Testosterone Therapy in a Transgender Male Patient as a cause of Acute Ischemic Stroke (P2-5.011)

Objective:
To share an intriguing case of a young transgender male patient receiving testosterone therapy who developed locked-in syndrome due to an acute ischemic stroke and to highlight potential risk factors for stroke in the LGBTQI+ community
Background:
There are many studies identifying risk factors for stroke in racially and ethnically diverse populations. However, there is little existing data for stroke risk factors in the LGBTQI+ community. Prior research has shown testosterone therapy in cis-gender men with initially low levels of testosterone increases the risk of stroke, especially in the first 2 years of use1. While testosterone therapy has been shown to increase the risk of venous thrombosis, its role in arterial thrombosis is unclear2. A proposed mechanism for thrombosis with testosterone replacement includes erythrocytosis, but the potential contribution of an independent pro-coagulant effect is yet to be determined3.
Design/Methods:
Literature review and case report.
Results:
An otherwise healthy 23-year-old transgender male on one year of testosterone therapy presented in an obtunded state. Examination revealed complete quadriplegia with sparing of vertical eye movements, consistent with locked-in syndrome. Imaging revealed complete occlusion of the basilar artery with distal reconstitution at the superior cerebellar arteries, and a large bilateral ischemic infarct of the pons. Computed tomography angiography did not demonstrate other large vessel disease or structural vascular abnormalities. Unfortunately, the patient was out of the time window for any acute stroke interventions. A hypercoagulable workup was performed but results were unrevealing and hematocrit was normal. Further investigation with transthoracic echocardiogram, transesophageal echocardiogram, and telemetry were negative for thrombus, patent foramen ovale, and atrial fibrillation.
Conclusions:
Acute ischemic stroke may be an under recognized complication of testosterone therapy in transgender males independent of degree of erythrocytosis. Further research is needed to establish a safety profile of testosterone therapy in this understudied population.

absolutely loved tonight’s episode of yellowjackets, it is a 10/10 for me. so unapologetically fun, extremely well-paced, had specific narrative beats that i have been wondering whether or not they’ll touch on, and that incredible danzig needle drop at the end as well! one of my favourite's so far this season. lauren ambrose absolutely killed it as adult van.

I wandered past La Ruche today because there is a lot of construction going on the entrance gate was open. There is an effort to restore the space. The complex is located on the passage Dantzig close to where I live in the fifteenth arrondissement in Paris. Today I was greeted by a friendly tabby cat but wasn't fast enough to get a better photo of it. He seemed to be guarding the site since the front gate since it wasn’t locked tight. La Ruche came out of the generosity of a sculptor named Alfred Boucher (1850-1934). He would grow from an artist of humble beginnings to an artist of fame and wealth. In 1900, he acquired the land on the Passage Danzig. It was at the time just after the 1900 Universal Exhibition of Paris was coming to an end. Boucher would buy at auction the Gironde wine pavilion from the universal expo. This was designed by Gustave Eiffel. Called the Beehive to help promote the idea of the intense activity of its artist residences. It would attract historic modern "Paris School" artistic occupants like:Fernand Léger, Chaïm Soutine, Zadkine, Archipenko, Marc Chagall, Michel Kikoïne, Jacques Chapiro, Jacques Lipchitz, Henri Laurens, Blaise Cendrars, Modigliani, and also its founder Alfred Boucher (he had been sculptor Camille Claudel's teacher.). My photo doesn't expose its lovely architectural elements that came from the Universal Exhibition like the main entrance gate, the caryatids on the Rotunda, or the Indonesian pavilion. Today the site is managed by the La Ruche Foundation and controlled by a board of directors. I think that around fifty artists are currently working there. On the one hand, some of this book is a little out of date depending on if you get the original version or the updated one (ttyl - 10th Anniversary update and reissue). Instant Messenger instead of texting and snapchatting and tweeting (and whatever comes next). On the other hand, has high school really changed at its core since this was published in 2004, or even since 1994, or 1984? I say no, and that’s why this book is still so amazing even as we rapidly approach 2024 (yikes).
It’s basically the story of high school for three friends who are close, but, also very different. Maddie, Angela, and Zoe. And it’s told with such humor and unvarnished honesty too which is pretty cool. Lauren Myracle may not be writing Shakespearean histories, but, in my opinion the books in this ‘Internet Girls’ series have depths that matter just as much as ‘to be or not to be’.

We had quite a riot at Bloodstock that year, with some truly unforgettable moments courtesy of meeting bands and friends amid many alcoholic beverages.This was actually the last show for both Lauren and Nick, who left the band amicably not long after the show for commitments in the real world.
'Hell At My Back’ is a precursor to our forthcoming appearance at this year’s HRH Metal III. Lyrically, the protagonist of the song is constantly haunted by his past,  hunted by the horrors he has confronted since time immemorial. Awful things he has done in the name of love. But it is again another song of defiance, of staying the inevitable, of the eternal struggle of 'being’. Of evading the clutching powers of Hell". 'Hell At My Back' is taken from the band's latest masterpiece ‘Devilment II: The Mephisto Waltzes’.

