfortunately such cancer has a high percentage of reoccurrence and more of its an adenocarcinoma, and the stage is also important to take in consideration (unfortunately the adenocarcinoma is very difficult to detect, majority is detected on late stages and only after a biopsy). So another possibility is that maybe they found a ‘high grade’ tumor (maybe in early stages or maybe not), and that’s why they decided to go for chemo, since it can have a very high possibility of reoccurrence.
Also, a question… if the cancer spreaded to the lymph nodes, you need to go for a second surgery? Or Chemo is the treatment?
That being said, I didn’t want to speculate, but yeah, I agree that something in that timeline is confused. I honestly hope that she isn’t with a very serious health problem tho, specially for her children’s sake. (I know you don’t care, but still)

I think there's something you're still not understanding.
When you have surgery, you have imaging done. The level of imaging that is available today gives surgeons and radiologists highly detailed images. The radiologist evaluates the scan and makes a report. Any suspicious masses are noted and measured by the radiologist for the report. The surgeon will read the report, but also look at the scans and come to their own conclusions. The surgeon and radiologist may look at the scans together and discuss.
Bottom line is, before surgery even happens, two different medical professionals have taken a look at your imaging to determine whether there are suspicious masses or not. If there are any suspicious masses, then the surgeons will tell you before surgery that you have suspicious masses that are likely cancerous and pathology will rule it in or out. To not do so sounds like malpractice. They tell you that they suspect cancer, and if it comes back non-malignant/benign, great. For example, a surgeon will tell you that "you have a suspicious mass of 2.5 cm in your descending colon that appears to be cancerous. Pathology tests after surgery will confirm or deny that finding." You don't go into surgery with suspicious masses without already having a discussion with the surgeon about the possibility of cancer. Unless your surgeon/doctor sucks, I guess.
This is what Dr. Reiner was getting at when he said that Kate's statement didn't "make a lot of medical sense." Surgeons know what they're cutting into. They know well beforehand whether the tissue they are operating on is cancerous or not. They don't just open people up and extract tissue for no reason.
As for your specific question regarding lymph nodes, if the surgery is planned out properly, they will identify any lymph nodes for removal prior to the surgery. For example, a man with a melanoma on his right shoulder may have a nuclear medicine lymph node scan done where the radiological marker is only injected into lymph in the area suspected in/near the right shoulder. Beyond that, it's going to be a case-by-case basis. Everyone is a unique snowflake. But generally, good surgeons & doctors aim to be prepared and get as much done in a single surgery as possible. Surgery is still trauma happening to someone's body.

@angelofthemornings Making this a proper reply-post because as a reply it might end up being too big.
The field is very vast, but the most common would be the person that runs the tests a physician asks you to and signs the paper to validate the results - we can be found doing your bloodwork, urine, biochem analysis (which all falls under clinical analysis/clinical pathology), and many other fields like radiology (x-rays, tomography, etc), acupuncture, embryology... here in Brazil we can have up to 29 licenses, each one corresponding to a different field. Some of these you can come out of university ready to work due to experience in your internship, but some others will require a specialization course or a master's - like acupuncture and radiology.
My license is in clinical pathology, but I've also spent a year in a specialization/expertise program (after getting my bachelor's degree) in laboratorial surveillance for diseases of public health interest, with a focus on diagnostic tools of immunology (serology tests) and molecular biology (mostly real time Polymerase Chair Reaction - PCR).
So I'm both licensed to work in any hospital or particular lab to run tests (with the objective of a diagnosis), and to work with the government in public health surveillance (where the objective here isn't a diagnosis, it is to confirm cases of a disease in a certain population and keep an eye out on how it behaves throughout the year, epidemiology).
I was about to decide between a Master's or a "direct" Doctorate (a Doctorate that "skips" a Master's and lasts longer than the usual Doctorate program) but then the pandemic came and many internship deals crashed, unless you were in virology, as that became the main and only focus of research at the time. I tried for a spot in the State's Strategic Lab, but it was interview-only and for only 1 candidate - I was placed 4th.
With that, my grandma's health also began to deteriorate, so I've been staying home since then to help care for her.
But 4 years later, I've started to job hunt again, and I'm now afraid I might be overqualified (and thus, "more expensive") to employers, as so far I havent gotten any return from my applications. I quite miss and wish to return to academia, sometimes, and with the State's lab, but I have a love-hate relationship with academia and it takes them forever to open up sign-ins for employees. Working in public health was my best time though, really, so if the opportunity arises, I'll be trying my chance there again - I did leave in good terms, and a lot o the chief researchers there wanted to work with me.

re: breast cancer ai, is the testing that would need to be done to make sure it's an accurate result invasive or expensive or both?
It seems I jumped the gun when I reblogged that article. Despite them using a mammogram on the post's images, this AI innovation is for pathology, not radiology. I was talking about computer-aided detection for mammography, but this is a different beast entirely.

