Did you know there’s more than one type of top surgery for transmasculine individuals? Depending on the surgery you get, your scars may not look like what you commonly see on photos of trans guys and fan art. Introducing the most unheard of variety, keyhole or periareolar!

This surgery is uncommon because you must have very little breast tissue to get it. Most surgeons will only operate on A cups, although some include B cups. The scarring for this is extremely minimal and sometimes unnoticeable!

My goal with this post is to show the varieties of transmascs out there, and how the most commonly shown type of surgery is not the only one available. There’s no “default” to show a trans man after surgery just like there’s no one type of natural trans body before surgery. 💖🤍💙

Things I learned post top surgery (peri edition) 5 days post-op
Being put to sleep and waking up wasn’t as scary I thought it was gonna be
Huge amounts of tightness and soreness in my chest area
Numb chest
Drains are annoying little bitches
T-Rex arms are real as fuck
I actually had a bit of trouble trying to pee the night I got home, it took my body a bit to get back to being able to pee normally
Neck pillows are both your friend and your enemy
Bad posture due to binder from hell
My back is sore from my bad posture and sleeping elevated
Applesauce is your best friend the first few days post-op
Liquid IV helped my sore throat from the tube they put down my throat
Car rides are a no
Help from others even if it’s just little things is a big yes
LOTS OF WATER
I got orange stained skin from something they put on my body during surgery, don’t know exactly what it is
I’m actually very slow at walking
Walking around every few hours is one of the best things you could do
You need assistance to bathe, and I couldn’t take a shower for 48 hrs after surgery
Wipes are your friend
Greasy hair is expected to
Flannels and sweatpants are the best clothing options
My hunger has been up and down, sometimes I’m super hungry and sometimes I’m barely hungry
Painkillers and Tylenol are your absolute best friend

it's been like 2 years since my top surgery and this is literally the first I'm hearing of massaging scar tissue lumps hello?
YEAH i dont exactly know what causes them, but if you had a larger chest prior to the surgery and also got lipo during ur top surgery it can heighten the chances of internal scar tissue! bc theres just More that has to be removed so the physical trauma is a bit larger ig? i believe this happens after any major surgery in a relatively fatty area of the body, its just how fat Heals i gues !
getting rid of any hardness after top surgery (whether it be scar tissue, hematoma or calcified seroma) usually involves a TON of massaging to help break down whatever it is and have it reabsorbed into the body.
dont get me started on the popping and the squelching sounds that can happen bc of these (i actually got a recording of it to send to my surgeon bc i was Scared LMAO). its kinda nasty feeling but relatively temporary and completely normal. im also only 3 months post op so im still healing:) what matters is my chest Looks good and i feel good n happy! aside from scar tissue pain,,,

periareolar scar reducing revision surgery experiences?
Hey, I'm looking for advice/experiences around a peri-areolar revision surgery.
I had periareolar 5 years ago or so. The shape and position is perfect (shoutout to Kneeshaw in the UK) but the areolas and the scars around them have stretched significantly. I don't have the confidence to go shirtless which was something I'd really hoped for from surgery. :(
I had a consult with Brendan Wooler, very jolly guy at the same hospital. He said they often get the complaint about scars/areolas stretching out over time post-peri, and they will perform revision surgeries 'at least a couple of times' in hopes of getting the desired look. I moved countries since then though, so I thought I'd try and find a surgeon here.
The surgeon here (van der Sluis) seems legit and sensible, super friendly, but he said 'I'm a lot less optimistic about the results than Wooler', and that 18 of the 20 times he's performed this sort of revision, the scars stretch back out. 'No matter if you keep the T-rex arms all the time, no matter if you stick to the exact instructions'. He suggested just getting the shape of them fixed with a smaller surgery under local anaesthetic, and getting the scars medically tattooed to camouflage them.
At the time I agreed, but I am now wondering if I should still give those odds a shot since I have no body confidence right now at all, and scar tattooing has to be repeated every few years. I would obviously prefer local anaesthetic and a smaller surgery (although I'm sure it won't hurt as much as the real deal did), but I don't want to regret that I would never have known whether a revision would have worked out perfectly. I am also a little concerned that the areola position could change, because like I said I am happy with that aspect of it.
For my first surgery, I was told from day one to perform regular physio stretches that were much too drastic (touching your hand to your back over your shoulder!?) which I blindly obeyed. One areola got infected and lost some skin. There was no scar tape or silicone strips either, which the new surgeon recommends. I think I had pretty bad conditions for healing the first time, and wonder if it's worth a shot under these better circumstances.
My question is: have you had a scar-reducing/areolar-reducing revision to peri? Or have you heard about any success rates for it? I want to know whose odds to trust - whether Wooler was too positive or van der Sluis is overly pessimistic. Thanks in advance!