Goodreads Monday is a weekly "challenge" started by Lauren's Page Turners, where you pick a random book off your TBR to highlight books that might get lost in the shuffle.
This week, I'm highlighting I am Providence by Nick Mamatas!
For fans of legendary pulp author H. P. Lovecraft, there is nothing bigger than the annual Providence-based convention the Summer Tentacular. Horror writer Colleen Danzig doesn’t know what to expect when she arrives, but is unsettled to find that among the hob-knobbing between scholars and literary critics are a group of real freaks: book collectors looking for volumes bound in human skin, and true believers claiming the power to summon the Elder God Cthulhu, one of their idol’s most horrific fictional creations, before the weekend is out.
Colleen’s trip spirals into a nightmare when her roommate for the weekend, an obnoxious novelist known as Panossian, turns up dead, his face neatly removed. What’s more unsettling is that, in the aftermath of the murder, there is little concern among the convention goers. The Summer Tentacular continues uninterrupted, except by a few bumbling police.
Everyone at the convention is a possible suspect, but only Colleen seems to show any interest in solving the murder. So she delves deep into the darkness, where occult truths have been lurking since the beginning of time. A darkness where Panossian is waiting, spending a lot of time thinking about Colleen, narrating a new Lovecraftian tale that could very well spell her doom.

feeling myself slowly romanticizing my 2015 self before i had surgery before i cut off all my hair and it still had pink in it & when all i’d wear were faded jeans, old torn up vans, and maxwell’s favorite sweaters. listen to nirvana’s entire discography on repeat every day and do ballet all week. stay up way too late to write pages to emilie and crush over lauren (who’s married now! wow!) and go home listening to black sabbath and danzig. back when that one coldplay song could make me cry and i still didn’t understand radiohead’s Creep or what it truly meant when that’s how i made ******* feel and back when i’d tell jacqui all about it and let her do my makeup, not knowing yet just how much i had loved her. when i was still collecting snk mangas and all of the saddest grunge songs were still my favorite. i’d text gavin all the time and my favorite jacket was the one that belonged to him. when all i could play was piano and i was learning all of my favorite classical songs and my favorite part of the day would be when i’d sit down and work on my sculptures… damn..

Glenn’s passion for film, the occult, and all things ghouls ‘n ghosts has long influenced his music, and now he’s taking these elements to the big screen. Featuring a plot shrouded in secrecy, a soundtrack of new Danzig music, and based on the output of his comic book publishing company Verotik—a compound of “violence” and “erotic”—this feature film anthology is a realization of his lifelong love of comics and the dark arts. We’ll melt your face off with the world premiere of VEROTIKA on opening night, with Glenn in attendance.

The festival will present its annual achievement award to none other than the esteemed Canadian actor Michael Ironside, who’s been a staple in the world of genre cinema for the past three decades. Ironside will accept his award following a special 70mm screening of the sci-fi action film TOTAL RECALL.

Cinepocalypse is rolling out the blood-red carpet for some of the most exciting voices in horror filmmaking of this age. We’re thrilled to host a special screening of CULTURE SHOCK, directed by Gigi Saul Guerrer, one of Variety’s 10 Latinxs to Watch. Her contribution to Hulu and Blumhouse’s popular Into The Dark series follows a young Mexican woman as her pursuit for the American Dream turns into the American Nightmare.
Minimum wage jobs are bad enough but a pizza delivery goes devilishly wrong in Chelsea Stardust hotly anticipated AFTER HOURS-esque dark comedy SATANIC PANIC. Produced by the production wing of Fangoria, who will also be releasing a special mini-edition of the magazine for CFF.

Caryn Waecther tackles isolation in the age of social media with her haunting sophomore feature DEADCON, following real-life YouTubers Lauren Elizabeth and Claudia Sulewski up against vengeful ghosts even more horrifying than their devoted fans.
Dark pasts come calling back in Veronika Franz & Severin Fiala’s Sundance hit THE LODGE as the fate of a young woman and her new, grudging step-children unravel in this star-studded psychological slowburn featuring Riley Keough, Jaeden Lieberher, Alicia Silverstone, Richard Armitage and Lia McHugh.
Pollyanna McIntosh, best known for her role on AMC’s The Walking Dead, moves into the director seat for DARLIN’, a follow-up to Lucky McKee’s 2011 film THE WOMAN, while also reprising her starring role in the debut that charged SXSW’s Midnight Madness earlier this year.

