What is Direct Messaging in Healthcare?
In health care, direct messaging is a way to exchange patient information between two or more providers. Similar to email, it uses secure messaging to transmit health data between healthcare entities. It is facilitated by Health Information Service Providers (HISPs), who provide the infrastructure and services needed to send and receive messages across their network.

HISPs manage the security and transport for direct message exchange, but physicians can also use standalone HISPs that offer a secure, webmail service. Using Direct is more efficient and secure than sending and receiving e-faxes.
Physicians have been slow to adopt Direct, and some cite workflow issues as reasons. For example, physicians who use an EHR from different vendors might not know that they can use Direct to share medical records. Even if they do, finding other physicians to exchange messages with can be difficult.
Practices that do implement Direct say they see a boost in productivity, as they no longer have to make copies or coordinate calls to ask questions about patients. In addition, practices can stay HIPAA compliant by ensuring all data transfers are encrypted and private.
Despite the benefits, physicians interviewed by Medical Economics say that most of their colleagues either are unaware of Direct or are uninterested in it. "I've asked physicians from other practices to exchange Direct messages, and most of them don't respond," says Medhavi Jogi, MD, a Houston endocrinologist who uses Direct with a few other groups.
