Do At-Home COVID-19 Tests Still Work? - Published Sept 6, 2024
These days, many people use at-home COVID-19 tests when they feel ill, rather than going out to get tested by a professional. (That’s when they bother to test at all.) But for all their convenience, the antigen tests commonly used at home have never been as accurate as PCR tests done in a lab—and the continued mutation of the virus raises additional concerns about their performance.
Rapid COVID-19 tests have never been perfect. How are they holding up as new variants emerge?
The U.S. Food and Drug Administration (FDA) continues to monitor the efficacy of the diagnostics it regulates—and as of August 2023, the FDA said none of the antigen tests on the market were expected to have reduced performance against Omicron or its subvariants. (All of the variants that have emerged since late 2021, including recent ones like KP.3.1.1, are relatives of the original Omicron strain.)
The FDA has also collaborated with a U.S. National Institutes of Health task force set up to monitor how variants affect tests. In 2022, well into Omicron's dominant era, that team concluded that DIY diagnostics continued to work well. Task force member Richard Creager wrote in an email to TIME that the tests still seem able to catch the Omicron spinoffs that are circulating now. “The rapid tests are having no issue detecting the variants,” he wrote, noting that the protein that antigen tests look for has remained fairly stable as the virus changes.
Even if tests have remained constant, our immune systems haven’t. Early in the pandemic, an infected person's “viral load”—the amount of virus in their system���tended to peak around the time their symptoms began. So if someone self-tested on the first or second day they felt sick, they likely had enough virus in their body for a rapid test to detect it.
Now that most everyone has prior immunity from multiple vaccines or infections, the timeline seems to be extended. Viral load now tends to peak around day four or five of symptoms, according to a study published in early 2024. That’s likely because the immune system is primed for action by its previous encounters with the virus, so it responds faster, even before lots of virus has built up in the body. A quicker immune response may mean a faster onset of symptoms.
“If your body has seen the virus before, you’re going to react to it and have an immune response more quickly,” explains study co-author Dr. Nira Pollock, co-director of the Infectious Diseases Diagnostic Laboratory at Boston Children’s Hospital. “That immune response can show up as symptoms.”
From an immune perspective, that’s a good thing. But it can complicate testing, because it means someone may not test positive for COVID-19 until they’ve already been feeling sick for days. In their recent study, Pollock and her co-authors estimated that a COVID-19 antigen test is somewhere between 30% and 60% accurate at detecting an infection on someone’s first day of symptoms, but up to 93% accurate on day four.
Based on such findings, Pollock and other researchers cautioned in a recent review article that people who are symptomatic shouldn’t assume they’re COVID-free based on a single negative antigen test result. Although it's a hard sell for people who no longer take precautions, the ideal scenario is for someone to stay isolated—or at least wear a mask when around other people—and test again around day four of symptoms.
“The instructions for these tests are to do serial testing,” Pollock says. “If you’re symptomatic, you should test early. But if you’re negative, you need to repeat it, because the amount of virus in your nose may not be high enough yet to detect.”
One other factor to consider: tests don’t last forever. Many people stockpiled tests when they were previously available for free through government programs and insurance, and those kits may have since expired. (You’ll soon be able to order four more free tests through the government.) The FDA’s website provides up-to-date guidance on determining how long your kits last.
Expired tests can still work, says Dr. Zishan Siddiqui, an assistant professor at the Johns Hopkins University School of Medicine. He co-authored a 2023 study on Abbott’s popular BinaxNOW tests, which found no major accuracy differences between unexpired tests and those that were five months past expiration. (Siddiqui’s other research suggests unexpired BinaxNOW tests accurately detect COVID-19 cases more than 80% of the time.)
But, although Siddiqui says he isn't too worried about using expired tests, it’s good to remember that their performance can suffer if they’re long past their recommended use dates. If the control line on your test strip doesn’t show up or looks distorted, that’s a good indication that it’s too old to use, he says. Even in Siddiqui's study, which found that expired tests still work, the lines on the old tests showed up more faintly than on fresh tests, making them harder to read.
Whether your tests are old or brand new, it’s good to use some healthy skepticism when interpreting their results. “I trust these tests,” even as the virus evolves, Siddiqui says—but a single result doesn’t always tell the whole story. If you have COVID-like symptoms but test negative, it’s best to be cautious and retest in a few days.

