Do At-Home COVID-19 Tests Still Work? - Published Sept 6, 2024
These days, many people use at-home COVID-19 tests when they feel ill, rather than going out to get tested by a professional. (Thatâs when they bother to test at all.) But for all their convenience, the antigen tests commonly used at home have never been as accurate as PCR tests done in a labâand the continued mutation of the virus raises additional concerns about their performance.
Rapid COVID-19 tests have never been perfect. How are they holding up as new variants emerge?
The U.S. Food and Drug Administration (FDA) continues to monitor the efficacy of the diagnostics it regulatesâand as of August 2023, the FDA said none of the antigen tests on the market were expected to have reduced performance against Omicron or its subvariants. (All of the variants that have emerged since late 2021, including recent ones like KP.3.1.1, are relatives of the original Omicron strain.)
The FDA has also collaborated with a U.S. National Institutes of Health task force set up to monitor how variants affect tests. In 2022, well into Omicron's dominant era, that team concluded that DIY diagnostics continued to work well. Task force member Richard Creager wrote in an email to TIME that the tests still seem able to catch the Omicron spinoffs that are circulating now. âThe rapid tests are having no issue detecting the variants,â he wrote, noting that the protein that antigen tests look for has remained fairly stable as the virus changes.
Even if tests have remained constant, our immune systems havenât. Early in the pandemic, an infected person's âviral loadââthe amount of virus in their systemâtended to peak around the time their symptoms began. So if someone self-tested on the first or second day they felt sick, they likely had enough virus in their body for a rapid test to detect it.
Now that most everyone has prior immunity from multiple vaccines or infections, the timeline seems to be extended. Viral load now tends to peak around day four or five of symptoms, according to a study published in early 2024. Thatâs likely because the immune system is primed for action by its previous encounters with the virus, so it responds faster, even before lots of virus has built up in the body. A quicker immune response may mean a faster onset of symptoms.
âIf your body has seen the virus before, youâre going to react to it and have an immune response more quickly,â explains study co-author Dr. Nira Pollock, co-director of the Infectious Diseases Diagnostic Laboratory at Boston Childrenâs Hospital. âThat immune response can show up as symptoms.â
From an immune perspective, thatâs a good thing. But it can complicate testing, because it means someone may not test positive for COVID-19 until theyâve already been feeling sick for days. In their recent study, Pollock and her co-authors estimated that a COVID-19 antigen test is somewhere between 30% and 60% accurate at detecting an infection on someoneâs first day of symptoms, but up to 93% accurate on day four.
Based on such findings, Pollock and other researchers cautioned in a recent review article that people who are symptomatic shouldnât assume theyâre COVID-free based on a single negative antigen test result. Although it's a hard sell for people who no longer take precautions, the ideal scenario is for someone to stay isolatedâor at least wear a mask when around other peopleâand test again around day four of symptoms.
âThe instructions for these tests are to do serial testing,â Pollock says. âIf youâre symptomatic, you should test early. But if youâre negative, you need to repeat it, because the amount of virus in your nose may not be high enough yet to detect.â
One other factor to consider: tests donât last forever. Many people stockpiled tests when they were previously available for free through government programs and insurance, and those kits may have since expired. (Youâll soon be able to order four more free tests through the government.) The FDAâs website provides up-to-date guidance on determining how long your kits last.
Expired tests can still work, says Dr. Zishan Siddiqui, an assistant professor at the Johns Hopkins University School of Medicine. He co-authored a 2023 study on Abbottâs popular BinaxNOW tests, which found no major accuracy differences between unexpired tests and those that were five months past expiration. (Siddiquiâs other research suggests unexpired BinaxNOW tests accurately detect COVID-19 cases more than 80% of the time.)
But, although Siddiqui says he isn't too worried about using expired tests, itâs good to remember that their performance can suffer if theyâre long past their recommended use dates. If the control line on your test strip doesnât show up or looks distorted, thatâs a good indication that itâs too old to use, he says. Even in Siddiqui's study, which found that expired tests still work, the lines on the old tests showed up more faintly than on fresh tests, making them harder to read.
Whether your tests are old or brand new, itâs good to use some healthy skepticism when interpreting their results. âI trust these tests,â even as the virus evolves, Siddiqui saysâbut a single result doesnât always tell the whole story. If you have COVID-like symptoms but test negative, itâs best to be cautious and retest in a few days.

Cassie Valentine discreetly glanced at her wristwatch beneath the table, wondering if there was still a chance to salvage her evening plans.
She had already changed into street clothes an hour ago and was ready to clock out when she received a page to report to the Diagnostics Team office. Dr. Ramsey had been clear from the start. The teamâs schedule was unpredictable, and all members had to forego time off as needed.
As the latest and the most junior member, Cassie had to flex more than others since she was juggling a tough residency and diagnostics fellowship. Still, there was an upside to all the time spent at workâshe got to do it with her boyfriend.
Not that anyone else knew the delectable Ethan Ramsey was hers, she mused with secret amusement. She knew, and that was all that mattered.
