I think a lot of scanlators use !? even though it's incorrect just so it matches up with the rest of the manga in cases where there's a word bubble that just says !? because WHO IS GOING TO TAKE THE TIME TO EDIT THAT? NO ONE, THAT'S WHO

News flash: you can write Japanese text vertically without having to make peoples' heads turn 90 degrees.

You can't do the same with English text.

Rotate that shit.

(And as an aside: the English order for punctuation is question mark first, exclamation point after.)