So some background, DCIS is known as ductal carcinoma in situ and is a pre-cancerous or a non-invasive cancerous lesion of the breast, depending on who you ask. Approximately 1/3 of people who have this develop actual breaat cancer aka it leaves the duct and start invading other things. So to make sure cancer doesn't develop, people are often recommended to excise it or even get a mastectomy - the removal of the whole breast - to treat it.
But doesn't that sound a little extreme when 67% of people may not even develop it?
I mean, look at prostate cancer, where urologists watch and wait to see if cancer will progress using labs, imaging, and occasionally biopsy. Because by the time you are diagnosed with prostate cancer, you're more likely to die from something else like a heart attack, old age, etc than from the cancer itself because it tends to grow so slowly. And getting a preemptive removal of the prostate - prostatectomy - has a lot of unwanted consequences including urinary incontinence and erectile dysfunction.
Wouldn't it be nice then if we could figure out what forms of DCIS would turn into full-blown cancer and what sort would behave more like prostate cancer?
Well, we do have some tech in the works, I believe, in the form of RNA sequencing, where you look at the cancer's genetic profile to see if it will go bad quickly. But RNA sequencing is expensive, and I don't believe it's common place yet.
And that's what the MIT research comes in. They are stating they can predict the behavior of DCIS, whether it'll act good or bad, by taking tissue samples of past known DCIS (and already having documented their behavior if they turned into cancer or not and how fast they do), using a chromatin stain to create slides, and feeding them into an AI to generate a predictive model that could tell from a tissue slide how DCIS will behave based on the model.
Staining and creating slides is cheaper than doing RNA sequencing according to the article ( I do not have the numbers on hand, but it seems like it'd be right), so this is a big deal as it both is cheaper for the patient and may allow a patient to avoid unneeded surgery.
Of course, the model needs to be validated to see if it works in practice, but it sounds promising.

As for what I was taking about erroneously, it's called computer-aided detection, and it analyzes mammogram images.
The CAD software can detect areas that are suspicious on a mammogram and alert a radiologist to them. The problem is that it can mark areas that may seem suspicious to the computer but seems normal to the mammographer. But what does a mammographer do with that information then?
The normal workflow of mammography is that you find a suspicious lesion on screening mammogram, ask the patient to come back to take diagnostic / magnified / compression images aka the breast getting smooshed views (more cost and discomfort), and if it's still suspicious with additional views you do a biopsy (invasive).
If the CAD is wrong but you test, you have done unnecessary imaging or worse even an unnecessary biopsy.
But if you the mammographer say that the CAD is wrong when it's right, you just missed cancer and put yourself in a very tight spot if they run the CAD software on your images.
To answer your question from what I was referring to, yes, the testing to confirm all the findings that the CAD points out would add a lot of unnecessary tests and biopsies for the patient.

Doc. Norma prescribed me an MRI at the local NCG Radiology Center.

Grade III tear of the posterior horn of the internal meniscus and the posterior horn of the external meniscus.
Bursitis of the patella
Tenosynovitis of the patellar tendon.
Hoffitis of the left patella. My left knee was basically ''gone'' but I didn't know it and I insisted on walking on it!! I took the report to Doc Norma, who - it seemed to me - didn't get too upset (it was characteristic of her not to get upset, perhaps deriving from the religious faith she adhered to: Mormonism). Norma told me that I needed a consultation with a specialist, identified as Dr. Jose' Alvarez Obregon of the Star Medica Clinic in Ciudad Juarez.

I saw him two days later and it was again a great surprise: a young and affable person, who, after having taken a look at the MRI, immediately put me at ease by saying that the report contained a lot of ''inaccuracies' ( not to mention worse). - Listen to me – he told – if you really had all the pathologies described in the report, you would be sitting in a wheelchair by now.
So why did they write all these ''inaccuracies''?
But to do business, right? The patient worried by the first report will want to have counter-proof, and will ask for new tests, always from them. By doing this the laboratory will earn more money!

Like Baja California for example. Cabo San Lucas was a destination on my bucket list, but unfortunately for various reasons I couldn't see it. Chiapas was also a state I would have liked to visit, especially with its ancient evidence of the Mayan civilization in Palenque. However, apart from the recurring trips to Mexico City, I managed to have some nice holidays in Yucatan and Quintana Roo. They are truly enchanting places, with vestiges of their important past, and splendid nature. The Mexican Caribbean is one of the top world destinations. Since my first trip to Cancun, in 1984, much has been done to make the Riviera Maya attractive to high-level international tourism. I particularly appreciated Tulum, where you truly live in a sort of ''magical atmosphere''. The Mayan archaeological site, well cared for by the local authorities, is something grandiose. The only site directly on the shore of the splendid sea of ​​the Gulf of Mexico. Its particular position, on the coast overlooking the sea, meant that it was the first Mayan city to be sighted by the Spaniards on March 3, 1517. Its favorable geographical position had previously made it successful, making it an important trading post for products such as fish, honey, salt, obsidian and quetzal feathers. The most important building in Tulum is undoubtedly El Castillo located near the landing, a small inlet that served as a port. A destination for tourists from all over the world, the charm of Tulum is also given by its beaches of fine sand, typical of the Caribbean coast.