I’m kinda tired of the “trans men have it easier” bit.
Especially as a feminine trans man. Even with years of hrt and top surgery there are times I’m seen as a woman for simply how I express myself. Testosterone is not some magical passing hormone that so many people make it out to be. And if anything, my chest makes MORE PEOPLE stare at me. Even though I had peri top surgery, my scars are very unique and that makes people stare.

A self-portrait honouring the scars of my top surgery.
Most representations of trans men post top surgery depict them as having scars from double incision surgery as this is the most common performed. As a trans man who was very lucky to be able to receive periareolar, I can feel a little left out of that representation.
While the scars are minimal, they can still be something that I'm hyper conscious of and still signify to me an incredibly important part of my life and who I am.

hello doc, in your opinion what is the best method for top surgery (ex. keyhole, double incision, fish mouth) ?
what i consider the 'best' method of top surgery depends entirely on both chest size and skin elasticity! theres no overall best method, just what will be best for the person. the surgeon will most likely chose the method or at least tell you which of the methods they will be able to do for you, and if you want a different method done for whatever reason that is something you will have to talk with the surgeon about.
keyhole and periareolar usually work best for small chest sizes, for a medium size your surgeon may decide on periareolar, buttonhole, double incision, or inverted t, and for larger chests usually buttonhole or double incision! but like i said, it will vary depending on exact size and skin elasticity. that doesnt mean you will get no say in the matter though, just that you will have to talk to the surgeon about specifics and what sort of results you want!

does casio have top surgery scars that we dont see or did he get that magic top surgery?
I actually thought about this a bit while I was recovering from my own top surgery! Personally, I don't mind "magical" transitioning as a trope or concept because you bet your ass if magic was real I'd be using it on myself, and we're all just trying to have fun here anyway.
That being said, in a non-magical or more literal interpretation of the fiction, I've been working under the loose idea that Casio had periareolar top surgery (which, I'm gonna be honest, I did not know about until last year).
For those who are squeamish/don't want to risk seeing medical photos in a Google search, periareolar top surgery is when incisions are made around the areola in order to remove breast tissue. The incisions and scars themselves are a lot less apparent when compared to double incision mastectomies (the other type of top surgery that leaves curved scars at the base of the chest), so they're functionally invisible when it comes to art and drawing Casio's chest.
Anyway, that's about as deeply as I've thought about it. Thank you for the question!

Hello! I'm sorry if this is an invasive question or inappropriate but your top surgery timeline looked really good. The scars were basically invisible; did you get top surgery different from the typical surgery to make it less visible? Are there more options with this stuff?
Sincerely, a inarticulate trans guy planning on stealthing it and getting a load of surgeries.
Hey! There are several different methods for top surgery, it's important to find a surgeon who will go through those different methods and work with you to find the one that works best with your body and your needs.
Pre-op I was about an A cup, so I qualified for periareolar or "keyhole".

So the only scars I have are around the border of my areolas. Periareolar is only an option for smaller chest sizes. Some B cups, but mostly C cups and up cannot qualify for periareolar due to the higher amount of breast tissue.