Continuing to set the bar for exciting new genre cinema, CFF brings even more world premieres to the Music Box. We’re not joking around when we say you’ve never seen anything like Eric Power’s ATTACK OF THE DEMONS. Three friends fight off a horde of demons in the apocalyptic film that more than delivers in carnage, gore, and cut-paper animation. Holly’s perfect-on-paper life is threatened by strange dreams she can’t explain in Dean Kapsalis’ THE SWERVE. Both a stellar portrait of depression and a horrific drama, Dean’s feature-debut easily marks him as an emerging talent to watch.
Indie horror vet Lucky McKee (MAY, THE WOMAN) returns with KINDRED SPIRITS, a twisted tale of two sisters, and the one who won’t let anything, or anyone, get between them. Eric Liberacki’s Chicago-set slasher THE LURKER pits theatre students against savage killer in a film that gives new meaning to “The show must go on!”
The festival will also play host to a number of exciting North American and Midwest premieres. Talal Selhami’s Morrocan monster horror ACHOURA, where old friends confront both their past and a beast of legends. Emilio Portes’ demonic border-set BELZEBUTH follows the investigation of missing children that takes a decidedly demonic turn. YouTubers try to crack a ghostly urban legend in Fabrício Bittar’s horror-comedy GHOST KILLERS VS. BLOODY MARY with equal amounts gags and gore. The Polish pagan epic THE MUTE, from award-winning Polish director Bartosz Konopka, explores a clash of beliefs in a thrilling, atmospheric heavy weight. Daniel de la Vega offers up a throwback to the days of black-and-white Noir films with the bewildering crime thriller PUNTA MUERTO.
Celebrated horror director Joe Begos brings us BLISS his latest pounding feature, that’ll drag you to the deprived underbelly of L.A. with the spiraling artist Dezzy Donahue, whose struggle for inspiration becomes complicated by her mounting bloodlust. Based on a tragic, true story Lucas Heyne’s MOPE follows two best friends, and low-end porn actors, who sought fame but gained infamy in connection to a gruesome, on-set murder. The tables are turned on two wannabe criminals in VILLAINS, from directors Dan Berk and Robert Olsen and starring Bill Skarsgård, when thievery runs a foul and the prowlers soon become the prey. Kirill Sokolov delivers a taste of bombastic revenge in WHY DON’T YOU JUST DIE, a delicious serving of Russian dark humour as each character tries to murderously wreak revenge on the next, presented by our friends from across the globe, Paris’ L'Étrange Festival.

Mild panic knocks on as I realise I should likely by this stage be der Guru von alles Berlin, but I scoop my memory spoon all the way around this ol’ dusty coconut and still don’t turn up the foggiest flesh-scrap as to where to whisk us off to. Fortunately I’ve a wad of attraction leaflets in a living room drawer, and - thanks to the much more logical Mama Walton - my loyal Lonely Planet Pocket Berlin. Lie-in and lazy ablutions licked, @laurenmary02 and I schlep off to the Checkpoint Charlie Museum for some chief Teutonic tuition. I forfeit yet more Smooth Host XP in taking us to the wrong U-Bahn station for our connection but try to shrug it off as a bonus tour of local subterrain as we recalculate a new Nokia-Snake-esque route.
Base briefly touched and further, fancier ablutions accomplished, we jolly our way to a rock night at Duncker Club. This time I have indeed done my research - rather a lot in fact - and am still wholly lacking confidence in selecting a suitable musical sesh that both myself and Lauren shall enjoy. We enter, hunt out refreshment, and hiply yet disinterestedly wallflower ourselves in a prominently dancefloor-accessible position. Our fellow dancers (initially, at least) land on the ‘reserved’ side of the “avin’ it” fence, so it isn’t long before Ol’ Bezzie double-dog dares me into hopping up to the DJ booth with a keen  ‘kannst du Danzig spielen, bitte?’. Our wee star of a DJ plays every song I request much to my devil-horning delight - though my comely statuesque companion seems to inadvertently trigger his Napoleon complex so it’s up to me or our new-found dancefloor chum to get the choons in. 
We’re oblivious of the AM whilst negotiating the post-ausstieg Prenzlauer Berg streets. Paper bag bulging greasily with stomach-soaking station stall scran, my sonambulant sidekick and I bedwards board a nail-gnawing crawler of a train, alternately noshing and nodding as dawn continues its soporific spread across the angular city skyline.

Assisted by long-time collaborators Lauren Francis (keys and vocals), Colin Parks (guitar), Nick Johnson (bass) and new drummer Matt Alston, Dani has arrived at the telling moment where Devilment become a genuine force to be reckoned with.  Formed in 2011 in the mystical county of Suffolk, England, Devilment burst into the vexed consciousness of the heavy metal hive mind with their debut demo ‘Grotescapology’ in 2012. Led by the unmistakably grim vocal gymnastics of Cradle Of Filth frontman Dani Filth, the band swiftly grabbed the attention of legendary music biz mover and shaker Monte Conner and were duly snapped up by his Nuclear Blast Entertainment imprint. In October 2014, these Albion-dwelling infidels unveiled their debut album, ‘The Great And Secret Show’: a whirlwind of pitch-black thrash metal riffing, multifarious gothic embellishments and a mean, mischievous streak a mile wide, it provided a telling and irresistible introduction to Devilment’s uniquely perverse, surreal and vicious musical world, wherein werewolves, voodoo temptresses and mutant fungal strains ran riot. Immediately embraced by the malevolent faithful, it was a sturdy and convincing statement of intent that paved the way for the sustained dissemination of its creators’ wildly imaginative and unsettling vision.
