Trump’s Proposed Healthcare Plan Explained

September 30, 2020
President Trump signed an executive order concerning America's healthcare plan on September 24, 2020. The purpose of the new healthcare is to provide Americans with "[G]reat healthcare" and restore "choice and control to the American patient." (1) Trump considered the previous healthcare plan, The Affordable Care Act (ACA), too costly and burdensome for typical patients. Most especially the low-income because if one could not afford insurance or simply did not want it, there was a huge tax burden. However, with Executive Order 13813, healthcare options have been expanded to more people by making renewable short-term, limited-duration healthcare plans more obtainable. Also, the Order provides patients with several different healthcare options that can be up to sixty percent lower than the least expensive alternatives under the Patient Protection and ACA. (2) Trump expects his Executive Order to cover 500,000 people who would otherwise not be insured.(3)
Healthcare reimbursement arrangements have been also expanded to cover 800,000 American businesses and over eleven million employees. (4) Within the Order, there is a rule that increases the availability of association health plans for small businesses, which will cover 400,000 previously uninsured people with possibly a thirty percent less cost, overall. (5)
The purpose of this healthcare is to not tax people but help people to save more of their money so they can purchase the healthcare they need and/or want. Additionally, those with chronic health problems can enroll in plans that fit their needs and “be paired with a tax-advantaged account.” (6)
Through “State Relief and Empowerment Waivers,” healthcare is more flexible, which causes coverage options to be expanded and the cost lowered for many Americans. For example, in Wisconsin, premiums were lowered by eleven percent, and in Maryland by forty-three percent. (7) On the other hand, the ACA spent “tens of billions of dollars” on electronic health records systems. But even with all that money, these systems are incapable of recording and communicating patient data correctly and adequately. However, the “America First Healthcare Act” allows patients to personally send their medical records to the healthcare provider they chose. In other words, Trump said he "cut [the] red tape" to save people in the medical profession time. (8) They need not spend their time looking for medical records because the patient will send it directly to them. The Centers for Medicare and Medicaid Services (CMS) and the Department of Health and Human Services (HHS) have calculated that this saves over 40 million hours of wasted time for providers and suppliers between 2017 and 2021. (9)
Prescription drug prices have been lowered, as well. It has been fifty years since prices have been this low. Plus, during Trump’s first term, a record number of generic drugs are approved, which saves patients $26 billion.
Executive Order 13937 will cause impoverished Americans who receive care from a federally qualified health center to have access to insulin and injectable epinephrine at low prices. (10) Executive Order 13938 allows the safe importation of a variety of prescription drugs from Canada, which translates into more savings for patients. Executive Order 13939cancels payments to middlemen. This causes prescription drugs to be offered at a much lower price without increasing the patients’ premiums or increasing Federal taxes. Historically,Americans have spent drastically more money on the same drugs that drug companies have offered to foreign nations. Except the foreign nations have been offered prescription drugs at a cheaper price. Now because of Executive Order 13948, Medicare patients will receive the same price for their prescription drugs that drug companies offer to foreign nations.
Trump has signed the “Further Consolidated Appropriations Act” which cancels the medical device tax, the annual fee on health insurance providers, and the "Cadillac" tax on certain employer-sponsored health insurance. (11) The “Cadillac” tax caused healthcare to be extremely expensive for most American families.
Executive Order 13879, which is called the “Advancing American Kidney Health Order”,gives patients in the final stages of renal disease more treatment options. An example of more treatment options is a program that promotes home dialysis and promotes transplants for patients. One hundred twenty thousand patients with end‑stage renal disease is likely to sign up. Additionally, the financial barriers to living organ donation through increasing“financial support for living donors, such as by reimbursing expenses for lost wages, childcare, and eldercare” have been terminated. (12)
Patients with pre-existing conditions are promised access to high-quality healthcare because of these Executive Orders.Trump said,
In an attempt to justify the ACA, the previous Administration claimed that absent action by Congress, up to 129 million (later updated to 133 million) non-elderly people with what it described as pre-existing conditions were in danger of being denied health insurance coverage. According to the previous Administration, however, only 2.7 percent of such individuals gained access to health insurance through the ACA, given existing laws and programs already in place to cover them. For example, the Health Insurance Portability and Accountability Act of 1996 has long protected individuals with pre-existing conditions, including individuals covered by group health plans and individuals who had such coverage but lost it. (13)
HIV patients are promised $226 million-plus, for treatment, expanded access to HIV care, medication, and prevention services, "We secured a historic donation of a groundbreaking HIV preventive medication that is available at no cost to eligible patients.” (14)
Executive Order 13941 supports small hospitals and health clinics in rural America.According to the previous “volume-based” policy, to get any financial reimbursement from Medicaid and Medicare, hospitals and health clinics must meet a patient quota. “Volume-based” policies were created by people who were insurance executives and politicians and their bottom line was to shape policiesin a way thatthey earned more money from patient’s health problems. Meeting the patient quota was a problem for rural clinics and hospitals because they struggled to meet the quota.However, this Executive Order will correct the problem because the system will be "value-based" in a manner which isto “meet the needs of [the] community” and not be “volume-based.” (15) An example of a “value-based" plan is the Rural Action Plan. The government's website lists its four-point strategy:
1) Build a sustainable health and human service model, including providing education and training to community health providers to increase access to quality healthcare.
2) More than eight million dollars in grants for Telehealth services will be provided to use on technology and to establish new, more flexible plans to improve access to healthcare.
3) Some states will not allow healthcare clinicians to finishtheir training. In other words, the quality of healthcare policies was extremely low. The fix to theselow-grade policies is the Rural Action Plan, which increases the quality of healthcare policies to the rural communities by issuing new policies that reexamine state-based licensing. Furthermore, this Plan invests $5 million to recruit and train more EMS personnel.
4) This Plan focuses on preventing disease and mortality in the poor, rural communities. An example of how they plan on doing that is with the “Healthy Rural Hometown Initiative,” which“identifies strategies to address the growing rural disparities related to the five leading causes of avoidable death, including stroke, heart disease, cancer, respiratory disease, and injury/substance use.” (16)
Executive Order 13890 affects senior citizens by making Medicare more affordable. An estimated $2.65 billion has been saved by seniors because their premiums have been substantially lower, and they have more options in the plans they choose. Trump said,
New Medicare Advantage supplemental benefits have helped seniors stay safe in their homes, improved respite care for caregivers, and provided transportation, more in-home support services, and assistance, and non-opioid pain management alternatives like therapeutic massages. Medicare Part D premiums are at the lowest level in their history, with the average basic premium declining 13.5 percent since 2016. (17)
Those struggling with substance abuse are affected by the “Substance Use-Disorder Prevention that Promotes Opioid Recovery and Treatment for Patients and Communities Act.” This allows the disbursement of billions of dollars of funding for programs that support prevention and recovery.
The “Sickle Cell Disease and Other Heritable Blood Disorders Research, Surveillance, Prevention, and Treatment Act of 2018” will affect the African American and Hispanic population because this disease greatly affects them. This Act provides financial support for research into sickle cell disease and authorizes programs relating to sickle cell disease surveillance, prevention, and treatment.
“The Right to Try Act” gives patients given a death sentence the "right to try" treatments that could heal them without Federal policies preventing them.
Finally, Executive Order 13944 covers the COVID-19 crises. It is designed to “protect Americans through reduced dependence on foreign manufacturers for essential medicines and other items and to strengthen the Nation’s Public Health Industrial Base” by synthesizing the federal and private sectors to develop and deliver safe vaccines, therapeutics, and diagnostics for COVID-19.