yea i talked about this a bit here but i would add for clarity:
this kind of narrative of 'rising rates of' [any dsm diagnosis, in this case adhd] is kind of misleading on the surface because these numbers, and cultural and medical attitudes toward these labels, vary widely. matthew smith gives a very abridged introduction to varying attitudes toward adhd globally, and points out that countries that have 'embraced' the adhd diagnosis and its corresponding drug treatments tend to be countries where pharma companies have pushed to expand their market for these drugs, and have been able to succeed in partnering up with local and regional medical guilds and practitioners' professional interests. which is to say that any 'rise' in 'adhd' should be interpreted with an eye to material factors, meaning, specifically, profit-seeking and broader patterns of imperialism and global market expansion.
none of this is to say that the impairments people experience in adhd are any less real, debilitating, or distressing. however, when we ask about those impairments becoming more widespread or severe, often the conversation becomes rapidly re-routed to cover only a narrative of individual cognitive or neurological 'failures' constituting a distinct 'disorder'. elided from this framing is the idea that an impairment of this sort arises not just from the individual's brain-mind-body, but from the extent to which that person is being accommodated by their social context, specifically demands for productivity, sustained attention, &c in the home / school / workplace.

i truly cannot overstate the extent to which it matters that when ritalin arrived on the us market in 1955, psychiatric diagnosis of and pharmacological prescription for children's behaviours were in a very different state to how they are today. it is quite common (in psychiatry but also in other branches of medicine!) that diagnostic definitions and categories change, or even come into existence altogether, at the behest of pharmaceutical companies who need a diagnostic label in order to ensure insurance coverage for patients interested in taking their patented drugs. this combined with marketing direct to patients, and paid promotion to physicians, is a critical piece of the history of the adhd diagnosis.
because i always feel the need to make this crystal-clear: i do not oppose or object to people seeking or using stimulant medications lol. i <3 stimulants. that's not what this is about. i want you and me both to be able to use white-market amphetamines whenever we damn well please and you don't need to justify that on any moral or medical grounds. xx

Challenge Day 276 of Year 9 Teb was a simple plastiform home kit robot that it's builder less than legally squeezed a grey market AI into. It was fairly sure that it hadn't been put together correctly, but didn't have the tech to do diagnostics on itself just yet.

Now more than ever consumers are buying items and having them expedited to their homes! For example, Amazon offers same day delivery for many items on their website! This allows shoppers to utilize their time doing other things rather than grocery shop, grab quick essentials, etc.…

When I shopping there is a better chance that you come across coupons. Oftentimes when shopping in store coupons aren't presented to you per the items you are looking to purchase. This means you get more bang for your buck as an e-shopper!

E-shopping comes with many other benefits as well, I am just here to highlight some of the most important or most relatable! E-shopping has continued to keep our every growing world connected to goods, products, and services. Something we should all be grateful for. With that being said, e-shopping comes with just as many risks and cons. Not that these risks can’t be avoided with proper precautions, they still must be highlighted. That being said…

Fraud risk
When shopping online and entering your credit card information there's a larger risk of fraud: credit card scams, phishing, hacking, identity theft, counterfeit products, bogus websites, and other scams that are common. The issues with this is you never know when credit fraud is going to occur.
To maximize your safety as a consumer it is best to go into any online shopping / e-shopping experience with knowledge of the company, an understanding of what a bogus website looks like, and the basic idea of what to avoid when entering personal information. Online shopping can be dangerous if as a consumer you are not careful with what you readily offer websites.

Shipping issues and delays
Another downfall to online shopping is that there can be issues with shipping, including delays, receiving the wrong items, and paying for shipping and returns. As a consumer online shopping can be frustrating for these various reasons. One of the hardest things as an E shopper to understand is that after making a purchase, online items can take quite a while to be received. Not often but sometimes after items are received the consumer realizes they were sent the wrong item, or wrong size, maybe wrong color, and the list could go on. This can be a frustrating process since many companies require consumers to pay for shipping and then pay for returns regardless of return reason. So after receiving items that were incorrect or items that you decided you don't want, having to pay to ship them back you are already spending 2x the money you had originally planned.

Spending too much time online
Lastly, already as a society we spend far too much time online. Now with e-shopping the screen time continues to rise. This online time can cause disconnected in many areas of individuals lives as well as actual branding disconnects with consumers. The tricky part about now having everything online is the brick-and-mortar business take a hit. Since consumers now don’t HAVE to go into stores human connections dim and phone screen brighten.
Either way e-shopping can be seen as the ally or the enemy. As the economy continues to grow, consumers do what consumers do best, CONSUME! Making e-shopping one of the latest and greatest developments regardless of your personal take.
Understanding boundaries and using online shopping but not abusing online shopping is key to societal success!