She side-eyed Ethan at the front of the conference table, facing the videoconference screen she hadnât known existed until today. His eyes were alive with curiosity and interest as they consulted with a colleague in California. The virtual consult would determine if the team was required on-site for the diagnosis.
Cassie wouldnât mind a couple of days of sunshine. Winter had Boston firmly in its grip. Of course, there was something magical about walking through a winter wonderland during the holiday season. Twinkling lights strung on palm trees didnât have the same appeal as snow-dusted oak trees.
âThank you, Dr. Amherst. Weâll review the patient file and get back to you tomorrow.â
Ethan wrapped up the call, and Cassie refocused her attention on the job at hand. She really hoped tomorrow didnât mean they would be working late into the night. But, with this team, you never knew.
âI donât know about the rest of you,â Baz said, stretching his arms, âbut I could use a break before we work on the diagnosis.â
âI agree,â June said. âIâd rather come in early and approach the case with fresh eyes.â
Cassie held her breath in anticipation. Ethan seemed to hesitate, but then he nodded in agreement.
âLetâs pick this up tomorrow morning. Not much else we can do today.â
Baz and June wasted no time gathering their things and heading out. Cassie wasnât far behind, but she stopped when she saw Ethan sit down at his desk and flip open the patient file.
âItâs late, Ethan,â she said, not hiding her exasperation. âAre you really planning to keep working?â
âI just want to go over things while theyâre still fresh in my mind,â he said, sliding on his glasses.
He looked up at her. âGo home, Cassie. Iâll be fine.â
âIâm not going home,â she said, settling into the seat across from him. âIâm heading to Faneuil Hall to check out the Christmas tree and holiday decorations. Come with me.â
Ethan smirked, amused. âIsnât that for tourists?â
âAnd for doctors whoâve spent fourteen-plus hours in a space that smells like antiseptic. I went last year, and it was nice.â
He rolled his eyes. âI see those decorations every day on my way to work. Iâll pass.â
âFine,â Cassie said, pulling out her phone. âIf you change your mind, youâll know where to find me.â
When his phone pinged, Ethan glanced at the screen. âWhatâs this?â
âI just shared my location with you,â she explained. âIâll keep it on until I get home.â
Almost ten minutes later, Cassie stepped out of the car, thanked her rideshare driver, and drew in a deep breath of the crisp evening air.
Just what I needed, she thought, feeling the tiredness fade away.
The scent of roasted chestnuts and pine mingled with the faint melody of holiday carolers stationed near Quincy Market. Strings of twinkling white and multicolored lights wound their way around the lampposts and tree trunks, casting a soft glow on the historic architecture.
Huddled inside her thick parka, Cassie trekked down the cobblestone streets, her boots crunching softly against the uneven stones as she admired the holiday wreaths adorning shop doors, their vibrant ribbons fluttering in the brisk evening breeze.
Tourists exclaimed excitedly at storefronts showcasing meticulously arranged scenes of snow-covered villages, glittering ornaments and festive garlands. Meanwhile, hardy Bostonians paid no mind to the spectacle, staying laser-focused on their holiday shopping lists.
Cassie treated herself to a steaming cup of hot chocolate and a bag of tiny, freshly fried donuts before settling onto a bench outside Faneuil Hall to people-watch.
A massive Christmas tree towered over the square, draped in a cascade of shimmering gold and silver lights that seemed to light up the night sky. Nearby, a family of four posed for a picture in front of an illuminated reindeer installation while a couple took a selfie, their cheeks rosy from the cold as they huddled close.
âGot room for one more?â
Cassieâs heart lifted at the sound of her lover's familiar voice. She turned to see Ethan and couldnât help but break into a wide smile.
âAlways.â She shifted to make room for him beside her.
âChrist! Itâs fuckinâ cold tonight,â he shuddered as the wind picked up.
âYou shouldâve grabbed a hot chocolate on the way,â Cassie teased, taking an exaggerated sip from hers. âHere.â
She offered him the bag of still-warm donuts, grinning when a dusting of sugar landed on his coat and the corner of his mouth as he took a bite.
âRelax, babe,â she said with a laugh as he muttered about the mess, brushing away the sugar particles with a napkin. âYou missed a spot.â
Cassie leaned in and licked the sugar from the corner of his lips. Ethan turned his head, his lips brushing against hers, and then he placed two fingers under her chin, holding her in place as he kissed her deeply and without hesitation.
Firecrackers burst in the distanceâor was it her racing heartbeat and the rush of blood to her head? Cassie didnât know or care as she fell into the moment.
All too soon, their lips drifted apart and the kiss faded into a whisper as their foreheads touched, sharing a quiet, unspoken connection.
âExcuse me?â a manâs voice interrupted. Cassie glanced up to see the other half of the couple who had been taking selfies earlier. âSorry to disturb you, but could youâŚ?â
He held out his phone to Ethan, then glanced uncertainly back at his girlfriend. Cassie grinned at Ethanâs hesitation and decided to take pity on him.
âIâll do it,â she offered, stepping forward.