The Mayan city of Chichen Itza is also spectacular, perhaps the most important archaeological site in Mexico. The Castillo or Temple of Kukulcan, dominates the ancient city, which prospered from 600 AD. to about 1200. Graphic stone sculptures survive in structures such as the playing field, the Temple of the Warriors and the Wall of Skulls. The journey from Cancun to Tulum, approximately 300 km, takes place along a modern highway totally immersed in an intricate equatorial forest.

I also had the opportunity to visit Coba', an archaeological site built by the Mayan civilization located 44 km north-west of the Tulum site. Coba' is a relatively ''new'' site where important archaeological discoveries have been found, but which is still being excavated and promises to reveal more hidden treasures. Cobá is located around two lagoons. There are some sacbé («saq′i b′e», white road), it is a typical road of the Mayan civilization that goes eastwards on the Caribbean coast and the longest covers over 100 km of distance going westwards to the Yaxuná site. The site has several large pyramids, the largest of which is in the Nohoch Mul group of pyramids, 42 meters high.

Another great place to visit is the local attraction called ''Xcaret''. It's an enormous natural reserve directly to the sea, where numbers of live animals can be seen.

I had also the opportunity to re-visit ''Isla Mujeres'' where I had my honeymoon in 1984. Lot of changes since, some of them not for good. The once tiny naif island has become a mass tourist destination nowadays.

Anycase its lagoon is still spectacular, and it's pristine waters provide a safe place to swim and snorkel.

Cancun has first-rate hotel facilities and a coastline washed by a beautiful sea. Its colors and shades are among the most beautiful I have ever seen in the world.

Final return from Mexico
The project - after many difficulties, was well underway. We had managed to have a good number of local workers who had been taught to work according to our parameters and specifications. Finally our Client was also convinced of the goodness of our work, and was showing us signs of trust. For example by removing sections of gas pipeline from other small contractors, and assigning them to us. But the unexpected was just around the corner. When I was convinced that I would complete the project, one morning in May, I had just arrived at the office, I got a call from Italy.

I know most of you don't know me well enough to find this interesting, but a few probably do so here's an update on my prostate-cancer journey.
Recap: on November 10, I got my biopsy results and learned that I have prostate cancer. Not only that, but it had progressed significantly since my MRI almost a year earlier (though still confined to the prostate which is really good news under the circumstances).
The reason for the year gap was repeated communication screwups by my urologist, for which the hospital's chief of urology and director of practice operations issued a reprimand (to them) and an apology (to me). Accordingly, I looked for and found another urologist - actually a chief of urologic oncology - at a different hospital. They had three requirements before I would technically become their patient with my first visit.
A new PSA (Prostate Specific Antigen) test.
A current MRI.
The biopsy slides.
The PSA test was easy. I already had that done the same day I talked to the new doctor's office. Since then I've been fighting to get the MRI. Insurance denied it, claiming no medical necessity. Yes, this was all around the same time that awareness of medical-insurer bullshit exploded into the public consciousness. Anyway, I used all the tricks I had heard about, had my primary-care physician do a peer-to-peer, all to no avail. Fortunately, the new doctor's office relented and said I didn't need a new MRI after all. I guess if they need that information the new guy can order it, and since he's quite likely a co-author on the guidelines they cited for the earlier denial he might be able to push it through. I'd love to sit in on that call, especially since the doctor who issued the denial is a breast cancer specialist ("practice limited to...") in California and might not even be qualified for this case. Still, would have been nice if the new folks had told me it was unnecessary before I spent so much time on a failed mission.
Today I arranged to pick up the (old) MRI data tomorrow. I also called the pathology lab about the biopsy slides. They seem OK with me picking those up myself, so I might as well do that as part of the same trip. Then, hopefully, I can take all three items over to the new office myself and be done with all this bullshit for the year. My consultation is on January 8, shortly followed (I hope) by treatment. So there's a decent chance I'll have this whole saga behind me before my 60th birthday. 🎉 Or maybe the cancer will have spread to my bones by then, but no sense worrying about that yet. It's always possible something terrible could happen and I have no control over it, so might as well focus on the most likely path.
Obviously this process has taught me to hate medical insurers. I'm also not fond of hospitals now. I had to spend hours calling around five different departments at the old hospital on Monday, trying to find the right office to get that MRI data. Then it turns out that the radiology library (separate from the main records department for some reason) is staffed by one person half time, mostly in the early morning, so I had to wait an extra couple days to reach them. The impression I get is that the front-line folks are good people doing their best - including the doctors except for That One - but they're operating in a system thoroughly devoted to gatekeeping and gaslighting for profit. Maybe when I'm all done taking care of my own medical needs I'll make this my cause. There are worse ways to spend my time.