She took a few photos as the couple wrapped their arms around each other and struck playful poses for the camera. The last photo had them kissing softly under the Christmas tree lights. Cassie thought it might be the best one of all, envying the openness of their relationship.
As the couple walked away, thanking her with bright smiles, Cassie sighed wistfully and settled back onto the bench beside Ethan.
âWhatâs that sigh about?â he asked, raising a curious brow.
âNothing,â she said, avoiding his gaze as she gathered the empty cup and paper bag to toss in the trash.
âDo you wish we werenât keeping our relationship secret?â
Cassie was always amazed at how astutely Ethan could read her feelings. She supposed it was inevitable when two people had been in an intimate relationship of one type or another for almost nine months.
âWeâre private people.â
âThatâs not what I asked, Cassie.â He took her hand, intertwining their fingers. âDo you want to go public?â
âI donât know,â she said. âOur families and my close friends know. It should be enough.â
âButâŚ?â
Cassie rolled her eyes. He could be relentless, like a dog with a bone. She knew he wouldnât drop it easily.
âBut I hate how the nurses flirt with you at work while I have to pretend it doesnât bother me. Or how, if we took a selfie right now, I couldnât post it on Picta because, as far as the world knows, Iâm single.â
âIâm not posing for a selfie on Picta under any circumstances,â Ethan murmured.
âWanna bet?â she shot back, glaring at him.
Ethan smirked, his lips twitching as if to hold back a retort. Instead, he leaned in closer, his voice low and teasing. âYouâd lose that bet, Dr. Valentine.â
Cassie raised an eyebrow, a challenge glinting in her eyes. âCareful, babe. I always play to win.â
He chuckled, shaking his head. âCome on, letâs get out of here before I freeze my balls off sitting on this icy bench.â
She grabbed her things and fell into step beside him, their fingers brushing but not quite holding. As they walked into the twinkling glow of the holiday lights, Cassie glanced at him, a quiet smile tugging at her lips.
Maybe the world didnât need to know about them just yet. Moments like this were enough.

thoughts on adhd diagonsis and the rising numbers of it? heard a couple different theories, including a school therapist saying that he thinks children are just getting misdiagnosed because theyâre cutting recess times, but interested in your thoughts! lol
yea i talked about this a bit here but i would add for clarity:
this kind of narrative of 'rising rates of' [any dsm diagnosis, in this case adhd] is kind of misleading on the surface because these numbers, and cultural and medical attitudes toward these labels, vary widely. matthew smith gives a very abridged introduction to varying attitudes toward adhd globally, and points out that countries that have 'embraced' the adhd diagnosis and its corresponding drug treatments tend to be countries where pharma companies have pushed to expand their market for these drugs, and have been able to succeed in partnering up with local and regional medical guilds and practitioners' professional interests. which is to say that any 'rise' in 'adhd' should be interpreted with an eye to material factors, meaning, specifically, profit-seeking and broader patterns of imperialism and global market expansion.
none of this is to say that the impairments people experience in adhd are any less real, debilitating, or distressing. however, when we ask about those impairments becoming more widespread or severe, often the conversation becomes rapidly re-routed to cover only a narrative of individual cognitive or neurological 'failures' constituting a distinct 'disorder'. elided from this framing is the idea that an impairment of this sort arises not just from the individual's brain-mind-body, but from the extent to which that person is being accommodated by their social context, specifically demands for productivity, sustained attention, &c in the home / school / workplace.
the core research methodologies & data interpretation in the psy-sciences embed social valences into neuro-psychological investigations, heightening the perceived contrast between, eg, 'normal' and 'adhd' brains / neurotypes / &c. susan hawthorne points out that this is a powerful feedback loop: social values are embedded in the scientific investigations, the results of which are then of further social interest, and together social and scientific values tend to converge, mutually reinforce one another, and strengthen the ideas and data interpretations supporting the concept of a discrete, pharmacologically actionable, transhistorical and cross-societal brain disorder.
i truly cannot overstate the extent to which it matters that when ritalin arrived on the us market in 1955, psychiatric diagnosis of and pharmacological prescription for children's behaviours were in a very different state to how they are today. it is quite common (in psychiatry but also in other branches of medicine!) that diagnostic definitions and categories change, or even come into existence altogether, at the behest of pharmaceutical companies who need a diagnostic label in order to ensure insurance coverage for patients interested in taking their patented drugs. this combined with marketing direct to patients, and paid promotion to physicians, is a critical piece of the history of the adhd diagnosis.
because i always feel the need to make this crystal-clear: i do not oppose or object to people seeking or using stimulant medications lol. i <3 stimulants. that's not what this is about. i want you and me both to be able to use white-market amphetamines whenever we damn well please and you don't need to justify that on any moral or medical grounds. xx

Challenge Day 276 of Year 9 Teb was a simple plastiform home kit robot that it's builder less than legally squeezed a grey market AI into. It was fairly sure that it hadn't been put together correctly, but didn't have the tech to do diagnostics on itself just yet.
